Definitely would not recommend. The staff is nasty and disrespectful. Its awful. My grandma came home with bruises, wasnt getting bathe properly and nobody was checking on her. When you call up their nobody answers the phone. Everyone talks about this place and somebody needs to do something to shut this place down. Nobody should be treated the way they treat people.

Care of our precious jewels requires a collective compassionate team. I have worked with Heartland Of Columbia for the past 8 years. Everyone (Ms. Hazel, HR-Ms. Q, nurses, CNAs, rehab, speech, etc) has been welcoming, supportive, helpful and demonstrated a caring attitude towards meeting the needs of their residents. The staff’s commitment to their residents was evident as they encouraged them to work hard and diligently in efforts to overcome the challenges they were facing. The staff was compassionate and knowledgeable as they willingly shared their expertise and answered any questions the residents or students may have had. The administration was very supportive and enthusiastically created an environment that promotes service excellence and the delivery of high-quality care. Thank you for opening your doors and partnering with us. I look forward to continuing this work with you for years to come.

My experience at Heartland of Columbia was awesome, staff were very friendly. Therapy was very patient and kind. The administration of staff was easy to talk to and very helpful. Activities were great it truly was a fun experience.

My experience at Heartland of Columbia was a pleasant one. The food was delicious and always hot, there was never any downtime because activities are seven days a week. The staff was kind and very attentive to my needs. Overall my experience was a positive one.

My husband had to go there, the staff was great, I would love to give everyone a thank you, Martina the nurse asst. And India were the only names I can think of but, they all were great. Thanks for the wonderful stay

BEWARE!! Most unprofessional group. Nurses arguing in the hall way. Having to wait for assistants for hours. Aid walking around in bedroom shoes. Staff calling home to tell you you have less then 3 days to remove family member and trying to charge 8k to let them stay in nursing home. Social worker works for Heartland and is of no assistance. They even didnt confirm transport to doctors appointments.

We have been working with this facility for nearly 4 years as a long term and rehabilitation center. From the receptionist to the administrator (who readily makes herself available) - its been an absolute pleasure. Hazel, Carmel, and Amy are my go-to whether its family or clients that we represent.

My experience at Heartland was very pleasant. The staff treated me EXCELLENT. The nurses, CNAs , activities staff and administration were so kind. Every morning staff greeted me with a smile and they asked if I needed assistance. The Beauty shop did my hair the old fashioned way and she even let me keep the rollers in my hair until i got ready to take my hair out.The food was real good and it was still hot when I returned from dialysis. This is how you are suppose to be treated. The staff is patient and they allowed me to take my time. Therapy was so patient with me. When I got tired they let me rest and they also did a wonderful job exercising my legs.Overall my time at Heartland of Columbia was very good. I would recommend Heartland of Columbia to my family and friends.

Never send anyone to this place. The upkeep and smell were horrible. Other than coming with food, i didnt see the staff do anything but sit at their stations. My dads insurance would have paid for 20 days, he was released early which was good, but the decision was made on a Friday evening with no warning. The social worker was particularly inflexivle, and over all rude. We had to scramble to make arrangements while no help or compassion from the facility. Thay didnt care about any of the circumstances of getting him transitioned.M

My friend was ignored most of the time. I had to go to the nurses station for a toothbrush and toothpaste. They couldn’t find any toothpaste! We never did get any.They did not give her the proper medication. When I complained they told me I wasn’t on the list. The doctor never came.Her brother removed her. She had to be readmitted into the hospital.Do not go there!

Absolutely horrible. We checked my mother in on the 27th of April 2019. The admission process is nonexistent. We arrived expecting to meet someone at the front desk, boy were we wrong! We spoke to a nurse and she said she couldn’t help us as she was ending her shift! Staff aren’t helpful at all. Finally some young man let us in, so much for security. We literally add to corner a nurse just to find out how a patient is admitted. We were sent to a room, full of people and left there for 10 minutes.Round 2, re-engage nurse’s station and asked again about the admission process. The nurse begin to question who I was. I semi-politely told her “who I am is irrelevant!” Does it matter. Is the admission process specific to each patient??? So for anyone checking in, here’s Heartland Admission process!1) Park your vehicle somewhere, no one really cares.2) Enter the building, please don’t bother the nurses as they are all super busy and can not provide you with any information-google it!3) Find an open bed!4) Be sure not to disturb your roommate.5) Sit there and wait for someone to notice you!Option 1, do whatever you can to have your loved ones sent to Rice Estate!

