My aunt was at this center and the amount of stress they added to her daughters is ridiculous. The staff left my family with not seeing her at times and numerous phone calls had to be made just to make sure she was getting the proper care. This is horrible and since then she has now past the family is destroyed and you guys did nothing to help ease that pain. You made an already hard situation even harder. Every patient should be treated as a loved one and the family should be treated with respect during the difficult time you as a center should of took the pressure off her daughters and did your jobs so they didnt have to do it for you.

DO NOT SEND YOUR LOVED ONES HERE!!!!!! HORRIBLE FACILITY! Its very dirty, they are horrible at communication! They dont answer phones when you want to check on your family member. You are only allowed to see your loved one once a week. The patients here will be cramped in a tiny room with many other patients! Its pretty much a hospice center where they leave your loved ones to die they do not care for their patients here. My aunt DIED here!!!! She was left unattended and always was left dirty. PLEASE DONT SEND YOUR LOVED ONES HERE !!!!!!

My wife has been a resident of this nursing home for 4 1/2 years I am generally happy with my interactions with the home. Elevate Care has been operating the home for about 1 1/2 - 2 years.We have had excellent assistance from Mawiyah Stewart, Financial Coordinator at the home. We had an unexpected bill for $550. Naomi was threatened with removal if the bill wasn’t paid within a few days. They did arrange to pay it off from her trust fund. But that would mean she would get no trust fund for 1 1/2 years. At my request Ms Stewart investigated the.charge and found that the charge had been paid by Medicaid. What a relief as our finances are limited. Thank you Ms. Stewart.

Hello my name is Dwayne and Ive been at elevate care Chicago north since November 2019 and I have received very good care and Therapy has been wonderful. I want to give a very special thanks to Michael Johnson who has helped me tremendously. He checks on me often and helps with whatever I need. He has solved issues with me and other staff and much more. I went out to appointment yesterday and he came with me . He was so helpful even the driver thanked him. My wheelchair was too big to fit the ramp but Michael took over and got me in the medicar He is so special and he never says no. Thanks for everything Michael I wish I could take him with me when I leave here hes the best

*Most Recent Update* My father has been here for many months now and I will say that communication has improved ever since I got into contact with Omar. I wish I could have gotten in touch with him in the early months but I guess better late than never. Omar seems to be giving a genuine effort to make this experience better.Elevate Care Chicago North is an absolute disaster when it comes communication. Recently they scheduled a care plan phone meeting, the only time I have hopes of speaking with the whole care team, to discuss the plans for my dads care and only one of the nurses was on the call. The social worker then had to reach out to others individually throughout the day and I still havent heard back from half of the team.If Elevate Care cant even hold up to the calls they schedule a week ahead, how are we suppose to get info about my dad? Also, havent been able to speak to his doctor all this year (2021). If you have a loved one, dont expect to hear for days or even weeks at a time. They only call when my dads condition worsens.Recently we had an episode where my dad wasnt responding or moving and we went a whole month and a half without anyone giving us any concrete answers. When they finally did a blood test about a month in they saw he had an elevated white blood count so he was sent to the hospital, a visit that probably could have been avoided if they would have listened to our concerns.Very few staff members have shown effort in helping us out.

I came to Elevate Care Chicago North on July 2nd, I came in on a ventilation machine and with a COVID 19 Infection. When I came hear I was told I would never be able to walk and talk again. I started respiratory and PT and OT therapy as soon as I arrived. I made lots of progress with this team. They are amazing and know what they are doing. I have been weaned off of the vent and began sitting up in my bed with in a week. With in 3 weeks I was on a Tracheostomy collar and using a cap to talk. I was also walking again! The customer service is exceptional, they go above and beyond to help you. They are very passionate about their job and I am very grateful to be going home being able to walk and talk again with out any machines. I am also COVID 19 negative. If you want to your family to have great care come here.

I think Elevate care is nice place and they take good care of you. I have been here 3 times for therapy and each times progressing more and more. The therapy team here is always helpful and make sure you get rehabbed , they work you. Hazel and Matt are just a few names that I can think of that are great. Every time I was here the Guest Relations Omar was always helpful every time I needed help or hand something hung in he’d be there t help me or find someone who would. The CNAs are helpful I know they work hard Sonia always goes above in beyond in helping me. Tony always has my room clean and Jovanni always made sure my TV was working those guys were always helpful.Kristen the Nurse that I had was very nice me, whenever I had questions she was come and made sure I had all my meds. This place has its ups and downs but which place doesn’t . As long as you let them know what you need they can try to work with you. I am happy to finally go home after this last rehab for my knee. Definitely a five-star! The facility deserves the reputation it has earned. A couple of weeks after my mother was admitted, her bedsore wounds that had bothered her for months have healed almost completely, and she appears clean and tidy with a fresh smell and higher spirits. Everyone of the staff, not just the doctors and nurses we met seems anxious to show he or she truly cares. Guest relations Omar got in touch with us immediately after my mother was admitted, and is always ready to listen. Katia is the most friendly and helpful receptionist. Kylie did everything she can to make sure our concerns are all taken care of and a most appropriate therapy and rehabilitation program was planned and organized. We are confident my mother is going to have steady recovery aided by the team of care-provides at this first-rate facility.

