I am very happy that we decided on having my partner move to Jacobs Hlth Care Cntr. The staff is very professional. They are aware of individual needs. They work closely with you to ensure that they meet the needs of individuals that are within their care. No place is perfect. But Ive found at Jacobs, communication is imperative in having to address the different issues that may come up, especially when you have so many different types of clientele/patients that they receive. Ive had no problem with the staff. Its all about finding solutions. Thank you very much.

The team at Jacobs has been phenomenal helping my wife recover. PT, OT, speech, nurses, sub acute section and SNF section all have been so proactive to help make my wife better. They are so dedicated, creative and keep patients and carer needs in mind while still being so so proactive.Would recommend anyone with someone coming out of ICU or a LTACH to go here for their next phase of recovery.

My Grandmother arrived on April 7, 2021, a hurried transfer from Arroyo Vista who had subpar service. From the beginning, with the tour, someone greeted us with a smile, the rules of visitation were clearly communicated. The spaces are clean and well lit, the energy is positive. Most important, my grandmother has been treated like a queen! She is our treasure so its nice to see the staff recognize her as such as well. Im looking forward to working with a supportive team to ensure my Grandmother recovers as quickly as possible. Thank you Jacob Healthcare staff! Special shout out to Claudia Barron, who has been an Angel helping us navigate the transition from Arroyo Vista.

All staff in Jacob Healthcare Assisted Living Facility, led by Administrator Amy J. and her Assistant Jacky I., are caring, compassionate, and considerate to all the residents. They always ensure the individual safety and physiological needs of each resident. They treated them as a family. The facility is clean and spacious.

In full transparency I have a management role in this facility; however, I have no reservation in saying that we have one of the strongest teams in the county. We do our best to put ourselves in the resident’s shoes to develop solutions that serve their needs. We are far from perfect but we hear staff and residents and resolve concerns as fast as possible. Staff are happy and I know that because we speak with them individually and the staff stay year after year. And as we know, happy staff give great care.

The staff people were helpful to me as a visually impaired person. I went there to visit a friend there for rehab. The place was very clean with no nasty odors at all. The staff were professional and friendly both to my friend and myself.

This facility is a nightmare. They do not answer the phones when you try to call, patient treatment is subpar at best. My father has been in this facility for approximatley 5 months and it has been a fight with them on almost a daily bases. He has been in pain and has trauma that is bleeding from a catheter . I have even spoken several times with Derek Orme the Director, I keep getting told they are doing this thing or another and nothing ever really gets done. Communication with the staff is almost non-existent and again what communication there is is just them telling you they are doing this or that and not doing anything in reality.We have my father here because they were recommended by University of California San Diego Hospital. I think maybe they should go in and talk to some of the patients if not all that are able to communicate and reevaluate this facility. If English is you first language you may want to learn Spanish or Filipino to be able to communicate with the staff on the floor.All I can say is I dont think the administration actually knows what is going on on the floors and just take the staff at their word and dont follow up on patient care. Only when you call (and that is if you can get them to answer ) repeatedly do you get any kind of answer and the its just to shut you up for the moment.If you can find another facility I would suggest it, but unfortunatley we do not have the kind of money that it would take to move my father to a better place.0 Stars for this facility.

I was contacted approximately one year ago by Amy, an administrator at Jacob Healthcare. She informed me of a resident who has used Mary Kay products for years. I will call her Ms. M. Her Mary Kay consultant moved out of state and Ms. M ran out of her Mary Kay products. After I met Ms. M, it was so apparent how important her skin care routine was to her. She will only use Mary Kay. She only uses three products of which she absolutely must have. She was so happy to meet me and I was so blessed to meet her. Although I no longer run my Mary Kay as a business, I have remained active to receive the products at a wholesale 50% off rate. Im happy to be able to pass my discount to Ms. M. and but my true happiness and blessings comes for the opportunity to continue my relationship with Ms. M. Thank you Amy! Your one phone call created a relationship that I will forever be grateful!

Staff hanging out not doing anything while a patient was yelling for help and being ignored. The smell is awful and to me thats a huge sign of a place that does not take care of its patients. I pray for the patients in there that have no voice and nobody to advocate for them. Im reporting this place to BBB. Do better for your patients people!!!

The staff are very pleasant and helpful. I am a home health nurse who sees a patient there. The staff are very knowledgeable on the condition of their residents. The atmosphere is pleasant with fish and artwork and volunteers meeting with residents.

