I was in this facility for post acute care from COVID-19. The day I arrived I was treated to cold cuts and potato chips. Food tasted really good. Rooms are fresh and new. Lily the nursing director is compassionate and alert. It took the nursing staff 6 days to get my RX doses correct though.

My mom was there in 2018 and 2019 on 3 separate occasions. The first admission was 4 days in length following surgery to repair a broken vertebrae. She ended up back in the hospital from an urinary tract infection caused by infrequent diaper changes. The second admission was about three and a half months later following an extended stay in a long term care hospital. They used a lift to get her in and out of bed and another machine to get her to the toilet. The third day of this admission the CNA failed to secure my mother on the machine, she slipped out and fell on the bathroom floor while the CNA played around with the commode.The VERY next day they tried to discharge her! The nursing home doctor assigned to my moms case was useless refusing to assist with an appeal to Medicare nor would he return my phone calls. I found out about The Jimmo Settlement - a lawsuit was brought against the Dept of Health and Human Services in 2013 and was settled in 2015. The settlement requires skilled nursing facilities to continue providing therapy in an InPatient setting BEYOND the 100 day Medicare limit when it is unsafe for the patient to return home or if it would be a significant physical hardship for the patient to received therapy on an outpatient basis. Please go to CMS.GOV website to view this for yourself. This settlement actually changed Title XVIII of the Social Security Act as well as Chapter 8 of the Medicare Benefit Policy Manual,. Please look up Subsection 30.7 titled SERVICES PROVIDED ON AN INPATIENT BASIS FOR PRACTICAL MATTERS: The new subsection also prevents the facilities from claiming Medicare will discontinue therapies because the patient stopped improving. They are now REQUIRED to provide therapies so that the patient maintains current function as well as to slow or prevent further decline. When I brought this new legislation to the attention of their Director of Medicare and Managed Care Services she didnt BELIEVE me!!! She had a representative hand deliver a letter to my moms room threatening to have her removed from the facility if she stayed beyond a specific date. After receiving this letter I reached out the their Chief Operating Officer to request they have their legal team look into the terms of the settlement. He claimed they did although the facility stood by their decision to have my mother removed after the specified date. His communications with me were quite insulting and denigrating throughout the series of exchanged emails. Finally after many, many emails, he realized the identified changes to the statute were valid. He never apologized, I never heard from him again. They failed to continue to provide therapies to my mom. My parents were forced to pay out of pocket 375 a day! I see with the name change theyre claiming a change in ownership although the person claiming new ownership, Sherri Medina was listed as an owner of Transitional Care of Arlington Heights. Be diligent when dealing with these facilities on behalf of your loved ones. I wish I could say theyre in business for the right reasons. Unfortunately I have the COOs written comment that hes been in the Medicare business for over 30 years. Gee, and I thought they were in healthcare!

This place is amazing! The staff is fantastic. The rooms are beautiful. Even the food is excellent. My sister-in-law received the best care and is now home after a major back surgery and successful rehab. My family is grateful for the wonderful care she received. We will recommend this facility to everyone.

First of all, the medical care is 5 stars. Everyone is caring, compassionate, and very welcoming. Staff makes sure I know when my medical appointments are, the nurses are always on top of things giving me my medications at the time needed. Therapy department is top notch; they are very skilled. Everyone here remembers your name. Management have payed attention to every small detail: providing for comfort, quality, top quality food. Management here is like no other place. I love the attention to minor detail in this building. The snacks here are great. The bed is very comfortable. Having my own shower and toilet in my room has made the greatest impact in my care here. Staff respects privacy. Ive been in other facilities that are horrendous. I highly recommend this facility.

I highly recommend this Rehab center! It’s clean, and pleasant! Everyone is so wonderful!It’s by far not a nursing home. Not long term, so everyone is welcome to do a quick transition back to life again. 😍😍Love, Love, this place!!!!