They need to be shut down and are NOT fit to be looking after elderly folks. How is it possible one of your patients is unconscious for TWO hours and not one of your staff members heard the machines that were going off for TWO hours STRAIGHT BC SHE WAS NO LONGER CONSCIOUS. How is it Within those TWO hours multiple ATTEMPTS were made to get in touch and We could never receive an ANSWER! But once we do get in touch, my relative is GONEEEE.. Please please please investigate before you think about stepping a foot in here . This place is HORRIBLE . Horrible service & horrible PRIORITY in taking care of their patients.

I came to Elevate Care Chicago North on Dec. 11, 2020 with chronic respiratory failure.This facility has a great respiratory unit. I worked with Nisha and her respiratory team , along with the speech therapist Nikki. I had physical therapy and occupational therapy as well. They worked with me to get me breathing on my own again. The speech therapist has enabled me to talk again after getting the trach removed. I was unable to walk and with the help of PT am now able to stand and walk with my walker. I am very grateful to the staff here, everyone does a great job. If your looking to have a loved who needs respiratory or therapy this place is good. Thank you again Elevate Care. I am elated to be going home soon without any ventilator or oxygen machine.

I had a huge promotion and my father is staying at Elevate. He isnt in the best shape to come out, but he was fixed up nicely the folks at Elevate and has gotten stronger with the PT. He was accompanied by Michael Johnson who took the BEST CARE of him. Michael got him there, put him in the right place to see his daughter get promoted and Michael even took pictures. My father had a great day and was super proud to be there. Without Michael and the crew at Elevate getting it done, that celebration wouldnt be the same. Thank you to all.

First off the pictures online are FAKE, if you would visit the center in person you would see it’s HORRIBLE. My mom was there 1 month and passed away. She was neglected there. She was supposedly attended last at 8am Alive and at 11They went to check for vitals and she was gone. How did they not hear her machine go off ? There is never nurses available they completely abandon their patients please don’t send your loved ones here. They just let my mom die and they didn’t check on her. She was left unresponsive for hours.

I came hear from the hospital with some heart problems. After round the clock care and monitoring my conditions improved remarkably. I have been doing better since I have been here. I was on a Dopamine drip and a Life vest and they are now discontinued. The Nursing staff on the 3rd are exceptional and CNAs did great job as well. I would recommend this place to anyone looking to get better. I am happy to be going home with out any machines now.

5 star, this place did a great job! This place is clean and have great supervision. I haddialysis here and they did a great job as well. I also had PT and OT which did a great job for me. I was unable to walk much but now with the help of a walker I am able to walk slowly but gaining some strengths. All the CNA and Nursing staff were very attentive. Guest relations,Omar was always there to give a hand to any concerns that I had, always jumped on it too. Thank you all.

I came into this facility on a stretcher, and its three weeks later and I am walking out. The nursing staff and the physical therapist took great care of me. The Social Services Director was very helpful during my stay as well. The CNA staff was very exceptional going above and beyond with my requests.

I received fantastic care from a employee yesterday Im not sure of the department but his name is Michael Johnson. He moved me and my bed and other belongings to a new room he was so pleasant and nice and he made me laugh. My husbad has been there for 3 years and just discharged today!!! Its a good place to be, the staff have been good to him for the time he has been there. A maintenance staff member Ed has always been very helpful to him. The house keepers are great in keeping up with the facility too.The food can use some TLC but other than it is very good. The customer service was good and helpful to him when he had times that where hard for him.The nursing staff is excellent! He really enjoyed working with Nurse Cadet who was very attentive to his needs.Thank you Elevate Care for everything you have done for husband.

