My mom stayed at this facilities for physical therapy and rehab for a period of about two weeks. We were pleased with the cleanliness, security, COVID-19 protocols and staff. They treated my mother well. Her recover was steady and positive. Some of the furniture is a little dated, but its in good shape and well maintained. The staff truly makes this facilities a good place to be. I recommend it to anyone who is looking.

My loved one was placed at Windsor Gardens and at first I was hesitant in having my grandmother stay for rehab. I was pleasantly surprised. The communication I received from staff was great. Any questions I had were answered. The social worker Annabelle was very helpful in communicating with us and setting up discharge for return home. Tomiko the business office manager made the process as easy as possible. Staff went above and beyond to ensure my grandmother was treated properly and with dignity. I would highly recommend this facility.

This was a very nice facility when l began working here back in the year of 2018 June 25th. It was rated as a 5 star facility then. At the time Administration was Lana Nixon. She is the reason the building went from a 5 star to a 1 star. If you have family there PLEASE take them out. At least until there is new administration. She is the Devil s child.

Windsor Gardens is a place that is filled with love and care for those that are entrusted in their care. Is Windsor Gardens perfect? No its not but what facility is. I grow up around long term care and now that I have the great pleasure of working in the field of taking care of those that are in need I can appreciate what Windsor Gardens represent, they represent commitment to those they serve. The Windsor Warriors is the team you want to be on when the battle begins, they in body strength, courage and love. At Windsor Gardens your not among co-workers your among family.

The staff at Windsor Gardens did an awesome job with my loved one while receiving rehab care. They went above and beyond with his care. The facility looks and smells great! The administrator was very helpful and pleasant. She gave my her number to call if I had questions. I would definitely recommend Windsor Gardens!!

Windsor Gardens is not a good place to put your loved one. It is understaffed and doesn’t do a good job of communicating with family. We placed my mother here after a recent hospital stay and thought she might be here until we decided upon Hospice. We were informed that because she would be placed on the side of the facility which houses patients who have to go in and out for dialysis, blood transfusions, etc., and due to COVID, we would have to adhere to very strict visitation rules. We could only have 2 essential family members on the visitation list for 2 hours visits and only if scheduled with 24-hour advance notice. We weren’t able to interchange the 2 people on the list and so any other family and friends would only be able to see her through a bedroom window or through an Exit Door down the end of the hallway.I understand COVID restrictions and limiting visitation, but we had to SHARE the visitation slots with ALL of the other families on her hallway. So, for example, if I wanted to visit from 9:00am-11:00am, I had to hope that no other family on the floor had already reserved that spot. Consequently, there would be days where I couldn’t get in to see my mother because all the visitation slots were filled. However, the day before we took my mother out of the facility, another woman who clearly knew the gentleman behind the desk personally, requested the same slot I already had. He gladly wrote her name down on the slot she requested even though I already had it.As if that wasn’t enough, Windsor Gardens was so short staffed that it was nearly impossible to get anyone to respond to a call for assistance. My mother, who is in a wheelchair, would often ring the button to ask for bathroom help, and she would have to wait anywhere from 30 minutes to 2 hours before anyone would come. One of the staff complained the patient to staff ratio was 16:1. Because it took so long, it was routine for my mom to start our visits by asking what time our visit was ending so I could be sure to take her to the bathroom before I left!!Our breaking point came from my mother never being sent out for her life-dependent blood transfusions which needed to occur every few days due to her medical condition. Windsor Gardens claimed they were never informed that she needed regular transfusions. Hard to believe that since she was specifically placed on the side of the facility where people routinely leave for medical procedures, visits, etc. Why else would she have been on that side? Windsor Gardens never followed up, or determined how this mix up could have occurred or how it could be resolved. At the end of 15 days, with no transfusions, we decided that it wasn’t safe for her to remain at that facility.Even getting her released was an ordeal. We asked that my mother be ready to leave the next morning when we arrived. When we got there, the nurses were unaware she was leaving and the Social Worker Annabelle, told us she was not “ready” to be discharged despite her telling us the day before she would be ready. We took her out right then and there and it was written up that we took her “Against Medical Advice!”The only positive thing I can say about Windsor Gardens is that the facility itself was always very clean and there was never a bad smell. But the attention to the patients, which is paramount for any healthcare facility, is sorely lacking. We repeatedly told social workers, nurses, aides, or anyone else who would listen what our concerns were, but to no avail.I know this was a very lengthy review, but I want everyone to understand how bad the experience was for our family. I saw all of the positive recent reviews and now believe those are fake. I learned a long time ago, that anytime you see a bunch of reviews, all around the same time period, all with glowing comments and from contributors with only 1 or 2 other reviews, they are probably fake.Let me assure you, that I am a real person, with a real mother that needed care, and we definitely couldn’t get that at Windsor Gardens!

