I am blessed to be in the role of Marketing and Admissions Director at Hamilton Trace. Of course, I have the role of telling everyone how wonderful our building is, but heres the truth. Hamilton Trace is a team of qualified, caring staff and friendly people. Throughout the pandemic we have had our challenges, but people have worked to maintain an optimism and support for each other that is admirable. Every place has its faults, we have struggles, we arent perfect. But we work hard to be certain that residents are well cared for, families are included in the process and staff know that they are appreciated. I honestly feel that Hamilton Trace is the best option when you cant go home.

Read the reviews, the good and the bad. Tour the facility and talk to residents and ask how their care is. But, here is our experience for the three weeks our loved one was there.What a wonderful staff this place has. My mother in law was treated with kindness and respect. She was sick, having dementia and organs shutting down.. extreme pain and very confused and angry. She wasn’t kind. She was delirious. We could hear the staff in the room being so sweet and kind to her. They were so patient with her. Gave her dignity, even when it was difficult to care for her. She even had a mattress slowly inflating in different areas to keep her from getting bed sores. They worked with our hospice team as well and kept in great communication with us. At least one staff member gave us her cell phone and told us to call her, day or night, even if she wasn’t working. Who even does that anymore?! That’s well above and beyond. My mother in law passed last night and is finally done suffering. Free and at peace. The staff and Beth Timko showed such grace and kindness to us, and have helped us coordinate collecting her things with no pressure to be out quickly.They accept Medicaid, and even the shared rooms are actually a small suite with two dry walled rooms dividing the two patients sharing the suite. Much more comfortable and safer during Covid as well. Even the art on the walls show beautiful nature photographs, very serene. They have a hair salon as well 😍If you love your person and want to be sure they’re loved and cared for as well as possible, don’t bother shopping around. You won’t be sorry you chose Hamilton Trace.

My father is a resident at Hamilton Trace in Fishers, Ind.The quarantine has been so very hard for everyone, but especially for our elderly loved ones. The staff at Hamilton Trace have just been incredible. He has hearing aids and is still nearly deaf so a simple phone call doesn’t work for us. They set it up so that he and I could FaceTime one another. They have done this for us twice a week throughout the quarantine. The facility now is allowing in person visits so that is SO awesome.Everyone has been so helpful. We got him a large screen TV for Christmas and the maintenance man put it up immediately for him. They have kept me updated on any change. The facility is beautiful and incredibly well maintained. None of us wish our loved ones to be in a nursing home, but if you are faced with this decision you can’t do better than Hamilton Trace in Fishers!

Our experience at Hamilton Trace has been amazing. Our 93 year old sweetheart moved in during the pandemic and was treated with such care, love and respect. Safety measures were just as I would have expected. I never doubted for a minute he would be well taken care of during this new phase of life.

I love this company and facility. I have worked here over 5 years now. Previously worked at hospital, Dr. Office, 21 years and 5 years respectively for other long term care companies. I do not struggle for resources to do my job. I feel supported and cared for as a professional in Healthcare. I enjoy my workplace and coworkers.

I have worked for Hamilton Trace for 10 years. As a nurse of 30 years I can honestly say that I feel blessed to work here. I work in an office with 2 other nurses who are both ex-director of nurses. Their wealth of knowledge regarding health conditions comes in very handy. During my time with this company theyhave always encouraged me to expand my education. They have paid for me to become a CPR instructor, TB instructor and obtain an infection control certificate. Currently they are offering training courses for borh nurses and aids. Once these training courses are completed and successfully passed you will be rewarded wirh wage increase. Hamilton Trace also believes in promoting from within. I feel very appreciated in my job and would recommend our facility to anyone looking for a new career.