I feel we are now in a even better position to comment on this facility which was recommended by my mother’s physician Dr. Ajay Bhatia for whom we are extremely grateful.We have seen with our own eyes that this facility is busy and extremely well run: Kusi as the administrator leads his team by command and by example. Under his leadership, the whole team at Elevate north appears highly dedicated, motivated, and proud of their very meaningful job, from the attentive Omar, Kylie, Katia, Rinjin, to the highly competent and professional therapists such as Nickie (speech therapy), physical therapists Deian and Arnell as well as David, Director of Rehabilitation who has devised a most appropriate therapy schedule, catering to the particular needs of my mother and our family. The nursing staff also deserves special praise for providing first-rate care day in and day out despite language barrier – my mother doesn’t speak English. As a result, my mother can now start to eat and drink and even begin to try to stand up which she couldn’t do when she moved in more than a month ago. Obviously, full recovery will be a long and slow process for my mother. But we can already see the lights at the end of the tunnel. So, we’d like to say “Thank you!”to everyone involved in the excellent care and rehabilitation of my mother, including those whose names I can’t but fail to mention here. I have been at Elevate Care Chicago since 2019. I came here on a Tracheostomy vent needing respiratory help. The team here took care of me as if I was family. I was capped and happy to say will be going home decannulated. The customer service here is great and the CNA Lintu always goes above and beyond. The nursing staff always made sure I was in great health. The Social Services director was always helpful with any questions I had. I would recommend this place to anyone looking to get themselves or their loved ones good care. I would have had a tougher time on my own. Thank you.

I came here on a ventilator on Nov. 2020, Nisha Saji the RT Director has done an outstanding job, she is very motivated to help her patients in getting decanulated. Her patience with her patients is outstanding. The RT team members Anthony, Vic, Joseph and Justin are awesome as well always going above and beyond what their duties calls for.The Director of Guest Relations, Omar, goes above and beyond his duties as well, ensuring I am always well taken care of. I have doing great and making progress, today I happy to say I am going home.Ben the liaison for the place has always been great with me. My name is lonnie Wilson Ive been here at Chicago north about a month Ive received good care while Ive been here. Today I felt extra special Ive wanted a shower but staff had been busy lately. Today a manager came to my room to help my CNA and I told him Ive been wanting a shower, without hesitation he said no problem he would handle it and he did. He personally got me out of bed and got me to the shower and he even helped me shower. I was surprised because hes a manager but he took time to help me personally it was so refreshing. Thank you Michael Johnson I really appreciate what you did for me. sadly I have since run into the business profit end of my stay I asked could they absorb the cost of one more day so I could get another day of therapy the answer no no no give us your social security check or get out not a week not a month one day and out came the fangs leave tommarow and thank God all of Evanston Illinois got my back because Im leaving tomorrow

I came to Elevate Care on June 18 right after my surgery for a fracture in my right thigh. I worked with Therapy to get better and walk again. I am happy to say I am able to walk again with my walker and very happy to go home after the great rehab I had. I recommend this place if your looking to have rehab done after a surgery. I would like to thank the staff for always being there when needed. The food was good and the place was always kept clean, I really felt like I was at home with family. Thank you everyone who helped me in my care during my stay.

My grandfather and I would like to say that this place is amazing. We were very nervous due to the circumstances of Covid, and rumors of rehabs. But we were so impressed with the amount of patience, love, sincerity, and hard-working employees that the physical therapy department has. My grandfather has already been recommending and so have I for this place. It is always clean, helpful, and we did not have any issues. My grandfather was there for three months for a knee replacement, and he has been so happy with his recovery. Hana and other employees that helped my grandpa, I want to thank you for everything you did for him whereas I couldn’t due to Covid. Thank you again!

I came to Elevate Care North on June 1, 2020 for rehabilitation. The staff was very helpful and the physical and occupational therapists were patient and kind enough to ensure my transition was successful. I had only one incident that was unpleasant and it was quickly resolved. If youre serious about being rehabilitated this is a great place to be.

Elevate Health Care Chicago North has been a great place. The staff has treated me like family. The attention to detail and staff friendliness is great. They are very quick to help you with what ever you may need. I would definitely recommend their services for PT/OT Therapy as well. My Grandmother was a patient at Elevate care and before the Pandemic we were able to visit make she had everything but when visitors were not allowed anymore Because of the pandemic , Mr Omar Arango and other staff members helped our family out by touching base with us and keeping us informed about my grandmother which kept us at ease during the no visitors policy .. I came here on July 14th for rehab PT, OT and dialysis. The personality of the staff is great. The staff goes above and beyond to make sure you are taken care of. The food here was excellent too. To begin with I was In four different nursing facilities. Furthermore, I would say this one was the best Medically. It was the first time, I remained healthy for four months. Next I have to say the wound care team is the best team I had the whole time in Chicago. Finally, the Medical Director, Doctor Gaziono kept me alive for 5 years. He did wonders for me and all of his patients. Overall, I would highly recommend Elevate Care. Especially, if you or a loved one has wounds.

