Words cannot describe how awesome this clinic is!! Dr. D and Jay are constantly going above and beyond for patients (current and new)!! We had an issue that we weren’t sure if it warranted an ER visit or not and I called the office and even though Dr. D was booked solid, he told them to put us on the books and he will see us in between patients. We expected to be there for a few hours due to us being worked in, but our total time there was a little under two hours; Dr. D immediately came in to greet us and decided it was a high priority that could be taken care of in the office. During the entire procedure he was so polite, entertaining and professional. You could tell he takes pride in his work as a Doctor and loves to teach others about the medical field just by how he was describing each and every step he was taking to the MA during the procedure. I recommend this office to any and everyone looking for a PCP!! Dr. D is the best!!

I would like to take the time to tell you how great Dr. D is. I have been going to him over 20 years since he had his practice on 103rd St. He has helped me through the loss of my husband, through my self having a heart attack. He has always been there for me and I value him a doctor. He is one of the best in the City of Jacksonville. I can always get an appointment when its needed. Its not that way with most doctors in this town. I have recommended him to many of my co-workers and they too have became his patients. If your shopping for a caring and highly knowledgeable in the medical field. I rate at the top of the chain for primary care doctors.

I have been a patient of Dr. DeLaHoz since 1996 and found him to be sincere, caring, compassionate and so involved with the health and well being of his patients.I have gone with other people to their doctor appointments and have been appalled at the way they are treated by their own doctors..its like they are on a time schedule and do not have time to listen to what you have to say.When Dr. DeLaHoz walks into the examine room he takes the TIME to LISTEN to what you have to say, he treats you with the utmost respect, he discusses your health care plan and explains WHY things need to be done. Dr. DeLaHoz attends seminars, lectures, classes and travels on occasion to keep himself updated on new technology and medicines. He passes on a lot of this knowledge on to his patientsI cant say enough great things about Dr.D. I have never in my 60+years met a doctor who will take his time and give it to you...he rates 10 in my book.

I have been seeing Dr. D since 2006. He helps with the issue at hand and then he actually wants to know about you. How is home, work, etc.Dr. D even found me a better medication for my long term disorder without me asking for it.I travelled for work and would be out of the state for months at a time, but would ALWAYS come home to Dr. D when I was sick.He is not only a caring g doctor, but he is a wonderful human being!

Dr De la hoz has been my primary care physician for over 29 years, he is very caring. I would not trade him for another doctor he has away cared about my health and he takes his time with his patients and don’t rush and he makes sure he answer all of your questions. I am going to him for years he could never find my issue I guess he got irritated because my last appointment he told me he was going to write my stomach condition as colitis when that is not been proven they can’t find my issue and I don’t believe he cared enough to look into it he just guesses.8-4-2019 this is the reply to what you said I’m being rude because you told me to go try something and I went to the emergency room listening to your advice and I had an allergic reaction to it and when I brought my grandmother in there to confront you about the situation you were acting confused and tell me that I need to find a new doctor as I agreed at the same time to say I’m going to take my money and services elsewhere!! And then you said you’re going to write my condition off as colitis knowing that you don’t even know what’s wrong!! This might work on the older group of people that you work with but not me!! You don’t have to worry because I’ve already contacted my lawyer about the situation and everything that happened from the emergency room all the way to every time that I’ve visited your office!! So be looking for my certified letter!! That makes me wonder if they took your license away from you before because you were not able and then all of a sudden you come back to the same facility

Doctor D and the entire staff are amazing. Doctor D has been our family doc. for over 15 yrs. There is no doubt that his patients are most important to him, when you become his patient you are part of the family and are treated like family. Terrific practice.

Great staff and healthcare providers that care about their patients. Insurance companies tend to make care more difficult requiring authorizations for everything. However this office take great care of patients .

Jax family care is truly Family care, they treat you and care for your health as if you were family!!!

They have made marked improvement in the customer service (and doctors are simply a glorified service provider so thats how they should be rated) but be careful of their charges. Physicals are preventative care and the law now requires that insurance cover them 100% with no co-pays. This office will attempt to charge you co-pays for physicals as will others. Check with your insurance provider or agent and make sure you arent being over charged.The new PA is excellent and really appears to care about his patients as much as Dr. De La Hoz does. Im alive today because of Dr. De La Hoz so Im a customer for as long as hes able to stay in business after the ACA kills medicine. I highly recommend his service and care.
