This is a Nursing and Rehabilitation facility,I think it is a great place for your loved one.Im also a Ministerial volunteer to the facility.Many of the staff members are new and theres a new Management team altogether.I have noticed nothing but loving kindness, respect towards the residents there at Paradigm Woodwind Lakes. I gave this facility a (five-star) rating! This is not my opinion during my tenure there, but of a truth!

I visited on business...Very clean facility, the smell was pleasant, the floors beautiful wood and the residents looked peaceful, rested and clean...rooms looked orderly. The staff very welcoming, especially the front desk ..very attentive and helpful.

I have a family member there and our family experience with the staff is not good by no means. The staff is not completely honest how our love is doing. They When you do get an appointment to visit your love the staff stands there the whole time like they are trying to hide something. Then they tell you have an hour but tried to cut it short. Our love health has clearly deteriorated in the several months she has been there.

The facility is kept cleaner since Paradigm management took over. Also the staff is very friendly and my dad looks very well now. Also appreciate all the staff that works there. Great job!

This place is absolutely horrible when you walk in with the patients or stay and it smells like urine the nurses are rude. I had an encounter with a nurse Julie Ann who is a nurse there that is very rude and non-professional. Another horrible nurse name Veronica wasn’t any better. When asked for their names to complain they refused to give them and wasn’t wearing name badges. I had to contact Human Resources for their names. This isn’t a place that you want your love ones at. My father stayed here briefly and it was a horrible experience for him and us. They don’t have security cameras to watch the patients so if an accident happens it’s your word against theirs. I saw people in their lunch area who couldn’t hold the utensils to eat and they were not assisted. When I reached out and asked one of the attendants could someone assisted the person. I was told that patient doesn’t like assistance so I continue watching. Lunch is over the tables cleared the person is pushed away back to the room with nothing to eat now this doesn’t make sense to me at all. This place is horribly ran and the people could care less they should not be open and whatever you do do not send anybody that you love there because they will not be taken care of they will smell when you first meet them but when you drop in you will see that the place is not being taking care of. Upon my father’s first arrival we walked into the facility without checking in with the receptionist. She was busy on her cell and didn’t ask any questions we just walk back found his room not good.

Wonderful place! The rehab department is top notch. You can tell the nursing staff cares. Ive worked in many nursing homes and this one is one of the best. No, its not the Ritz Carlton, but it is a nice place. Thats the honest truth.

I give it five stars only because it is housing [_one of my most favorite person_] in the world.....but i would have to say that some of the staff there is very unkind and careless...but i thank my allmighty God for Paradigm....i wish all the luck to them on replacing the bad and bringing in the new....Good everything is hard to come by now,a, these days...i have faith in this time that we are living that the changi is allready a working progress..

Great place with great workers.Edit: absolutely horrid since new management. Covid cases were out of control.

Its extremely hard when youre placing a love one in a home and terribly difficult when youre moving them from another state. Last thing you need is zero communication or being ignored from a facility youre considering. Not sure if this is the way the new owners practice or if the staff isnt trained correctly but I find it rude and very unprofessional. I never received such awful treatment from Monica or any other facility I visited. Zee doesnt return phone messages or emails. Im glad that my mother wont be at this facility. If this is how the staff treats you during the intro stage, I can only imagine my mothers stay being an unpleasant experience.

They took good care of my mom which is the most important thing. Physical therapy did a great job. Thanks to the staff.

This place needs to be shut down and investigated from outside agencies... my Grandma just passed there yesterday a day after being there and was just released from the hospital out of imminent danger only to be brought there negligently left to die alone... family visited the first day and she was good and looked forward to seeing her family the next day but something unexplained happened before the next morning and she was only there supposed for rehab... Wether you kill someone or let them die negligence or not it’s still murder and at yalls hands...No stars and YALL NEED TO BE SHUT DOWN... Horrible experiences for my mother . The facility basically pawned her off to the ER several time and then refused to accept her back. Now two weeks later still waiting for St.Joseph to find a place for her. I told the administration I was going to report them to the state . Shes said , go ahead ! This happens all the time .

Do an open records request for their 911 calls. Their patient care is sub-standard.

If it was possible, they would get no stars. This place was horrible. If I kept my mom there any longer, she would have died. With a one month time span, she had over 7 falls. A couple of them they claim they had no idea who put her back in the bed.

I don’t think it was proper for (whole milk)ice cream to be served to my diabetic dad (has some dementia)I don’t think the nurse’s response was appropriate- “someone gave it to him “ and “he must have asked for it “. My godson arrived in a coma and a week later he woke up thanks to the care, affectionate and respectful care of the nurses and technicians who are very professional, in turn we are grateful for the help of Dr., The Head of Social Work, the Directorate of Nursing , the Administrator who have actively cooperated with government procedures.Special shout out to Trishia the head of social work 👏🤣🤣🤣🤣 and receptionist 👏👏❤️❤️❤️ for all the compassionate and emotional support for the whole family.
