I just want to take a moment to give a shout out to Kathy Giba for the tremendous help she has been during my mother-in-laws stay. Due to covid and our inability to visit with my family member it was hard to have a good understanding on what her treatment was like, how she was doing, etc. While we were able to call into the facility and talk directly with our love one, we never got the chance to speak with a doctor. Kathy was very helpful in making sure we were up to date with important information pertaining to my love ones insurance and discharge process. My mother in law was here about two years when recently decided to house COVID patience. They didn’t let my husband(her son) who is the one in charge if he wanted to take her home before they housed the covid patience since she was on oxygen and very poor health condition. She contracted COVID and two weeks later she passed away. I would not recommend this place at all. Should not house seniors with COVID patients one or the other. Just a horrible place! I wouldn’t even give it one star

The staff is really hard working and over worked but they try there best to still have that quality of care not number of care. The administrator puts so much stress on everyone and because of that stress towards the management and other departments they feel the need to act like everyone is the boss of every department so the supervisor house keeping thinks he can boss the nursing department the supervisor for medical records think she can boss everyone that walks in her path. Even the admissions department thinks they can walk the halls and try to push that im better than you. There is no team work there is no happiness but negative vibes everywhere. Which in all effects the quality of care needed for residents.

I used to work here for many years and all i have to say is take your family or your self else where, this place has no consideration of staff or patience, the administrator is not fit for the position, i feel bad for the patients that have no choice on being there.

Visted my grandma here and all i have to say is sad.All the staff looked like they are forced to do there job, my grandma said that she complained to the administrator and he did nothing for her, and i dont want to be that person but it seems like all the department heads and administrator is Filipino says alot about this company.

The best nursing home in the area. The staff is courteous and friendly, going beyond what is expected. I would recommended this nursing home to anyone that needs rehabilitation in the AV.

For it being what it is.. I was happly surprised by how Harm and Caring the staff is . if i had a question it was answered if i didnt understand something they explained. .But to be honest.. It makes me want to be with her more.. I wish the page gave u information on becoming caregiver/primary caregiver / and disability,living and or finical help

I loved this place because of the nice employees that are working hard and keeping the place tidy

The worst place to take a family member. My father was there for months. Constantly getting sick. Everyone in that place had cdif. Once one patient got it, everyone got it. Obviously the nurses were not cleaning hands between patients. There were many other issues but this one was one of the biggest. I found a medical rehab in van nuys, of Sepulveda. I cant not stress enough how much better this place was. I know its a drive but well worth it!!

The place is not clean, disgusting

Please do not send anyone you care about to this place. Patients are given cold, tasteless food, they never receive their medication on time, drivers who transport the patients are wreck less, nursing staff are rude to the patients, and the list goes on. Even starting with making a phone call, the lady who answers is extremely unpleasant.

*12/8/16: Not given heart meds in the morning and mom had heart problems at 10:30am.*12/9/16: Meds not given before breakfast.As of today Primary Well Springs doctor hasnt seen mom. Later in the evening wrappings on legs not changed and wounds not cleaned. Doctors assistant never came when they said he/she would.*12/14/16: Found out that they dont have my mom on her propranolol that she has taken for her heart for years. May have contributed her heart pain episode she had which brought down her health. Nurses didnt give my mom time to get into bed. Twisted her left leg (the damaged leg), below here knee cap.*12/15/16: Spoke with administrator and found out that A.V. hospital took my mom off propranolol that she has taken for her heart for over 5 years. Propranolol not given at dinner time medication. Nurse said it would be giving next morning. Mom received Musinex at dinner for her cough finally.*12/21/16: Mom was sent to see doctor on 10th St west. She sat in her wheelchair for 8 hours and got sick from the cold and rain. Sent to doctor just for a consultation. In my opinion that could of been done at the nursing home.*12/23/16: Mom wasnt given meds so far its 10:10am. She takes heart medication first thing in the morning every morning.*12/27/16: Mom wasnt put in bed properly by the CNAs. They got her out of her chair with her bad left side against the bed. Was informed by a therapist that it was the wrong way to get my mom from her chair to her bed which resulted in an injury of her left leg. Mom was given a norco and had no xrays done to see if her previous femer fracture or current hip fracture was damaged.*12/28/16: Mom wasnt cleaned at all during the day shift and sat in her urine all day. Considering moving her to a different facility now because this one isnt treating her properly or good at all.*12/29/16: mom was given bell pepper in her eggs again after telling staff 3 times that she has no gulbladder and cant digest it. This week she has been given spicey food several times and mom has a stomache ulcer. Mom never got to talk to the dietition even though they said she would.*12/30/16: Got to nursing home at 11:55. Mom had no wrap on her right leg and her wound bled all over her sheet. Noticed blood and dried blood around her wound. Called for wound care doctor to come clean her leg and look at the blood. 12:35 wound care still hasnt came to take care of my mom.12/31/16: mom called me at 5:30am telling me that she wouldnt be getting her Nexium. Called the nursing home very unhappy and spoke to the pill nurse. She was very rude to me and then went to my mom after I called and told her she doesnt appreciate when relatives step in. I want her out of this place but no other physical therapy nursing homes in antelope valley. Contacting lawyers now to see what our legal rights are in this situation. 2:30pm, mom still hasnt received her prescribed Nexium and was given tums as a replacement which arent even close to what she takes. This is unexecptable. I wouldnt advize anyone to put their loved ones here.1/3/17: Moms hip and left leg are still hurting from the nurses getting her in bed wrong on the 27th of december when I and her best friend were there having Christmas. We both witnessed their actions. As of now no x-rays if her left hip and leg have been taken.

My Dad was sent there from the Hospital from a Amonia he was there one night next Morning end up in Emergency Room dont know till this day what Happen the night, worts place in the AV is a Sad why with so many bad case why still Open

Horrible place, the staff allowed my mother to be badgered, abused and neglected while in their care. If your family member or friend is there long term or even short term, go check on them PLEASE!!! They have changed their name twice within the last 6 months.

If i could go below zero with a negative I would. I am always on hold. I get hung up on. My grandmother never gets my messages. The staff is rude.

I regret that my father was ever in this place.

I have a friend who is residing there for rehab with a broken hip..She was given a sling wheelchair without a rigid base in the seat to support the hip..or a cushion in the sling back wheelchair.She has a bedsore now..and still doesnt have a proper wheelchair with solid seat to sit on….She says she would never recommend..this place..She has to wait two hours to get a pain pill last night...
