This should be a zero star faciity. There is not enough space to detail all of the mistakes and horrific care my father received here. Their memory unit does not have capable nor skilled staff and should not be allowed to operate until they are. While my father was on hospice and dying, there was very little care or compassion shown to him or our family. I would strongly advise you not to allow your loved one to go to the facility. The photos I have would not be appropriate to put on the internet.

My mother was at liberty for almost 2 years. For the last year I was trying to get her moved as I wasnt being heard by the doctor there or the DON. The DON wasnt letting me move my mother and it had taken me a while to realize this. When I called him out on it, he immediately quit after, I was told. They kept my mother on Ativan which was making her very paranoid and suicidal. I asked for them to take her off of it after one of the nurses said they would use Ativan to sedate her. I didnt think this was the way to go when it was altering my mothers mind so much. I asked them to stop giving her the pills until i could talk to a doctor but they instead increased the meds. When the DON was gone I finally was able to move my mother. A nurse at the other facility told me my mother wasnt acting right and they wanted to decrease to med. I believe it was too late and Liberty keeping my mother and overdosing her led to her death. The other facility had her in isolation and when I received her belongings that wasnt unpacked yet, her belongings were stained and her wallet that had a couple rings in it was missing. Now, they want to charge me when court papers say Im not financially responsible for her, just her guardian. Please demand to check on your loved ones here. I was too late.

Liberty Nursing Center of Lima is a beautifully decorated and clean facility! The staff is friendly and helpful! So many beautiful rooms and spaces. The amenities include, a salon, library, therapy gym, and multiple inclosed patios!!

Liberty is a great place to work, with an amazing environment for residents. We are working on may community outreach programs, such as the adopt a grandparent program with Lima Senior, community training in partnerships with local agencies, and many more opportunities arising. We take pride in making sure that Liberty is an environment where residents feel loved and the community feels welcomed.

A lot of the reviews posted here are at least one, if not two, years old. The past is in the past. Liberty Retirement Community of Lima is now under brand new ownership, with a nearly entirely new senior management staff. The new staff is devoted to providing outstanding care for all of the residents and their families. Please, take a moment and look at when the reviews were posted.

One of the best-paying places for a nurse or aid to work. The new owners have really done a lot to make it a great place to work and take care of people. I worked for the old owners and the new owners. What a change for the better!

I want to note that I myself have never stepped foot inside this facility, though through my own experiences of hearing things myself said by staff, and being told by others family and more who experienced it first hand, I came up with the following review and my opinion of this facility. While Michael Johnson, reached me in order to get it removed or altered I have decided to leave the below as my final review after speaking further with family and reviewing the facts.My Best Friends Mother went here as a hospice option, and her going here has been living in hell on earth for her and her family. This may sound extreme but those who add drama to those going through so much just to make the experience worse can only be described as so. They treated the family member with little to no compassion by the aid staff. They called her names, refused to transport by wheelchair (When even ordered to do so by doctors) and other things I cannot verify as factual so wont place here. Hospice was supposed to be working in tandem with this facility to help support her care but it seems that not only did they not work in cohesion, but seemed to push the workload off to hospice when they were present rather then providing extra care. This was witnessed by family members and expressed by the one in care herself. According to Michael Johnson, The director of admissions and marketing, the State of Ohio investigated allegations of abuse and neglect and nothing was found, however we all know the state cant charge anyone without direct proof. Its near impossible to prove verbal abuse, refusing transportation when wanting a smoke, and really just overall basic human compassion. I will note that abuse was found prior to the investigation against said family member, and 2 of the staff were reportedly fired, however no indications or press releases were performed to apologize to not only the family but also the general public for the egregious behavior. While I hope this was a 1 time occurrence, and that the others in this facility are not under this type of care, the fact that it happened once, is still one to many.I hope this review serves to advise all families to be extra vigilant when choosing end of life care with any facility, and to always trust your gut and to do your homework.I do commend Michael Johnson for reaching out to me on my original review, and I know this may not be the outcome he was looking for, but I must hold any and all accountable for actions that occurred. It is Michaels Job to assure that this company is successful, and while he did his due diligence in some parts, and some areas were out of his direct control, its ultimately up to the company as a whole to provide the 100% error free care we expect for our loved ones. It would go a long way to help ease my discomfort of this facility if the company wishes to make a public apology to the family and general public in written form.

