I had been looking for the perfect place to put my mom and from the first phone call I knew Cross Care was the place! I was able to go in a tour the day after I called. Within a week, my mom had a bed. The staff is so hard working! My mom is so happy and I know I have found the perfect place for her! I highly recommend if you are needing a place to put your loved one! You can be rest assured that they will get the best care!

If I could give negative stars, I would! Most of the staff is very nice & compassionate, but ONLY face to face. Literally not 1 person was friendly on the phone!After my mom passed, I went to get her belongings. They claimed there wasn’t anything, so I waited in the parking lot for 45 minutes for them to search around. The social worker finally came out with a box. Not only was she cold & completely oblivious to the fact that my mom just died, but she argued with me about whether it was even all hers. Come on lady, I’m white & there are pictures of black people in “her” box!! So now the 1 & only item of any importance to us is gone, because THIS PLACE IS THE WORST!!!!The social worker we had needs to be fired! She never returned texts or voice mails the same day, if even at all. Please please please DO NOT take your loved ones here. I wouldn’t even bring Hitler here!!!!

The facility was beautiful. Everyone is as very friendly and nice! The social worker was so helpful, she answered all of my questions and was very knowledgeable in Her field. I’m am a very satisfied family member

Caring, knowledgeable staff with low turnover. I learned the hard way not to be seduced by a fancy chandelier and piano in the lobby...yes Cross Care is short on frills but they have what counts, which is a staff that is there year after year so they really know the residents and can tell when something changes. We had a wonderful experience with them for four years, until our mom passed. I would recommend them above any other facility in Jacksonville, including much more expensive options, and I know that from experience.

Rehab was great but when mom decided to stay and move over to the nursing home dept she seemed happy but then there was a change she was found in bed wet at 2pm her breakfast tray still sitting there then whenever you called she was in bed or they couldnt locate her or never came back to the phone then in July 2020 nurse called said my mom was goin to hospital said her vitals were great she was very responsive ate good then hour later found unresponsive sent to hospital and passed no one at the cross care knew anything why or how then they later refused next of kin her med records had us sign a paper for them and said theyd call when ready they never called we went back and they said shes no longer a resident here so youl need to get a legal representation to get her papers tgen they gave us a box with her stuff the clothes had someone elses name on them her jewelry was gone then they suddenly changed over all personal after we threatened with a lawyer then we found out that some residents were found with covid but never taken to hospital or reported cause they didnt a want that on thier record my mom was 96 but only prob she had was her bp and when she was on rehab side she was every where with her walker when moved to nurse home side they removed her walker and put her in wheel chair for rest of her stay she was alert we tried to find a lawyer for wrongful death but lawyers didnt want to touch it becareful rehab there is good but get your parent set up for home after rehab dont let them lead you into nurse home section its very very different there also breakfast eggs hard cold an the food was bad well they gave us a box of stuff they said was hers all the clothes were someone elses her jewelry was all gone brand new quilts her knick macks all her new clothes she went there with expensive antique doll they even took a large picture big print Bible that her church brought her they calling it cross shores now but its still cross care

We had to put a loved one in a long term facility at the age of 73. Very hard to grasp.After a long search and tons of prayers Cross Care was our choice. The offices to the aids have been nothing short of awesome! Caring about our needs as well as our loved one.When Michael hit the panhandle this year with a vengeance. Those people in long term facilities had nothing. I witnessed Cross Cares staff open there doors and hearts to help. They filled every available bed with sick people. Cots in the Chapel for staff members in need. Cared medically for these people but way beyond that. Spending time with them-finding loved ones for them even washing staffs cloths at night at there homes!These people really care about there residents with dignity and grace. We are grateful and blessed to have each of you in our life’s..... Thank you. The Mattson Family

If you truly value the well being of your loved one DO NOT bring them to this establishment. The owner of this establishment uses this facility solely for profit. There is NO care at this center. No direction, staff has been in and out the door. Most of the employees I have talk to fear and hate the owner for the ill treatment he provides to them. Unsanitary conditions riddle this place like a plague. Its a wonder this place hasnt been shut down. In my findings there has been several investigations on illegal activities performed here. INCLUDING A RAID BY THE FEDS! BEWARE AND BE WARNED BELIEVE THE BAD RATINGS! Dont let the lies draw you or your loved one to this horrible establishment. REPORT ABUSE.

My mother and my mother in law have been here...this “Care Center” is more concerned about money than caring for their patients. Find somewhere else to take your loved ones.

I wish we could get my mom out of there and into a better place! It smells like urine and the sewage backs up thru the floor all the time! My poor mom is miserable there. The staff will lie when I call to talk to her and say she is playing Bingo or in the dining hall when I know she is not!!! They just don’t want to be bothered to find her. Not like she could be far! The staff comes and goes daily. It is an unorganized mess in there. Half the time they do not even know where the sign in and out book is located. I have called for hours at a time when NOBODY answers the phone and have been on hold up to 20 minutes when I have called to speak to my Mom. This place is unfit and nasty for humans!

Shifty administration that takes money from patients. Sewage backup often. Rude office staff that does illegal signing on documents. They practice in fraud medicare and social security has yet to catch on them. The head administrator has been there so long that he has become insensitive, mean, and arrogant...John Duval and his assistant are CROOKS . DO NOT TAKE YOUR PARENTS TO THIS FACILITY. Many staff persons are NOT certified.

Went to visit a friend and her hair was dirty, her nails long ragged and she had bruises on her arms. They brought her breakfast and the eggs were hard and cold. I saw an aid getting another person up by just yanking her around. When I said something I was told to mind my own business that I was not a family member. Shame on them for taking money to mistreat seniors!!!

Its horrible. The staff are rude you cant get a hold of anybody after 5:30 .there are some nice people there but overall its horrible.

I selected Cross Care because they would take both of my parents and put them in the same room and they would be near a Catholic Church which was very important to them. One parent died before they could both be transported there and the other parent died 3 weeks later after living at the facility. They were going downhill anyway but the care at this place was horrible. The staff was transient, sometimes he got his meds other times they said they werent prescribed. He was on a feeding tube and the wound around it was starting to look infected and when he finally went to hospice they had to pump all the liquid food out of his stomach because he wasnt digesting it but no one seemed to know or care at Cross Care. Once the supervisors left at the end of the day it was hard to find anyone that could make a decision. The place needs to be rehabbed and the CNAs were not friendly or helpful like we had seen at other places. We were in the process of finding another location for him but we werent in time. I feel sad that he spent his last weeks in what I came to realize was a substandard facility. I couldnt post this without selecting a star but I really have to give them a 0 star rating and would suggest that you find somewhere else for your loved one
