Please DO NOT put your loved one here for care. This place is awful. If I could give zero stars, I would. The staff barely communicates with you and when they do, they do not follow through with anything. It was clear my father was not taken care of while in their care. He was there a little over a week for transitional care and declined rapidly. They kept telling me he was declining 50% of his services because he just didnt want to do them, which after finally seeing him upon being re-admitted to the hospital by his cardiologist, I could see why he could not participate in therapy! The edema in his legs was well controlled by oral medication leaving the hospital. They allowed the edema to get so bad, that the top layer of skin on his foot peeled away when he removed his sock. He could barely walk because his feet and legs were so heavy from the water weight. The rehab director tried to tell me it was because it could be from not getting up to move around. FALSE! Now that he is back in the hospital his edema using oral medication is under control and guess what...he hasnt done much walking! Makes me question if he was even getting his meds correctly while there or why they would have not adjusted his dose. Not only that, the top layer of his skin in his buttock area was so raw that every time he moved the undergarment would catch on his skin and cause immense pain. The last straw was when I specifically requested the nursing staff to ensure his hygiene would be taken care of and in a clean wardrobe for his doctor appointment and upon meeting him to transport him to an appointment, his pants where damp and smelt like old urine. The smell was so foul, I almost threw up. This was not an accident that just occurred as his nurse tried to tell me. I made the nurse get me fresh clothing and upon her return she told me I needed to sign a piece of paper stating if my father needed care past a certain date, we would need to pay the fee since Medicare would not cover any longer due to his non-compliance of therapy. Thank the lord his doctor saw what was going on and would not allow him to return. If anyone employed here reads this..WAKE UP! These are people going thru a very difficult time. Be humane and have empathy. My father may not have been the easiest to attend to, but he had just had a third heart attack, already in heart failure and confused. You treated him poorly and recorded it as if he caused it all to himself and there was nothing medically stopping him from therapy.

My grandma LOVES her new physical therapist!! She is newer at the facility but you would never guess it!! She is seriously the sweetest, so patient, and very caring. I feel so happy knowing my loved one is safe and cared for here!! All the staff are super kind!! It is not like those bigger facilities...my grandma gets one on one attention and I feel so great knowing her needs are met!!

The personal cares of my grandma were neglected here! Being short staffed is no excuse when it comes to giving care! Not only was her grooming neglected, but she lost 15lbs during her stay. Thankfully, she is back home now. Think twice about Woodlyn Heights!

I am not going to give this place any stars. very unprofessional from management to staff. Always under staff. Staff do not like to help each other therefore residents go hours waiting to be changed or any help. residents have to wait for lights to be answered due to staff hiding, on the phone or gossiping or out smoking. I wouldnt recommend anybody to work or put their loved one in this location at all. apparently i have to click a star to post. very disappointed in this place and how it is run but i guess its what you get when you have unprofessional running it and click.

Certain to get my husband, 63 year old Vet out ASAP. Would never want any one I love or know to be subject to the Staff or so called cares. This place has a very low star rating for big reasons.

If you have to leave a loved one in this facility watch over them very intensely!! Pain meds are not given on time, physical therapy is pushy and unprofessional (uncaring of Residents pain level). Lack of attention to get timely pain meds are delayed causing more discomfort.

Megan, charge nurse, actually told my Dad when he went to get a nurse to help his wife go to the bathroom, I told you to go to the room and stay there . All the demonstrations that several care givers gave at the capital to respect the elderly just set us back. Megan, please adjust your thinking to care for all elderly