I performed my clinicals for CNA training here, and every time I walked into that building I was appalled. There were a few nurses that knew what they were doing, but most of them hardly cared for the residents!! The residents would always be so happy to see us when we were there for clinicals, and it’s probably because the actual staff doesn’t care for them! They get yelled at, left alone to sit in the hallway for hours, and overall completely ignored. It was almost impossible to even get a nurse to properly show me how to perform skills. As a person who wants to excel in the healthcare field, I still cannot comprehend what I experienced at this nursing home. I still worry about those residents everyday, and I finished my clinicals over a year ago. Please, never put your loved one in this facility. No matter what review you read, it is overall an awful environment riddled with neglect.

This place is not a place you want to put your family member. My fathers first visit here, the nurses were great at communicating and calling back. But the Doctors dragged their feet until my father was SO sick that he had to be readmitted to the hospital and almost died because the doctor “knew better”Since my father didn’t have another facility to go to AND because he contracted Covid at Hamilton Trace we didn’t have any other choice but to send him back to Hamilton Trace. This time was worse. I received a call from the nurse the first full day he was there. But after that I have called to speak to PT, social services and my Dad’s actual nurse. I NEVER received a call back!!! Now they call me to tell me he is being discharged and do I have a place we want to send him....WHAT THE ....!!!!! I don’t even know my Dad’s status to know what kind of facility he needs to go to!! I would stay away from this place.

We would not recommend Hamilton Trace Rehab. Our dad was there after a hospital stay (was walking when he left the hospital, but couldnt get out of bed by the time he left Hamilton Trace). Rehab rarely came in to his room to work with him. When we brought him home, hospice found he horrible bed sores that had not been treated, which means he was neglected (not turned enough). I contacted Ericka at Hamilton Trace, who apologized and said it was unacceptable. She put me in contact with Lindsey, who indicated he had a rash and cream was sent home for this. It wouldve been nice to know he had bedsores also (sores were terrible, and I have pictures). I emailed Lindsey regarding bathing our dad (he was not bathed the entire 14 days there), and Lindsey did not respond. Dont rely on Hamilton Trace Rehab if your loved one needs any assistance getting out of bed.

I have been watching these reviews and I dont know if any of you have experience at other places but Hamilton Trace is by far the best I have had yet!!!! My father is currently there in the short term rehab program. So far they have been very attentive with my concerns for my fathers wellbeing. I have been trying to find out if there is any way at all they can let my Father watch Heartland either on Netflix or Amazon Prime. I can use my log in and password, hopefully it will work! Also, hands down, Beth even has a fax number and she does reply. So far their assistance with my father has been superb!! I have zero complaints for Hamilton Trace!!

The moment I walked into Hamilton Trace I knew that this was the right place for my husbands Rehab time. The Admittance Director Kassandra was so very kind and caring.Since then we have had an opportunity to experience overall caring personal to help in getting my husband ready to return home. The Director Bryan is always ready to give a helping hand and to listen. The transportation to different appointments provided by Hamilton Trace has also been outstanding. The driver Gary is a jewel.We talk to many people who praise the Rehab facility with their many Therapists and have chosen to return sometimes as many as 3 or 4 times on their journey to better health.Hamilton Trace is all together a place where people care about people!

During this time of Covid outbreaks this place has NOT been in accordance with CDC requirements. There are now at least 5 workers here who have tested positive for the virus and it goes up every day. Yet there is an aide here that continues to go into my mothers room wearing mask wrong,and no gloves or gown. My family and I have brought this up with Soc Services and nurses countless times and still nothing has been done! My mom has had to wait over 40 minutes for someone to help her to bathroom. She has stage 4 kidney failure and her kidneys cannot tolerate this especially since she has to take a water pill for CHF. There are a couple of good nurses and aides but it only takes a few bad ones to make life miserable for someone already suffering. Shes had to call family constantly to try to get help with toileting and pain meds. We have called sometimes and been told someone would go right down yet 15 minutes or more will pass and still noone comes. She has been told she will have to hold it while they pass out and pick up lunch trays. What happened to patient needs coming first??? Shes barely gotten any physical therapy because she cannot walk on the floors in room without sliding. They tell her its all in her head. Really??? I was told that treads would be put down to make it easier for her to walk but of course nothing has been done. Instead she sits in a broken wheelchair with her legs dangling down because the footrest is broken. This has been bad for the swelling in legs. This place used to be ok but my mother will not go here again until someone in charge actually cares enough to know what is going on. Praying that she will be well enough to walk and go home soon.

