I would never have surgery here. I filled out all the paperwork. Consultation day (over the phone) came. I had to pay $35 for who knows what before I could talk to the doctor. When I did talk to the doctor it was literally 5 minutes. Nothing about the surgery. Only my insurance and gave me a referral to somewhere else. I spoke to the office staff to ensure my insurance wouldnt be billed for anything since I paid them and no actual bariatric consult took place. 3 different times I was told my insurance wouldnt be billed. They never return phone calls. Almost 2 months later I get an EOB from my insurance. They billed them for $550!!! Ill be calling my insurance to alert them to the fraud. This place is a scam. So glad Im going elsewhere!

I had a wonderful experience with BMI! Dr. Cottam did an amazing job and I’m so happy with my results! I spoke to tina most of the time and she was very kind and helpful with all my questions. I was very skeptical about coming here due to some of the reviews. But everyone has a different experience. The only negative things I have to say. is that I was told that the $200 Non refundable consultation fee was included in my procedure and it wasn’t. Also I would have to agree with other people’s bad reviews on this place not calling back. But I’m a pretty impatient person when I really want something so I ended up calling back 20 or so minutes later and they would answer. But overall, I highly recommend this place. And all the staff were helpful.

Worst clinic I have ever been to specially for something so serious such as weight loss surgery Dr Rishards was my doctor and left me with a very very disappointing service and 13 cuts in my chest when they normally do 4-6 cause she never notice my torso was so long.... really we saw each other for 6-9 month in a row and you never notice???!. I always felt neglected by her and she was super busy on her laptop typing. I wish I would have been treated with empathy and compassion and a lot more personable service rather than feel like a cow in the slaughter house just get me in cut me and NEXT!The process was hard as it’s supposed to be but never made easy by her. would never recommend her “expertise” find someone else.

I will never recommend this place!!! At first I enjoyed my experience I was able to learn things about my metabolism I didnt know. The staff was very nice the doctor wasnt pushy for any specific surgery or date, and listened to what I had to say. Than the insurance lady came in and i explained to her that my insurance would end in a couple months from the time of the appointment because of changing jobs. So they needed to bill them quickly for that visit and then I would get them my new insurance info for everything else. We Spent almost an HOUR talking and making sure I was covered. But come to find out they waited to bill my insurance until it was to late. And when the insurance said I was no longer covered they sent me the bill. I was surprised to say the least. when I called them they claimed to have lost my insurance information and had me give it to them again. So I gave them the old insurance information and the new insurance information. But to make a long story short I now have to cover the cost. I realize going in a couple of months before your insurance ends is not the best move but they should have been honest and told me to wait until my new insurance arrived. And unless your extremely over weight I have found Jenny Craig works really well.

Had my surgery on 11/13/2017. Dr Richards is great but her staff is disorganized and very non empathetic to the patients they treat. From Jesse their benefits specialist to Paula the one who submits the paperwork they repeatedly give you the same comments over and over, “no we haven’t received that information yet.” Over and over until you threaten to go someplace else then they jump in to action. Dr Richards MA Kristin never answers emails or calls you back, she doesn’t pay attention to detail and could careless what your name is or that your a patient. I’ve been trying for two weeks to get LOA paperwork corrected because of her negligence but she doesn’t care to return or answer emails let alone just do it. This doesn’t even include the amount of effort it took for her to actually submit the paperwork to begin with. The gal that does the IV therapy has a horrible bedside manner. She didn’t hardly say two words and was very unpleasant. When she disconnected me she said nothing, at all. Just moved on to the next person in the room so I assumed I was done. They definitely need a better staff that is more in tune to treating patients and realize that their patients are people and deserve excellent care especially considering the nature of they’re business. Overweight individuals have a hard enough time in society they don’t need it from the staff that is supposed to be supporting them.

