Doesnt deserve ANY stars! There is NO care at this facility! Not enough skilled staff, Management never addresss any issues. Staff is rude and uncaring. Return phone calls to Administrator or Social Worker are never returned. Patients never have their own clothing even though they are labeled. My family member waited 4 months for a podiatrist to be scheduled to the point where he couldnt put his shoes on because of the pain. Lack of personal care for patients. This place should be shut down! For the cost of this facility the staff should be making above minimum wage and the patients should be able to have a one on one aide. Medicaid pays an exuberant amount of money, but its obviously making the Corporation is lining their pockets with the profits.Family member has been removed from this H--- H----.

Id give ZERO stars if I could. Its unfortunate that nursing staff purposely doesnt identify themselves so you dont know who youre speaking with regarding your loved one and their care. I document everything, report issues immediately, apparently this is the norm here. However, its against policy to be unprofessional and withhold vital information about a patient. Nurse Sherry in the Pavilion, one day Gods going to spin the block and youll be living in the shoes of your patients. Ill happily smile when your nurse hangs up on your child when they desperately just want to know youre well.

After a bad experience at another long term care facility, Silver Healthcare Center was a breathe of fresh air. They took excellent care of my father. Communication from physical, occupational, and speech therapy were strong. It was easy to schedule outdoor visits. The business office was friendly and helpful with any needs we had. I would highly recommend Silver Healthcare.

After arriving at Silver Care from another horrible facility. Its been a total nightmare. People are using Covid as an excuse not to do their jobs. My loved one was sent to the emergency room yesterday on top of all the other crazy stuff that has been going on. Now I cant get anyone at the facility to give me any information on how hes doing. Everyone is in a Fake meeting. And all of their voicemails are fulI I do not recommend this nightmare of a place to anyone.

Horrible. Worst of the worst. Dont believe that they have changed at all. No communication. Apparently the staff spend their entire shift in one long meeting.I would bet a lot of money that most of their positive reviews are paid for, or by staff. You cant go inside that place and then give it anything more than one star.

My mother and my brother are staying here. This facility truly cares about the patients that live here and the family members that are related to the patience. I live in Los Angeles and I truly love the fact the the facility cares enough to allow a member of our family that lives in the area to come there and the facility brings out my mom and brother on the porch area of the facility and allows them to be face timed so that I can SEE and TALK to my mother and brother in assuring me they are being very well taken care of. And we are so so so very happy to see each other that it brings about smiles and tears of joy just knowing that they are being very well taken care of. I love this place and the staff members that work there. God Bless them ALL ,SINCERELY!

My brother is a resident at Silver Care and has been for almost three years. I am very pleased on how they have been caring for the residents at this time of Covid. They appear to go above and beyond to keep them safe and update me regularly. They have made every effort to help me be in constant contact with him via facetime, etc. He receives his lifesaving treatments without fail. The administrative staff is very professional and helpful. Update: I as well as my brother are experiencing long delays in getting responses from the staff on problems that come up both administrative and day to day care.

I have a relative who has resided on Court 2 for about 2 years. This floor is for people who have psychological issues and often require a place to live long term. This facility has improved over the last year. I appreciate the dedicated workers who show up everyday and attempt to improve the quality of the residents lives. Special mention to Regina (Nurse), Ron(Head Nurse), Tai ( appt coordinator), Ceria (receptionist and all around mediator). The current doctors and nurse practitioners are excellent and work well with the patients and family members. The activity personnel Courtney and Agnes, really try to engage the residents , especially during this time of covid. The kitchen attempts to meet the residents nutritional needs and preferences.The new director Karina, is trying to employ excellent personal and improve the facility overall. Although, If you need to speak to her, she refuses to return a call or is just always to busy to talk to you directly. Another problem is staff turnover in the social work/counselor department. The people they hire seem to be sincere, and then ,IF they stay long enough to become effective, they leave. It is important that this department stabilizes. I used to do my relatives laundry because items where frequently missing. Since covid, all of her laundry is done there. There continues to be challenges to return clothes and bedding to the correct resident. This continues to be a source of frustration for my relative and myself.Since coming to Silver Healthcare, the overall quality of my relatives life has improved greatly, but there is still room for improvement. As with any facility it is imperative that a resident have an advocate.Silver Health Care, Thank you for your response. You asked me to call you directly with my concerns . I have , many ,many times.I have called about the concerns previously stated and more serious concerns I did not feel comfortable publicly stating. The problem is when situations have not been resolved and I call The Administration, My direct calls were not returned . I was repeatedly told you were in a meeting or on the phone. This is not a new situation. My number is listed as a contact..Previously, I have left it for you...maybe you should care enough to return relatives calls.

