When I got to Cornerstone I was taken care of really well. Jose is such a great attentive Assistant and really took great care of me. I wish I had him taking care of me everyday. The nurse got me food since I hadnt eaten all day, but I didnt see her again. I really appreciated that. The Respiratory team was great checking on me all the time especially Kat, she kept encouraging me when I was not feeling like I was healing fast enough. The Physical therapy team helped me get stronger and Lexi and Laurie did a great job. The Nurses were great Lori and Sal always checking on me. The Assistants that came by all the time were appreciated especially Michelle and some at night but dont remember their names.

My husband came to Cornerstone on a ventilator after being in the hospital for 3 weeks. He received amazing care from all the staff and our family felt so supported by everyone we interacted with each day. We were prepared for a 4 week stay but because of the wonderful care, my husband left after 2 weeks. We really are so thankful to the staff but especially RT, PT, PCT and all the great nurses.

Have worked here for over three years and I love working here. All of my night crew work great as a team and care about the patients and how the facility runs as a whole. My charge nurses (Robert and Vanessa) care about their staff and make sure everyone is treated fairly and with respect. I am grateful to be a part of something that helps people get better and back to their families. Look forward to being with the company for many more years to come.

Thank you to, RT Kat, Zoila and Doreen, PT Adel, Lori and Aaron, Speech Kris, CNA Ashley and Andrew, RN Renae and Vanessa.

First off I would like to thank the people and staff at Corner Stone Rehab, but most of all I want to thank the respiratory therapist and their assistants, when my husband got out of the hospital they informed me that he would not be able to breathe on his own without the ventilator trachea. When he got to Corner Stone, immediately the respiratory therapist started doing their job; little by little my husband was making progress. They were making my husband work his lungs on his own with many exercises, and before we knew it he was off the ventilator, thank you for putting all your effort in making this happen because without the respiratory team from Corner Stone it would not have happened. Second I want to thank the he nurses and CNA on hand that helped my husband come this far.. I as a wife to my husband who was at Corner Stone Rehab, I can honestly say you can take your loved ones to that rehab and would be well taken care of. You all helped my husband and myself and for that I want to thank you all from the bottom of my heart. God Bless you greatly.

My friend is currently at this particular facility. If I could have rated it any lower than the one star that was required to post this I would have without hesitation. While I have become very knowledgeable of the practices of facilities, such as this one, throughout Arizona, this one is one of the worst in terms of the methods that they implement to secure individuals as patients. They purposely avoid communications with those under their care as a means to not only hinder their departure, but also to manufacture an excuse to deem them as being non compliant if they even attempt to make the simplest of request or ask a question that makes too much sense. And heaven forbid if the individuals who are under their care understand their patients bill of rights. They will be subjected to the exact form of passive aggressive disrespect that is verfiably known to trigger a relapse. Which I believe completely, and consistently violates their mission statement. While I have made sure not to discuss my friends personal interactions in too much detail I will, however, suggest, plead, warn, to others who may consider using this facility to do your research and to PROPERLY vet the situation before you or your loved one find themselves in what might feel like a hostage situation. Using covert legal methods to insure that someone stays is beyond immoral, amongst other things. I dont hide. My picture, I believe, is with this post.....On Wed. 7/21/2021 my friend was finally released from this facility after I had to talk to them about how they got her to sign away her rights while she was not totally coherent. They had purposely drugged her the previous evening after our last phone call...I was supposed to pick her up upon her release time, as she had asked me to, as a means to prevent her from possibly hitting the streets to self medicate again. She was supposed to be submitted into another rehab facility or implement a very aggressive Intensive Out Patient treatment plan. But because the facility had failed in their bid at manipulating her to stay as a patient they released her back onto the street without any immediate support system. They can deny this if they choose to but it is the EXACT truth. She is now somewhere probably using fentynal, which is the most recent drug of her choice. I blame myself for initially going to the wrong facility, but I also acknowledge and understand the vile move that was pulled by Cornerstone Hospital. Just having her wait in the lobby, especially after I had just gotten off of the phone with a staff nurse, Diane, and their receptionist would have been the moral and proper thing to do....I had read that they could be pretty dishonest, but I actually now have first-hand knowledge of their tactics and practices......7/25/2021....This morning I discovered that the number that was provided by Cornerstone Specialty Hospital, included in their historical form response to a valid complaint, was nothing more than a source for a 3rd party answering service. That in itself speaks volumes. It has been my business experience that any entity which chooses to cloak itself within so many unnecessary layers is usually more than just suspect. In regards to my complaint or concerns I have chosen to forego the other unnecessary layer of contacting the state of Arizonas Atty. Generals office and instead file through the Dept. of Justice. Sure, this may somewhat lengthen the process, but the results will assuredly be better......Just an update....As life would have it my friend who I have been mentioning has landed back at Cornerstone after running herself ragged for 3 months after they released her without a support system...While I totally agree that my friend needs help, Cornerstone is not the place....After first trying to lie in regards to there being a signed R.O.I.form which allows for communication between patient, support person & staff they have already begun their gaslighting routine...They are beyond unethical....

