This place leaves a lot to be desired. My grandmother was there for two weeks after having a heart attack and a stent put in. She lives with me and does not have 24 hour care or even someone home ten hours a day to sit with her. Her health is at the point that she cant do for herself like she used to. I expressed this to the social worker at Rex Rehab because I know the steps that have to be taken to transition her into long term care. The woman was rude and unhelpful, and told me my grandmother didnt want to go into long term care. Of course she doesnt. Who does? But when your safety is in jeopardy, it pretty much doesnt matter what you want. I had to pick her up and bring her back home - worrying every day for the past week if shes going to set the house on fire with her early onset dementia, or fall down the stairs since her room is on the second floor. Now, she says she cant even get up to go the ten steps to the bathroom. I told the social worker this would happen, and all she had was attitude. And to top it off, she told my grandmother that I just simply didnt want her living with me anymore.And of course while in rehab, my grandmother said her call bell went unanswered for hours at a time. She only had sink baths for two weeks and no hair washing at all. They are doing construction inside of the building and making all kinds of noise that is disturbing to patients. This place is a joke.

My mom was a rehab patient in this facility. Due to Covid, they have a litany of restrictions for visitation, which is understandable. There policy states that you must call 24 hours in advance of your visit. Ok. I call 24 hours prior to my visit. No one answers but I am prompted

Not only do they say theyre gonna give you a bath at 1pm and dont show up until 4pm. When I try to call so I can visit my grandmother nobody answers the phone. Is there anybody working there. Maybe if the owners would stop hoarding the millions upon millions they make and actually pay their staff a living wage they might actually have enough employees to do things the right way. This place is a joke

This is the worse place ever!!! We thought because Rex Hospital was such a nice facility that my mother would really get good care there. Boy were we wrong. There are so many things wrong that I dont know where to start. I guess the first thing is no doctor. They claim that there is one there, but I asked everyday to see her and out of 13 days I never did. Next they are so under staffed that they couldnt possibly take care of them all. They just kept receiving new patients daily but couldnt take care of the ones they already had. I stayed with my mother everyday for 13 days and still she didnt receive good care. The nurses just give out medicine and that is all they do. The nurses assistants are nowhere to be found and you never knew who it was. They are suppose to write the names up on the board but that was rarely done. I never knew who to ask for to come and assist. I am giving them one star because of the rehab people there. I guess they are the only ones there who do their jobs. I know Raleigh lacks in good rehab facilities which is surprising because it is such a big city, but I would do my research before going here. There is so many other things I could say, but I am trying to be nice. Also I want to mention that on the discharge papers they said that everyone did a great job and that daughter and patient were satisfied with the facility and staff. That is falsifying records. Watch out and beware!

My dad has been here three weeks. He goes home this coming week but I wanted to say my dad has gotten the best care. All the nurses and doctor and social workers have been attentive and nice. I will always say good things about the PT the OT and everyone. These people care and I am thankful my dad had great care for the three weeks hes been in their care. I will tell everyone how great it is.

My mom was here for two weeks. She had absolutely the best care. The physical therapist were great, my mom loved Joshua! He had so much patience and really helped her a lot. Everyone was so nice. She was not happy about all the turkey sausage, turkey bacon and turkey everything. But the care for sure out weighed that. Thanks for great care and getting my Mom stronger!

My mom stayed here for 2 weeks. I wouldnt want her to be anywhere else either here or homeThe nurses are great so are the aides and other staff. Excellent care.