This is the worst place my mother came home with no coat in below 40 degree whether no shoes I sent her wheel chair and she came back without it need to specialize in care and love out the door I am filing a major complaint !!!!!Horrible place for a live one! Don’t sent your love one sent clothes for her to change never once used them The worst !!!!!

I was a patient for several years here, upon my recovery the staff was so helpful and encouraging. I lost my father through the COVID and staff helped me through my loss. Staff is thoughtful and encouraging they really care about their patients.

One of the dirties facilities I have ever seen. The entire place smells like a mix of cigarette smoke and urine. There is trash overflowing and it has to be the most unfriendly staff I have met. Im not sure how they are open and I wouldnt put my worst enemy in there.

Ive been a patient at here since late August 2017 and have posted before. Id like to say I stand on integrity. Ive had my share of ups and downs here and every time I have an issue or complaint its addressed immediately by supervisors , management and the administration . This place isnt perfect . To say otherwise would be a lie but I can say that the above mentioned strive for perfection everyday and in my opinion genuinely care about every patient here . The social services department here is great as is the receptionist who greets you when you walk through the front door. Silver health care gets two thumbs up from a guy who started out in a coma and on life support.

I came to silver health care ventilator dependant and had no use of my right arm or legs after suffering a stroke. I am now able to walk and and am even gaining use of my right arm and hand again . The administrator and nursing manager are so helpful and quick to address any issues or concerns Ive had. The staff are very helpful as well .

If you care about your loved ones don’t place them here. My grandmother with dementia who’s a fall risk was placed into this facility through Kennedy Hospital for rehabilitation to get stronger, after about 3wks in she was hospitalized with a skull fracture, swelling on the brain and a fracture in her eye socket. (She fell). When going to pick up her belongings staff was unfriendly accept the nighttime supervisor that was on that night, most of her belongings where already missing (It wasn’t even 24hrs), we where suppose to get a call back from the administration (Never did), I am now without my Grandmother whom passed away because you don’t have a full staff or enough staff to watch the patients or whom are fully knowledgeable of elderly dementia patients whom are at a risk for fall.I work two jobs and my mother and I cared for her better than this facility did

Once again the silver health care center is still operating? How can this deplorable and disgraceful location still be in business? the staff is worse than the services that they provide! Please God dont let me end up in a place like this.... Will make it my mission to make them accountable even after I remove my loved one from this HELL!. The state inspectors and social security department should be ashamed of themselves and investigated because someone in those institutions have to be receiving kick backs to allow their behavior to continue for so long.

This place is HORRIBLE. It’s like a ghost town. All the workers are nowhere to be found and you don’t really see the patients. We chose this place because they have Dialisys unit . Well my son had to be transported to his usual dialysis center because they didn’t have a spot for him. Well then they never should have accepted him. He needed a stretcher and the nursing supervisor changed everything to a wheelchair. He just had knee replacement and was not able to bend knee just yet. Night shift staff does not want to be bothered. It stinks so bad in the rooms. When u first come in and on the website it looks fine . Rooms are so old fashioned. Beds look like they are from the 80’s or 90’s. The bathroom floor in his room was so sticky that my foot came completely out of my shoe , which was stuck to the floor. This place is a HOT mess. The one nurse Tammi is the only one that seemed like she genuinely cared. The night nurses have an attitude of not wanting to be bothered . This is no exaggeration, it is ALL true !!!!!!!!

Everything is good. Dialysis is awesome. Therapy is good also. Part of the reason I came here is because my uncle was here before on long term for about six years before he passed. He got great care here. So I knew I would get good care.