I came to this facility on July 20 and I have worked very hard with the speech therapist, Nikki, who was very pleasant and always went above and beyond in taking really good care of me. The housekeeping was very attentive and always keeping my room and facility clean. The CNAs Sayja and Lintu were very attentive to my needs, always asking if there was anything else I needed. Matrena, the Social Services director, made sure I had everything I needed to successfully go home. I have been away from home for 7 months, rehabbing. I am so excited to finally go home! I would recommend this place for anyone looking to get rehabbed and looking for great care.

I arrived at Elevate Care on October 19th 2020 after hip replacement. I was in lots of pain and agony and the staff members were very kind and patient with me. I was greeted by Guest Relations, Omar, who if I ever had any concerns would get them solved expeditiously. I am very grateful for the TLC that I received, especially from my CNAs, Bernica, Sanja, Karina, Lintu, and also the overnight Nurses Sophia. and Faith who were always attentive to my needs. I would also like to thank PT Deian and OT Lourdes who got me going from point A to point B when I was at my worst, always being patient with me. I was originally here in 2014 and am glad to have come back to the facility. The rooms are really nice and comfortable as if youre in a Hotel. Last but not least, for the ones whose names I cant remember , Id like to thank you all very much for your kindness and great service you all provided.

My girlfriends grandpa was staying here for a while and received amazing care. The staff really take care of their residents and make sure that all their needs are taken care of.The nurses were so attentive. He was on the first floor which is beautiful with all the decorations they had. SS staff Kristina and Denyse were who I met. They were so nice and they even introduced me to the Ron, they were upbeat and got us some food too. Very nice of them!

My uncle fell and hurt his hip and asked for emergency care. Two hours later and still no response. When I called to complain, I mentioned that he recently had a hip replacement and needed immediate care. The supervisor stated that they had no recollection that he had this procedure done so recently. My question is, what do you ask your patients upon admission.She claimed he is being monitored and that the doctor was notified. My uncle is over 80 years old that went in for something simple, but yet he has been in their care for over three weeks Now that he has experienced a fall no one seems to care, this just does not add up.I will be following up with your corporate office, and see what answers they will have at that point. If this is a healthcare facility and a patient asks for emergency care, your response should be a lot faster. They only looked at the outward appearance of the fall, again he is over 80 with a recent hip replacement and they dont seem to care.

A job well done by their team- they work very hard and do the best that they can with the amount of workload and residents that they have to take care of.Guest relations is very friendly and Ron will make sure that you or your family member is happy and satisfied. I love this place so much. I was at Glen Crest nursing and rehabilitation center after a knee replacementsurgery. every one was so nice and up beat. we even had a Halloween party. the doctors, nurse and physical therapy and office staff were great. I almost did not want to go home! I would not be as far along in my recovery with out the great therapists. I can not forget the cna and how caring and helpful they were. Great staff- they do an amazing job caring for the residents. The social services staff are very involved and make sure to keep family members in the loop.Every facility has its kinks, nothing is perfect, but I think that this facility does a fantastic job of keeping residents and family happy.

Adrianna is one of the nursing assistant in the 4th floor and she is very helpful to me. She is very trained well and accommodating. Barbara also who is ony of the nursing staff was also good. Kristina from Social Services is also very helpful. Allan my nurse is awesome! Hannah from PT helped me feel at home. All in all the customer service red jacket are good!

My mother was here and moving her was probably the best decision I probably saved her life. She had a shaking twitch from one of her medications and would ring for her nurses assistance and wait and never get helped fell and fractured her ankle and her toe still didnt care and wouldnt respond to her light when she would press ur for help. Cockroaches walking around smells like nothing but pure feces on the 2nd floor of course thats common but not the way that place smelled. Very ignorant rude lazy staff. Called and called multiple times to speak to administrators haa what a joke they were never available either. When leaving they wouldnt even give her medications that place mite as well be called the morgue their ridiculous and a big joke

Disheartened with Glen Crest Rehabilitation Healthcare Center my brother was transferred from Kindred Hospital to Glen Crest for extended treatment for an open wound. Upon arrival to Glen Crest I was disappointed with the presence of his room. The room was dark and extremely dingy, the furniture was old and unkempt, the television was an old box TV, and there was an extreme smell of urine on the unit. It is nothing like the brochure demonstrated on the website. Then later to find out my brother was placed on a psychiatric unit vs. medical due to lack of room availability. The following morning his wound care was not done until my brother addressed it to someone in administration. This facility was not our preference due to the distance, however it was recommended by his physician at Kindred Hospital since the doctor is also a member of the staff at Glen Crest. My family is really disheartened by the situation.Response 4/23/2018My sister addressed our concerns with Mr. Kusi the first day and he said he will address this the next day with the admission director since it was late in the evening when my sister met with Mr. Kusi. The next day when my mom went, she seen the situation remained the same, so we had him transferred out. Now he is at Symphony at Aria and we are pleased with the immediate care and services that is being provided.

