My mother was being transferred using a slide board and called out multiple times that she felt the chair slipping. The person assisting her reassured her that it wasnt and that she was just scared and to trust her. The chair wasnt properly locked and shot out causing my mother to be dropped and her knee had an actual dent in it. Neither my father or myself was notified even through she complained for a month that her knee hurt. She was never evaluated with imaging either, and she is there because of a severe spinal cord injury that had her hospitalized for two months.

This is a wonderful facility not only did they take excellent care of my dad but they also provided amazing and compassionate help to us in navigating his recovery during a very stressful time. Everyone was exceptionally patient and kind especially Nicole.

My aunt has been here for a few months and seems very happy and comfortable here. The staff is cheerful and friendly. The place looks busy all the time, but in a good way. This doesnt seem like a hospital-type setting, with everything feeling grim and medical all the time. Theres a nice feeling to the place. I liked the big, open dining area and the way they decorated for Christmas.

I have to say I was very impressed with this facility. My Mom was in the hospital for quite some time and needed to be released into a rehab facility. My Sister and I were given a sheet of paper with about 10 different facilities listed and were asked to make a choice. It was also suggested that we go and visit some of the facilities before making a decision. Well, with no knowledge of any of the places listed we had no idea event where to start. I pulled up both Google and Yelp reviews however there were not very many reviews. So we ended up touring quite a few of the different facilities and Lemay Avenue was the last on our list. From the moment we walked in we were impressed.Lemay Avenue was very clean, bright and inviting. The Admissions Representative was very informative and gave us a tour answering our questions. We ultimately decided to go with Lemay Avenue and afterwards were very glad we did!There were a number of rooms available and we were very pleased that we were given the choice of which one we liked for Mom. The care that she received during her stay was exceptional (except for a small issue with one therapist whom Mom didnt care for and after speaking with the Supervisor, she did not have to work with him again). The facility was very accommodating since Mom liked to go sit outside in the sun during the afternoons and we were also allowed to wheel her around in a wheelchair so that she could enjoy the grounds. The food selection and quality was very good as well from what Mom told me. Also, my Mom is on dialysis so 3 times a week she needed transportation to the dialysis clinic. Lemay Avenue (Columbine Health) was able to get her transported to and from each day without a single problem. (They even packed a lunch for her if you can believe that!)I highly recommend Lemay Avenue Health and Rehab if anyone is looking for a rehab facility in Fort Collins. The facility is top notch. I hope the day never comes where I am in need of a facility such as this for myself, however if it does, I know exactly where I will be going!
