My wife admitted to San Jose on 3/31 and I have gotten the best sleep possible since her illness started in January. After not being able to see her for weeks the facility welcomed us in with open arms. The staff had on mask and the facility was clean. They explained the scheduled visitation to keep residents safe but allowed me to make sure my wife got settled. I’ve called the facility multiple times to check on her and she continues to say everyone is so nice. Thank You San Jose for making me feel confident that my wife is well taken care of and safe!

I have been to alot of facilities since 2014 when I broke my leg and it healed wrong .....but this is the best one Ive ever been to and Ive been to Palm Gardens In Winter haven which is a wonderful place in 2015 but this place is so amazingly friendly and I want everyone to know that ! If you need something they go out of there way to help you and the best part is they make you feel like we are all one big family I love them 💗

My dad has been @ San Jose for 2 years and loves it! They have taken excellent care of him and provided his every need. I’m very grateful for the outstanding staff!!

I am so glad I made the decision to bring my mom here! About 4 years ago my dad was here and it was horrible. When I heard they had new management I decided to give it a try for my mom. They have not disappointed! The ED and DON as well as the floor staff are always responsive to my questions and my mom’s needs. They take very good care of her and her wounds have done better than they ever have in any other facility. Please don’t believe the bad reviews you see, they are simply not true. And even though they aren’t perfect if there is and issue I have been able to speak with upper management even on the weekends and my concerns are immediately addressed and resolved. They are the definition of Healthcare Heros!

DO NOT LET YOUR LOVE ONE GO HERE.My mother is current a patient here. She arrived today.Hasn’t gotten her heart meds. She was suppose to get at 7 pm it is currently 10:12pm March 30th 2021. She hasn’t gotten her breathing treatments she was suppose to get at 4 pm and 8pm. While I’m on the phone with her. A staff member came in and told her she will gets her meds someone time tonight. Her heart feels like it’s jumping out her chest. No staff member is answering the call light since she got there. I Have called 15 times and no answer. I already called the complain line. She said she about to call 911 for her self.

San Jose is simply a great place! They are wonderful to their residents and have handled the COVID situation as best as they could. I would definitely recommend this place for your loved ones.

I hope this review helps in your search for a nice Health and Rehabilitation facility for your loved one during lifes difficult times.After extensive research and personal visits we chose San Jose Health and Rehabilitation for my wifes brother who is suffering from a terminal brain tumor. He was admitted under a Medicaid Hospice plan. Without the assistance of the entire staff it would have been much more difficult. From day one they took over the process of qualification for Medicaid and other resources. They arranged transportation via ambulance from the hospital to the facility.The day he arrived we were overwhelmed by the compassionate and professional staff. He had a nice clean room with all the creature comforts of home. We stayed for a few days while he acclimated himself to his surroundings. We live in Naples so we had to trust he would be taken care of.Fast forward...that was 8 months ago, We get up to visit him about every three to four weeks and speak with him and his care providers often via phone. Never have we felt out of the loop or ignored. The facility has much to offer, lots of activities, movies and other social activities. The big plus is that the food is actually quite good.Heres why Im writing this review....the staff, the Executive Director, Director of Nursing and the unbelievably caring nursing staff. Even the housekeeping members are honestly good, decent, compassionate people. This is not your typical Medicare, Medicaid warehouse. This is a facility that runs smoothly, efficiently and with empathy for the patient and family members and like I said staffed 24/7 with folks that I am happy to call friends now. Again his nursing and care team are unbelievable, the supervisor at his nursing station is a saint. I could go on and on.In closing, if you are in need of this type of service dont pass this facility over. They provide an invaluable service and keep in mind its a learning experience so dont be afraid to ask questions, theyll be more than happy to help.A big thank you from our family to theirs.

My aunt was recently here and it was the worst experience ever. When first getting there it took 3 hours before anyone came to check on her. While talking on the phone with her one evening she called 5 times for someone to come help her to the bathroom and no one came so I called the front desk oh were going right now was the response. Only to still NOT come help her until an hour later after she had wet on herself. DO NOT TAKE YOUR LOVED ONE HERE!!

They will be reported to Medicare! Absolutely the worst! Do not let your family members go there!

This facility has gone far above and beyond for my brother in law that is living there now. The Admissions person was so helpful. The Administrator (Mrs. Cribb) is so sweet and makes you feel so welcomed in the facility. They take their time with you explaining everything. I am very impressed with all of the administrative people. The nursing staff has been nice to my brother in law and gets him what he needs. He does not have any complaints. Thank you so much!

I give San Jose 5 stars because it is a great place to work. Our staff and employees work as a team, we have a great support system. Out staff are kind and excellent in assisting our residen. We r family with a big ❤.. Coming To work at San Jose is a joy and a privilege. We strive on compassion, integrity, passion, and honestly and the most of all the golden rule of thumb. This place moved my father with out his or my mother’s permission to a facility of their choice about 2 weeks ago. My mother talked to Shonda/Director of the facility and they completely took away my family’s rights disregarding our wishes. The social worker told my mother they were moving him because we filed grievances. On a wed the facility social worker told my mom my father was out and by that Friday a new facility called stating they had him. They through my father without legal just case. They should NOT have a license to care for families love ones in my opinion. Their actions and some attitudes are just disrespectful and disguising.