I recently placed my brother into Hamilton Trace (HT) to assist in his recovery from a stroke. I was a bit apprehensive concerning the level of care he might receive in any nursing homes these days -Covid, staffing levels and staffing proficiency all being of concern. Approximately eighteen months ago I chose one of what I had understood to be the best in the area for surgical recovery for myself. That was a disastrous experience. Quite to the contrary was my brothers experience at HT. I admit I and my family struggled with the rigorous Covid protective protocol that made it difficult to visit my brother -the facility was Covid virus free. The staff was responsive and most professional. HT will be at the top of our list to consider when we need to look for a facility to provide for assisted living level of care..

Dont put you loved one in this facility. Last Friday was the beginning of my moms stay. She is in there for subacute rehab.Beth, the marketing director, made false promises from the beginning. She said mom would be assessed by a nurse practitioner the next day. She said sometimes the doctor herself comes in on the weekends. I, working in healthcare, thought this a bit odd but I did need a place for mom to get stronger. Mom was more weak from her surgery than I anticipated. Beth, also said mom would have therapy assessments and social service assessments on the weekend. Those assessments except for social services were never done and certainly not reported to me in the care plan (which by the way the rehab team didnt even attend).I chose to drop mom off myself. While they knew we were coming, no one was prepared for us. Michelle, the receptionist, didnt even acknowledge me when I came to the inside door. I understand Covid is happening and we are all trying to be careful but you could at least acknowledge me. I am already upset enough having to make these quick decisions for my mom. It took Michelle at least 2-3 minutes before I could even get her to the door to even get a wheelchair for my mom. My mom in the meantime had digestive issues and had an accident. I had to quickly take her to my office to clean her up because Michelle was so slow.I finally got mom back to the facility. They did at least have the requested wheelchair. They whisked mom away. At least, I could have window visits. Beth once again said my mom would be in one room but they assigned her another one. They told me the new room number as I was finishing up the paperwork. Beth assured me my mom would be in a room close to the nurses station so she would be better monitored. The nursing station is not designed to look into moms room. Also, the rehab nurses would not answer the phone at all. If they did, they would hang up on me. I didnt get someone to answer the phone until I threatened to pull my mom out then. The pulling my mom out conversation got a phone call from Beth and we talked about how my mom needs to press her call light. My mom would press the light but it still took someone 40 minutes to get to her. There was a night her vitals werent even taken.They also didnt elevate her bed which by the way is supposed to happen when a person has a shoulder replacement. I question if they really know how to help a person rehab.My mom did get help changing clothes the first couple of days. She did get a shower finally on Monday but this is the only shower she has had. She has been told multiple times by the aide that she would get back to my mom. My mom is very alert and oriented.The aides didnt know how to help my mom with her brace. I am sorry but I think if you are working on a rehab unit you should be used to all types of devices.She did get some physical and occupational rehab.She has not had her bed sheets changed all week. She had to learn to elevate her own bed.The only 2 silver linings in this whole ordeal is she at least got to eat 3 times a day and she did get some rehab so she can get stronger. I will be pulling her out this weekend. She was scheduled to leave on Tuesday anyway.Dont put your loved ones in her especially during Covid when you cannot go in and do for your loved ones yourself. It is glaringly apparent the staff in this place doesnt care at all about people. They only care about the Medicare dollars they are getting.By the way, I work and live in Fishers. I have a thriving business in which I get to talk to people. Not one person I talked to had anything good to say about Hamilton Trace and this was just this past week!

