The Team at Pathways Memory Care at Villa Toscana has made the transition of our mom into their facility a very reassuring and comforting experience.The staff at Villa Toscana has been nothing less than warm, welcoming, caring and patient with us. The team truly has given us a sense of ease and relief in making this critical decision. Where do I start. The first night my husband was there, his new electric razor was stolen. During his stay which was 20 days, his shoes were stolen, his chocolate I brought him and some of his clothes. That is the least of it. This was a horrible experience from beginning to end. This facility is also suppose to be rehab. My husband of course had memory issues. I was hounded daily that he could not focus to do the rehab. So, I told them no problem, I would take him home. They immediately starting No, NO, NO. Of course they did not want to lose the Medicare payment. Since I lived very close to the facility, I decided to go one morning at two oclock. Once I entered the foyer, before I went into the door to the memory care unit, I heard very very loud rap music. Once I got in, I found my husband on the bed without any covers, not even a sheet, and he was in only an undershirt and underwear. He was very very cold. I found the help in the dining room dancing to the very loud music. On another occasion, a friend of mine whose husband was also there, found my husband on the floor in his room. When she told a staff member, her reply was He is always on the floor. My nephew went at 8:30 at night and found the same situation with the music, very very loud. He looked in my husbands room and He was not there. He went into the living room and found a staff member asleep on the couch. There was a man who he thought was my husband, curled up on a sitting bench next to a wall with half he body hanging off as if he was going to fall. He woke the staff member up and angrily told him to help that man and where was his uncle. They found my husband about to fall out of his wheelchair, locked inside the nursing station. His bedtime was suppose to be 7:00pm. As far as the PA that would come in, my husband was having lunch on day when she was making her rounds. He bit off the tongs of a plastic fork and swallowed it. I reported it to her and she looked at him and said not one word. This is a horrible horrible situation. They people that have given it high praise were probable not there everyday as I was. He was horrible. I took my husband out 1 day before his 21 days. They argued with me, telling me Medicare would not pay for the stay if I did that. My sister and I put him in a wheelchair and wisked him into my car. Then I was sent a bill which of course I did not pay and sent it back with a note. I have never heard from them again since I told them in the note my husband and the nephew that came to visit were both lawyers. Please heed my warning about this place. It is awful and dangerous.

I would never recommend this place to anyone for the famiy or work. It is not memory care! The clients dont get change like they are supposed to. Understaffed! Management/admin is vindictive toward the staff.Just stay away from this place.Feel bad for the ones that dont have family.

We have been so lucky to have my Mom at Pathways. Im not able to visit at often as I’d like but we know that mom is well cared for. There are daily activities to keep the residents active and the nursing staff is generally wonderful.I would recommend Pathways if you are looking for a good, clean and caring place for your loved one.***** update***** COVID has been SO difficult for all of us but especially loved ones in nursing facilities. We haven’t been able to visit mom in so long because Pathways is really taking every precaution with the residents health. While it’s been terrible for us not to be able to hug mom the nurses and the aids go above and beyond to help us keep connected. I can’t express our gratitude to these wonderful people for providing the most wonderful care for mom. They are always updating us, and are always willing to bring mom to her room when we want to do a video call with her through the Alexa device that we had them set up in her room for us. We are able to “drop in” on the device randomly as well and can see how well the staff is caring for her. (We provided this device but they also have tablets for families who want to do virtual visits)Of course these amazing people are undoubtedly feeling the stress of this pandemic and we are so impressed at the precautions they take to keep everyone safe. There hasn’t been a single case of covid that I’m aware of and the residents are all in the process of getting vaccinated !!

Most of the staff were compassionate and caring. The nurses gave excellent care. The Activities director worked daily with residents to engage them in various activities. He brought in people to stimulate/entertain the residents. The food was very good. Linens were somewhat limited. Had to wait for clean sheets sometimes. The doctors were concerned and listened to residents or loved ones. They were available to be contacted at all times. Overall a good place for a loved one. I highly recommend Pathways.

I live out of state. Lately, the nurse cant seem to find the phone so I can talk to my mom. So nice of them. Bad enough I dont live closer. I will make it an issue when I visit in October.