I had a gastric sleeve done by Dr. Cottam five days ago and, so far, has been a great experience. The doctor took all of the time necessary to answer all of my questions - much better than my PCP. The staff were all very competent and reassuring, going out of their way to help us. One of the best things I’ve done for myself.

Im now two years post-op with over 100 pounds lost with the DS procedure. I was taking 3 oral medications and night-time insulin and meal-time insulin. I no longer take ANYTHING for the diabetes! I had sleep apena and it is now gone. High blood pressure and high cholesterol both gone! I feel like a whole new person! Love the staff and the Doctors! Nothing but great things to say about them!

This place is a scam! Never go here. The docotrs and nurses are horrible to deal with and are rude and conceited. They are of absolutly no help or support and dont even keep their word. So if they tell you something dont go to them. Ive never been treated so poorly.

They were miss leading about the cost of the consultation. The nurses and nutritionist were kind. Dr Cottom lacked bedside manner and compassion. He seemed to be going through the motions, and was arrogant. He had zero interest in helping really explore options and feel good about my decision.

This company is using insurance fraud to run. My insurance company doesnt cover anything related to weightless so they offered to check for a hernia. If I had one, they would do a hernia removal surgery and just happen to also perform weight loss surgery.Additionally, dont believe their lies about only paying your copay and they will pay the rest. They will change your diagnosis code to be something your insurance will cover, and if they are out-of-network, youll be left with a bill (in my case, for a info session that most places offer for free)!Beware!

I felt ok about the surgery but when I was following the calorie intake and exercise suggested by Tracy and had a fainting spell, Tracy would no longer return calls or email and left me hanging. The nurse didn’t know what to do to help me start to lose again. I felt like they threw their hands up, gave up on me and as I was not getting my calls returned I gave up on this clinic and manage my weight now on my own. I was extremely willing to do what I was asked and very disappointed in the clinic as a whole.

I knew Dr. Cottom from Vegas. I went to him for my lap-band surgery about 2004, and it freaked me out when he wanted to do a breast exam on me. Why would you need to do a breast exam for a lap-band surgery???? I asked him that question, when he asked me to remove my shirt and lay down, after the nurse left the room. He stated because the band will be next to your heart. NOT!!!! It gave me the most creepy feeling. Ive asked three other barbaric surgeons about the practice of doing breast exams for a barbaric surgery, and they said NO, why would we need too? My point exactly! What a perv! I wish I could erase that moment from my life.

Yesterday, 11 Oct 2018, was my first appointment. I am very impressed with how organized is BMI. I learned a lot on their website beforehand and was able to fill out a lot of paper work online. The staff and Doctor seemed knowledgeable and supportive. My questions were answered and I felt heard. I like the idea of lifetime support. I am looking forward to meeting with them again.

I went to BMI for the gastric sleeve surgery. I have been nothing but satisfied with the care I have received from the PA’s, Andrea and Tracey! They all truly care about their patients. I’ve had great success and I’m still going strong in the right direction to hit my goal weight and these ladies help me everyday! They are always available and always supportive, even when it’s suggested that you will no longer lose weight and you should have an extended surgery done, they build you up and support you! I couldn’t do this without them! The clinic is clean and friendly and I’ve made great relationships with the folks there! I recommend this clinic to anyone who is struggling with their weight and just needs someone to support them! Grade A+ care!

I had a little trouble finding the office because my GPS told me the wrong side of the street but I made it there before my appointment time and saw me immediately.

I had my first appointment with Dr. Cottam. He seemed eager to answer all my questions and ready to work with me and my insurance. I was pleased to be scheduled for my first diet and exercise appointment. I am glad to get the process started quickly. Parking is very hard to come by at this office but the staff seemed friendly and happy to answer all questions.

This is the first time I’ve ever written a review on anything. But, BMI deserves it right now. I started the process in September. Gastric Sleeve on October 3rd. So, I’m 18 days post-op. Dr. Medlin has been nothing but amazing. The surgery went well. The pre and post-op diets have been brutal but they work. Still recovering but I’m happy to date. I look forward to working with their team and working their follow up program this upcoming year. One of the reasons I chose BMI is because of the support I’ll receive on a monthly basis this year...and many years to come. Fingers crossed my experience continues to be as amazing in the future as it has been so far.

