If I could give zero stars I would. Don’t waste your time sending loved ones to this facility, you WILL transfer them elsewhere. They should be shut down!

I had a family member who scheduled an interview he works nights drove there in the morning after working all night! Was very rudely told the position was filled. Very unprofessional the interview should have happened anyway! This is why you can’t keep staff! Missed out on a great employee who is early for work happy all the time give 110% every day!!! Your loss!!! And your reviews are horrible.

Facility is not equipped to handle care for patients. Building has not had an upgrade in over 30 yrs. Very depressing and dull and dingy interior. Website does not do justice, it is actually a FAKE enticement. FOOD prep is frozen meals from refrigeration. Not clean at all. Essential medical supplies you think would be in stock in-fact they dont even know what some of the medical terms are. NON English speaking. (1) Nurse on floor, who basically is not trained other than giving out meds. I would not recommend this facility to anyone. Please Be- AWARE and do your homework first if you even plan to send anyone here. Misleading advertisements.- And AVISTA, dont answer this review with Oh we are sorry to hear that, because if you were truly sorry you would do something about your in adequate facility!!!

As of right now, I will not give a great review. I will be there in a week and will decide if I want to change my mind about this place or not.

Giving this place 1 stars is way too kind. My mother was there and got the WORST service. They forgot her medicine, didn’t help her go to the bathroom and just plain neglect. We had to call 911 to get an ambulance to take her to another facility as they didn’t answer the nurse phone. Every time we called for help they let the phone ring. Shame on this facility for still being in business. It needs to be shut down and investigated! Please don’t let loved ones near this terrible facility .

The staff is incredibly unreliable. They leave patients sitting in their feces for hours. Do not put your loved ones in this facility. One star is still too high of a rating.

This place is amazing!!! Over the last 9 months a new company took over and hired all new staff and they are the best! My mom was in a horrible place in Philadelphia and she had 14 bed sores when she arrived here and her hand was closed shut. She was here for 1 week already and 3 of her sores have healed, and they starting working on opening her hand already! She looks happier and cleaner and healthier being here. The staff are friendly and very helpful! I highly recommend this place. If u want a place where your family will be treated the way u would take care of them then this is your place!! Seriously best nursing home out there. Thank you to Jack and all of his wonderful staff for taking such good care of my mom and all the other patients.

My mother was here and it was bad. I had to get her out of there. The food was horrible, inedible often and NOT because they were following a Kosher diet. It was just bad. I checked on my mom everyday and she still had things stolen from her and was treated poorly. Some of the aids were nice but some were not. PT left a lot to be desired. Do not recommend this place at all. Send your loved ones anywhere else but here.

Please dont place your loved one in this rehab! My mom had a brain surgery and was fine after the hospital, for 3 days she spent in this facility, she didnt open her eyes, I suspect she was given meds, which was not prescribed to her and knocked her out completely, so she wouldnt bother anyone.....nobody cared when I tried to address it.

If you have a loved one that is immobile I do not recommend this place. Every morning when I check on my mom she is soaked in urine pajamas.bed sheet and even the pillows that I place under her arms. Night shift do not change people and morning shifts dont do until after breakfast. They expect patients to sit in urine while they eat breakfast. Asked to speak to a supervisor and they never showed up. Its a disgrace in the manner they treat patients.

The initial time we tried the facility was for mom staying in respite care, which was a wonderful experience the aides on the A wing were awesome and mom was very happy. Approximately 1 month later mom returned as a rehab patient and again was on the A wing, and the experience was the same. The aides were very attentive to mom’s needs and although her health had changed from the previous stay any issues or concerns we had were discussed and resolved accordingly. With mom’s health rapidly declining we unfortunately made the decision to transition her to long term care once rehab was completed. We’re currently regretting the decision- she transferred from A wing to the Saltzmam unit and it’s been nothing short of a frustrating inconsistent nightmare with both the 2nd & 3rd shifts. They’re constantly short staffed leaving many residents not attended to in a timely manner, we always have articles of clothing missing, there’s lots of fraternizing at the nurses station while room lights are ringing, majority of the staff has a poor attitude and all they do is complain & barely take accountability for anything just finger pointing & shifting blame. The majority are rude and demonstrate passive aggressive behavior. It’s sickening to hear and see and the visits to mom have become more of damage control then actually enjoying her in the moment. We’ve reached out to administrators, supervisors & management on several occasions and while some issues have been addressed things seems to be getting worse instead of better. Where’s the professionalism? Respect? Dignity? For all of the residents. We’re just sick and tired already but we will continue to be mom’s voice no matter what. We shall see what’s to come in the days ahead..

This place was terrible. I would not send anyone there. I my nana suffered being there. They treated her like trash. I have officially opened a case with the state about this place. Food was terrible. The director of the kitchen was very rude and condescending when asked about the always available menu. They never had no fresh fruit unless from a can.

JUST RUNNNN!!!!Tbh they are nice people. They patients are not taken care of as they should be my grandmother got left on her bed pan for a long time. She ended up in the hospital twice the first time was because they didnt know how to get her out bed the right way with the Hoyer or whatever its called. The second time she was throwing up for days before they sent her to the hospital again!!! Also she had a case of water that was only missing a few bottles we went to pick her stuff up THEY WERE GONE....If you have a choice go to Barclay rehab or search for one better then this!!!

This place is unbelievable. They gave my family the run around regarding documents they needed for my mother to be accepted as she was not able to go home and needed to be transferred from another facility. We are in the process of getting her on Medicaid and were told that as long as we had an appt with Medicaid, they could take her. We got the appt and then we were told they needed all of her financial records for the past 5 years before they accepted her. We provided the records and then they tell us the day before she needs to leave the other facility that they need copies of a police report regarding a theft she was the victim of. The day we were expecting the transfer(today)- we are told they didnt accept her. We were told WEEKS ago that they DID accept her and all we needed was an appt. We provided every bit of information they requested and every few days they wanted more and more only to deny her bc she was a victim of theft (who also has Alzheimers)Im beyond disgusted by this. Avista should be ashamed of themselves.

My father in law was neglected, and fell ( after repeated requests to have his alarms on, the aids there just said he aint goin nowhere. He now has a refracture of his hip, a new fracture of his spine, multiple bruises, sepsis, and all they can say is, I dont know anything about it. Since they cover up what happens there, do not use this facility. Dads roommate said he was ill and dying and they did not care, and he died last week. If they dont like you, they treat you like dirt.

My grandmother came into this facility because she fell at home. We took her to the hospital first and they found two fractured hips. They then moved her to this place. Five months after being there she magically broke the hardest bone in the body, her femur. This was not on the initial x-rays. They are aware of the femur now and instead of helping her, they keep feeding her heavy pain medication so she cant complain. Do not allow anyone you love to be placed here. They do not truly care and they cover up serious issues.

Awful place. My friend has been at this place for 3 weeks and has yet to be given a shower or bath. He has been in the same clothes for the last 3 weeks. The nurses are never around, the aids do not follow up and give the impression that they just do not care. Like the other reviewer stated, this place is horribly neglectful.