Wow just read Lisa’s review, it’s spot on. My uncle was on court 2 and the residents would wonder the halls with no supervision, clothes soiled from head to toe. I came to visit my uncle and his room had feces smeared all over the walls and in the air conditioning unit, he told me one of the resident had wondered into his room and made this mess. My uncle was unable to get out his bed and pushed his call button for someone to get the wondering resident out but help never came. When I got there his call bell was still going off and he was screaming for help as loud as he could. I left several messages for administrator sandy she never returned my call. I spoke to the DON Liz and she brushed the issue off, I was actually taken back a little about how rude she was. Thank God my uncle is no longer in that place!! Every time I would come there to visit, call bells would be ringing while the staff was hanging out at nurse station. Please take your love ones somewhere else. AND PLEASE IF YOUR LOVE ONE’S ARE IN THERE GET THEM OUT!!!! There is no respect for human life In that place. No dignity in that place and absolutely no leadership In that place.

My mother-in-law has been a resident here for several years now and she loves the staff so thats why she is still there. Fortunately, she has very good care from her aides because if she wasnt happy we would have taken her out of there a long time ago. The so called leaders in the building are far from that. I complained one time to the administrator who runs the building and she never responded to me like she said she would, I guess she is too busy harassing her good staff. It really is a shame the way she talks down to her staff as she walks around in her hiked up dress with her face in her phone or saying curse words with her friends while laughing. Its terrible to pull up to a building and see a pow wow of smokers that includes her and to see her actually kiss one of the male aides after flirting with him. She works the poor aides and nurse to death just so she can get a good bonus because the safety of the nurse:patient and cna:patient ratio is uncalled for. What aide can do hourly round with more than 20 patients and give good care?? Something needs to be done about her or you will lose all of your good staff. I asked her boyfriend for a number to corporate and that was never give to me. Thanks to the aides and nurses who take care of Momma and God will bless you.

My Mother was in the short-term area and everthing was excellent. She was losing weight when she went in talked to nutritionist and formulated a program for her, she gained back 16lbs. Aids Dee and Liz were exceptional, nurses were all excellent and answered all my concerns. Second shift and weekends seemed to be a little short handed but were very good. The rehab staff were all great and got my Mom strong to come home. The food was good for institutional food, if you go to dinning room they got a choice of two different entrees if they are on a regular diet. Overall everything was much better then other places Ive had my Mother. Would definitely recommend for short-term care, care, Mom was in for over two months.

The people were nice. Nursing staff and administration especially. Dialysis was very good, I was happy to have dialysis in the same building so I didnt have to be transported. My only complaint is I wish I could have more therapy.

There is not even a word in the dictionary to describe this horrible place from the minute you walk in the door. The smell in the facility was horrific, so bad you gagged. When I opened my Uncles room door, I saw him laying there in the dark, with the curtain hanging off the wall. It was just inhumane, and the smell in his room, I almost threw up. The next day I called to complain, where I was told that he didnt want his room cleaned, he told them he had asthma and couldnt take the smell of chemicals, My Uncle has Dementia, so I said and you listen to him and didnt call the family, he does not have asthma? And if he really did have asthma, wouldnt you have an alternate solution to clean his room? So basically instead you just havent cleaned it? The next day my Cousin goes in to only find him eating in the dark, no working light, non working TV. We are working on getting him out of here, I am in disbelief that this kind of place even exists. Also, this was not the floor my Cousin was showed that he would be on. The floor is dark, dingy and beyond depressing. Please do not put your loved one here.