Recently visited my granny whom is a resident here. All I can say is « wow. » The place has changed for the better and in such a short period of time. It looks better, smells better and staff is a lot more responsive than I remember. Although, there is still a lot of room for improvement, I am impressed.

San Jose is the most beautiful place and also I enjoy working for San Jose the team here at San Jose is very excellent they make sure that the patients are kept well and they also just go about caring and loving and San Jose is a private setting and San Jose is the most wonderful place to work is also a wonderful place to live Im so blessed to be at San Jose and Im so blessed to be able to work with the patience as well as our staff members and I rate San Jose as number one because I enjoy my job and I love the people here. thank u! Administration plays favoritism. Director of nursing is very shady. Management back stabs each other constantly. The state and DCF is always in there on issues and complaints. Toxic place to work. Stay clear!

I would never let my loved ones ever stay here. They do not provide filtered water or proper care for their residents. Medications were not passed out at proper times. Bathrooms were never thoroughly cleaned, where there was feces present on the side of the toilet prior to my loved ones arrival. The bathroom is also where a nurse provided more water for my loved one as well. Some employees you could tell actually cared and provided decent care but I wouldnt take a chance here ever again.

My grandmother has terminal cancer and is under hospice care. She left a bad situation at a nursing home in Orlando and we brought her up to Jacksonville to spend her last days closer family. She is thriving at San Jose. She went from refusing meals and tube feedings to feeding herself with assistance from staff. Its amazing what a little TLC can do! My family is so thankful for San Jose H&R!!

While visiting my mom 2 employees were yelling and arguing right in front of everyone. Made us feel extremely uncomfortable. The lady even used the N word. Both employees were black but still rude to yell like that in front of us.

The nurses DO NOTHING BUT TALK TO EACH OTHER . Im going to call corporate on them because they getting paid to do nothing but DISRESPECT SMH

I am a patient here. The care is very lacking. Meds are frequently late and a lot of times there meds missing because no one ordered refills or the pharmacy never sent them. It can an hour or so before someone answers your call light. Meals are delivered late, cold, and not what is supposed be served. Some times just sandwiches instead of the regular entrees. I am still rarely given a bath, though I am suppose to get one 3 times a week. Some employees are great, but it is usually workers from employment agency who give the best care. They are on the 3rd management change since I have been here and it still has made little to no difference for me.

Horrible food. Slow call light response. Had to wait for pain meds.

This place is dirty and shameful. I have never seen a place where the nurses and staff are so blind to the uncleanliness in the individuals rooms. My grandmother is in this facility and the floors in her room have some sort of throw up on them it is very sad for me to have to see.

Please do not trust this facility with those you love. We recently removed my mother-in-law from this facility. We found her 3 or 4 times drenched in urine. She was attacked by a CNA and had a horrible bruise where she was hit in the eye and on the chin. A week later, she fell out of her bed and they didnt even call an ambulance! Thank God she was okay and thank God we found a better place for her!

I dont write reviews but this one warrants my time and attention. WARNING WARNING WARNING, the facility, staff, and executive director, Lordins Geffrard is absolutely POOR. It is beyond poor for the following reasons.1. My mom was admitted on 2/15/18 at 5:30pm and didnt receive her time-sensitive medication until the following day 2/16/18 at 6:15pm therefore missing 3 doses of antibiotics and 2 doses of seizure medications. She also missed an antibiotic and her blood pressure meds.2. She did not receive care after 11:00pm during the night shift. Therefore she was not changed by the staff all night, nor was turned in the bed as she has wounds and need to be turned every 2 hours.3. She was given a medication on 2/16/18 that we previously refused to possible allergic precautions. The night nurse attempted to give it to her again but I was there to refuse it. If I wasnt there the nurse would have given it to my mom and who knows what could possibly happen.4. They do not take proper sanitary precautions. The reuse the same blood pressure cuff several patients with out sanitizing it .They do not have an inventory of caps for catheters therefore they are using the same caps which can lead to infections. (I have pictures). The 2 days we were there we did not receive housekeeping care. The trash was backed up, floor never mopped, nothing wiped down to ensure a sanitized environment.5. The executive director Lordins Geffrard was very rude. He reprimanded an employee right in front of me which made her cry and he began to raise his voice at me about asking for more toast for my mother.6. The average wait time for a response to a call button is 20 mins. I took a log of every time we made a call.7. After I witnessed abuse and negligence I was not leaving my mom and was adamant about staying overnight to ensure proper care. I was refused the opportunity to continue to observe care because I was calling them in improper care. I was forced to leave. Well of course I did not leave my mom. We requested discharge.8. He never provide a written policy on visitation or visiting hours after I requested it multiple times since he was trying to force me to leave.When you tour the facility of course you will not be privy to to the negligence, the abuse, lack of proper care, or rude staff, but that is what this review is for. Dont just take my word. Check their ratings .It speaks for itself. Its just too bad we got caught up. Hopefully this helps someone else.

No stars. They are hugely irresponsible and negligent. My mother was attacked there twice, and they did not deliver the appropriate treatment. Horrendous. And now they wont respond to my phone calls. I have been transferred to medical records and now not anyone is answering and I have been on the phone for well over a half hour and not anyone is answering.
