I had an appointment May 25th solely to get a refill on my medication. The refill was never sent. It is now July 15th. I called a week ago to fix this problem. I was told my prescription would be “high priority” for the doctor. I went on vacation so I could not pick it up from the pharmacy that day. I went to pick it up yesterday and the pharmacy had never received the fax. So I called AGAIN. I spoke to several people saying they would make sure the prescription was faxed. So I called today to see if my prescription was ready. NOPE. It had never been faxed. So the person I spoke with today said she would personally speak to the MD and they would call be back to let me know it was faxed. I was never called back. I am now completely out of my medication. I called the pharmacy to see if it was faxed, and to no surprise, it was not. This is not okay. This is not good patient care.

I am upset with Hazel Ann David practice for a long time been wanting to change I finally did it I broke my right shoulder in April on the weekend called her office to let her know the office help had no response or record of it until my sister came down to help me. We passed her office we went in there no concern thought she would get me going on Physical therapy on the paper work it took over 3 weeks and to one week to see the surgeon I was in pain for 2 and half months and when I need my medication I cant get it due to the pharmacy is trying to contact her

I am very happy with Dr. Hazel Ann David. She listens and is thorough with her examination. I feel she really cares about my well being and takes the time to educate me regarding my symptoms and what to do to get and feel better as soon as possible. I recently moved about 20 miles north of her practice but Im keeping her as my primary physician because she is worth the drive. I highly recommend Dr. Hazel Ann David.

I had an appointment schedule for 4/22/19 at 9AM. Which was scheduled several months out. There shouldn’t be any reason why I should be waiting 45 minutes to see a doctor. I told them I needed to be out the office by 9:45AM because I needed to get to work by 10:30AM. I ended up leaving the office without being seen by Dr. David because of the horrible wait time. What’s the point of scheduling an appointment and not being seen at your schedule time or even 15mins after arriving. This is ridiculous. I would NEVER return to this office.

Dr. David is a wonderful physician that I would recommend to anyone. She takes her time and is very patient. She will address all your concerns one at a time, never making you feel like she is in a rush to see the next patient, or like your concerns are petty. And she is not afraid to say she needs to do a some research on something and get back to you. Plus, her staff is very caring and sweet. If you want a doctor that is a real honest person Dr. Hazel is for you.

Dr. Hazel Ann David is thorough and one of the best doctors in Orlando. The proof is in the improvement of my health.I like that she tells me the consequences and gives me options. She believes in preventative care; for this reason, I tolerate office staff.4 stars were given because of several staff. Tina who sets appointments and assist with patients is difficult at times and hard to deal with over the years I have made many complaints and patients have left because of her attitude. I am tired of the rollercoaster ride.Lizzette is a staff member that refuse to put on gloves when dealing with me. I have asked several times for her to wear gloves. It should be mandatory to wash your hands in front of a patient, due to Covid-19 prior to touching them.The office staff makes and breaks the office. How many complaints does it take for change to come? Everyone is aware this isn’t an isolated problem.

My family has been visiting Dr. David for over a decade. We are all extremely happy with the office, the service provided and our over all experience as well. She is very kind, cares about her patients and will take the time out to listen to all your concerns and help you in every way she possibly can. Wait times are not long, the staff is professional and respectful and they truly do care.

Dr. Hazel takes her time with patients, and addresses all their concerns. She spent nearly half hour with me and helped me through all the concerns I had. She is a dedicated Dr. and a wonderful one too! I would recommend her time and time again :-)

Dr. David is wonderful and is really on top of my health issues. Im happy to be a patient of hers. The staff is also as wonderful as the doctor.

I can not believe this doctor. I went in for a physical earlier in March and they billed my insurance over 300.00. I then go in for test results which only took about 15 minutes and she billed the insurance for 50 minutes and another 400.00. She even referred me to a cancer center for them to find absolutely nothing wrong with me and for them to bill my insurance hundreds and hundreds of dollars. I swear this is highway robbery and it has to stop

She so down to earth! She will take her time to talk and examine you. She not an abracadabra MD

Dr. David is very much concern about your health and she even took the time to call me back rather than waiting until Monday morning for information I wanted to know...she doesnt rush through your exam/visit. I love her and recommend her fully...never in my life have I had such wonderful experience with my Primary Care Physician before...

No one came to the front desk for 15 minutes. The nurse left the door wide open when I was in the gown. She made me change back in to my clothes in front of her, rather than stepping out of the room. The doctor gave me misinformation about my BMI and EKG results. Then they refused to give me blood test results, breaking HIPPA. You can only get test results by going in and talking to the doctor. In other words only by paying them again. I have been to other Physician Associates doctors and was satisfied. However, I would not recommend this doctor to anyone.

Been going here at least 10 years or more the staff is horrible I need a refill on a asthma inhaler that I need daily but instead of putting in a refill for me over the phone to my pharmacy they make me come in. I need this medicine ASAP and they want me to wait 10 days for a appointment. Do they not care at all about their patients? Do they not understand I need this medicine DAILY. Horrible staff.
