Ive been involved with this location for a number of years now and I have to say, my treatment team is quite professional and on topof things Id say... a one stop shop for all my mental health needs including a GENOA pharmacy inside the office.

The staff was very nice and the service treatment was fast and just kept it very simple for me.Im not a regular patience there but if ever was I definitely request NP Martha.Thankyou Janice H.3.09.2021

I happen to enjoy my clinic. If I have an issue I know I can get ahold of Jamie Seymore. Shes the best. My whole team is great. They are a cheerful face when I go in. Ive been with sw network/community bridges 8 yrs and I still enjoy going in.

I have been on benzos for 4 years for serious anxiety I suffer from bpd last week I had to flee my home to avoid covid and I stayed at a hotel the first night and left my meds on the night stand. I lasted as long as I could and then I called my case worker. I have just been told I am being taken off my benzos cold Turkey no tapper because there has been to many alerts?? I have never had this happen before and these people are supposed to help you in times of crisis and Im being ignored by my caseworker. Do not put your mental health or medication in the hand of this provider.

I gave them one star and thats too much. Incompetent from the receptionist to the supervisors.If you need mental health or drug addiction help dont go there! Youll get stuck and they will keep making money out of you.Incompetent and heartless.

Terrible customer service. Called all day to try to speak to someone NO ANSWER all day. Automated system is horrible and nobody will return/answer calls.

Psychologist is pretty good but need to have the ability to change patients medicine when requested.Nurse and receptionist staff are cool and receptionist are quite helpful.Some Case managers need to be more timely and prompt.The case managers also need to be more prompt at returning phone calls.The company needs to do an update with case managers on timeliness and policy and procedures now that there no longer part of Southwest network.Case managment staff needs to improve proficiently and effectively on clients issues.The Facility is clean.

Id give them 0 stars if I could. My experience with community bridges is HORRIBLE. First off they hospitalized me against my will, when I told them countless times I didnt WANT or need the services. Well spent two days in the ER waiting for them to send me to a psychiatric ward, then had me ride in a ambulance two hours away to another hospital. Spent four days there. Got out and had NO way of getting home, had to call a buddy to come get me. Then heres the KICKER left me with ALL the bills and cost of the services I didnt want. Now Im over 9k in debt because of them. Ive contacted them via email and phone trying to get this sorted out and nobody will help me now, they just keep transferring me to different departments. So now Im left with no choice but to seek legal counsel.

This place was brought and is now CBI. Sabrina is super nice at the front desk, but other than that what can I say? Its your typical Arizona outpatient clinic servicing poor seriously mental clients. The building also houses a parole and welfare office so its a shining example of modern America. You have the nanny state + prison industrial complex + Big Parma all in one place!

Very clean at the time I was a client at CBI that was like almost 7 months ago my clinical team had done some bezare things and some of it was affective .I was homeless and I slept in the streets until they found me a shelter but some of them were even kind of rocky but I pulled through and adventulaly I was placed in my own individual housing and even its having its probably probably because of this clinical team not assisting me very well they didnt even meet with my landlord.Im at a new clinic now and its already not going good there iether seems that I might be back at CBI at a later time within this year.

So far I am happy with them since they move from Hampton and country club. But that is not them in the picture. That is an x-ray building. Southwest behavioral is actually behind that building in a totally different 2 storybuilding

