This is a clean, yet personable place. All equipment is modern, new and obviously well maintained. All items for residents are well stocked and easy to get to.The staff at this facility is amazing. Everyone is professional and very good at their jobs. I feel that there is an overwhelming, genuine dedication to the care of each resident. Everyone is always eager to assist in whatever way possible, no matter the intensity of need. I noticed that the facility supervisors and more senior staff can often be found taking a personal interest in the care of each and every individual. Care like this is exceptional, and I would recommend this location to anyone.

The nurses and CNAs are wonderful, mostly. They are often short staffed and leave one CNA to do the work of 2-3 people. Many of the nurses are weary of the doctor at the facility and his recommendations. Regardless of how well intentioned and good at their job many of the medical professionals, this has led to a lot of neglect, especially for late stage dementia patients. Many residents do not get their teeth brushed or nails cut, especially diabetic patients. Residents who require mobility assistance to leave bed have been left in bed for many, many hours. The worst I saw at a resident who had been left in bed for over 24 hours before I clocked in for my shift. Management is second-rate and I would be hesitant to send my loved older adults here. It is no question why they fail to keep staff, both management and medical, working at the facility. Management has lost multiple sets of paperwork, including banking information and other financial paperwork. Management ignores my emails and even when they do respond, they are generally unfriendly and unhelpful. I had to talk to several people in order to obtain help, eventually I communicated strictly with the administrator. He seems to be the only competent person on the management staff. Perhaps this will change, as they did transition to being owned by a new company in late September. Regardless the building is in shambles. There are frequent issues with sinks and toilets within the building, many of which do not get fixed for days or do not get fixed at all. The building is infested with flies and other insects. The rooms are very small, but most contain two people anyway. There is very little sunlight in the building and very little to do during the day besides sit in the lounge or their rooms and watch TV. Most residents Ive interacted with have stated that they would like to find a way to get out of this facility.

One of my favorite facilities to work at! There is always a positive work environment and the residents really benefit from that because we are always able to go the extra mile for them since we have that good teamwork.

Understaffed, aging facility with the odor of urine and disinfectant. Unskilled workers friendly but spread too thin. Facility doctor talked to my mother from the doorway; did not do a thorough and professional exam. But they sure billed Medicare for the visits. Mom had to return to PVH; their lack of care was a contributing factor.

Proud to be part of this family. What a difference the staff has made this last year. The culture is like its never been! Being able to experience the changes and movement that is taking place has exceeded my career expectations. Really excited for future residents and staff to come enjoy our team.

This home has focused on the quality of care and compassion for its residents. The residents smile and are active in their home. While it may not be the newest facility, I constantly hear that it is the one that has the most caring staff.

The therapy department in this building is extremely compassionate in treating all patients. They go the extra mile to ensure patients achieve their maximum potential and feel confident in their recovery. They treat everyone as family and put every patients needs first.

This facility should not be up and running. As a past health care professional, the staff and administration at TFCHCC are untrained, understaffed, lazy, and exhibit no genuine care towards the wellbeing of their residents. I would HIGHLY suggest looking into all other options before ever contemplating putting a loved one in this very neglectful home. Our mom was left in bed (she is wheelchair bound, not bed bound) for the first 48 STRAIGHT HOURS she was in the facility. It was claimed that “it was close to a holiday” and “it was a weekend” as well as “the first few days are like a trial period,” as if any of these reasons were acceptable excuses to leave a human being in a bed for 2 days straight. The Fort Collins Heath Care Center does not provide ethical, appropriate medical treatment for any resident left under their care.

I have called this facility THREE TIMES looking for information for a room for my Dad and not one return phone call. That speaks pretty loud and clear for me.

Neglect, abandonment, untrained staff, the list goes on and on. Not a facility I would EVER recommend to anyone, under any circumstance. These patients deserve much better!

“This is truly the home that cares.” The staff go above and beyond to help the residents. Between the residents and staff it feels like family.

I want to thank Matt and his staff for working with me to solve some pressing issues with one of your patients who is a close friend of mine. Accommodating and pleasant are two words that come immediately to mind.