My first appt was 1/29/19. Things moved quick and had surgey 5/20. Dr. Richards was nice. I had some complications after surgey she took great care of me. I had to have a CT scan 6/21. Never heard anything on results. Called her office many times, no one ever called back. Had a follow up today and everyone seemed confused on why I was so mad. Her staff is ridiculous. But, I liked her. One star is for her staff

Today, 10/19/16 was my first visit. I was anxious and a little nervous. Filling out the paperwork took me a while, I had to remember things and surgeries. My memory isnt what it used to be.I thought everyone was nice and Jessie was a great help to me. I like Lois and the nurse Tina.Dr. Cottam was so relaxed and didnt seemed hurried and wanting to get out of the room.So far Im set up for a diet class and there are things for me to do. Im looking forward to a continuation of my journey for a life change.

It took over one year to get my surgery, I was approved for three different types of surgeries and had 2 dates change (after time was taken off work and people from out of town were coming in town to help during recovery) no thought was given to me or my needs EVER. I finally had surgery in Sept of 2015 & I feel the surgery went well, only 1 week out of surgery so not positive. I would rate 5 for surgery itself. But the HELL leading up to it and the who cares about you attitude. I get that everytime I deal with them and I have got this the whole year. It does not make it worth it. They also charge this $350 education fee Scam. I had no clue what to expect, what to do after our now. They give you a memory stick with info on some odd their surgeries. Mine was not included in the info abd no one knew what I should be doing. So threw away $350 pretty much worst Office I have gone to. I also would still be making then get on.it but I had to finally call an attorney and get office manager involved so.they finally helped, only after a year and being forced. Bad part is this is only place my insurance covers. So I have no choice. Good luck if you go here, its a good place to go if you want to be treated like junk.

Let me just say Dr. Cottam is a liar. Here is a excerpt from an email sent to me before my consultation. If your insurance does not pay for your consultation with the surgeon we will accept your co-payment, deductible or co insurance as payment in full. Doing my research before I went to my appointment. I found several people posting about this same problem. So, I asked Dr. Cottam before we even got started. If my insurance does not pay for my consultation with the surgeon will you accept my co-payment as payment in full? He said.. Yes, absolutely it will be covered in full with your co-pay. Well I just got a bill for $158.53. When I called billing I was asked if he noted the question I asked in my chart. So we will see how this goes and I will edit my review if it gets handled to my satisfaction.

Over the course of about 3 years I had lost control of my weight. After seeing the scale reach a number that was not acceptable for myself I started to seek tools. I had several family and friends who had Bariatric surgery and it was the best decision of their lives. I set up a consult with Dr. Cottam in June of 2018. The entire staff was so welcoming and friendly. I ended up getting an Esophagoscopy done right then and there. Dr. Cottam stated I was a great candidate for a Duodenal Switch surgery. I knew this was the tool I needed to get myself back. After less than a month of waiting for PreAuthorization from my Insurance, I was scheduled for surgery in August 2018. The surgery staff at City Creek Surgical Center was absolutely amazing! So supportive and caring. I had surgery with a minor heart complication and ended up staying over night. They were so patient with me as I could not get comfortable enough to sleep. Since my surgery I have lost 157lbs and feel amazing! My monthly check-ups are so rewarding and helpful!!! A huge thank you to Dr. Cottam, Tina, Andrea, Tracey, Emily, and so many others for helping me through this journey!!!!!!

