Very professional yet welcoming. Covid-19 protocols in place, but even with masks, the eyes were smiling.I felt like a person that they were interested in and not a number.

Love Dr Broughton shes the best! Very sweet and takes her time and is very fast to send any referrals or take care of anything you need. Some other offices take forever but her and her staff are on top of it. I miss my Doc that retired Dr Underwood but I absolutely love my new doc, plus she will handle all women exams which I didnt do prior so this is an added bonus.

Dr. Okie is a really good doctor. He likes to make sure you are completely understanding why he says something should work for you. He takes the time to explain everything in terms you can understand as well.

I was greeted by a very friendly staff then was called back to an exam room within 10 minutes of my appointment time. I met the for the first time the most friendly physician, Sanad Alshareef. Since I was looking for a new doctor this person just fit what I was looking for. I will definitely be coming back to her.

Fantastic experience as a new patient! Facilities are beautiful, very clean, very mindful of distancing. Staff could not have been any more friendly or helpful from the front door to the lab! Dr had a whole plan for wellness and was very thorough. Highly recommend this location. So glad I switched to them!

I think the staff are great and treat patients with concern and respect. The facility was clean, in a location that was easy to find, and had plenty of space to distance from other patients. The PA I saw along with the nurse were very friendly and I did not feel rushed through the appointment. I was a little unimpressed with the initial visit care, though.I went in with a couple very specific things I wanted to address, but instead of having them addressed they were just entered in my chart and I was told to come back in 1-3 months to discuss the issues I brought up with the doctor. One of the topics I wanted to discuss could have easily been initiated at that first visit, so I didnt understand why there was literally no effort at all made to initiate some kind of care plan for it. Would have loved to walk out of there at least with some ideas of a plan forward for the items I brought up instead of just being told to come back again.Other than that, overall pleasant experience. I would still recommend them for care.

This was my second visit and it was excellent! The staff is really on the ball and Has a wonderful system in place for checking in and out and for referrals and medication. I met with Jared Anderson who was wonderful. He was intelligent, compassionate and so helpful.The office itself was pristine and comfortable. All Covid requirements were in place perfectly.

KARA TRZASKO, PA-C, is my “DOC!”She is one of the most, if not the most caring and professional medical person I’ve ever known in my life. She actually “listens” and CARES about her folks she sees. All the Ladies at the front desk where nice and friendly, the Dr I saw however not so much. Im not anti vax but he told me to go get the vax without any reason too. Especially since apparently natural immunity is better than the vax. I may seek a 2nd opinion....He pretty much spent 5 mins talking to me then booted me out.

Dr. Broughton is a very sweet, kind and truly cares for her patients. I am glad that I found her to be my doctor. The MA was wonderful as well.

I have been going to Family Care Partners since my daughter was born 11 years ago and have always appreciated their time and care. Now as a patient myself I am happy that I am getting the same excellent treatment. Everyone at the front desk and checkout are always very nice and patient. Thank you to my provider that is very sweet Mrs. Jackson whom also sees my daughter and listens to all of my concerns. I would recommended this practice/ office to anyone.

I am so pleased with my new Dr. She is so understanding & helpful. She has a wonderful bedside manor & Natasha is amazing. All her stall are very kind & helpful.

I felt like it was professional and well cleaned and I love that it was attached to a lab for testing. My appointment was in the afternoon on a Friday and that may have been part of why I felt rushed. But it felt to me like everybody just wanted to go home.(Not that I’d blame them.) I will go back but the next time I will make it mid week. There were several concerns That we didn’t get to discuss. They have a choice I would suggest making an appointment at the first of the middle of the week and hopefully that will take care of the problem.

Dr Broughton is caring and efficient, asks amplifying questions and gets to the heart of the matter. Both times I have seen her, as well as the additional two labs that were taken prior to the office visits, the practice is efficient and well run. Very good experience all around.

So thankful that my Care Provider Erica Shanks empathizes and listens to her patients. Felt like I was the only one there and even the lab technician was friendly and professional.

Just a couple of “little things” to bring up after my recent visit. First, the ladies at the front desk I would simply give this little piece of advice: when dealing with a customer in front of you don’t stop mid-sentence and tell me to hold on in order to have a long conversation with somebody else walking by about a cell phone. Then, after the side conversation is done, come back to me and say oh well you didn’t have to wait anyway. Second, please check the discharge papers for patients before you hand them to them...keep them up to date.

My lab work has never been so painless and quick.

Ive had the past 2-3 appointments in a row get lost and cannot get a refill for my meds. dont come here

Dr Broughton is an excellent doctor. She listens to my concerns, even when I have more than one problem to discuss. She orders whatever tests are needed to get to the bottom of things. As someone with multiple health issues I need a doctor who goes above and beyond and Dr Broughton is that kind of doctor.

I have been a patient with FCP for over a decade. I have always felt that I am heard by the healthcare professionals and the support staff. Healthcare and health planning/management is a team effort and this is the best team I have worked with. It is also a huge benefit having the lab and critical care onsite at the Intracoastal location. The facility is very nice and large enough for safe distancing in the waiting room. The staff is pleasant and professional and I highly recommend them to my friends and family.