This is the absolute worst place! From the Admin to the CNAs. The smell is horrible and bedbugs are all over. No one is helpful, they act as if asking questions is bothering them. Hazel, the Admin is very unprofessional & very nasty! Please dont take your family members here. The CNAs need skills one on one ASAP. The worst facility in Columbia

Heartland off of Forest Drive requested that I delete this post because it reflected poorly on some nurses and 3rd shift. However, my mother, has requested not to delete the post based upon her care at Heartland. She was released on April 24, 2019. This is a review of my opinion as I visited my mother daily while she was at Heartland Rehab.The PT/OT Therapists (Loteria, Marcus, Melissa, Tabatha, Tamera) were excellent and I appreciate all that they have done to help my mother. I would rate them 5 STARS!!!Some of the nurses were excellent and the pain medication given on time.(Tammie, Melissa, Meaghan, Donald) I would rate them 5 STARS. Other nurses such as Martina H., and the others were rude, argumentative, and uncaring, I would rate these nurses with 0 STARS. Example: My mother’s pain medication was scheduled for every 6 hours. There were times when she did not receive the pain medication for 12 hours. Nurse Martina H. argued with my mother that she had taken her pain medication. Nurse Martina H. favorite words are, “It was documented.” When another nurse arrived, my mother asked again for the pain medication, we were told that the medication had not been delivered to the facility. Nurse Martina H. insinuated that my mother was confused, when actually Nurse Martina H. was confused, and we never found out who had documented that my mother had received the pain medication when she had not. The nurse never apologized for her mistakes and she speaks in a loud, bossy tone to attempt to intimidate others. Another huge issue of not receiving pain medication on time is therapy. When someone has not had their pain medication, they do not want to have therapy and the only reason she was referred to Heartland was for the therapy. Nurse Martina H. told my mother that if she refuses therapy even when not receiving her pain medication she would have a kick back from her insurance company. That was not true. Other times the pain medication as scheduled was given up to 2-3 late when the facility had the medication.The 1st and 2nd shifts CNAs were excellent! India, Kim, CNA Martina (different), Sandra, and Erica are compassionate people who love their job. I would rate them with 5 STARS!! The 3rd shift CNAs were terrible. When my mother call for help at night on the 3rd shift of April 23, 2019, she had to wait for 2-3 hours before anyone would come to see what she needed. After I spoke to my mother, and I called Heartland, then and only then did she got some help. Once the CNA arrived, she was rude, disrespectful and complained about how many other patients she has to take care of, and questioned my mother about why she called her daughter. I would rate 3rd shift CNAs with 0 STARS, and hopefully someone will make some changes with the 3rd shift CNAs jobs.The Administration staff (Admission Office, Business Office, Social Workers) along with Ms. Margaret in the Beauty Salon were all excellent and very pleasant to work with. I would rate them 5 STARS.PT/OT Therapist - Loteria, Marcus, Melissa, Tabitha, Tamera - 5 STARSAdministration (Admissions, Business Office, Beauty Salon) - 5 STARS1st and 2nd Shifts CNA’s -– India, Martina, Kim, Sandra, Erica - 5 STARSNurses - Tammie, Melissa, Meaghan, Donald – 5 STARSNurse Martina H., and all the other rude nurses - O STARS3rd Shift CNAs - 0 STARSJessica and Donald - 5 STARSMy suggestion to improve the ratings would be to replace those nasty day nurses with more caring people and work on improving the 3rd shift nurses, and CNA’s by replacing them as well. It is evident that they do not like their job. I would also like to suggest that Heartland work on obtaining an in-house pharmacy and not waiting for medication to be delivered from Florence once or twice throughout the day if delivery arrives at all that day. This procedure is ridiculous for those in pain. Rehab patients need pain medication before therapy. There should be no excuse why patients cannot receive their pain medication on time each time it is scheduled.

Great therapy staff horrible administration, nursing, and CNA staff!! Nurse Janae is very rude, nasty, unprofessional, uncompassionate, disrespectful, and she does NOT care what happens to your loved one!! No need to go to administration about anything because they dont care either! If you want your loved one CARED for definitely do not send them or let them get se the here!! Medicare website will show you a 5 star rating but thats very inaccurate scoring!!

Really nice people, but either understaffed, underpaid, or overworked, because many of their clients/patients complain of lack of service, infection due to neglect, and experiencing poor treatment overall. Perhaps the staff is exhausted, or in a staff emergency, but the complaints have been pretty consistent for the last 5+ months.

Here as I am writing this. Terrible nursing staff and CNA tonight.Waited 4 hrs plus for drink of thickened water tonight. Still dont have it. The Nurse tried to give me 2nd shot of insulin when already had been given one.Cant wait to go home. Pt Ot Speech therapy are wonderful.

The nursing staff was ok...the Therapy Team was great...their administration staff is awful....they have no sympathy when it comes to the family...they were very disrespectful and not helpful at all in my fathers Discharge...in particular The discharge social worker Carmel...she was dismissive from the beginning and she lied to me to my face about services she would help in preparing for my father...in the end because of her lack of communication we were left scrambling days before to prepare for my fathers discharge...I went to file a formal complaint against her and her boss did nothing to correct the situation did not write anything down...pretty much took her side in the situation and brushed me off as if my complaints were not valid

My mother just got transferred there couple days ago and already staff is giving her the wrong medication twice she has a room mate thats coughing all the time staff there is rude at night shes getting sick not getting better Ill trying to move her asap
