Kelly was wonderful. Helped us with all sorts of stuff that was not really their problem.Place is clean and pleasant. Very happy to help my mom move into this new phase of her life.

One Star, Not Five Star! I am a family member of a memory care resident and I am very upset by what I saw Friday last. Now that they have begun sunshine visits I am allowed to sit in my parents room for 30 minutes. I went a tad over my time and I heard someone talking loudly and rudely from outside my parent room. I opened the door and saw the memory care Director complaining to charge nurse that I was over my time limit and taking advantage of the situation. I was also then told by one of the nurses on duty (more like off duty since she did not get off her phone once) that I was causing the sun down behaviors because I stayed to long and made my parent nervous! The staff that day (Nurse and Director) are very rude and uninviting. I am not sure what they have to be upset about since neither ever appear to be doing any good, except be rude and boss people around. The Aids are wonderful and calmed my fears of what I saw and heard. The Main Director of memory care (Rose) is lovely and always kind and professional.

My mom, who had Alzheimers, was in the memory care unit of Church Creek (CC) for 5 years, and had a wonderful experience ... The atmosphere was clean, caring, loving, alive, and homey (not institutional). The nursing staff and aids were amazing ... professional, attentive, warm. They communicated regularly with our family, and went above and beyond just doing their job. CC had entertainment regularly, special holiday celebrations, wonderful buffets for patients and family for major holidays, and celebrated each residents birthday. Cost was reasonable compared to many other less desirable local facilities. Also available were many convenient services ... hair salon, podiatrist, physical therapy, bus service, informative lectures on various topics relevant to seniors. My mom passed away at end of 2019, and was happy and well cared for up to that day. Our family misses seeing all the wonderful staff there ... they had become like family members.

LOVE this place. Nice, clean, updated facility. Staff is extremely accommodating, food is fabulous, more affordable than most senior housing options. Independent living with add on services options available works great for us. in house doctor and nursing services are wonderful to work with. We couldnt be happier!

I am writing this review one day after my father sadly passed away at Church Creek. While he was only there for less than a week after suffering a stroke on Christmas Eve, I was very grateful for the compassion, attention and care he received. We experienced a team approach in which my father came first not the job description. I spent a lot of time researching SNFs because I anticipated that this would be where by beloved father would spend his last days. I am writing to help others who are searching for guidance and insights into this very tough decision. While no SNF is perfect, I was happy with our choice and appreciative of the staff.

I truly believe my grandma died here due to neglect. She was in fair health, compared to other residents there, I’d say she was in excellent health. She was simply staying here for rehab after surgery. She’d wait up to 2 hours for staff to be available to take her to the restroom and would end up sitting in her own excrement when they failed to tend to her needs. I would never recommend this facility - it is severely understaffed.Response to owners response: I wish that I could change my review to negative stars after receiving a comment from Church Creek. I gave no timeline or personal information about myself in my initial review. It was extremely inappropriate and unprofessional not only for you to address the timing of my grandmothers death, but also to go as far as to contact my Aunt based on my concerns. As if somehow because you claim she didnt have concerns, that diminishes the validity of mine. I dont wish to speak to you. Theres nothing you can do to fix this. I know youre more concerned with legally protecting yourself and not actually addressing my concerns, but no need to worry - Im not planning to do anything further. This review is simply for other families to see and take into consideration when searching for adequate care for their loved ones. However, I do suggest that you learn to practice confidentiality and empathy in future interactions with your customers.

My husband was at Church Creek for several months. He was very disabled and required a lot of care. The treatment he received was wonderful. The doctors, nurses, and other staff were knowledgeable and empathetic. I was very glad that he could be at Church Creek.

Wanted to thank everyone at Church Creek for the wonderful care that was provided for my mother during her respite/rehab stays. She really appreciated the caring and friendly manner expressed by everyone!

Do not put your loved one in this facility. (Rehab). Staff underwhelming, care is negligent, very poor outreach with arrogant approach to clientele. We made a big mistake here. And it ended up costing a loved ones life.

The rehab and nursing area in this building is terrible. This place has horrible leadership who are full of excuses.The head nurse Bonnie is worthless and a liar. Talking to staff many are unhappy but try to be hardworking. We couldnt be happier our family is no longer there. AVOID AT ALL COSTS
