Horrible place to entrust with the care of your loved one! My dad wasn’t even there 24 hours and sustained a fall while in the care of this facility! My fathers very condition makes him a fall risk and anyone with even an iota of a fragment of a brain cell can see he needs assistance with his mobility and cannot be left alone or unsupervised for extended periods of time! Either they didn’t take the time to read his medical history or they did, but didn’t care enough to ensure and implement effective fall prevention strategies

I really am happy that I chose this facility for my husband. He really this place. I have never visited a rehab facility so clean. The staff are all so friendly and helpful. His family is so pleased with this facility . I rate it a 5 star..

My Mom was well attended by the staff and the location on Fullerton was very clean, no bad odors or heavy fragranced detergent. They had a Starbucks in their lobby, but it doesnt open until 9 am. The only thing I noticed missing was a few hotel/travel sized toiletries, that would be really helpful, and take so very little to remedy.Mom cant wait to get back home, but meanwhile she is in a safe place and I am thankful.

THIS WAS HORRIBLE! if there was 0 stars i would put it at that. My grandmother was there for about 2 weeks and they treated her terrible. They pushed her around they forgot to feed her at times.They treated her so terrible that she cried so much! AND THEY DIDN’T EVEN CARE?!?! So don’t let your séniors go in there. I am disappointed! And feel so sorry for my grandmother.

I want to thank the 3rd floor staff for the empathetic care they gave my brother Im glad he was able to end his days there they seemed to have looked after him very well and I truly appreciate it he was not one of Jehovahs witnesses but I Im and I can truly say that he was kept clean and he never smell nor did the floor he was on like some places do. One complaint I do have is the food needs improvement but that has nothing to do with the nursing or cleaning staff. My brother and I was able to bring him some foods that he likes and he was ok with that. But glad he is now asleep in death and not in any pain.Thanks again for all you did for him, I truly appreciate it from the bottom of my heart.

As my mother had spent time in several rehabilitation facilities, many of which were unfriendly, unsafe and unsanitary I was skeptical about Symphony.Well let me tell you, this one is a breath of fresh air, literally. Other places had that nursing home unpleasant smell which was not detected here. The rooms were bright and clean. The staff was friendly, accessible and accommodating. The nurses were spot on with administering the proper medications. Another popular, ritzy rehab center gave my mom the wrong meds causing her to hallucinate, so of course this is a big deal and a great relief.It is stressful enough seeing your loved ones needing an extended nursing care and rehabilitation placement without having to worry about the facility’s ability to deliver excellent care. Symphony of LP takes the stress and worry out of the equation. I highly recommend them.

My sister is currently receiving rehab at this facility and its very nice and clean. Personnel is receptive and helpful. Im impressed . Personnel very helpful and answered my questions and concerns.

Some workers are nice others not so much. It takes extremely too long to answer call lights especially for fall risk/bed ridden residents. I have assisted on many occasions because the aide just never showed. On two separate occasions the person i was visiting as well as things I left for the next day for myself have been missing. This is unacceptable as the resident doesnt have any food restrictions. So either the roommate is taking the things or the workers.

Debbie (Administration) has been so good to myMother - who was not a good fit initially but has fallen in love with everyone at Symphony. Let’s be honest, skilled facilities are not easy on any of our loved ones and they cannot do all the work alone! Families and loved ones have to participate too but Covid has made it impossible to do. Therefore, we must work together toward advocating success - it can be done and Symphony is helping me to do this mission!!GCA

As part of my training through the Nirvana Institute I have done my clinical work at Symphony Lincoln Park. The staff is very professional and provides excellent care to the residents. I would feel comfortable having a loved one reside at this facility.

Mom brought here Friday after being at Masonic battling a staff infection. Nurses very helpful. Therapy helping mom get a little stronger. Want her to eat more. Think that will increase her strength level. Goal is for her to walk with walker with minimal assistance. That was her baseline at home. Looking and hoping for more progress in this coming week!