I Spent over a month at Transitional Care with my brother needing very specific care and quality control. The facility, nurses, and especially the management were simply the best I have ever experienced. My brother came into the facility a broken man and left with great hope for the future and his health was dramatically improved. I would give Transitional Care 10 stars if possible. My sincere thanks!

I had back surgery and my two weeks at Transitional Care contributed greatly to my recovery. The most positive experience was the physical and occupational therapy. Except for their days off, the same two young ladies worked with me for my entire stay. They were most thoughtful and professional. They knew just what exercises I needed with the result being to prepare me for independence at home. I could not have done as well with in-home or out-patient therapy.The entire staff, especially the nursing, was very professional, friendly, concerned, responsive and compassionate. This too contributed significantly to my recovery. The level of staffing was more than adequate. Another important aspect was the food. It was fresh, healthy and in appropriate portions.The facility was kept spotless and cheery, as you expect of a new building. Lighting and corridors were generous allowing for easy walking. The overall environment was very pleasant. Many of the patients were very welcoming to newcomers and friendly throughout my stay.The bottom line is that I recommend Transitional Care of Arlington Heights most highly.

I stayed at Transitional Care after leaving the hospital to recuperate from an motor vehicle accident.The facility is spotless, brand new. Each suite is private unless you request a two bed room for you and a family member or friend.Staff was attentive and friendly. OT, PT and Speech therapist were very knowledgeable and compassionate.I feel my recovery got off to a good start at Transitional Care. I would definitely go back if the need ever the need arose again.I have felt confident recommending Transitional Care to several families since my stay.

My 96 year old mom came over to this facility as a private pay due to the fact that my brother was unaware of her secondary insurance. The day before the money was collected, I called the facility to let them know my mom had a secondary insurance policy that would pay for her stay there. I was told by Chris in admitting no they wont . That week I contacted my moms insurance to verify once again that they would pay and called back to Chris to insist they contact my moms insurance. It has now been 4 months, countless calls to insurance and this facility. And now they have received money from our insurance( not all of it because they dont send in the right bills) and they are still holding onto our $4900.00. This has been a nightmare for us!! This should never have even happened had the intake person contacted our insurance company from the moment I spoke with her. Also, the facility is beautiful on the outside, great rooms, nice tvs, a refrigerator, but staff on the floor is slim and overworked. If you bring an elderly person here you best be around for therapy. My mom acquired bruises on her legs from leg weights and when she left the facility her fractured pelvis was healing but she could hardly walk due to severe sciatica from strained muscles which then took us to another doctor. And if you should still decide to bring an elderly person here, a camera in their room would be a good idea.

The physical therapy and occupational therapy was priceless! Everyone of these professionals are hardworking and compassionate. They know when to push you and when to back off.I had total knee replacement and because these professionals were so wonderful I actually LOOKED FORWARD to my sessions with them. Hands down, I would have never gotten on the long road to recovery without them.The food I thought was pretty awful. But then, Im picky. This is easily solved by having visitors bring food. Each room has a mini fridge.

This place is a great find! Not sure why the negative reviews but our experience was exceptional. My 78 year old father was admitted pre and post OP before and after a surgical procedure. Upon admission he had a difficult time walking and performing every day functions. It wasnt safe for him to be home alone with his cardiac and thyroid issue.TCs staff was very accommodating making sure things were just right and his medications which was quite extensive were properly dosed. Rehab with the PT occurred on the floor facility just a few doors down and was very effective.Dining hall is also at the end of the hallway which was a very short walk away and the staff is great treating diners more like hotel guests than patients with each of the courses brought out after detailing out what was on the menu. Food was healthy, portions appropriate, and my picky father approved of its taste.We were also very impressed with the Nursing staff. All were very responsive and friendly. Most notable was their willingness to work together to help out. We had the utmost confidence in their ability.Father was discharged when he got to the point of being independent again. He only had good things to say about this place which is gold considering hes not an easy guy to please! I would not hesitate bringing anyone here.