I would not recommend this place to anyone who is going through insurance to pay for their surgery. Maybe they are good at accommodating patients who privately pay the $20k for the surgery, but they have been anything but helpful during my process of trying to actually get the surgery.I have done everything that they asked of me as well as all the things required by my insurance company, and they have been dragging their feet to help me get the surgery done on time. Everyone in the office is always passing duties along to someone else who doesnt ever get back to you. I met with the surgeon once for a brief moment and have spent the last 3 months in the dark.Overall it has just been a horrible experience to work with this office. Unless your insurance is like mine and will only cover the surgery through this particular office, I would steer clear of the Bariatric Medicine Institute.

I had surgery 1/11/18 and am very happy with the guidance that I was given by the staff as well as Drs Cottam and Belnap. My weightloss is up to 85 lbs since surgery and I love clothes again. My friends and family always comment how happy I have become since surgery and the fellow pts always tell me how nice it is to know that I am on staff and know what they are going through because I have been there. So far my experience has been on par with what the rest of the reviews show. The office management SUCKS!!! No one returns calls, no one picks up the phone, no one does anything! I have almost NO requirements from my insurance and they still havent even requested my records from my family practitioner!!! I dont understand why they treat clients this way; even if youre only going through insurance, doesnt that represent income for the company?? I am beyond fed up and very close to trying to find a different office. I cant believe that any doctor would allow their patients to be treated this way.

This is a great place to go if you like long waits people that dont return your calls. Dr Richards need to find new staff. she did a great job. the real problem is i only talked to her at the first appointment, the appointment just before the surgery and the follow up.I had tons of problems with her staff . i was scheduled for an appointment when i show up the doors where lock and when i finally got a hold of someone they where having an office wide meeting and she said that they would call me back to rescheduled I Still havent heard form them and that was two years ago. this is just one of the many storys i have about this place. I told two friends about where i got my surgery and they went there and had the same problem that i had. DR RICHARDS GET NEW STAFF THEY ARE RUINING YOUR PRACTICE.

Honestly, I wish I could give 0 stars. And I wish I would have read reviews before hand. This office is a joke. I had my consultation in September 2018. Its now about to be December and I still dont have a surgery date. They have a completely unprofessional and irresponsible insurance rep. She doesnt do a damn thing. She didnt eve submit all of my needed paperwork uninsurance so I was denied the surgery. At this point shes supposed to schedule a Peer to Peer review and that hasnt happened yet. Its been a week and I was told it would be scheduled within a few days. I jumped through every hoop they and my insurance need and they dont dont anything but string you along and drag their feet. At this point Im considering looking for other options because this is a joke.

I have just started the process with BMI and I havent really had issues with the staff. I can actually say that LORI HARMON has been pretty good to work with so far. It seems like they (Paula Larsen) wanted to drag their (her) feet getting my file submitted to insurance but I call and email quiet frequently. They know me by name because I am in contact so much. I am not going to let myself get lost in this process because I have worked far to hard up until now to not make it to the end. My file has been submitted to my insurance company which they say should take 2 to 2.5 weeks so I will start bugging the insurance company after a week. Hopefully once the insurance responds it doesnt take to long to get a surgery date because I do have to do the 2 week liquid diet.

I had my first appointment on 07/18/2018 and it was an absolute joy. I was able to fill out my paperwork at home and only had a couple things to fill out at the clinic. I also absolutely love their patient portal it helps with finding out what steps you are on and when your appointment is.Once I arrived at the office I felt calm and was brought back within 5 minutes of signing in. I got my vitals and weight checked and then was brought back into the office to wait to speak with Dr. Cottam. The nurses are so kind and personable, I felt like a person and not a patient.I finally met with Dr. Cottam and I told him what I wanted, he explained that with my height to weight ratio the procedure that I wanted would not be as successful as doing one that he preferred. (20% compared to 90%) He answered all my questions and even comforted my mother who started crying. The only aweful part was checking my stomach for damage from Acid Reflux and I still have some throat pain and a headache from that but oh well.After meeting with Dr. Cottam I met with the dietitian and she was so nice. She seemed to answer any questions that I had more then any previous ones that I saw and made sure I understood what type of diet I would be on.I loved my visit and I was treated with respect the entire time, which is really hard when your been overweight your entire life as I have. Im super excited to get this procedure done and ready to start my new life.