One of the best places that I have worked as a nurse. I worked 7-7pm. The head departments are always there for you. And also there are always nursing supervisors on the building. Great pay and also they are always educating the nurses. I highly recommend this nursing home

My cousin has received excellent care at this facility. The social worker is very compassionate; at one point she took my cousin on an outing outside the facility. The MD psychologist has been very careful to calibrate dosages of medicine to avoid over-medication and has also responsive to the familys concerns. Floor staff has overall been helpful and patient.

Last Month I was given the responsibility to find my best childhood friend a place to stay for short term recovery and rehabilitation. It is not an easy task because you want the best for your love one, but not sure which facility is best for you or the patient. After visiting some I was impressed and comfortable in choosing Symphony Of Lincoln Park for these reasons. First the tour guide made me feel welcome and relaxed. She also was training a young lady name Leah (activity mgr) who said if you do decide to bring your friend here she’s going to be loved. Now I expected my friend to receive quality care but to also receive love, I have to admit I was doubtful. Today I agree with Leah with no doubts Symphony of Lincoln Park has something special besides excellent care and rehab. They have a Symphony of warmth, love, interest and concern. Her Dr who I call Dr Z is patient, takes the time to listen and explain the patients health diagnosis and plan to help the patient and allows you to understand in layman’s terms. The social worker Amanda is kind as well as informative. She will help you over and above her duties. Physical therapist Adrian is dedicated, fun and encourages Nadine who’s now motivated, excited and believes she’s gonna get better. Nurse Corey and Louise keeps me informed so I feel a part of Nadine’s healing process. I am so thankful for the employees at Symphony. They are a great mixture of excellent care, encouragement, kindness, motivation and love. They are interested in your mental assurance as well as your medical health. My friend has improved so much since she’s been with Symphony. If I ever have to gave care and rehab take me to Symphony of Lincoln Park so I can get some good care and love.

Transplanting a loved one from their home, security, family & warmth to a nursing facility is devastating. Upon visiting several facilities it turned to be highly traumatic and depressing. A friend suggested I visit The Symphony acute care network, what a difference! Five star service, smiling faces, highly trained, concerned/caring professionals.The staff, without exception, is supportive, encouraging, enthusiastically helpful and positive.Thank you Symphony, for relieving the guilt I felt. Thank you for the peace of mind, for treating the ill with dignity and respect, and for providing 24/7 conscientious, considerate, sympathetic care!

So far I’m extremely disappointed! I went to visit my aunt who recently had a stroke...and at no point during my stay...did ANYONE come in to check on her...reposition her...make sure she was ok...Nothing!!!! I had a previous experience with symphony of southshore that wasnt so good..so I was mortified to find another one of my Aunts placed in a Symphony facility...the best thing about this place is the aesthetics...which does NOTHING for the patient! Im going to make sure Im present as much as possible...otherwise I know they won’t be doing their job!

All nusing staff are vary professional,and friendly.The people who work with my brother are as carful as theycan and still accidents happen. So far my bother as had his ups and down with his battle with his illness.Ill take the time to thank any and everyone who came in contact with my bother .thank you from the bottom of my heart.thank youfor your patient lord thepatient,the people in the kitchen they try to acomidatemy bother because he cant eat everything.after radiation and or his chemo treatments.they do try and feed him.thank you. The second floor where my mother is always smell like body waste. It took them 3 days to unclog the toilet in my mothers room and I had to bring my own hand soap and paper towels because they didnt have any. The people that WORK there (hahaha) are always in the nurses station listening to music or on there cell phones or just talking. They take forever to clean the patients. And today they had a woman that looks like she is in her 80s cleaning the patients and beds. She didnt know how to handle my mother. I had to clean my mother myself. This place is a joke!!! I cant wait until its time to take my mother home. Todays date 12/30/18.