My aunt was a patient in the sub-acute wing at Silver Healthcare Center. She went into the facility totally unable to walk, after suffering a stroke. I cant say enough good things about her (and my) experience there. The nurses, and aides are extremely attentive. Anytime she needed them, they were there. Elizabeth and Delores were especially helpful. They always tried to keep my aunts spirit up so that she would keep working hard to get better. The physical therapy department was WONDERFUL. In the 4/5 months that my aunt was there, she went from no chance of ever walking again, to walking out of the facility on her own. It was a very emotional day for all of us. You always hear terrible stories about these places. I had many reservations about sending my aunt to any facility. I spoke with the Administrator, Sandy, and she could not have been more pleasant and accommodating. I was really impressed with her overall demeanor. To say that she put my mind at ease is an understatement. She was awesome! I felt as if she valued the care of my aunt as if it were her own family. By the time we left, I really did feel like the staff at Silver was part of our family. I honestly cannot say enough about everything that we experienced at Silver. I have read some of the reviews and I was really surprised at the things that I had read. My aunt did not have any of those issues during her lengthy stay. I am not really big on writing reviews but I wanted to let everyone know how wonderful this place really is. If you have any doubts, you should really go and see it for yourself. Angie in admissions gave us a wonderful tour of the facility and I am sure that she would do the same for you. I could not have asked for a better place to care for my beloved aunt. You wont be disappointed!

Hello, this is really a great place to stay, the nurses and aids are delightful , they really seem to care tbe patients, please bring your loved one here , its really peaceful! To a point, but if I dont get my money soon I am going to make so much trouble its not funny! They have a total nut case sneaking into my room, this morning while I was taking a shower and I caught her ! She belongs upstairs with the other nut cases !And they keep sticking up for her and she keeps asking me personal questions and so does her mom about when I am leaving? Its really none of their business!!I want my money now !!!

If I could give less than 1 * I would. Please do not put your loved one in this facility. There is no compassion or care involved especially when theres a 43 to 3 patient to caregiver ratio. Ive never seen anything like this place. From the time we arrived its been a nightmare. Upon our arrival My mother was not even placed in a room and was greeted by a ranting nurse who was upset that the hospital did not call ahead of time and give him a report. While my mother and the EMT transport is waiting to get her settled hes running up and down the hall asking family and EMT for the hospital phone number because someone is about to get cussed out He continued ranting about how they dont know how to do their job. This was totally unprofessional and uncalled for. And very unsettling for someone who is very sick and just coming into a facility already with anxiety. A second nurse sat idly behind a computer unresponsive to the EMT when asked what room the patient was going to. She totally ignored the EMT. When we finally found her room it was disgusting. I had to go out purchase swiffers and Lysol wipes and a pillow to accommodate my mother and to clean her room. The pillow they gave her was not even suitable for an animal. The floor had sticky substance all over it and there was trash on the floor and overflowing in the trash can for 3 days so clearly no one is cleaning the room even as the nurse stated that the rooms get cleaned every morning this clearly was a lie. I visited my mom 3 days consecutively and her trash is overflowing. She went without medication for two days because they had not ordered it. They forgot her meal six times. I understand mistakes happen but this is unacceptable. Then when they finally brought her a meal it was not fit to eat. My father and I had to bring her food. They also tried to keep her confined to a bed when she is completely mobile and able to move about. Medications for gotten meals forgotten this is unacceptable. Shes there to get better and these situations just contribute to making her feel worse. This place is terrible and I would highly recommend that anyone avoid it at all cost.

Recently went on a tour, this place doesn’t exactly show like the pictures. I met with Angie for my tour, she was really nice and professional. Although this place didn’t show like it was pictured I was really considering putting my father in there. Until I met administrator Sandy, I was extremely turned off after that. She was rather rude and was talking In the hallway about non work related stuff while a call bell was going off for quite some time. After seeing how the administrator carried herself, I knew this wasn’t the place for my father.

This place is definitely NOT recommended!! If you actually love your mother/father or sibling, please do NOT take them to Silver Care! My mom who is a diabetic, was given 2 hot dogs after they skipped breakfast!! After I complained it was repeated the very next day!! Not to mention she had a lung operation and they were not taking care of her wound properly! Her wound had a dark color along with some discharge!! After so many complaints and negligence towards my mother, I put in a request for her to be discharged ; instead of 24 hrs, it took days before they did anything!!