I had a loved one there on long term care the staff was so helpful and nice not being able to see my aunt during this Covid quarantine. Everytime I called all my questions where answered. Sentura the nurse very professional the administrator Lana Nixon very helpful and nice I recommend this facility.

Its never easy when you have to leave your loved one and during a pandemic is even harder. The staff at Windsor Gardens, especially a nurse named Qwaye made for a smooth transition. She answered all the questions we had about any COVID related restrictions and provided the next steps in my Aunts new journey. Thanks Qwaye and Windsor Gardens for exceeding my expectations and most of all reducing my anxiety!!!

Unfortunately, Annabel this facility’s social worker is not professional. I can also say that she does not care about the residents. She does not have the proper knowledge or resources to properly care for the residents. She does not even try to gather information to make sure each patient has everything that is needed upon discharge. She will not answer a call and when she does she would rather place you on hold and let someone else take the call rather than deal a patient or their family members.My mother’s personal medicine that she had with her when she was admitted to Windsor Gardens, was used while she was there and was not given any prescriptions upon discharge to make sure she was taken care of until her next doctors visit. They also sent her home without home health care. My mother was told by Annabel and Tim that she had to go home because she couldn’t stand. However, they would not assist me with getting my mom in the long term side of Windsor Garden. I am so disappointed in the work ethics of Annabel and the other directors who saw fit to send my mother, who can not stand nor walk, home without proper care.

My aunt is a current patient there and for the past 3 weeks my family has been denied visits to check on her. Her health is questionable. Since she has been there she has gotten worse. We try to check on her but they hang up the phones. Or they leave us on hold for 30 min or more. We were told by an employee that she isnt doing well., but the nurse says she is up and happy but we know that not to be true. They are failing to take care of her. She can not speak up for herself because she is deaf.. They have given us the run around. They stopped communicating with us when we said we were going to move her closer to home. The management and staff are horrible and lie. They do not care about the patients. WE NEED HELP!!! I FEAR FOR HER LIFE!!

Mother was here for about 3 weeks for rehab . This place is not a good place to put someone who is not able to get around on their own. When my mom was discharge half of her clothes and personal items were missing ,she ended up having to be rushed to hospital for severe dehydration Doctor said she seemed to have been dehydrated for about 7 to 10 days. With the strict rules that they had on visitation your not able to stay in touch with you love one and yku are just left to trust that they are caring for them correctly. I sent a charger up there with my mom for her cell phone I had to call almost everyday to get someone to plug it in I went almost a whole week without talking to my Mom because your only allowed to visit 1 day every week. One good note the rehab did loosen her up but I almost lost her last night due to dehydration thats not exceptable.

My mom went here after having surgery on her arm. They only changed her dressing twice and she was there at least 2 weeks. They didnt send her home with any instructions or pain meds and one of the nurses, Camilla, told me that her bandages being changed wasnt her problem. Um super unprofessional and rude...she was her weekend nurse and should have made it her problem. Never again sending any of my loved ones here. I reported it to the social worker but have no idea if anything was done.

Recently, a friend of mine was admitted for rehab. First of all the facility is decorated beautifully. I have visited another facility owned by Cantex and the Lancaster facility is awesome in comparison. My friend was there for a week. I visited each day at various times and each time I visited the facility was very clean and well cared for. The staff was always very friendly and accommodating to my friends needs. The room had all the conveniences: individual bathroom, microwave, sink, cabinets, and a refrigerator/freezer. If I ever need rehab or long term care, Windsor Gardens will definitely be my choice.