The staff at Symphony Lincoln Park are the most attentive and caring group of people I have met working with those of us that may be depressed, short tempered or just plain scared of enduring the pain of growing older. They expeditiously follow-up on questions and concerns about your care and next steps. Guess what ? This is my second time here and I am not Susie Sunshine and I have high expectations...After having a knee revision by Dr. David Hoffman specializing in Orthopedic Surgery at Advocate. I would like to take this opportunity to thank the administration, team of doctors ,nurses Occupational Therapy , Physical Therapy , case workers and kitchen staff at both facilities for their due diligence and hard work. I was patient at Symphony from Oct. 24 thru Nov. 5th for their rehabprogram post reverse ball/socket and re-repair of the rotator cuff ofmy left arm/shoulder. As this was the 3rd ortho procedure for me, I knew I wanted to go back to Symphony(aka Imperial) for their rehab program. I was not disappointed! The facility itself is pleasantly cheerful and bright.The employees are very helpful and if there was a problem I knew who to go to for information All in all it was a good experience in getting me back on my feet. My father spent 2 weeks at Symphony after being admitted to the hospital from a stroke and then diagnosed with Vascular Dementia.Before he was admitted, i read some of the previous reviews like many people do, and i was hesitant. But i knew he needed skilled nursing to recover from a stroke and that i needed time to plan out relocating him to Los Angeles to be near me.The first floor is fancy and the upstairs floors are old and not updated. So again i was worried.His first night was rough for him, he was in a room with a tv that didn’t work. This was brought up to staff and he was moved to a nicer room within 24 hours. Every single staff member i came in contact with was compassionate and knowledgeable.I flew out of state 2 days later and every single time my father would call me with a issue, I’d call them and speak to a nurse and every single time they would address all things immediately (that rarely happens with anything in life).My father felt cared for while here. That says a lot.I felt confident he was cared for and that also says a lot. I dealt with Daphne who helped me place him here, and then Cassandra who literally was in contact with me every other day assuring me that all things that needed to be lined up for his departure were being handled. And they were. He left yesterday and I’m happy to say is near me now in California. My father likes to complain and he had no complaints during his stay except having a roommate. Having a roommate is a given in a skilled nursing home.I recommend them and can say with all confidence if you are willing to help along side Symphony you will have a great experience with this facility. I’m not affiliated with this facility in any way, i just find it nessesary to voice my positive experiences when i have them.

Great place to be! The place is like a 5 Star hotel inside. Very clean and has Starbucks! The staff are accommodating and friendly. I visited my aunt on 2nd floor and I was guided by friendly nurses. My questions and concerns were addressed accordingly. Nurses here are competent. CNA went to my aunt’s room and asked my aunt if she’s ready to go to bathroom. I like that. Thank you for taking care of my beloved aunt!

The patient to nurse/CNA ratio is lacking. The nursing staff, primarily CNAs are rude, not attentive, and attempt to make their laziness to do their jobs the patients fault in some way and pretend the patient is confused and incorrect. The culture at this rehab facility needs to change. Some staff are excellent but the others need to understand their job is to assist and serve the patients. I know of two families who hired a personal assistant to assist with their loved one in the faculty because they staff did not do their job. I know three other families who moved their loved ones because even after speaking to everyone including the administrator nothing changed. The therapist do great work but look for another rehab facility if you are considering Symphony of Lincoln Park and want your love one to be treated with dignity and respect. I am certain we will never again use this facility.

This is got to be the most unorganized understaffed facility Ive ever seen my father-in-law ,my grandmother and a friend of mine have all been in these type of facilities and this is just horrible from urine on the floor to to throw up that nobody came to clean for hours just disgusting ....they do not return phone calls nobody can ever give you a straight answer on any of your questions .they are remodeling and the patients are walking through the hallway while they are sanding the walls ,fiberglass insulation is hanging down from the duct work as the air blows on it ... the only reason I gave them one star is so I could write this review otherwise I would have gave it a negative 2 stars ... if you care about your loved ones dont send them there..the sad thing is my mother wanted to go there because she was there before but it was under different owners then...

This place is a joke my friend was there and they always trated him with disrespect. One time the cnas left him a soiled diaper for over 3 hours. And trying to speak to someone or get there attention is like trying to talk to a brick wall