My wife has been at Pathways Memory Care for almost 4 years. Her previous facility was unable to provide the care she needs and recommended Pathways. From the very beginning we were greeted and welcomed to the facility. It is a very nice facility, clean and well maintained.I haven’t had any complaints the entire time we have been there.There are four separate units with three currently occupied.There have been turnovers with staff, nurses, and care partners. Although consistency would be great for the residents, we recognize this is a fluid industry. Still there is a core nursing and care partner staff that have been there since we arrived.I would highly recommend this facility’s to anyone who wants their loved one to get great care.Some former family members who have left and moved their loved ones to another facility had unrealistic expectations. Some expected a personal care partner for 24 hours a day. The facility is staffed in accordance with state guidelines.The dining facility is well maintained and meals are se bed at regular hours. They also provide room service dining for meal times.There are daily activities for the residents, based on the cognitive abilities.If you feel you would consider Pathways as a place for your loved one, call and make an appointment to visit and tour the facility. I highly recommend it.

Maria Johnson was very rude. Stated that she could only accept medicaid pending. Would not accept my father who has already been approved for medicaid. Had this info been disclosed prior to my 2hr trip to tour the facility, i wouldnt have even wasted my time.

They appear and present to be a Memory Care Facility gow ever they are listed woth the State as a Nursing Facility.Being they Sister property located a few feet away is a Nursing facility...why two that does the same thing. Staff some good, some bad, some caring and others could care less. Very short staffed, not consistent, short on supplies and linens. I have reached out to their Corporate office and no reply. Just check on your love ones regularly.

This has been a pleasant and safe place for my parent who has succumbed to a progressively degenerative neurological condition that has taken away independence, self-care, cognitive and emotional functioning and regulation. Any person in that situation requires solid 24 hour care from adept staff of all levels of expertise as well as the dedication and advocacy of at least a single or team of family members or friends when at all possible (an extremely demanding situation for the resident/their family and the staff!). We have experienced a variety of facilities and know there is never perfection (4/5 stars) due to staff turnover and changing owners and the occasional rotten apple of caretakers/providers and even residents/roomie challenges - BUT this group of staff and leadership has consistently been responsive to any concerns or problems that have arisen, working with us to provide a continuum of care and interventions to assist my parent through this journey and to maintain dignity as immediately as possible. Just make sure you recognize and introduce yourself to all players at all shifts if at all possible and never hesitate to speak out directly to folks on behalf of your loved one. One story and small feel of neighborhood setting and courtyards are the bonus! (from experience - not an institutional feel) Cameras and phones are optional, private and medicaid pay, in house Physical therapy and maintenance, activities, dental, podiatry, open visitation with security codes, mobile X-ray. Responsive doctors, NPs, psych staff, social worker, etc will work with outside providers as needed. Support Group and Family Counsel participation is encouraged if possible.

Reporting to state today. Controlled substances “disappear” having to reorder prescriptions for patients because class II meds walk off somehow, rude staff - low amt of staff on floor most of the time - staff texting when on floor, taking a NPO bedridden nonverbal patient to the breakfast area in a wheelchair to eat breakfast....(again not checking patient info. 1 patient cannot eat 2 patient is at risk of falling...so let’s put them in a wheelchair and walk them to the breakfast area and walk off). Patients void in rooms and minimal surface cleaning performed - feces remain on floor. Lice breakout in facility. List can continue.

I just recently started reaching out looking for a facility that could help me care for a family member of which this is one of the hardest decisions our family has ever made. To trust a care facility with our loved one. To know without a doubt that they will be cared for unequivocally with the most pristine surroundings, staffing with a positive attitude making safety one of their most diligent priorities making sure that every door is locked behind them so that residents don’t wander off. This administration at this facility spares no cost setting up an arts and crafts rooms so residence can be wheeled up to a table and assisted doing crafts, flowering an assortment of other things totally amazing. The kind of hands-on care me and my family are really looking for when we look to facility such as this one I want to thank you for such a fabulous tour of your facility and will let you know that you are our top consideration for the job. 🙏 this is truly been an answer to prayers.
