My injector left Evolve for a better opportunity. When I found out I requested that the product they had me pre purchase in the amount of $2647.50 in January of 2021 with the intent of the injector that I have used and feel safe and confident with. Yet I was told due to the fact that I didnt use the services with in 90days they are saying they will not refund or provide me product. I never signed let alone received a receipt for the product I purchased over the phone in good faith. I have reached out and have asked for a refund or at the least product. I cant imagine a business that could keep money that no services were provided for????

The staff was all very kind and knowledgeable. Andrea was the nurse that did my procedure. She was very professional and easy to talk to. I will definitely be back and would definitely recommend Evolve to anyone needing quality care.

Ashley at Evolve Medical did a great job on my Dysport. I was pretty nervous to get it done originally, I am very young and didnt want my face to look frozen, unnatural, or have my eyebrows drop etc. I do not think that a better job could have been done! What wrinkles I had are gone while still maintaining a very natural look and still being able to move my eyebrows and show emotion through them! Plus Ashley was so nice and extremely knowledgeable!

The ultrasounds on my legs were done very professionally, keeping me private while necessarily going to private places. I’m very impressed by their kindness.

My dad and I just went in to have our varicose veins removed. I wasn’t too worried about it but my dad was a bit nervous. We both had a flawless experience. Very little pain more like pressure and tugs while numb. Never realized the importance of getting the veins taken care of until I looked into it. A Dr. Whitehead and the nurses were awesome.

To whom this may concern,I came to Evolve Medical for aesthetic services that my Aesthetics Nurse Kayla Papez in whom I have had treating me for the past 5 years. Evolve was offering a promotion so I called and spoke to the receptionist whom advised me that I had to purchase the products because the promotion was ending and they would no longer be offering it. So, I pre purchased product in December of 2020 with the 50% off promo they were running over the phone to use at a later date that tallied over $2500.00.When I found out about 2 weeks ago that my injector left Evolve due to a better opportunity. I called Evolve Medical requesting a refund due to the fact that my injector left and I made this purchase with the intentions of HER treating me. Evolve Medical is now saying that I had only 90 days from the date of purchase to use the several fillers, botox and Morpheus 8 (please keep in mind Morpheus 8 is between 4-6 weeks apart from each treatment in which I purchase several) After purchasing the promo over the phone, I did not sign, nor did anyone discuss a 90 day? Never once did anyone verbally report to me nor did I sign any paper of ANY kind stating these terms and conditions?They are refusing me a refund on my balance and will allow me to come to the person they have replaced in place of MY INJECTOR? They are saying they have some plastic surgeon that can do these injections which I can find no injectable credentials. And why would they want to take someone elses clients? I trust my injector due to her skills, level of experience in this field and knowing she is the best.I feel very taken advantage of by Evolve Medical and NEVER have I or anyone that I have spoke with heard of such practices. I work hard for my money and I save so I am able to have such services. I feel that they are preying on innocent individuals whom in good faith trusted them.At the end of the day, it should be our choice whom we want to treat us at any level.

I have been suffering with lower back pain for over 10 years. I am happy to report that Dr. Frodsham is the first physician to pinpoint the cause of the pain. He also provided a treatment plan to eradicate the pain and thereby avoiding major back surgery. I would strongly suggest meeting with Dr. Frodsham prior to having back surgery. I came to Evolve for some vein procedures. In the past I’ve had the same procedures in other facilities. I came in expecting the same procedures and the same type of staff. I was so pleasantly surprised with the type of experience I had. It was way far beyond my expectations.. The staff there made me feel comfortable and within a couple of minutes I was completely relaxed. John, Ali and Brooke were amazing and so professional. The atmosphere was very organized, immaculate, no waiting and from the minute you were taken back to prep for the procedure you are treated so special. To them you matter and you are their main concern from the beginning to the time you walk out and leave. The bedside manner was so impressive, each one of them giving 100%. Those procedures can be tough, but all of these qualities John, Ali and Brooke have made my time spent there a positive experience. I can assure anyone contemplating a procedure these people perform should go no where else but here. These people are amazing and such an asset to to Evolve. Kayla is phenomenal! She takes the time to fully explain the procedure and what to expect throughout the process. She is extremely knowledgeable and always makes me feel comfortable. I trust her judgement completely, 100%. She is the only one I will go to for Dysport/Botox, lips, etc. Shes is totally transparent and honest. Kayla always has an amazing attitude and you can see that she truly loves what she does. My treatment results are always PERFECT!! I highly recommend Kayla.