This hospital should be avoided. There is a lack of leadership from the very top. The Nursing leadership is an absolute joke. And the chief executive officer knows it, yet does nothing about it. Hands down 5 stars goes to the RESPIRATORY department only at Cornerstone on Rosewood, specifically Kat and Robert the Wizard!. I was admitted to Cornerstone after spending 62 days at the VA in ICU following a dx of Covid...and had already been on a ventilator for 60 days. The goal from the beginning was to get me off the ventilator and trach...which started the day after I was admitted. 20 days later I was officially off my ventilator and trach!! I dont know what I would have done If it werent for the respiratory teams support, encouragement, and creative approach to treat me!! Another shout out to Lori at Cornerstone with PHYSICAL therapy...who never gave up on me, who encouraged me, and was there for me when I took my first steps! Today is officially 8 months since I was put on a ventilator, and there are simply not enough words to express my gratitude to those who were apart of my recovery to get me back home to my family.

This place may be good for rehabilitation, but by no means would I consider it an ICU. My dad had been at Banner in Tucson (a true ICU) for a few weeks before Banner let us know he would need to be transferred. We pushed back all we could as he was getting the care he needed at Banner, we were seeing improvements, and just didn’t feel Cornerstone could meet all of his needs. Unfortunately, he was moved. He was there for four days before he passed away. I just don’t believe they were set up with the level of care he needed. The nurses didn’t have nearly the level of knowledge I would expect from an ICU nurse (they have a much lower level of knowledge than all of the Banner ICU nurses we interacted with) and felt the same of the doctors. It was simply a much lower level of care than we needed and, because of that, I will always wonder what would’ve been had we not moved my dad.

My Dad died while in the care of Cornerstone on 1/22/21. Once he entered Cornerstone I never saw him again. Here are the details of our experience with Cornerstone at the end of my Dads life. My Dad was admitted to the Covid ICU at Northwest on 12/14/20. He was medically paralyzed and intubated on 12/16/20. Our family received daily updates, and more when needed. The nurses at Northwest connected us, via Facetime, to my Dads room daily. He was on the road to recovery. My Dad was moved to Cornerstone on 1/13/21. He was going to begin the process of coming out of sedation and rehabilitation. While in the care of Cornerstone his condition began to deteriorate. Cornerstone insisted that they only speak with one family member, and that only once a day. They never connected a Facetime for our family, despite daily requests. Every request was met with an excuse or unfulfilled promise to call us back. We filed complaints with CIHQ, the accrediting body for Cornerstone, CIHQ considered the matter resolved after a Cornerstone employee called to tell us our requests for daily Facetime were too difficult. Cornerstone denied my family all contact with my Dad until his last day. When his condition had gone past hopes of recovery, his wife was allowed to be with him while he passed. I dont know if my Dad lost his life due to Cornerstone incompetence, but we lost our last week with him because of it.