Dr Collado is useless! I laid in bed for 3 months, broken legs, hip, vertebrae and pelvis. The day I began learning to walk again they were out of my pain pills that I hadn’t even been taking! This is day seven without pain pills because I’m waiting on Dr Collado to sign something. This is patient neglect! I’ve never even met him.

This is a wonderful place to bring your loved ones. Being in the healthcare industry myself I have had the opportunity to experience the attentiveness of each staff member at Jacobs, the place is clean, friendly, and theres always fun activities to keep the residents engaged and social. I would highly recommend doing a tour to appreciate all it has to offer.

I am so thankful my best friend was placed here for skilled nursing care. The staff are nothing short of amazing. Eugene the nurse is always very positive, kind, and uplifting. Asma and Samira the sisters who work together take their time and work as a team to ensure patient safety. You can feel the caring and compassion. Giselle the incredibly experienced and efficient CNA works hard, communicating with patients,letting them know I will be right back to take care of you. We are hoping for a miracle. All the healing energy, and faith filled people we have met are so appreciated. A million thanks to all who give 100 percent everyday.

Was going to just let my opinion drop. But after they reported me to Adult Protective Services, I will submit a review. Stay away from here if possible. Basic things - like getting a shower - is not always provided. If that is put on the back burner, you can only imagine what else is scuttled. No communication.

Was not prepared to accept me as a new patient because I arrived in the evening. Didnt have meds on hand for scheduled dosing, unable to perform wound care as needed. Loud with people yelling in the halls after 11 pm.Place is disgusting, like something you would see on the news. 90+ degrees in patient rooms but no air conditioning on and no fans. Everything old and run down. Same single wrist blood pressure monitor used on all patients, total lack of hygiene. Not even privacy curtains in what seems like a single room with extra bed crammed in a corner. Lobby looks nice enough but it is to cover for deplorable conditions within. Had to check out against medical advice because they could not provide me with scheduled and approved pain meds. Was threatened with the police being called. Had to have 911 called and returned to the hospital as Jacob Healthcare Center was completely ill-equipped to handle a single new patient. No supervisor, no doctors at night, just a single person to handle intake and any problems.Avoid at all costs. This place is a literal and figurative nightmare. If you love yourself or your loved ones save them from this place before you even consider leaving them there.

They failed to watch her.they walk around doing nothing it is a very dirty place trash is piled up high in the hallway trash on the floor.Also trash on bedroom floor. This is one of the worst healthcare clinic never come here.And if you do you love your family stay whith them all day

WARNING!!! WARNING!!! WARNING!!!Do not let your loved one into the HOUSE OF HORRORS that is Jacob Health Care Center. Pretty fish, nice patio with trees, nice chairs, nice piano. Ugly operations and Uncaring Administration driving Range Rovers and BMWs. Very short staffed, one evening only 1 cna per 13 patients. In the space of one week 2 elderly patients in one wing alone suffered falls due to no supervision and left alone and unattended. Another week, 3 more patient falls due to staff shortage, no such thing as personal care, luck of the draw if your loved one has regular diaper changes. On a rare occasion, 3 clinical staff members were present but they ignored an elderly gentleman who needed help getting to his room. Its a shame that underneath the somewhat nice exterior there is ugliness and money hungry management. Terrible Place. Ive never written a review before, but I would like to warn anyone who is looking for a rehab, place for their loved ones.1. VERY SLOW at responding to the emergency button located next to beds. over an hour most times.2. Scheduled medications are not followed. the patient has to press the call button to call for it, (if they are even cognizant) and have to remind nurses.3.Nurses are all either new or not informed or not capable/allowed or some other excuse. Point being, next prescribed medications are not ordered and so there is a 2 day gaps without NECESSARY MEDS. because of their lack of attention/4. My loved one was not showered/bathed and wounds were not seen to for OVER A WEEK! his staples and open wounds became infected and was only tended to when I begged nurses to give me the supplies to attend to him.5. Many of the employees will lie on file and say that they did physical therapy and showered patients when in REALITY it was not done. Aides claimed he was seen for Physical Therapy every day since he arrived when he had not been attended ONCE!6. The bedside commode was not cleaned regularly. and the room was not cleaned ONCE in the 2 weeks he was there,... trashcans were emptied regularly... cool.Luckily my family member was not alone, and mentally aware, otherwise Im sure he would have been basically abandoned and ignored as was the fate for the disabled people left on display in hallways...In conclusion, there is no structure, very little training and NO SUPERVISION resulting in little/poor care of patients.There are good people there! But not enough. A nurse, Darlene, delivered socks and body wash as gifts on Christmas Day ( FROM HER OWN MONEY!!!!!!) to all patients in her wing. Several employees shared with me that they sometimes purchase supplies from their own pocket (gloves, wet wipes) because the facility does not provide what is needed.Please dont bring your loved ones here, unless youd like to see them suffer.