Silver Care has gone down hill drastically since administrator Sandra took over. The buildings have become bed bug and scabies ridden. The Court 2 unit is covered in feces. If you take a walk around court 2, you can see feces and urine bleeding through residents clothing. In the pavilion unit they abuse patients physically and over medicate them. The heat in most of the building is broken, very cold especially for elderly residents. The parking lot out back of the kitchen is filled with trash. This trash contains food, dirty diapers, hazardous waste such as needles all thrown on the ground in a huge mountain like figure. The trash is built up because the compactor is filled and won’t be picked up due to non payment. This facility is a complete disgrace. Nursing fraudulently staffs the facility to meet state staffing requirements. There are several spots around the facility that leak and have mold growing. I’ve also seen mold inside the ice machines along with slime, it’s an absolute disgrace. The elevators are always breaking down often times leaving visitors, residents and staff stuck. The solution to the ongoing elevator issue, is a sign taped to the door stating “out of order”. Imagine being transported from court 2 dialysis and the elevator breaking down leaving you stuck In it with no way to call for help because the elevator phone is broken and the emergency bell doesn’t work. The vent unit could probably use more nursing aids to change patients this floor has a horrible smell at all times. I honestly can’t see how the administrator ignores these issues and keeps her job. For that reason keep your love ones far far from this place !!!

My aunt is a resident at silvercare subacute. The nurses are good. The attitude of a couple of the black nurses was unacceptable according to my aunt who is a patient. They have no personality. But everyone else was very good. The aides are especially attentive and sweet. They make her stay here bearable . The food is horrible. The family was always bringing food. Other than that she would n I think have eaten. You really need to up your game with the food. There is so much waste. As far as financially you would save money by having food that is edible and pleasing to the eyes.

They almost killed my grandfather. If you care about the person you are seeking care for, send them anywhere else. The staff is inattentive and does not make any effort to help their patients rehabilitate. We had to buy our own depends for him and still he would sit unchanged for hours. Their alarm system was broken for two weeks and nothing was done. We were told he would never be off the ventilator or go home, and since moving him to a different facility, his trach collar was removed in a month. A few staff members made an effort, but it was not enough to redeem this horrible place. Please do not overlook these negative reviews. They are all an accurate reflection of this poorly run health care center. Anywhere else is better than here!

Let me start off by mentioning if your loved one cannot fend for themself and needs to be monitored closely, this is NOT the place for them. Although my experience was a short one I quickly learned that patients go HOURS without being checked on.My mother was hospitalized in February for 3 weeks. While there she had a trach put in. When she was well enough she was moved to a specialty hospital to be weaned off the ventilator. She spent a month there. She was doing great. So well that they felt she was ready for her next step before going home, a rehab where they work with patients on vents. I chose Silver care because they had what I thought had the better ratings according to Medicare.gov (unbiased) in the area.My mother entered the facility on a Monday evening and sadly expired on Friday morning. I honestly think it was due to neglect. I cant get answers from the staff and I have yet to hear from a doctor.If you love your family member DO NOT place them in this facility.

My mom was in there 3 weeks for theraphy n ended up in the trama center in the hospital. With brain injurys n swollen eyes so she cant open them due to a fractured eye sockets n a tube to breath from.if u value your family or realative dont put them in Siler care on brace road .because of the incompatint workers i may lose my mother.

My next door neighbor has been in this facility for approximately 6 months now and the other day she was upset and called me asking me to please come in to help her. I immediately came in to be of her assistance being that when she was at home I took care of her every day. The CNA happened to be on their lunch break so I asked someone else for help. The lady name Helen L. immediately asked me if I was ok when she saw me in the hallway and I explained the situation and asked her if she was and aide and could help. She explained that she does the scheduling but is a certified aide and would be glad to help. She then stopped what she was doing and cleaned up my neighbor and got her everything she needed. This was such a blessing and it was so nice that she didnt hesitate or complain when I needed her. Some times you ask people and they say oh thats not my patient or thats not my job, but not Helen, she was ready to help and it really made me much more satisfied with the building and I feel much better knowing that the nursing home does have some genuine help. Please send her thanks as I have not seen her this weekend to thank her myself. God Bless you and all the patients and hope everyone gets strong and has such good care that my neighbor received that day.

