My 4 year old daughter was a tad bit hesitant about going to the dentist.. well it’s safe to say she no longer will ever be scared of going! She had a wonderful experience, and ALL the staff was kind, and sincere! Waiting time was absolutely reasonable, and her dentist was super friendly and allowed her to touch the different tools he would be using. She loved being able to be super involved in the process and guarantee you will not be disappointed in bringing your child here! Thank you guys for being so friendly towards us, but especially my little girl. We look forward to returning in 6 months!

Ive been going here for at least 6 years and have never had an issue. They are reasonable with scheduling and very helpful when dealing with insurance/payments. All the techs that have worked on me are professional and considerate. Dr. Dhake is a great dentist who always tries to help the patient. She has never been judgmental or negative and has a wonderful personality. I highly recommend her and Dr. Daisy. I wont go anywhere else for my teeth.

Dr. Dhake and her staff are amazing! We have been fortunate to have them oversee our dental care for many years now. They continue to uphold a safe and professional environment, which is very crucial and reassuring, as we learn to deal with COVID virus.

I went to this dentist to repair a tooth that hurt with the cold, well I went and they repaired it but they told me it may hurt even if we repair it but it will only be for two weeks but it is normal two weeks passed and I came back and they told me that it was normal that I was going to last a month the pain was going to go away. And so I left it after a month of enduring the pain, I returned a month and it continued to hurt and they told me that they could no longer do anything, the truth is I do not recommend this place, what place does it tell you that it will hurt even if they have repaired it?

I have been coming to this office for 5years. The staff is pleasant. You dont have a long wait time. They are extremely patient with children.

I went to get my tooth looked at for an extraction, I was taken care of by Dr. Daisy she was excellent and provided so much information and explained everything in great detail. She treated me like a friend and was super kind. I highly recommend this office and you will be lucky if you get Dr. Daisy!

First time back since COVID began. Glad to see all the precautions being taken to keep patients safe. Not surprised though as Dr. Dhake and her team have always put patients needs and safety as their top priority.

I had been in different dentist but this one it’s the worse I had one my tooth hurting me and I decide to get it fix so I went and they fix it then they told me you’re going to feel pain in a a week or so but it’s going to be ok it past two weeks and I went back they told me it’s normal it’s going to go away also they make me change the type of paste to a sensitive one I did then after a month went back with the same paint they didn’t want to fix it now I have to look for an other dentists because they didn’t fix my tooth it’s just waste of money I don’t recommend this place at all

Forever Dental is a welcoming place full of professionals from front staff to the assistants and the dentists. My appointments are always on time, cleanings are always thorough and staff always takes their time to answer any question or concern that I may have. They make you feel welcomed and my dentist Dr. Dhake is a wonderful person and amazing, wonderful, excellent dentist! She has an amazing personality and always so positive, love that! She is the reason along with the hygienists and front office staff that I always go back. I’ve been there for over 8yrs, that’s how comfortable I am with them.

I found this place online for my Dad. He had a pretty specific denture issue with limited coverage, and this place was awesome! The staff is extremely patient and friendly. They genuinely took the time to find and explain ALL of the best possible options available to my father and they even followed up with him after his care to be sure he was feeling OK and that everything worked out smoothly. Very impressive. I would recommend them.

I have always had a wonderful experience here— I’ve been coming to the office for 3 years. I have routine cleanings twice a year and the ladies always do a very thorough job. I got my Invisalign here because it was the most inexpensive in the Chicagoland area for amazing service by Dr. Daisy. I will continue to come here as long as I live in Chicago

We had been coming here for years, but it seems they’ve gotten so busy that the appointments are so far apart. A dental cleaning can take up to 2 months to complete. The doctors are great, but the front staff seems overwhelmed that they forget who is paying the bill. I had an appointment but I never confirmed it through their system because I wasn’t sure I could keep it. I never received a courtesy call to remind me like they usually do. I finally missed a call from them telling me that they were going to charge me $35 dollars for missing my appointment. I was ok with that since I was guilty of missing the appointment. I called to reschedule but reminded them that I never got a courtesy call and that I never comfirmed. She lied to my face and said that I did. I put her on hold to double check with my wife and the texts that I had received. Turns out that I was right but the girl wanted to play the you’re wrong I’m right game. That to me was the last draw. Taking my family somewhere else where I’m treated better. It’s a shame because my kids like it there but they forget about their old customers.

This place has lack of service. My husband had an appointment at a certain time and one hour later they come into the room to begin the procedure. They first started the deep cleaning then left the room for about an hour and came back to finish. Never coming here

Be leary giving them your phone numbers! I just made an appointment and they sold my cell number to an outsource agency that texts you! HIPAA!!!!!! I called them as soon as i got a text saying I did NOT authorize them to give my number to ANYONE!