I want to start this review by saying that I have personally met the Owner and Administrator of this facility. They both have Nursing back grounds and it shows. They love their employees and it shows.Now on to the review. My Mother was admitted here for rehab of 2 things. The first was removal of a knee replacement ( she has temp. Plastic in her leg at this time), the second being an ongoing wound that was half the size of a tennis ball. From the very beginning I met Christine (wound care nurse). She was the primary nurse for my Moms wound treatment. I want to personally acknowledge how happy we are with her. She knows what she is doing, should be trusted to always do what is best for her patients recovery. Christine not only performed her job well, she helped my Mom heal the wound almost completely in 5 weeks. I should add that my mother was under the wound care of a very specific high-ranking individual at a nearby hospital that had three and a half months to attempt to help to heal the wound. Not only did that person not help heal the wound but in fact made it worse, so bad that my mother was admitted to the hospital with a severe knee infection which caused her to have her knee replacement completely removed. Christine is a tremendous asset to not only TCA , but to everybody she will ever come in contact with during her professional career. Thank you Christine from the bottom of my heart. Now onto the others who helped, hugged, treated my Mom like family during her 5 1/2 week stay. Emily (PT), You are awesome as well. Patient, attentive, courteous and also a tremendous credit to your trade. Ray and Lauren (OTs) were also awesome OTs. Rayna, Ashley, Zenona, Aweda, Cynthia, Caleb (RNs) were terrific, attentive fast responding Nurses. Andi, Donna, Angelica (CNAs) were fantastic as well. In general I can not count the number of times one of your staff hugged my Mom, wished her well or generally treated her as a family member. For those reading this, this is a facility that is under new ownership since July of 2019. Im sorry if your loved one had a poor experience prior to this. I know with complete certainly that my Mother will be back here at TCA when the time comes for her next knee rehab. I cannot say enough positive things about this Facility. Sheila (Administrator) cares very much for her employees and it shows in their attitudes and performance. Keep up the good work TCA , with love gratitude and thanks.Karl E. Streitenfeld

EVERY room is private (only one bed per room) and nurses, OT/PT staff are all friendly and efficient. Construction was completed in January 16 so no nasty c. diff nursing home odors. Also, their dining area looks similar to IKEAs restaurant: electric fireplace, brushed steel furniture accents, black framed artwork on walls. Plus state-of-the-art equipment in their therapy rooms.

A mixed bag.The physical and occupational staff were thorough and highly skilled. That was my reason for being here, so I have been basically satisfied.On the negative side, you get little information about basics like when you will see your MD (I have now been here 30 hrs and have not been seen by the MD), when you will see your nurse, and the basics of the process here. You are told that certain things will happen, but they get forgotten. They direct you to an app, which is vague.Many staff have been very helpful and cheery. Some appear to be passive-aggressive.Food is very good for a rehab facility. TV is outstanding but Wi-Fi very weak. Facility is relatively new. Mercifully NO smell of urine. Nicely informal atmosphere.

While licensed as a skilled nursing facility, this center only focuses on the short term rehab patients. This is not your typical ‘nursing home’. There are patients of all ages. The concept is a hospitality (think hotel) focus, with highly skilled medical team and patients that are here for just a few days. It’s an opportunity to more fully recover from surgery, devastating illness or injury before you go home. While this concept is not unique, it is new to our area.

This place is horrendous. The nursing staffing is short staffed and burnt out, the admissions office is pathetic and the so called business office is clueless.Building is good looking but all that glitters is not gold.Consumers beware of the SNF you place your loved one.

My Mom had a terrible experience at Manor Care in Northbrook in 50 minutes Transitional made room for her and had nurses and dinner waiting for her. She had heart surgery and stress was not what she needed. Thank you so much

My wife was recently discharged from your facility after almost 3 week of rehabilitation as a result of a turn for the worse with her fight against cancer. Her goal was to get back home again and get back some of her independence. We found your facilities to be modern, clean and patient friendly. The food way exceeded our expectations. The nursing staff, CNAs and OT & PT therapist were very positive and supportive of my wifes needs. She left in a much better place then when we started.I wanted to acknowledge one of your CNAs, Seon Duquesnay. She went above and beyond with her level of patient care. She always came to work with a smile on her face and a positive attitude that shined. My wife felt very special every time Seon was with her. I trust that you have some form of employee recognition system to reward those who make a difference for the patients they serve.We would not hesitate to recommend your facility to others in need. Thanks to everyone at your Arlington Heights facility who had a hand in helping my wife, Dorothy get back home again.