I have been here for one day to rehabilitate after ankle surgery. While the PT and OT have been wonderful to work with, the rest of staying here is simply horrible due to missed medication schedules and defensive lies.After sitting awake in pain for three hours due to a late medication in the morning, the next medication was missed at one pm and after hunting for the nurse for hours in pain I was given a second dose at the time of the third, and then they said I could not have a next dose until midnight and I refuse to sit four hours in pain to cover up nurses missing schedules. I’ve had to call the complaint line now twice in one day.Then the first night they were standing by my bed and I woke up terrified in the middle of the night. They wanted to do invasive procedures totally not called for medically and never planned with a doctor . . . and in the middle of the night. I was terrified. It was more like a Hollywood nightmare movie. Procedures should be discussed with a physician and the patient given a chance to have alternatives and not shocked in terror in a dark night. I still have not met this doctor.Unlike other bad reviews I find the international staff delightful and have enjoyed meeting them.Last has been the terrible food. While making a good effort to accommodate a wheat allergy with a gluten free diet, there is only a bare bones list of a few plain foods. The actual food is served with plastic utensils on plastic take out containers. The food is standard prepackaged fare with over -cooked vegetables and meat like shoe leather, served cold although it is supposed to be a hot meal.FIVE DAYS LATER: I have come back after five days now to say the people here acted fast to fix some things. Medication schedules are okay now, and medications are correct at least. The food is not better - truly awful.I traced down the source of the myriad problems and it is actually handwritten information manually filed for notes. The place is not computerized - like back in the 1900s. No data software. Manual tracking of everything. Changes not entered manually on paper are not made and no records exist.MENU SELECTIONS: these were never entered and I was served food to which I was allergic. I was told “we don’t know your allergies, the staff that took down those menus were here yesterday and they went home, we do not know you.” This is manual data entry, if not written down, the next day staff does not know it. I caught the gluten filled items in time to avoid a severe allergic reaction but someone else may have been killed. I am very serious. I asked The head dietitian and she did not know what gluten intolerance was and there are almost no menu items I can eat. This is about lack of training. The main administrator apologized to me but this situation is a serious one and allergic shock can kill.The administrators who introduced themselves on day 3 were all white. The nurses you see only for medications. The assistants do most everything else and these people are like being in the UN, every nationality represented. People have been kind and well intentioned. This is not about the people. It is about the lack of accurate record keeping.BLOOD PRESSURE vitals are taken every shift. They use little scraps of paper to write the numbers down. If they do not manually enter them on paper notebook sheets, they come do it all again the next hour. One day it took three tries and still finally they came back the end of the shift to take blood pressure - no records had been entered. Data falsely recorded like this causes endless problems.I have documented all this so that these extreme problems can be addressed - bad data is bad data and it causes mistakes in so many aspects of a facility that it is sincerely unsafe here. Though administrators apologized, until this facility is computerized, you risk your life here.

Rex Rehab is a lovely and well run facility with very kind professionals.I have recently had a very close friend at Rex Rehab and was withher daily. She had no complaints during her stay to rehab from hip replacement after falling; therefore, the sudden nature of her havingto have surgery with no forewarning or planning added to her plight.She was totally dependent on assistance for several days and staffwas always there to help and very kind with many smiles. The roomwas spacious and clean.After reading some of the other reviews, it concerns me that theycould be from another facility due to the fact that the picture thatthis web site has posted is not from Rex Rehab and the rating shouldaverage should be higher than 2 1/2 . I would go there myself anytime I needed in-house rehabilitation.

My father spent a month here. The facility is definitely not new (it was clean!) and the food was not the greatest (especially compared to Rex Hospitals fantastic fare), but the staff more than made up for these shortcomings. They were responsive, helpful and always cheerful. A dramatic improvement compared with the horrible conditions and staff at Crabtree Rehab (now closed) from my fathers prior surgery recovery experience.