I am one that doesn’t trust many people nor doctors, but here at Family Care Partners my family and I only see one person Dr. Kara Trzasko. She’s our Dr and she’s best Doctor in Jacksonville, Fl. She make sure your heard and well taken care of wouldn’t trust another with my health and care.

Dr. Underwood is excellent. The billing department is awful. I have trying to access the payment portal and get denied, and when I call the billing department, I either get a voice mailbox or get told the mailbox is full. I got a final bill and I still cant access their portal or reach the billing department to pay my bill. I have paid many medical bills and NEVER had issues like this.

I really want to make this Doctors office work but there is a constant communication problem. The last telehealth appt for a family member they called my phone # even though when I made the appointment I confirmed the correct #. After they called me I once again gave them the correct # to call..and they never did. When I called to find out why they said they did call and no one answered NOT TRUE.We waited a week for this appointment and now have to try to get another appointment.

My appointments are timely- no sitting in the waiting room for long periods of time. Front desk associates are friendly and knowledgeable. Samia Jackson has a lovely disposition and really knows her stuff. She is very easy to talk to.

I’ve been going back and forth with this office about some prescription I was supposed to have BEEN got since beginning January 2021 and it’s now February. I have been the one calling my insurance and my insurance has been EXTREMELY HELPFUL. They have clearly told the doctor what prescription my insurance covers and this office still keeps sending the wrong ones and on top of that they wait a whole week to send my prescription to my local Walgreens. When I call I’m told “mam we take more patients” but there should be no reason why I have to wait over a month for a prescription especially when my OWN insurance has called them DIRECTLY. I understand that there’s more patients they need to attend to but if they got to many then just say that 🤷🏽‍♀️ I need to find one that can fufill my needs. I’ve been in pain this WHOLE TIME too. A whole month of waiting and some more in PAIN. P-A-I-N.

As always, my experience is most gratifying....All staff members are so cordial and strive for excellence in all aspects of patient care.I especially praise Ms Jackson for her most knowledgeable, experienced, dedicated and most caring treatment of all my medical needs.

I was in and out with a referral, prescription and Flu shot in 30 minutes. Less than 5 minutes in the waiting area, 5-6 minutes with nurse answering questions and taking vitals, doctor came in like 2 minutes after nurse left if even that long, looked over everything asked a few more questions, checked a few things out, stated his recommendation, told me he would be writing a prescription and referring me to the chiropractor, and then recommended a flu shot (less than 10 minutes total). He was very clear and to the point. A few minutes after he left the nurse came back and quickly gave me a flu shot, then the doctor came back with my paperwork and prescription. It might not have even been the whole 30 minutes with all issues addressed. I wish all doctor appointments were this quick and to the point.

Doctor Sams an amazing physician. Always professional and knowledgeable. And still there for his patients in this time of uncertainty.The Family Care Partners staff is also thorough. I was screened upon entering the facility and the entire time I was there I viewed staff cleaning and sanitizing every area of the office. EVERYONE was wearing masks. GREAT JOB.

As always my experience with FCP was on par. My service is always prompt and professional.

This should be a zero. After having blood in my urine I had to wait 24 hours to be seen. Decided as it was getting worse to go to Ascension St. Vincents Urgent Care - Town Center, was seen right away. Was seen the next day by Dr. Sams, came up with a course of action and follow up. That’s where it all stopped. They never got the test results from the St Vincent’s, faxed authorization for the CT scan, sent authorization to Urologist. I have had 2 follow up calls from Urgent Care Doctor but my own Doctor has not called me in 48 hours from a requested call. If you expect a Doctor to care, go elsewhere!

What ever happened to the days when a doctor actually took the time to listen to their patients?I was just seen by Dr. Green and he spent more time talking over me than he did listening. I asked for a natural approach versus taking meds and he just replied, I dont know any.This is 2020, as a doctor you should want to provide your patients with multiple options and not just put them on pills. Your patients are putting their lives in your hands. You should treat that privilege with more respect!

Dr. Underwood is THE MOST thorough doctor I have ever had the privilege to call my doctor! I’m NOT big on any doctors and I was extremely lucky when I was able to get an appointment with Dr. Underwood! He spent more time with me than any other doctor I have EVER been to! He actually listens to the patient and what the patient has to say about their own bodies! I can’t say enough great things about Dr. Anthony Underwood!

I like going to Family Care Partners, they are always willing and able to work with my schedule, they have switched my schedule for me at last minute from Office visit to a teleprompt visit with no questions asked, the office is always clean and they are always on time. I see doctor Sams who is military and what you see is what you get. He is a good doctor Who says it how it is, you can always count on Doctor Sams to give you the truth, the whole truth with no sugar coating. Some might say bad bedside manner oh, I say no guessing games and in the medical field that can actually be a blessing. All in all I have to give them 5 stars and even with Doctor Sams Ill give him 5 Stars as well, Im sure hes not that bad I just dont know him that well...

Best healthcare staff and services.