After working within the hospital system for over a week my PCP referred me to Dr. Frodsham and his staff. The word EXCELLENCE doesnt fully cover the care, treatment, and concern for my well being. This was perhaps the best medical experience I have ever had. I hope I do not need their services again but if I do they will be the first call I make. Comprehensive Interventional Care is just the best place to go! Dr. Frodsham has the most pleasant personality and you can see that he cares about each of his patients. I am so grateful to have found Dr. Aaron Frodsham and Evolve. I have never received more compassionate or professional care. I have suffered with pelvic and leg vein issues for more than 20 years and received treatment from my first interventional radiologist in Seattle 17 years ago. I didnt realize I would be receiving care again. Having additional pelvic veins embolized was scary after so long, but Dr. Frodsham answered all of my questions and took his time to ensure this treatment is as beneficial as possible. I felt completely safe in his care.I have now been treated by 4 interventional radiologists and I cant recommend Dr. Frodsham and Evolve enough. Even though the procedures are minimally invasive, many of us have struggled with lots of pain, fear and trauma prior to arriving for the minimally invasive treatments. He and his staff are amazing! I loved my first IR, but not all of the supporting staff were as amazing because it was a traditional hospital I was treated at. Because this is Dr. Frodshams clinic, his staff are hand picked and the absolute best as well. They are on par with him. I completely trust and admire Gina, Brooke, Dewey and Patrick. Diane and Aspen are great as well. The scrub techs were great, but thanks to the Versed, I dont remember their names. :)

Dr. Frodsham performed an ablation procedure for me in 2014. He did a terrific job with the procedure, and his accessibility during follow up consultations was fantastic. Recently he did an interventional procedure on my leg, and the results were terrific. He is truly a terrific doctor.

Dr. Aaron Frodsham is MAGIC! If you are dealing with pain it is worth a consulation with him. He is THE Expert in all things veins - he thinks outside the box and is unconventional in his tretament approaches. He discovered a condition with me I didnt even know was happening & a big root of my pain origins. He was able to change the quality of my living with 2 treatments. I am FOREVER GRATEFUL!!! He and his staff are extremely knowledgable, compassionate, patient, the follow up is wonderful. AND THEY CARE!!!! The staff at CiC is phenomenal - they are friendly, knowledgeable, and it is obvious they enjoy their jobs. I have had chronic migraines for over 15 years and this office performed the only treatment that has ever provided immediate relief. Dr. Frodsham took the time to explain everything in addition to answering my questions. My prior authorization was approved within 3 days and the procedure took less than 1 hour from parking to leaving the office. The staff followed up with me the next day to see how I was doing and to reiterate that they are there to help if symptoms return. I have worked in healthcare administration for the past 3 year and never have I encountered such a welcoming office; it really feels like an extension of home.

I had been struggling with a Desmoid tumor for more than 3 years. From the time the tumor was diagnosed, I had chemotherapy as well as having it surgically removed 3 times by 3 different surgeons all of whom thought that the surgery they performed would get rid of the tumor. None of them seemed to know much about a Desmoid tumor. Each time it was removed, it grew back larger and faster until i was told it had invaded my ribs. The last surgeon removed three of my ribs as well as the tumor and was ready to remove it again when I found out about Dr. Frodsham. It was by accident that my daughter sent a U-Tube video of a patient of Dr. Frodshams who had had her tumor removed by cryoablation and was pain free without any re-occurance. I went to see him right away and luckily was a candidate for his treatment. After three years of severe pain and surgeries, I was pain free and the tumor has shown signs of disappearing 4 months after the procedure. I view finding out about Dr Frodsham as a miracle and to be treated by him and his staff in such a caring way was more than I could have hoped for. I will always be grateful to him because he and his treatment has changed my life.

Dr. Frodsham and the staff at CiCC Salt Lake literally changed my life. I broke my L3, L5, S1, shattered my sacrum and broke both of my hips 8 years ago. I have seen countless doctors and had countless procedures over the past years and I have to say, Aaron Frodsham has made the most impact on my pain in the least amount of time, with the least invasive procedures.