First, the location is on Rex way which is just off of Lake Boone Trail in between the main hospital entrance and the McDonalds Shopping Center. The address posted on Google Maps is the mailing address at Rex Hospital that the rehab facility is associated with. Once you turn onto Rex way it is the first building on the right however it is quite a ways down before there is anything on the right. The sign says Rex Rehab & Sleep Center but the Sleep Center has moved to another building, they just havent changed the sign yet.I have had two family members come here in the past year and after the first ones experience here, I insisted that the other one come here as well because they both received wonderful treatment. Im not sure if those who had very negative experiences were maybe in long-term rehab instead of short-term rehab as my family members were since they are different parts of the same facility but all of the nurses, medical staff, nursing assistants, etc were wonderful both times. I have also heard bad things about Rex rehab in Apex though Ive never been there, so to specify, this review is about Rex rehab in Raleigh.I would say I have about 3 weeks experience with this facility as I visited every day or every other day. The nutritionist made up a heart healthy option for the patient who had problems with her GI system and couldnt eat the amount of fat that would be in a regular diet, though out of the menu given to them patients can choose whatever they would like to eat for that day from a list of options.Neither patient has complained about the food being cold or late. And the quality of the food taste-wise has been agreeable not just satisfactory.When full mobility assistance was needed for anything that required getting out of bed, the patient only got a full-on shower twice a week as they needed a nurse to help them in the shower, however they were given the opportunity to have a in bath in bed bath given to them or done themselves every day. Occupational therapy and physical therapy would be scheduled the day before and the schedule for each patient written on the board in their room the night before so they could let any visitors know what time they would be busy getting therapy so we could plan our visits and each day the patient got a calendar with the days list of social activities.There are activities planned for every day, usually several activities each day like an ice cream social or a game time so that residents are not isolated and bored in their rooms. Each bed comes with its own TV and the option to use headphones to watch TV so you dont interrupt anyone else if you are in a double room and there are sufficient electrical plugs so that patients can keep a phone and tablet and another device all plugged in to their bed and have access to them from the bed without having to have them unplugged. I think on three different occasions some agency brought in therapy dogs to interact with the patients in their rooms if patients wanted to interact with the dogs and that was a highlight for both family members.There is a lovely little garden outside with paved walking trails for residence who are able to get around by themselves in a wheelchair or if they would like to go outside with their wheelchair or Walker with a visitor. There is even a fish pond and a small vegetable garden.

My Mom went there last year and called a few hours after we left. she was terribly mistreated! she also said she couldnt sleep because the staff was making so much noise in the hall like they were having a party. she was yelled out and ignored when trying to go to the bathroom. needless to say, we picked her up and got her out of there the minute she called. there is a lot more to the story but please dont take your loved one there... a lot of discrimination goes on as well... there were bugs on the floor. my Mom had a major brain surgery and they would not allow her to have her bed rails up. this is total bull. she just left ICU and it was mandatory her bed rails be up there. she would ring the bell and they would ignore her. so sad. they need to close this place down soon!

Horrible, horrible place...rude staff even some nurses to my surprize, call bells ring for hrs, nobody answering. Nurses not able to assess a pt. You gave to come and push for things to be done. Drs orders waiting for hrs (8hrs) to be specific b4 urgent IV could be started....we had a horrible experience!! Had to get out of there, who is the DON..here? Are they educating their staff??!!

Terrible placeYou can tell everyone hates their jobsEven fundamental nursing care is lacking.I agree with another poster--this place needs to be shut down. I had to ask for basic nursing tasks to be done, e.g. checking vital signs. These requests were met with hostility and sometimes even refusals, Theyll check it tomorrow..One nursing assistant was on her cell phone the whole time she was supposed to be helping a disabled patient in the shower.Other responses: Im not your nurse. Were changing shifts.

Dr. Zinner is inept, the facilities reek of feces and urine, and so few nurses have even the most basic grasp of English that communication becomes problematic at best, dangerous at worst. As a physician and loving son I was appalled. This place is abysmal. Do not send your loved ones here to die.

Awful from beginning. Unsanitary, got bad infection while there. Incompetent doctor, P.A. Doctor tried to order a medication that would be catrostropic if I hadnt caught it. Unsanitary, rude staff, wont change meds even with bad side effects. I had to ask the nurse what each pill was to find out that Dr. Zimmer still ordered it. Rex Rehab should indeed be shut down. Nursing care Is nonexistent for 95% of nurses. Food is worse than prison food and left to stand for hours and is unedible. Fire alarm went off in middle of night and no one was notified that it was a false alarm. Administration doesnt care about misbehavior. I informed administration about employee smelling like marijuana at 7am and admin did not do anything. Many more horror stories but not enough room. Nurses will lie to you for no reason and humiliate you. Again, dont go there. Expensive transportation.