My mom came here to rehab after open heart surgery. I am very thankful she was there and not at home because she developed pneumonia. Everyone there was super nice and caring. She wasn’t to happy with the food but she managed. Thank you to all the staff who helped in her (Sharon Priest) recovery. Highly recommended. Very clean inside super nice staff!!!!

Putting a loved one in a Memory Care facility is tough. After searching several facilities, we felt that Hamilton Trace was our option. The first 3 days there, my Dad had 3 falls. The 3rd fall, he was found in his room on the floor. We still have no idea how long he had been there as they told us from the beginning that they check there patients every 2 hours however, it’s not charted. An ambulance was called and he was taken to the ER. Thank goodness no broken bones. From the very beginning of admission, we told the staff that he did not know how to take his clothes off or shoes and that he needed help using the restroom to put back on a Depend. This seldom happened. While visiting him, he would not have on a Depend and had soiled his clothing. I would clean him up and put a Depend back on him. On 2 other visits, he was in his room with a shirt, socks and shoes and no Depend or pants. This was totally unacceptable. The staff neglected him the entire month he was there. We had 5 meetings with the executive director, the head of nursing and the social worker repeating ourselves over and over. Nothing changed. The staff was not properly trained for Alzheimer’s patients. They did not redirect or re-approach when my Dad was sundowning. They would call an ambulance to have him taken to ER. The staff never smiled or engaged with my Dad or other patients. They lined the patients up in front of them in their wheelchairs or chairs and had them there all day so that the staff could text or socialize. When you put your loved one in a facility, it’s a state law that they have a chest X-ray. My Dad never had one until last Friday when we told them that we were taking him out of their facility. The social worker was harassing the new facility to get him out. I honestly could go on and on. The bottom line is this, do not put a loved one at Hamilton Trace in the Memory Care Unit.

With an aging parent I was at a loss on what to do. I looked at several buildings and just wasnt impressed but when I found Hamilton Trace, I was so thankful. When coming into the facility the first thing you see is a beautiful fireplace and a friendly receptionist. When given a tour of the building I was in all about how many staff members stopped and introduced themselves. The rooms are gorgeous compared to other facilities that I went to visit,Hamilton Trace had by far the largest and cleanest rooms. As a dentist I am well aware of the things that could go on in a nursing facility and I would never subject my parent to neglect but at Hamilton Trace I felt at ease and comfortable putting my parent there to get the help they needed to return home. I would recommended any of my patients to come to Hamilton Trace for great care and excellent food!

Absolutely stunning facility. I toured many facilities while searching for a nice fit for my grandfather. Everyone I met with was spectacular and very friendly and made my grandpa and I feel very comfortable with the new adjustments. This is the only facility I would recommend!

As the Executive Director of Hamilton Trace, I have been impressed with the high quality and professional staff Ive seen in action. The facility is clean, new and comfortable as an alternate location for patients that are recovering away from home. The therapy team is amazing with coverage 7 days a week. I have also been impressed with the meal service where residents are served directly from the kitchen rather than from a cart where food trays have been stacked up and sitting for too long. If you are looking for a place to receive rehabilitation, skilled nursing or assisted living care, I would highly recommend coming and seeing for yourself what Hamilton Trace is all about.

My grandfather was a resident for about 2 years. He had great care and the nurses were always there to help him when needed. The therapy team was outstanding- they catered to his individual needs to improve strength after his stroke so he could be as independent as possible. Towards the end of his life, my family received great support and empathy from nursing and social services. For not being able to be at home, I couldnt have picked a better place for my grandfather to call home.