Unprofessional staff, nurses , and CNA . Unorganized facility and irresponsible wouldn’t recommend this place at all . Director and coordinator do not return calls back until 4 days later. You can never get in contact with any nurses to get information on your loved one. This facility needs attention or get reported to state no use of calling someone to report all this if they don’t answer your calls .

I live out of town but am legally responsible for my aunt. Our family was suddenly caught off guard when her caregiver became ill. I had to make a choice quickly and chose Windsor Gardens. I have to admit that I was nervous at first. But the staff was so compassionate and sensitive to my situation. After speaking with Tomeiko Seigle who assured me that my aunt was in good hands, I made my decision. The transition was flawless and my family is happy. We frequently visit and are very pleased. I was not looking for perfection, only a well balanced staff who knows their job. I got that and so much more.

Theyre unprofessional, it does have a strong smell of urine, they dont bathe the patient on a regular, hardly answer call button, i thought it was a great place. But clearly its not. Recently visited this place yesterday and it smelt like straight piss in there I was talking to my mother n law she went from talking normal to saying numbers and wondering off into space which I didn’t agree with after she was given some “pain pills” she didn’t need I asked the nurse what kinda pain pills was she giving her and why was she giving her pain meds if she said she wasn’t in pain those pills make the patients act unusual and I’m definitely gonna get to the bottom of it because no one deserves that even her roommate that she has in the room with her was complaining about her bottom hurting and no one came to shift her or sit her up and me and my girlfriend was there with her mom for about 2 hours which is ridiculous

They started out okay, but them are some lazy workers, they dont feed the people right. When you know people have high blood pressure and diabetes you feed them right. Dont give them a lot of bread, food full of salt, etc., Its look nice on the outside, but Lord, they need some professional help and workers who care about the people and not just getting a pay check. Those workers be hiding out and also talking rude to the patients. Clean up your act. If you dont like working with the elderly then dont abuse them go somewhere else and work and get out of the healthough care business.

Dont expect your family to get daily vital signs or get cleaned up. This place reeks of urine. Night nurses are asleep at the nurses station orwatching movies on the tablet with the earphones on. One of the night nurses actually accused us of stealing a wheelchair that I brought for my mother. The aides are asleep in the break room and in the residents room. Social Worker has never visited me once. Wheelchairs smell like urine, therefore making residents smell of urine. Air mattresses are not cleaned or disinfected. Soiled wipes were left on top of a bureau in my mothers room and in open trash cans. My mother was served burnt food.The cafeteria personal are rude, and the meals are always late and cold. Unit Manager makes up rules for her own convenience. Managerial staff are disrespectful. Staff retaliates when complaints are made against them. Executive Director is RUDE RUDE RUDE. Corporate makes excuses for their employees, they are always on the side of the employee. My mother fell five times when she was there. She also went to ER twice. Residents that are not able to press the call button are ignored. Some of the residents are left in wheelchairs all day. Residents are taken to the bathroom. The nurses at the station are loud, and shows a lack of decorum. There is one nurse in particular T.T., is extremely confrontational and tried to force feed my mother when she was in danger of aspirating 2 days before she died. She also almost gave her medication to the resident in the next bed. If you are considering placing your loved one here, Please make sure to be there every single day. Its pretty on the outside, but dont let the looks fool.

The facility is nice and their introduction is awesome. No one is on the same accord. Some nurses do not take vitals and medication not always given at a timely manner. PT/OT are very nice and take care of business.