This has not been a positive experience for my family member. Breakfast was served at 8:45 am. For a diabetic, this was rather late. My family member was not dressed until 10:40 am. Her bed was not made up at all even though she was out of the room for PT. Urine remained in the standing toilet from late morning until supper time . I cleaned out the urine container myself. This place is appalling. My father was sent to this facility to recover after knee surgery. He ended up getting worse. The people there are incompetent and as another reviewer put it, they clearly dont like their jobs. They staff is giving titles like nursing assistant but they are nothing more than bad maids at best. There is nothing medical about them. To give some examples, I came to find my father in the middle of a bed changing. The nursing assistant, more like prison orderly in my opinion, basically was trying to have my father hold himself while rolling onto his bad knee rather than calling for assistance. His gown was completely hanging off of him to where he was completely exposed and the nursing assistant didnt even care. I covered my father and asked her if she could be a little more mindful of his exposure for next time. Which I was met with a cross look. Another incident was in walking in the next day to visit with my father, I saw a pile of sheets that had blood on them sitting on a chair beside my fathers bed. I asked my father how long the sheets had been sitting there and he said for a couple of hours. I immediately called to have the dirty sheets removed. The next day I come in and ask my father how he is doing. He says he has been calling for help and no one has come by to help him. I took a look and found that the call button that they had plugged into the patient station was frayed and when I pressed the button no lights lit up. I immediately yanked the cable out of the patient station as I used to work with these and I knew it would throw an alarm. Sure enough, about 15 mins later a nursing assistant walks in to see what triggered the alarm. I explained to her about the bad call button to which her response was to tell me that she would have to see if they had any more and she didnt think that they did. So in the interim, my father could pull the cable out of the wall himself to try and trigger the alarm. Now, my father is doped up on pain meds and can barely summon the strength the sit up and hes supposed to somehow yank a cable from behind while in the bed and unable to turn around to see the station. I was in shock. So, I went and found an empty room and asked another nursing assistant if I could get the cable from that room. She didnt care, and so I did. I connected the cable back to the station and tested its functionality for myself. I really started to become seriously concerned about the care my father was receiving here so I took off a day and spent pretty much the whole day with my father. Not once did anyone come by and check on him on there own. He had to call for everything and then wait 15-30 mins before someone would show up. Now thats just the day care. As for the medical care, there doesnt appear to be any. The doctor only saw my father on the day of admittance and has not been back to check on him since. The nurse has also not been by to check on my father on any kind of regular daily schedule. They let my father it in a chair for a whole day which caused more swelling and when asked about why they are not putting the ice machine we had been giving by rex hospital for his swelling, we were told that he is supposed to request that. Now Im not sure, but I dont believe my father went to medical school and Im pretty sure hes not affiliated with medicine at all. So the very notion that he is somehow on his own suppose to determine when he needs cold compress and when he doesnt kind of baffled me. What made it completely jump the shark for me was that they were expecting him to be doing this while he is doped up on pain meds?!? They tell you not to drive a car when you are on pain meds. Yet somehow these ppl are asking my father to be his own doctor while on them??? It is crazy and I really believe this place needs to be shut down and anyone associated with this place no longer allowed to participate in medicine any capacity whatsoever. Up to and including a job just changing the bed sheets. Horrible experience!!! DO NOT GO!!!!

Have not been there yet, but can anyone explain why it shows the address as 4210 Lake Boone Trail but people who have been there tell me its a building on Rex Way? I can not find a nice map by Rex labeling all the buildings, so every person who is a first-time visitor or patient has to bumble around and waste time searching for office buildings because somebody is too lazy to make one clear map. Thanks.