Therapy is great.Care on the other hand is not. My mother has had to sit in her diaper for an hour for someone to come change it several times and was sick for three days and no one addressed it. She was also assigned a dr and never saw them and they have screwed up her meds and one day forgot to give her one of her doses. Don’t go here unless you only need therapy. They are completely understaffed.My advise to them is if you don’t have the staff to care for your patients only accept the amount you have staff to care for.

I am patient at transition care (1-18 through 2-8, 2019). The actual patient care is very poor. Nurse response time is often as long as 25 to 30 minutes. Wait time for pain relief mediction ( as needed ) is 20 minutes on average. Even when a medication is listed as offer every 4 hours it seldom is offered in a timely manner .There are some nurses that have compassion and a sense of responsibility and urgency, but notmany.The physical and occupational therapy department is very good. The food service and wait staff are also very good.All that being said, I could not and would not recommend Transitions Care to anyone. One of the most important reasons to go to a Rhab Ctr. is its medical/clinical care. Transition Cares lacks compassion and a quality staff. ThE clinical staff is also very understaffed from 11 PM and 7 AM.Allan DeForest

Call light wait time during pm hours is about 15 minuets. My grandmother was transferred to Transitional care from Northwest Community Hospital. On Saturday 6/16/18 around 8 pm it took CNA 15 minutes to respond. She called LPN Jorge who never came. It is nice and clean, but not stuffed appropriately. Per nurse on duty, patient to nurse ratio is too high . Would not recommend. As NP for Advocate Healthcare I know that staffing in rehabs and LTF could be tricky, but it still their responsibility. Wait time for the call light response at Advocate Hospitals is less than 3 minute per policy . Sad that such new facility is going the same trend as the rest of rehabs in the area.

Nursing home did not have the medications for a friend when she was transferred on a Saturday afternoon - 1 PM, 3/4/17 from Alexian Brothers after knee surgery. She not have pain medications or her anti blot medications by 6 PM. She was In pain 10/10 and the nursing staff said we dont have your medications, the hospital usually sends the medications with you. Nursing staff didnt answer or delayed answering the call light.. She could not get out of bed by herself because her walker was the wrong size and the staff did not know how to adjust the walker. She was told she would have to wait until the next day, Sunday 3/5/17 when physical therapy arrived at 9:40 AM. She reported the only thing the staff knew was how to instruct her how to use the TV. She is extremely unhappy, in pain, not getting answers to her call light for help and MEDICARE is paying for her stay. She has been told the nursing home does not have the correct size of walker for her. At 9:37 AM 3/5 AM she reported she had not gotten her medications and again in pain 10/10 and no one was answering her call light.

My mother was in there , the care was horrendous , I was not allowed to talk to Nurses , was supposed to go through the Cardiac head nurse only she did call or keep her word to me , I had to practically beg for a shower , and she wasnt always there . I felt no compassion from her..Forgot to weigh her for 2 days , she was complaining about her oxygen not being right , their answer was stress . When her body was going up and down on the bed , and her ankles were 3x the sixe of normal I demanded blood work chest x-ray, spoke to the Dr myself she was rushed to the hospital where she remained for 5 days . gave her 140mgs of Lasix in emergency room , dont feel she would have survived if she stayed there. Found your nurses to be very below par . Please dont have Jason call me , because I talked to him already . If your loved one needs more than orthopedic care do not send them here!. Please be there for your loved ones as much as possible . One other thing if you feel you want to spend the night , they told me it would be $80 a night , never heard of this before !!!