This place is horrible and if I could give it a negative stars, I would. The staff are unprofessional and if you complain they retaliate. Not sure what school the nurses graduated from, but they need a refresher course. Nurses dont take vitals on patients that are on hospice, I was told they dont do that. A nurse continued to force food down my grandmothers throat even though she lost the ability to swallow in her late stages. Was told my the hospice nurse not to give her food because it was a choking hazard, I guess no one gave this nurse the message that she was transitioning to die and didnt need food. Another nurse stopped my sister in the hallway and told her that she was bothering my grandmother on hospice ( I keep repeating this so that you can see the level of insensitivity) because she was holding her hand. My grandmother fell 5 times in this facility after having hip surgery and was told my the night time staff that they couldnt watch her all the time. There were no bet alarms used and her room did not have fall risk on her door like most other facilities use. For the couple of weeks, they were giving her the incorrect dosage of pain medicine and then moved her out of rehab early even though she still had 10 days of rehab left on Medicare. Complaints go unheard, the Social Worker and the Executive Director response was if you dont like the treatment you can leave instead of asking, how can we make it better. Not realizing that it doesnt make sense to move someone on hospice, this was the best response they could come up with. I question all credentials of the people they hire. The carpets are filthy and it smells like urine. If you are looking to send a love one here, ask them how many residents die every week and see what type of response you get.

If I could give this place less stars I would!!! 12/31/2018 this place ALMOST KILLED MY MOTHER! She was recently discharged from Baylor Hospital on 12/24/2018, to this HELL HOLE OF AN ESTABLISHMENT! For starters the place smells like death and dust, they lack staff availability and the requests patients make go unanswered for HOURS AND DAYS! It seems as if the employees there are so clueless and are just there to collect a check, no one cares about your love ones here. My mother complained on the following days: 12/29, 12/30 and 12/31 complications of feeling dizzy, nauseated and fatigue. Her charge nurse Samuel... Did nothing!!! From day 1 my mother told me that he made her feel uncomfortable and he worked the later shift. I seen him for the first time 12/29/2018 when she first complained of being dizzy, he did nothing! I tried asking other staff members for help and they just starred at me like a deer in headlights! I figured my mother could just sleep it off and be better the next day so I watched after her until she fell asleep then I left her at 1:30 am. I spoke with her on the phone the next day and it she still felt the same. Her therapy nurse came in 12/30/2018 and my mom hung up the phone with me to tell the representative what was going on. I didnt hear from my mother again until 12/31/2018 and I could hear in her voice she was sounding BAD! I advised her that I would come to her as soon as possible.SHE TOLD ME THEY MADE HER SIGN A CONSENT FORM TO RESUSCITATE!!! LIKE THEY KNEW SHE WAS DYING! I got there and as bad as my mother looked it terrified me! Her pupils were huge, She was so weak she couldnt sit up at all! My baby sister kept trying to keep her head from hitting the bed railing and my 10 year old daughter felt HEAT COMING OFF HER BACK! I hit the call button and two ladies came to the door, but one just peaked in I guess trying to get a glimpse of the scene for GOSSIP! I explained to the representative who came in that my mom said she needed to use the restroom, but due to the tight fitted room the wheel chair could not fit on the side of the bed my mother was slumped over in. Out of nowhere came the charge nurse Samuel, who has a very strong accent that NO ONE UNDERSTANDS CLEARLY asks my mom who was VERY LETHARGIC at this point whats wrong with her. I explained to him what was happening and he tells me You cannot be Dizzy and Nauseated at the same time. I told him Im calling 911 because you dont know what you are talking about! The ambulance responded promptly took her to Baylor where she had a FEVER OF 101!!! AS OF TODAY 01/02/2019 SHES BEEN GIVEN 5 BLOOD TRANSFUSIONS AND SHE DIDNT KNOW MY NAME, MY DAUGHTERS NAME, HER NAME, OR WHAT YEAR IT WAS! She was very pale and talking to herself. FROM THE EMERGENCY ROOM THEY ADMITTED M MOTHER IMMEDIATELY! If it wasnt for my quick thinking she wouldnt have made it another day AT WINDSOR GARDENS! ( Shes still in the hospital by the way) and has had 2 procedures and counting! DO NOT TAKE YOUR LOVE ONES HERE!!!!

O star rating. This place is disgusting very unprofessional no structure..charge nurse wearing flip flops. All the tech sitting at the desk laughing,playing and talking on their call phone. Dont care about the patients.this place needs to be investigated ASAP!!! Check on your love ones at all times please!!!!

Not a caring place!! Constantly giving false, inaccurate reports, cover-ups regarding incidents. Director is rude to all even her workers. Find out what the ratio to resident/patient is.... Constant turnover!!
