My father in law is constantly being sent to the e.r under their care, twice for dehydration. The staff members bring covid into this facility. I dont understand how this place hasnt been shut down

Had a really horrible encounter with the receptionist Autumn. She is not helpful at all. Why is she even there? Please caution this staff she does not deserve to be a receptionist. Very horrible first impression. Rude and unhelpful. There are many people who will do the job properly. She is doing a horrible job and representing the organization poorly. I also saw how she treatedother staff, she was not helpful to them either. She acted like they were a burden to her.

A very nice facility and staff. Help is always available and patients always alright it looks. Bart is seemingly ok with Mosaic on Lakeshore, and going for walks or watching TV is available whenI am visiting Bart. I am confident Bart is getting help he needs, and if slow progress he does not suffer, thankfully. All in all a very nice place.

The staff is rude and inconsiderate over the phone. Calling to speak to a loved one is a task! Phone rings for minutes before being answered...HUNG UP ON! Its ALWAYS an excuse! Per my niece its smells of urine!! Listened to my Mom beg a nurse for assistance while speaking with her and the person kept walking.

If you want your loved one to have bed sores and no medical assistance after having a stroke on their clock this is the place for you. The doctor at Mosaic did not believe my cousin had a stroke and it took another family member to recognize it and call 911. Unfortunately the damage was done since the doctors at the hospital predicted the stroke had occurred days prior. This place is unconscionable! I wonder how many bedsore cases insurance companies charge them with.

It is difficult to be a skilled nursing facility in Chicago. My father has received exceptional care and devotion from the nurses, aids and CNAs. He has been in their care for almost 7 months and the staff really care about his well being.There are institutional issues that make care difficult. We have come to accept them as governmental and the bottom line of corporate entities. The shifts are understaffed. The medicaid patients do not receive physical therapy that may enhance their function and mobility. The facility could use some updating and perhaps some expansion. These however, are limitation of the state and the private company that makes administrative decisions.The staff respects the dignity of my father. There are several staff members that I would name, but I dont want to identify them openly on the internet. My family thanks you all and we are very pleased that you have listened to us and supported the end of his amazing journey.

Not a relatively bad facility compared to the rest of Chicagoland, but I have definitely never been blown away by the staff. They cluster at the nurses station and will do everything in their power to do as little as possible, including getting up to assist EMS during emergencies or coming to the patients room to answer questions so the EMS crews dont have to go back and forth. They complain about filling out paperwork required by Medicare/Medicaid and give EMS crews a hard time about everything.That being said, I have not come across the kind of gross negligence in care that can be commonplace at other facilities. So, way to be just above that very very low bar, Lakeshore...

My dad has been there for several months & the nursing & rehabilitation staffs are wonderful! I couldn’t have asked for better people to have to take care of & trust with my father!!!

Worst in communication. Staff appears doesn’t communicate with each other department. Appears doesn’t care what patient come in with and most staff just come to work for the sake they work even doesn’t do much with their patient lack of caring.