Had to give them 5 stars! I am one year out and loving it. I can do so much more than I have been able to do so much mor than I have been able to do for years. I feel great!I had one problem right after I got home after surgery. I contacted Dr. Cottam immediately and he got back to me within five minutes and very willingly took care of the problem. I just want to thank the staff and Dr.s from BMI for their excellent care during the past year!

****update** finally ended up having my surgery.. I would still and I hate to say,suggest another office for your weight loss journey. The hospital I had surgery at check on me 48hours after. My surgeon office did not...I was scheduled for fluids 48hours. I was expecting to at least meet with a PA or the doctor. You just get fluids. The lady was very nice but was only there to do your IV. I left a few messages with my doctor MA about some things I wasn’t sure was normal! I never got a call back! Seriously the staff at this office is making the facility go down hill. Either they are taking on too many patients and being greedy about not hiring the support they need. Then to add salt to injury the help they have can be down right forgetful about patient care. The doctors are great and knowledgeable, but you honestly deal more with their staff which is seriously a hot ass mess!!! And the office manger Just makes countless excuses for the lack of response of his staff.*****I am a revision patient. Doctor cottam put my lap band in 2009, due to issues having to have it taken out. Mind you the issues started last year. Kept weight off until last year. As of today doctor richards is doing the removal surgery. I have seen her on past follow up fills on my lap band. The one star is their staff specifically their insurance coordinator needs to be replaced. Paula on voicemail says please give 24 hours to callback, more like a week. Then when you do finally get a hold of her she says she going to submit your information to insurance, asks you facility you want to use etc., says email is best contact. Another week goes by I contact my insurance directly to see if anything is submitted, nothing. Email her, takes another week for her to respond,for her to tell me something out of her control happen and she will submit today. Then she asks me all the same question I already not only answered over the phone, but also sent her in a email. That was last week, as of today my Insurance still in has nothing faxed in. When I first dealt with their staff in 2009 for the process I had nothing but amazing things to say.. as of today and I will update I have zero faith in their staff.. I can only speak for doctor cottam and richards, they are amazing doctors. I am seriously thinking of going to another facility......

WOW what an experience. BMI understands how to treat patients like paying clients. From the time I made the appointment to my first appointment today, I have been extremely impressed. I loved not having to spend an hour prior to the appointment in the office filling out paperwork. I did most of it online and then the rest printed and completed from the luxury of my couch and brought them with me. I loved that they sent the dietary info so I could have all my questions ready. The day of we were greeted by a friendly receptionist and taken back almost immediately. Even getting on the scale was a joy (which never is) with a note posted that this machine can only tell gravity it cannot tell you all the beauty inside you etc... The nurse took vitals and then the dietician came in and was extremely thorough. Then I saw Dr. Cottam who was awesome. He explained everything well and listened. He answered questions and concerns from both me and my husband. After describing my almost debilitating GERD, he took me back and did a scope to see that indeed I have a Haital Hernia. He explained how he can fix that at the same time as my sleeve. I cant believe I may also have relief of my GERD as well. Then the insurance representative came in and answered all of our questions and set up to request authorization and explain the process in detail. Then the super fun exercise consultant came in and normally I would feel intimidated but she was awesome. She explained how she will work with me to make sure I am getting the right exercise for my body and goals. Essentially, I get a dietician and a personal trainer for life after this surgery - unbelievable. I love that they are so comprehensive and can see the big picture on how to help me for life! Finally, they took some blood (really nice ladies too) and I was on my way. I cant wait to hear back from BMI on my approval and scheduling my surgery.