THEY ARE ALL COMPLETE MORONS WHO ONLY WILL MAKE YOUR MENTAL HEALTH ISSUES WORSE. 2 YEARS AGO I STARTED WITH THIS CLINC AFTER WEEKS AND WEEKS AND WEEKS OF BEGGING THEM TO GIVE ME AN EMERGENCY APPOINTMENT.PLACE IS FILTHY. IT STINKS. THE PEOPLE CASE MANAGERS ARE STUBID INCOMPITENT AND OUT RIGHT LIARS. DR IS RUDE AND PATRONIZING. THEY DO NOT CARE ABOUT YOU YOUR HEALTH OR YOU ISSUES. PROBLEM STARTED WHEN MY IGNORANT CM MARY ANN, STARTED LEAVING VOICEMAILS ON MY PHONE ABOUT A PATIENT NAMED VINCENT. HOW HE WAS OUT OF COMPLIANCE WITH HIS COURT ORDERED CARE, AND APPOINTSMENTS HE WAS MISSING. DUDE MISSED APOINTMRNTS BECAUSE MARY ANN IS NOT COMPENTENT ENOUGH TO RECORD A PTS PHONE NUMBER IN THE CORRECT FILE. I CALLED AND FILED AGRIEVENCE AGAINST THE CLINIC AND MARY ANN FOR VIOLATING HIPPA. INVESTIGATION SUPPOSEDLEY FOLLOWED WHERE IT WAS DETERMINED I WAS WRONG, THERE WAS NO WAY I COULD BE GETTING CALLS FOR VINCENT AND HIS CASE MANAGER. SO, I QUIT GOING THERE. I ALSO QUIT GOING THERE BECAUSE AFTER CONFIRMING MY ADDRESS WITH 3 PEOPLE THEY STILL SENT TRANSPORTATION TO MY OLD ADDRESS. IF THEY CANT BE TRUSTED WITH VINCENTS PROTECTED HEALTH INFORMATIN WHAT WERE THEY DOING WITH MINE. CALLS AND VOICE MAILS CONTINUED. IVE BLOCKED EVERY NUMBER THRY CALL FROM, YET STILL SOME HOW THESE MORONS STILL MANAGE TO LEAVE ME VOICE MAILS. IVE CALLED AND LEFT MESSAGE AFYER MESAGE, IVE CALLED AND SPOKE TO THE DESK AND ASKED THEY CORRECT THE ISSUE. FOR 2 YEARS I CONTINUE TO GET VMS FROM THEM. NO LONGER FOR VINCENT BUT NOW FOR CINDY FROM WENDY. I LOST IT TODAY. 2 YEARS!!!!!! I CALLED AND TOLD WENDY HOWS TUPID SHE IS. IM DONE. I SWEAR TO GOD AND THE WORLD IF YHRY CALL MY NUMBER ANYONE FROM THAT CLINC ABOUT ANYONE AT ALL, I WILL TAKE ALL THE MESSAGES AND REJECTED CALLS I HAVE COLLECTED AND SAVED OVER THE PAST TWO YEARS AND FILE A SUIT IN MENTAL HEALTH COURT.

I have been to numerous locations for service before . Here I found case worker, Keira F. She is the ONLY person who has taken the time to do her job because she cares about her job. I cannot say enough great things about her and they are blessed to have her, as are her patients. I have been unable to reach out to her bosses but I know though are Heather Y, Ryan P, and Diane C. Please fully understand what you have in Keira. She is literally one of a kind and had she not become my case manager, I would have quit the system. I have never know anyone with such a true compassion. She is the real deal and gives nothing less than her best. I would like to write a long letter of commendation for her heart and her help, but need an address. Her works need to be recognized! I trust her and feel safe with her. I know she has my back. God bless her, as she blesses all of us she comes in contact with. Please acknowledge her as she is so deserving. Keira, you are a lifeline and heaven sent. I believe in earthly angels because I know you. Thank you! Simply, thank you! You are reality and a rock-star! I could not, would not, be able to do me without your help. I just visited CBI for Half moon. This place had stolen my property, and committed acts of violence against me. DO NOT GO HERE FOR HELP

Horrible place ,very u nprofesional uncaring case workers.,disoorganized no one knows anything. People are working there for the money not because they care about patients.

All I have to say is that my experience with this place has been horrible. Case managers are never available, they do not have most services that you may need, they are very rude, and I definitely will be transferring some place else.

They are great at doing a bad job not once did they get in contact with me when my brother was there let him go right outside in the rain and lied to him about his placment

Look heres the thing. theyve gotten Better since they were hampton, but they were an Absolute Disaster when they were hampton so thats really not anything to be excited about. im calling them and have been for the last ~20 minutes and they havent picked up their phone. my case manager has gotten way better at making appts and stuff for me, but its still impossible to get ahold of her, and the fact that they have Mandatory Meetings for Everyone Including The On Call People at 9am every day is a disaster. yall need someone on backup. also its still impossible to get a referral to anyone through them lmao
