I was in a previous Rehabilitation Center but Evergreen is definitely better. They are into their job and very encouraging! This is a great place for short stay physical therapy. The physical therapist who helped me were very great. The CNAs are real attentive and helpful. Nurses and staff are all great. Staff has great attitude. I would highly recommend Evergreen.

I would have to say that evergreen is definitely a great place cause the staff and all the CNAs are really caring and attentive to the residents. The physical therapy staff is excellent in the details of rehabilitation. I stayed short term after a car accident and everyone including residents are super nice. Helped me to get back on my feet again.... literally! Thanks .

I just wanted to give a review on how great the activities department is there at Evergreen care center. They go out of their way to make sure the residents are happy and well taken care of. Once a year they take the residents on an outing to the big Fresno fair. For birthday parties and special holidays or occasions they have live entertainment for them and the residents are always so happy for that. They take the residents on nature walks On a weekly basis through the neighborhood where the facility is located. The Activities department is one of the best I have ever seen.

Evergreen was a very good place to go when you need rehab. I really enjoyed myself. Staff was very good. Nurses good, activities was fun! Great roommates! I know no one wishes to go to rehab but this is a great place to go. I highly recommend this place.

Evergreen has been my moms home for 14 years. She is healthy and vibrant because of the Evergreen staff. The nurses, social workers, activity director, janitors, etc, all do their part to create a loving, clean and safe environment. They work to keep residents healthy and emotionally stable. I highly recommend it.

I am giving a 1 star only cause you have to put one in order for a review to post.. we had my mother with dementia in there for a few weeks and nothing was ever done no rehab no therapy etc etc there answer to that would be “ she didnt want to or she was asleep” .. we would leave multiple messages to the therapist and never once got a call. They always seemed to be confused when we would call and ask how my mother was. We decided to take her out. I work in the medical field and i know exactly what needed to be done and how things work..I would not recommend this place...

Evergreen has good staff. Whenever I felt like giving up, staff was always there for me to never give up. Therapy was good to me, they help me in ever step of the way, I will always say thanks to them. They did heck of a job. Nurses were always there for me too. Overall, I recommend Evergreen for short stay rehab!

I’ve been working at this place since November of 2020 and I absolutely love the staff and atmosphere. I was welcomed with opened arms and everyone made me feel very comfortable. All the staff are very polite and caring about everyone.

My stay at Evergreen Care Center was positive for my health and State of mind. My Admission person was very attentive, kind and helpful. Yvonne Checked up on me daily making sure I was comfortable. The staff was great in general when I needed them they had my back. Activity dept. kept me going staying busy with hobbies I didnt expect to like it. Got to say my stay was great i highly recommend this facility. That i think of getting a job there when i recover. i will miss everyone at the facility.

Evergreen Care Center provides great care to the residents. The Staff are friendly and offer the support the residents need while in their care. Great place to receive wound care and therapy.

My aunt has been a resident here for 7 years...although long term care is now mixed with those receiving short term care/physical therapy...this is home for many. If it werent for the Activities Director, Carolyn Daniels I dont know how any resident would have made it through this past year during Covid. Ms. Daniels makes sure that each person is engaged in an activity: crafts, Bingo, coloring...each holiday she puts together a festivity with all the things to put a smile on these individuals faces. When I couldnt go in to visit she made sure we Skyped on a regular basis...Carolyn Daniels is a hero in my eyes...she will sacrifice her holidays with her family to be there for the residents....thank you❤more of you are needed in this line of work!

I would love to give a positive shout out to the Activity Department. The Recreational Services Director (RSD), plans and coordinates most to all daily/monthly group activities and outings. Residents enjoy everything from Nature Walks, Range of Motion exercises, Garden Club, board games, yearly fair outings, you name it, they consider it. They also provide 1 on 1 room visits for such residents whom choose not to participate in group settings and prefer a visit at bed side. The RSD along with her assistants and support from her Administration team make it possible for this facility to have such a successful activity department. Truly making everyone feel like family while providing them a beautiful quality of life based on every individual residents needs, wants or requests. Great job team!!!

Staff at the facility takes very good care of their residents. Activity director does a lot of different activity that includes staff and residents. She finds new activities to engage staff and residents.

I like this place, it’s a good place to live at. I like how activity keeps me busy. There’s good people here. I would recommend Evergreen to anyone.

The residents are offered a wide array of activities. They often have taken trips to the zoo, movies, Senior Day at the fair, lunches and shopping. The Activity Director is very creative and ensures the residents are engaged in groups or onan individual level.

I have been happy with the activities and facility in 2021. The staff has made it a safe place for my mother to be and I’m thankful for their staff to go above and beyond.

I had the loveliest time these last 2 months at evergreen. Staff is very nice. The CNAs are wonderful. Activities always keep you busy and fun. Nurses are very nice. Department heads are very friendly and help meet needs, in a timely manner. Housekeeping helps keep the room smell clean. Kitchen makes lovely food. Therapy helped me improved physically, willingness to help. Evergreen makes you feel like home. I give Evergreen a 5 star, I recommend this place to anyone. I will miss Evergreen.

I was a patient at Evergreen Care Center in Jan. 2021 for approx 3 weeks.I had the best care given to me by the staff, PTs and OTs. The nurses and Drs were always available if I needed something. Last year I was in 3 different Rehab facilities, and without a doubt, I would request to return to Evergreen Care Center. I would recommend Evergreen Care Center to anyone who asks.

Ive had a wonderful experience with evergreen staff especially Belinda and Yovonne. If I needed help in crisis they stood up for me and helped me through it all. They also stayed in contact with my family. Caroline is excellent person who spent most of her time running activity services.She monitored most of my family visits and I love the fact that the facility is like a home.The CNAs are very respectful and accomodations to personal needs.I highly recommend the facility to individuals who are in need of care and support.

Ok this place has more than one name ...huh but anyway I left another review at the name Raintree Convalescent home as well. I looked into the situation I last wrote on and Nurse Dianna was completely clueless. The truly faulty Nurse got all of the messages of my condition then left for the day and didnt find any of the residents important enough to update Nurse Dianna of thier conditions.This Nurse is absolutely not someone that I would ever want to be in charge of my or anyone elses care. This Nurse is Lanissa .

My name is Errol Marshall, Ive had a great experience at Evergreen with a great staff that was great to me in my hardest times.Yvonne & Belinda are wonderful social workers. almost couldnt leave cause Yvonne made so comfortable here for me. And therapy was one of the top reasons why I healed up so great with my wounds, especially Taylor and Latesha was definitely the best Top Nurse taking care of my wounds also Claudia. I would recommend this facility to anyone who wants the best CNAs to care for your every needs.im gonna miss all of them like family and the Cooks cater to you as well..God bless them all