Crappy office. When I called this practice they said a consult with the doctor will cost $200. They charged a rate of $550 to the insurance company.They took my blood pressure, weight and height and I spoke to the doctor for 25 minutes.I didn’t like this doctor. First, he kept trying to push me towards a more invasive, unnecessary and expensive procedure. Second, he kept getting me to say I have symptoms I don’t, trying to convince me if I showed medical necessity insurance will pay. Third, he seemed incompetent with the procedure I wanted saying he had only done a few. I called his office the next day to say I didn’t want to do the procedure with is practice.

Had the lap band done about 10 years ago as you may or may not know your not suppose to throw up with this, I ended up with the flu and for three weeks I could not get any one to return my call, i was scared and finally ended up calling my family phician for help. I quit going after a few insurance issues as well. A few years later I started getting a rash and my stomach was hurting went back to find out I had an infection around my port my husband and I wanted it all removed but they kept saying no no you just need the port removed. Had surgery went home and a little over a week after that I ended up in the emergency room with sepsis and almost died. The problem was the lap band had massive infection around it. Ill never go back.

This has been once of the worst experiences of my life and I would not recommend this clinic to anyone.I have to drive three hours, one way to get to this clinic in the first place...I had been advised my phone consultations would be included in the cost of my procedure but have since been charged for them. I had to call the insurance company to get my approval because their benefit rep, Paula, doesnt know how to do anything but drag her feet. I was scheduled at the wrong hospital & was notified two days later that my surgery would then have to be postponed for 1.5 months. I was charged a deposit by the hospital and BMI for the procedure. I already have my refund from LDS Hospital... I am still waiting on my refund from BMI and their billing department. I paid my portion of the claim in April, I have called and emailed A MULTITUDE of times and I still do not have my refund of $550... which I have now been advised is going to be only $450 because I am being charged for 2) less than 10 minute phone calls from the doctors. Which should have been included...and one of them is because THEIR DOCTOR CANCELLED MY IN OFFICE APPOINTMENT and then called me afterhours :( Not to mention, my pre-op consultation with the surgeon contained ZERO helpful information.Go somewhere else. Do not go here.

Dr. Daniel Cottam from the Bariatric Institue of Medicine. DO NOT GO TO THIS MAN/OFFICE! --In my appointment, he kept scratching his junk, and acting very strange. Completely unprofessional. After the appointment he reached to shake my hand...Gross! I used a whole lot of Germ-X after that. I thought that I could live with the junk-scratching/strangeness because, Ive seen his patients before and after pictures. But his office is AWFUL. After paying a $35 consultation fee for what was to be a FREE consultation, they proceeded to bill me $250. I called many times to resolve that issue, and each time, I either got hung up on, put on hold, or completely ignored. When I finallytalked to their billings section, I was snapped at by the lady, after she she fixed the $250 charge (thankfully) I was charged an additional $10. After that, because I am so desperate, I foolishly tried to schedule surgery. My mother (who is helping me with this whole process) and I called and called to see what was our next step, but each time it was the same, put on hold, completely ignored, hung up on, and yelled at. We left a message saying that somehow We kept missing each other in an effort to mend bridges. I called, and finally got ahold of their scheduling manager. She proceeded to tell me that:We havent missed each other, It was MY fault that they havent gotten back to me. She told me that they havent contacted me because I wasnt following their instructions! Completely pissed by now, I asked what process I was supposed to be following. After that I told her that Id call back later after the process was done, but in reality I was done. Dr. Cottam also kept rubbing my mothers thigh throughout the consultation. She would move away, and he would scoot closer in his chair. Gross.I found out that there was a Dr. Christina Richards that also worked there, and decided to contact her to see if it was any different. When I asked to change doctors the nurse asked me why I told her I havent been impressed with Dr. Cottam or his staff. Theyve been acting very unprofessionally. She proceeded to tell me very snottily Yes. It is the same staff. and she was rude to me the rest of the phone call. I am NEVER recommended this place, and I am going to file a official complaint because this is NOT acceptable.

Very hard to communicate with and get return phone calls .Do you only seem intrested in getting started with the patients follow up is hard .