The new owners and managers really care about the patients. We had a very small problem, and it was taken care of that day. The head cook even comes to everybodys room to ask them what kind of food they like I will definitely go there again ... hopefully I wont have to. :)

With the new ownership and all new staff, the place is really turning the corner. It is becoming a great place to have a loved-one stay for rehab or long-term. I would highly recommend.

Not what I expected after talking several times with head of admissions. Positives place is nice and clean. Negatives, during Thanksgiving holiday very short staffed. Several times Meals were delivered was ice cold and had to go to nursing station to have it warmed up in microwave. Night nurses are 5x better than Day staff with exception of head nurse. Didnt know we had to bring our own wheel chair, cmon man. My father had made arrangements to have my moms hair done and was told we could use the salon here. That would be great if someone that worked here actually had a key to the place. I tried for over 2 hours to get a key and was told when head nurse got back she would unlock it. Head person got back and no key either. So after borrowing a wheelchair from another resident we had to stand my very weak mom over the sink to wash her hair. And what makes me madder is the crew at nurses station just didnt give a dang. Not one person asked if we needed assistance. Also I ended up making coffee at home and buying a carafe to bring it to the room since you cant even get any here after dinner even though there is a huge coffee pot but no coffee or filters. When I asked for some around 8pm I got a shrug of shoulders. As a business owner who believes in customer service I hope the powers to be actually take notice of this review. Ill be doing another one in near future.

My family member passed away here. She was found WITHOUT bedsores or rash. She appeared comfortable in her final 24 hours of life, by her CNA daughter. HOWEVER, she was verbally abused there. BUT, the abusers were terminated, immediately. Furthermore, there were residents rights violations in question, pertaining to safety ,dignity, and the quality of life with Libertys refusal to assist residents by wheelchair, when they go out doors to smoke. A hospice patient, with a doctors order for assistance by wheelchair, including outdoors, to smoke was refused by Liberty employees. Allegations were made against the DON for refusing that care directly to the residents hospice nurse. Lastly, we believe that she was not given medication on a timely bases, when ordered by hospice to be routine. These cases are to be investigated by the Ohio Attorney General.

I have worked with the owner of Liberty for years. She is very professional as well as a very caring person. I would recommend Liberty Nursing Centers of Lima to my friends and family.

I wouldnt go anywhere else for therapy after my knee replacement surgery. Even after I went home, they called just to check on me. The new owners have completely turned it around.

Buster Rozier Jr. received substandard care. The majority of times Buster had to be hospitalized were due to infections which were prevalent. Of course, they never wanted to admit their incompetence. I even had to hire someone to stay with him at night to make sure Busters needs were met.The message I want everyone to hear is as follows: Beware of Baton Rouge Health Services Community! If you are looking for a nursing home for your loved one, think twice before considering Baton Rouge. Aesthetically it is beautiful, but that is the only positive statement that can be made about the facility.

Baton Rouge Health Services is a very well designed facility inside and out. I am sure millions of dollars were used for the interior and exterior but THEY FORGOT they were suppose to deliver healthcare to people. People who may need help because they may be old, sick, and/or disabled.OOPS!! This review is suppose to be about a healthcare experience. I CANNOT SAY ANYTHING POSITIVE ON THIS SITE that would reflect an overall positive experience that my son, Dan, had at this facility.He was in the hospital often for acquired preventable infections. Do your homework and investigate diligently when searching for a nursing home.