Dr. Sams is by far the BEST Doctor I’ve ever had! He saved my life. The 1st time I came to see him I was a mess, psych wise. He was able to diagnose me immediately, an even prescribed me the RIGHT meds without the whole trial an error that most Drs do before finding the right meds. Nope! Dr.Sams was on point! I have never felt better and I thank Dr. Sams for saving my life that day almost 2yrs ago. I won’t forget about the staff either, all of the staff I have encountered each time I’m there is absolutely wonderful, great customer service, an they truly care. I am so very glad I chose Family Care Partners!!

I had a great experience here UNTIL I saw the actual doctor, DR.GREEN. I would 100% NOT RECOMMEND anyone to see him and believe he actually is NOT suite for for the job. He is extremely rude and quite obviously has no respect for his patients. I’m sure if you’re reading this you’d love for your doctor to be kind and so concerned for your health and happiness, so it is definitely in your best interest to request anyone EXCEPT for DR.GREEN and to spread the word, because no one should pay to see such a terrible doctor.

I have been very sick for months and months. I was discarded by one dr after a year and how I was notified was by his nurse calling me and she told me he didn’t wish to see me. Very unprofessional. Then the second dr did the exact same thing. I have been basically bed ridden and now abandoned by them. It’s terrible.

After being a FCP patient for many years at another location I had my first appointment with Dr Green at the FCP Intracoastal office today. From reception to checkout it was a very good experience. Dr Green listened to me and my concerns. I couldn’t have asked for a better experience.

A very efficient office. Did not wait at all. Hagar (MA) and Dr. H. Green were both kind and friendly. Explained process for a new patient and talked with me about specific concerns. Ordered bloodwork was done right on site, very convenient. Although the office was a ride from my home, it was worthwhile.

Im very allergic to dog dander and cigarette smoke.I was exposed to both in a very short length of time.When that happens breathing becomes challenging.I called, they got me right in and gave me the treatment I needed.The ladies at the reception desk are always kind and welcoming; always respectful and patient when asking for additional information.The doctors and staff are professional, yet caring as they follow through with the best event-specific care for each visit. I always leave feeling like my symptoms and needs heve been met with the best care possible.If further attention is needed Ive always been referred totop-notch care providers, able to address my specific needs.Im really thankful to have such a dedicated group to assist me with my goal of being the healthiest I can be.

This place is awful when trying to get a refill or anything faxed. I amost didnt get my scheduled Surgery because my physical and EKG were not faxed to my surgeon even though I had it done a week before surgery. I had to call FCP several times, fax, and went by the office the morning before surgery to get results sent in time I love Dr. Underwood and that why I put up with this facility’s Incompetence.

Dr Underwood and his staff are amazing. Everyone was polite and helpful. Dr Underwood is always patient understanding and helpful. He is always attentive and efficient. My whole family loves him.

This intracoastal location was AMAZING! From the receptionist, to my new doctor, to the check out ladies. I had a sick visit today and was unable to see my regular Doctor because she was booked. So the staff scheduled me with Dr. Green. Dr. Green is the worst Doctor I have ever been to. He was extremely rude. He did not care to hear about my history and what I had to say. He talked down to me the whole time. This place has some great doctors, but he is NOT one of them... The way he talked to me was un provoked and I don’t know if he was having a bad day but it was VERY unprofessional!!!!

My husband and I have been patients of Kara Trzasko for a few years now. Before, we both went through a multitude of doctors—both trying to find one that we “clicked” with. Kara is that person we click with. She is so knowledgeable, informative, and kind. Neither of us ever feel rushed during our appointments—she always makes you feel like you’re her “only patient in the world.” 🤣 We love her, and recommend her to all of our family and friends. Since Kara is in such high demand, sometimes getting an appointment with her is difficult if you don’t book it in advance. With that being said, we both have had to see Dr. Sams a couple of times and he’s been great to us, too. He knows his stuff and is very informative. The check in ladies, the check out ladies, LaTara (the medical assistant), and even the ladies in the blood lab are always SO sweet and helpful. We love the Intracoastal location of Family Care Partners, and will continue to be patients for as long as we are able to!!!! Thanks for an always pleasant experience!

Very nice waiting room fast service friendly staff.6/3/20This trip was great, but last week I saw Dr Green he was rude unfriendly and did even talk about my main reason for being there I ask him to sign my disabled parking form which was for a year he mad it for 6 mo when I ask why he said to com back in 6mo and I said I don’t think my heart will be better in 6mos and he just got up and walked out without another word. This is the third time I’ve seen him and it will be the last I changed Drs

The Pediatrician is wonderful, very attentive and shows care and concern. Allows child and parent to talk and offers suggestions. Wonderful staff from first time entering the building until checkout.

I have been a patient of Family Care Partners for over 20 years and, after my doctor retired, I stayed on as a patient and am so glad I did because they continue to provide me with the best care! I also like the ability to be seen by a doctor after hours and on weekends through their ERGent Care office on Dunn Avenue. I have never been disappointed with their service and highly recommend their practice.

Saw Dr Sam and loved him! In 10 minutes he noticed and addressed issues i deal with and had more conversation than my previous Dr had in 6 years. The entire staff is not only friendly but very kind. Highly recommended!!!