This place doesn’t even deserve one star. My husband was in the Alzheimer’s wing for three weeks. I was constantly turning up the thermostat in his room because he was easily chilled in air-conditioning but the aides always turned it right back down to a frigid 65. (They don’t seem to understand that room climate is for the patients’ comfort, not their own.) Nobody knows the chain-of-command; if you need something special like a wheelchair restraint you get “Oh, you have to speak to so-&-so” only to find out that they’re not the one, it’s somebody else (who also turns out not to be the one...etc.) Any forms you need to fill out never can be located. The rooms have mosquitoes and gnats. The water taps in the bathrooms all drip or run continuously, splashing a fine spray of water onto the floor, leaving hazardous puddles. Toilet seats are loose and skid back and forth. The laundry is often late in sending up clean linens, forcing residents to lie exposed with no blankets for hours; one morning I found my poor husband huddled naked and shivering on his bed with no covers anywhere in sight. Pilferage is rife: all my husbands little toys disappeared the first day, his traction socks were stolen. He received little help with eating and after he was no longer able to wear his denture, I had to plead for two weeks to get him changed to a soft diet. Then, despite “soft diet” noted on the order slips, his meal trays never changed. I kept him alive on soups and baby food. The nurses kept giving him his pills with Glucerna which has soy. I explained that he was allergic to soy but even after he broke out in a red, blotchy rash they kept giving it to him. Nothing was done to alleviate his itching; I’m the one who wiped him down with Aveeno Bath. The staff seems poorly trained in matters of hygiene. Apologies if this grosses you out, but I have to mention it; I watched a nurse (yes a NURSE, not an aide) wash my husband’s anal area then, without changing her gloves, she reached up and began patting his face to wake him. So, what do you think–at $12,500/mo, were we getting our money’s worth???In all fairness, the unit supervisor did her best to help but she was up against the same problems: ignorance, apathy, incompetence, and a poor chain-of-command. Everyone was very nice; I have no complaints about attitude. But it’s a hell-hole, no question. Do NOT put anyone in here if you can possibly avoid it!!!

The location is awful staff only works when outside visitors are around. I have witnessed male staff changing female patients in the bathroom alone. Hope not to end up in a location like this when its my turn....if you have a relative at this location I advise you to drop in as many times as possible at different times to see whats really happening. Please Please Please DO NOT SEND YOUR LOVED ONE HERE! Its a dump, while the staff try really hard the conditions are horrible . I read some mixed reviews and thought this would be an ok place. Its dark and dirty, the staff ignore call bells , they fail to administer meds as scheduled , they seem to be out of supplies that should be in every facility. There were no hand sanitizing stations or bottles placed in common areas. I observed nursing staff go out to smoke and then back to patients rooms without washing hands. One evening one of the nurses could not obtain a basic supply of bed linens and wrapped a sheet around a pillow instead of a pillow case. RUN!!!

MY GRANDFATHERS ASSIST BUTTON WAS TANGLED BEHIND HIS BED SO HE COULD NOT REACH IT AND SCREAMED FOR HELP YET NO ONE CAME TO HELP HIM. WHEN I GOT THERE I HEARD HIS SCREAMS FROM DOW THE HALL. Also, can someone please tell me why I was not offered any help after my review posting. Nevermind, I know the answer. The responses this place is giving for your reviews are the same for many reviews. Not customized, which means they are not actually listening to your complaints. It has been a year since my loved one left this facility. I have reflected every night for the past year, and after a lot of time I have realized my judgement on this terrible place has not changed at all. I was in pain watching my family member being abused. I cried every time I visited, not only because I loved them, but because of the tourture they were put through. Here is my review from last year.The place has good physical and speech therapy, but besides that, nothing good comes out of this center. I had a loved one at this facility. They sucked all of the hope out of all of the elderly.The nurses yelled at my aunt and I. This was only the beginning. Rooms darkened. No light. No care. Neglected. Abused. Left alone. Silver Health care does not care about their patients. Do not send a loved one there.

I was there visiting a friend, and just being there for 1hr was enough for me to see how they treat patients. A patient was yelling for help, then a staff member just yelled back press the call button. How about you go find out if they need help. Another patients call light was on for twenty minutes before somebody came to check on them.Im a nurse, and have worked in and out of many facilities, hospitals, rehabs, nursing homes, etc. And this is unacceptable. Staff was loud and rude when speaking to patients. As a case manager, I will try my hardest not to refer anyone to this facility.