Horrible. Ultimately Caused the death of my mother in law. Took over an hour to respond to her asking for help. She died because they took over an hour and a half to respond to her dying and claimed she died a quick death seriously. She was recoverable. She wasnt sent there to die. All she had to do was to get her breathing normal.

I am a current patient about to be kicked out to the streets and homelessness. Im 62, was a licensed for 23 years and have never been treated with so much disresr treated like worthless nobody in all my life!!! Ive been lied to and been ignored and treated as though I have no idea whats going on like an Alzheimer patient. I just had a talk about all this with the administrator and told her I was taking this to the Better Business bureau and my Ombudsman and in a cocky e she said go right ahead like she could care less.This place has been referred to as the House of Horrors, I dont believe that even tells half the story.Be very careful, please dont put your loved ones through through this ive seen and heard Seniors screaming for help with no response!!!Reverend J.M. My experience with the administrative staffI am only giving this one star because it is required to leave a post. I can not complain about the nursing staff other than what my father said which is that they do the bare minimum if that. Also the heavy stench of Human urine and feces that sometimes permeates the air can be a bit much.The issue that is of great concern is as follows:The incompetence and intemperate behavior demonstrated by the Director of Administration Mayleka Parker. Who ultimately has a profound affect on the stability and accountability of the entire staff and organization.Her complacency and reprehensible laxity have in my opinion absolutely no place holding this position. A simple task such as insuring theproper updating of a patients chart with the correct contact information seems to be a task to difficult. Which can lead to a potentially horrifying situation for other patients and family members, as it did ours!My wife, two young children and myself arrived for a visit to find the room and bed occupied by a stranger! Accompanied with blank stares from the nurse staff only to find out after having to ask two different nurses, that my father had been admitted to the hospital several days prior to my visit. I was shown the phone number on his chart that was being contacted I explained that the number had been changed and should have been updated on his file . The information was then updated for a second time now with the nurse and still managed not to make it to my fathers file as I later found out on a call with Maleka.Im so glad I found this piece of paper with your number on it Malekas exact words. Your father has been sent to another facility, He went for a visit and liked it so much that he didnt want to come back.I was told the name of the facility but was not given a phone number. I was told that the facility will be given my number and I will hear from someone. Several days passed and no call was received. After attempting multiple unsuccessful google searches trying to find contact information for this facility I again contacted Maleka and explained that I had not heard anything from the facility where my father was now staying I then requested the phone number. To which in a uncivil and mocking tone of voice Maleka responded whats your mailing address? along with a glut of other irrelevant questions. At that exact moment I felt as if my father was being held HOSTAGE somewhere and if I didnt comply than lI would never find him. I again asked for a contact number and was given an address and told again that I will get a call from the facility. I terminated the call and once again began the search for my father. I had a legitimate address but still no contact information.Several days passed again and still nothing.I then drove to the address that I had been given to find my father comfortable but distressed. He explained that he had been told that no one was able to locate me. I spoke with the nurse on site and was recited the number that was on the contact form. Low and behold the same incorrect number still had not been replaced. I was told that no other numbers were provided for them to contact me.I was given the number to the owner/operator and confirmed that no other contact information was provided. I was also informed that Maleka stated to him that she was filing an accusation of elderly abandonment.When I attempted to contact Maleka to verfiy the accuracy of the claim I was sent in what seemed like a whirlwind of evasiveness. Finally a vcml for Katie. I left a message since admin was gone after 5pm. I was then contacted by an ill mannered, repugnent, foul mouthed Maleka. I was threatened then had the police called on me with false accusations of threats and harassment. I spoke with the officer and was assured that the matter would be resolved accordingly.The audacity!!!Unfortunately my post is too long and will not post, but the level of buffoonery displayed by Maleka is inexcusable and in poor taste!

Im a Wound Care Nurse at JHCC. I love my job. The patients I see are phenomenal. Their cases become apart of my daily life. We struggle together, we heal together and that is what JHCC is all about. At least thats how I think.As a nurse I couldnt ask for a better company to work for.(they arent even paying me to write this)This place needs some updating and some new paint. Would love computerized charting and pagers for CNAs to shorten call light wait time.

Excellent place! I highly recommend it, the service is excellent, all the staff is very friendly, the facilities are very good. On two occasions I have had a family member there and they have always looked after him very well. I always recommend this place to anyone who needs it.