The place has bed bugs and is filthy. They also dont seem to care that its unclean. I personally know one of the cnas there and shes treated like garbage and because she wasnt warned about their bed bugs my house now has some because of their unwillingness to take care of the bugs.

I was a resident there last year Jan - June 2015 one night a cna took my phone and money. They gave me a check for 100.00 saying it was a deposit. Later I asked for the balance and found the receipt for the phone 875.00 the administrator said that I agreed to take the check and that was a lie. Be careful dishonest cna on staff there. I got no reason to lie and it hurts me I would have never left. Therapy is the best program in the building on 1st floor. Your not safe on the other floors with non working security cameras.

I have been a RN for 33 years and never imagined that at age 52 I would find myself requiring subacute skilled nursing care.Unfortunately I suffered a severe fall with a head injury and secondarily a GI bleed, and was hospitalized for 11 days. During that time I also experienced 2 seizures and underwent many diagnostic procedures. I spent 5 days in ICU and was very weak and debilitated.I was unable to return to work and my prior living situation.Fortunately the Attending MD at the hospital recommended I go to Glen Crest for Nursing and Rehab Care. The very next day a representative from the facility came to meet with me toshare about Glen Crest and facilitate my transition. I was so grateful to be accepted and felt hopeful!I was admitted to Glen Crest on 9/29/17 and upon admission could only transfer from my bed to a chair 2 feet away with maximum assistance.As soon as I entered the facility everything I required was provided immediately. I believe the coordination of services is the cement that binds and promotes healing. Allow me to elaborate: The Nursing staff was always courteous, professional and timely. They contacted my MD briskly if I required additional orders. The CNAs were kind and helpful,and always readily available to assist me. I also had the care and guidance from their Nurse Practioner, who was available 5-6 days a week in the facility all day. Next was the dietician who I met with 3x during my stay. She was always concerned that my menu was accurate ( had prev. gastric bypass surgery). And the food was very good. If I felt I wanted an alternative the substitute menu was very solid. I have to say it was my first experience eating Kosher, and I liked it. Upon admission I was provided a customer service booklet and it explained all I needed to know about my new dietary experience. Next was my assigned Social Worker. She was awesome! She was a good listener, compassionate, and went out of her way to help me. I certainly was emotional for a while! And last but not least, THERAPY!!! I had the most dynamic, motivated, and creative Physical and Occupational Therapists. I have been here 18 days and with the superb direction of my therapists I am close to my pre fall baseline. I receive therapy 6 days a week and it has not been easy, but ever so rewarding. Their techniques to improve my overall strength and balance are challenging. Everyday I look forward to my time in the gym, which by the way has extreme state of the art equipment. But it is not even that, my OT would take me outside regularly to do exercises that refreshed my senses, just by being outside and do something innovative. My PT would play music for me because he realized it was a great motivator for me too.I will miss them both so much. I am totally independent, doing laps outside and have recouped so much, so fast! I cannot say enough how this place has brought me back. From activities to the receptionist staff....all caring. Almost forgot to mention housekeeping and laundry. Both services are very good. My laundry was typically done and delivered back to my room, and folded the day it would go downstairs. The housekeepers on the 1st floor were standouts too. I hope I havent left anyone out. I have so much gratitude for Glen Crest in my recovery. I am discharging the 20th and am amazed how far I have come with Glen Crest. The service in this place is terrible. Patients have to wait hours to be cleaned. Always excuses. Cnas walking around sitting in a waiting area on cellphones, chit chatting and watching tv. Some even complain if you ask for help. Do not recommend to anyone looking for decent service.

I do not and I repeat do not recommend Glencrest at all. It smells some of the Cnas have attitude and dont want to do there job the just sit around on there cell phones in the dining area I cant stand this place the only good thing they have is valet parking

This place stinks. I calling the nurse in charge and she ignores me. I am in bad pain and no one cares. I just have a hip surgery in my hip. And I cant no move. My name u will see me in court.

My stay here on the premises was great! The nurses and the CNAs looked after me very well and were kind. They were always ready to help.