The experience was horrible. My sister was at Cornerstone L-TAC in Tucson, AZ for three weeks. She had been in a terrible bicycle accident that caused life-threatening injuries. She was in an ICU unit for four weeks and then transferred to Cornerstone for Long-term acute care. Her care was so below standard. For instance, she was left sitting on a bed pan and had to remove it herself, and the pan was left sitting on the side of the bed until a tech came to remove it, at the request of a visitor. She was never cleaned until hours later. I was supposed to receive daily updates from the nursing staff, which rarely happened and then only happened after sending multiple text messages to the admitting staff and the Chief Nursing Officer (CNO). I went to a meeting with my sister and staff a week and half after she was admitted to go over our concerns and found that my sister had not been cleaned until I made an issue out of her hands being covered in ink (she had to write to communicate) and dried feces on her thumb. One night nurse told me, We dont want to talk to you. You ask too many questions. The same night nurse had no idea why my sister was in Cornerstone because , according to her, I dont read patient charts, so I dont know why they are here. Another nurse said to me, I was told to give you an update. Tell me what you want to know so I can tell you what you need to hear. A third nurse told my sister to stop ringing the bell for the nurses. The weekend staff were useless. They did as little as possible to care for my sister. I complained throughout my sisters stay and very little, if anything, ever changed. I was told to call the CNO if we continued to experience problems and I did call him. He did not call me back. I did, however, get a call from the charge nurse and he was fantastic. This was an awful experience for my sister, myself and her visitor. If you are not an advocate for your family members care, who knows how he/she will be cared for.I gave this review ONE star and only because there were a few nurses, Charge nurse, and one or two techs, that were very helpful. Otherwise, I would not have given Cornerstone L-TAC any rating. The front desk staff was very friendly and helpful as well. Otherwise, the staff was far from good. For the most part, Cornerstones standard of care was far below standard. How do I know this, my sister was a nurse for over 40 years and as injured as she was, she was well aware of how poor the patient care was. Think long and hard before sending, or allowing, a loved one to end up at Cornerstone.

A loved one was a patient there and they were very negligent. Very nice staff, but they were understaffed. It would take them 20 minutes to answer the call button. They were happy to leave the patient with his oxygen turned up all day so they wouldn’t have to worry about him. They also kept him there as long as Medicare would pay even though he could’ve come home sooner. Not satisfied at all with the experience or the care that was received.

The nursing staff is professional, pleasant and caring. The office/administration staff are borderline incompetent. My father was a patient there for almost two months and his case manager gave us so much misinformation and made insensitive remarks about my dad that we called to complain about her. The director of nursing couldnt be bothered to return my call. The person in charge of getting admitting paperwork completed should be retrained or fired. My dad was admitted in early June and unfortunately he had several setbacks to the point that we were advised to stop all treatment and let him die. My mother and I sat there every day for almost a week watching him slowly pass. On August 1 when we entered the hospital we were given a stack of forms to complete. When we asked what the forms were for, we were told that they are the admitting paperwork. He was admitted June 12, and he died August 1. They were basically trying to cover their assess.

A few years ago, my friends father was a patient here, after recovering from surgery. He also suffered from dementia. Because of his mental cognizance wavering off and on, he wasnt always able to speak up when he needed changing, etc. On the 2nd day he was there, he had already developed IAD from sitting in soiled Depends for hours, and had started showing signs of bedsores. There seemed to be a shortage of staff, because I only saw one or two employees moving about, and this was during midday.I worked in a hospital at that time, and was familiar with the proper procedure of entering/exiting a room of a patient whois in isolation. I had noticed the room across from my friends dads was one of these iso rooms. On his 2nd day there, I witnessed a staff member enter that iso room without putting a mask. She did don a gown and gloves, but did not remove either upon exiting. She then tried to enter my friends fathers room across the hall, still wearing the contaminated gown and gloves. I refused her entry by physically blocking the doorway, and insisted she accompany me to the nursing station to speak to her boss. I dont know if anything became of my complaint, however, because the very next day we moved him to another facility (the one located behind St. Josephs Hospital). I am happy to report that the level of care provided at THAT facility was top-notch.
