My husband was there in first few hours he was ignored being paralyzed he had to maneuver himself with the bed tray to the bathroom and back to bed then again later no response to his call button then all his meds were not ordered. I am removing him from there before the first 24 hrs are up . I would not let them care for my dog let alone some one I love sad state of affairs

I stayed at Horizon for a septic hip and intravenous antibiotics and it really was a good experience.The whole staff was wonderful to me and treated me with the utmost care. From the activity staff to nursing staff and everybody in between were great!I loved all the meals and when brought to me were ALWAYS hot.The only problem I ever had was with a rude, narcissistic resident.

Worse place ever!!! Never would take my parents or family here. They never answer phones the nurses or CNA most of them are rude when u ask for there name they never give it to you and it’s so hard to understand them. They just discharged my father they didn’t even send the medication over to the pharmacy. A nurse not sure of her name treated my father so bad. Horrible horrible place.

This place will not give your loved ones the care they need. My grandmother went here after leaving the hospital and they wouldnt give her the medicine she needed. Ended up calling 911 to get her out of there. They only care about billing insurance. If you love someone, dont EVER send them here.

Horizon is a beautiful facility! The staff is awesome, they truly care about their residents and have kept them safe during this pandemic!

The staff is awesome, and DON is making lots of great changes. You can see the pride in the way the facility is maintained. Since a new dietary Director started there have been lots of compliments about the meals. I would definitely recommend .

My grandfather stayed here, the staff is courteous, the food is good, the nurses are knowledgeable and the DON is amazing. The nursing manager of rehab Stacy is awesome and I would recommend this place to any of my family members or friends.

My grandmother came here after her hip replacement surgery. She has to wait over an hour just to have someone come by to help her use the restroom and by that time she already wet herself. Same wait time to have medication administered. She was supposed to have physical therapy every day. Sometimes they do it, sometimes they dont. We complained about this to the director of therapy (Sam) he straight up called my mother a liar to her face. How disrespectful! I took my grandmother out early to save her the misery. How is this place still open?

DO NOT SEND YOUR PEOPLE HERE FOR CARE. All the Director of Nurses and Management here only cares for money, and could careless about the health and well being of patients.

My dad got sent here from the hospital was here for 2 days and he passed away today. They where suppose to have him doing therapy and all he did was lay in the bed and they did nothing for him. His oxygen levels dropped sent back to hospital and none of family was called. When we got to hospital my dad had already passed away.I DO NOT RECCOMEND THIS PLACE TO ANYONE. We will be looking into this further

My grandmother stayed here for the last two years of her life. The staff was always so sweet and helpful.. I have some great memories with my grandmother thanks to Horizon. The food is good too!

I have a family member in their care. Staff has been wonderful in caring for him. And always polite and courteous to us when we visit.Thank you!

Worst place I ever encounter! Front receptionist has so much attitude, if you ask something they feel annoyed and all they do is just dress up and sit around. Dont pay the CNAs much. So many people quitting and I dont blame them and surprised at all. The administrator is so rude and he thinks he is the only one educated person in this whole world. The way he walks pretty much everything about him. Never respond if you greet him morning someone needs to fire him as well.

Family member was there, didnt get admitted until 2 days before being discharged (after 3 weeks of being there). Medication wasnt given on time or not at all. Nursing staff didnt even know what they were giving. The call button could be on for hours with no one coming in. Hardly ever took to the bathroom. If I wasnt there my family member would never have help out of bed, into bed, showered, taken outside, or to activities. Never had a warm meal served to her room. She ate in her room because no one ever took her to the dinning room. It was the worst experience ever. I wouldnt recommend this place to my enemies!

I found the management to be gracious and kind. The staff is very compassionate. Thanks Horizon for all you do!

Horrible experience with Horizon Subacute Center. The staff missed her scheduled bath several times, even after I talked to the nurse manager. I had to talk to the administrator to get the staff to give her baths for the scheduled twice a week baths. I dont believe they ever cleaned her dentures in the near 30 days she was in Horizon. She developed a UTI while in Horizon and had a fever for several nights and I am unaware of the nurses doing anything but treating the fever until the infection got really bad, ultimately developing a sepsis (according to Clovis Community Hospital). She had to be transported back to Clovis ER and returned to Horizon after a few days of IV antibiotics. Then Horizon business department called me telling me that she owed a copay that she in fact did not owe. Horrible experience with Horizon. I give them a D- in taking care of my mom.

2 stars for some of the staff being helpful. The rest of the staff walk around like careless zombies waiting for a paycheck

The staff doesnt know what theyre doing.The administrator Silas Acosta is the rudest, most unprofessional person I met my entire life, hes a bully. hes new. A couple months. ( staff turnover). Hes never there, doesnt know anything, about anything. The nurse director didnt know anything, was smoking cigarettes 20 feet from the door, and was drinking a big starbucks, looking at me all confused when I was asking her questions. The administrator screamed me, and said he was gonna call the police, when i was telling him his staff was abusive to me. So, its ok for him and his staff to abuse me? Yell at me in front of my mother? I asked for his boss, he said too bad, your not gonna know.This is the worst experience Ive had dealing with my elderly mother. One day the staff says something, the next doesnt. No one knows what they are doing around there.Its horrible, a nightmare!!Her medications are all mixed up, wrong meds given, the nurses are notNOT R.Ns. they do the know anything. Wrong foods, that can interact with her meds!!! That can kill her!!Its pretty on the outside/ inside. But the staff turnover is high, according to staff, and even one of the Drs. That come once a week, told me, no one cares there. As soon as you leave, the patient is left all isolated, and they dont watch if they eat or drink, they just dont care. If you want your family member to eat, check in them,you gotta go yourself and feed them! And they lost her dentures!!Her hair is on knots!! They dont bathe them. The staff is low pay, so they just dont care!!If I wasent there everyday asking to give her water, juice, food, This and that, check her oxygen level, etc..etc..Shed be dead by now!

Stacy is an excellent nurse and Nancy in medical records is always so sweet!

This place is disgusting the employees and supervisor are trash and so is the place.Idon’t recommend it at all, everyone is rude and everything is so unsanitary.

My uncle is there now and I give thanks for being so far away I return calmly because I know they have a nice team of workers who take care of him.