My mother was a resident of Hamilton Trace for two years. She passed away on February 26, 2019. Her care prior to her becoming bedridden eight weeks ago I would rate as good, not excellent. After becoming bedridden I would rate them as fair. I found care meeting to be a waste of time. Social workers would listen to your concerns, take notes and act like they would be addressed but, they were not addressed. So I learned to rely on myself to make sure all her needs were met. Luckily I live close so I was able to do this.After becoming bedridden, I then truly saw the care that was given. She was not being offered water and I always had to ask for water when visiting her. Many times her oral care was not being addressed. This is someone who can not move, not even her arms. Hospice, Paradigm, was brought in for the last three weeks. I dont know what I would have done without them! Emily her nurse was fantastic.Emily tried many different ways to get the aides to offer water and oral care.Her final 12 hours, no one came in to change her. I finally got an aide to do this. Wouldnt this be standard procedure to do this?Hamilton Trace, you need to make sure your aides are doing their job!I do want to thank the RNs. Especially Reagan and Lisa. They always took very good care of my mom.A final note...after my moms passing, to this day, I never received a call or card from Hamilton Trace with their condolences. I found this to be pretty sad considering she was a resident there for two years. This kind of behavior is what makes people believe that the dollar is their main concern.

I brought my father here for rehab after a short stay in hospital. He needed to transition to skilled nursing .Prior to admission I interviewed several facilities to make sure they would help with placement from Rehab to SN.First the good. facility is fairly new and laid out well. Rooms are large and comfortable. Dad was pleased with the food. Location was great for us.The BAD - communication was very poor. Never was able to get anyone to actually call me despite several messages and stop by requesting this . There care team showed up an hour late for scheduled care/plan meeting. Was advised that dad was scheduled to discharge in 4 days at this meeting. Even though he has been on site for over a week we were not made aware of this and lead to believe he would be their longer. Problem was they had not started to look for placement yet. I scrambled and found a place for him.Staff was mostly pleasant but not engaged. Common rooms were dirty. Ground up mud and m&Ms on floors . Despite fairly new facility common room furniture was in pretty bad shape.Overall if you have other options I would try else where. Where the management of the facilities have some engaged, caring folks that take pride in the work they do and the company they work for... . From what we experienced This is not that place.

Absolutely appalling. I in conscience would never send any of my relatives here for care. Nurses were supposed to be trained on dialysis procedures. Machine was turned off before the end of therapy, consequently leaving fluid in the body. Not once but several times. This is extremely dangerous.

Was there for rehab therapy for my leg. Broke my tibeal plateau. I was the youngest patient. I got along great with everyone. Staff seemed very friendly and customer service oriented. They greeted each patient with a smile. The director, whom I had not met, knew who I was by name which to me shows he cares and knows what is going on. Place was clean. I feel that the therapists know what they are doing considering why I was there and how quickly I improved enough to go home. Food was good. My main negative is that they could probably use a couple more cnas to assist patients quicker. Otherwise, good experience with my first nursing home and rehab facility and I recommend.

First, most of the 5-star reviews that have no additional comments are posted by employees, former employees, employees who are now at other Car-don facilities, the parent company of Hamilton Trace, and in some cases, the family and friends, of Hamilton Trace employees. (Check the names and then look up or call and ask for those people, or employees with the same last name who worked there during the time the review was posted.) It is a desparate attempt by the employees to artificially raise the ratings which were staggeringly low and cover up how truly bad this place is.My mother fell in the bathroom (where she wasnt to be left alone), pulled the emergency cord for help and was still lying on the floor when I came in an hour and 45 minutes later. Another patient fell from bed, and screamed in pain, completely ignored, until I finally went in search of an employee to help. Hours later this poor person was sent to the hospital for broken bones. His family wasnt notified until days later.I could go on with horror stories I experienced or witnessed. Patients ignored or left wet and dirty while employees surfed the internet, shopped online, or, on more than one occasion, slept during their shift. The real shame is that the therapy department is actually fairly good. Unfortunately, relatively little time is spent in therapy so most of the time patients are resigned to the clutches of the medical staff, mainly ill-trained aides who have been rushed through a hasty course and bring to mind parolees not qualified, caring medical personnel. Actually, that is what strikes me most, the near complete lack of caring by anyone at that facility even when problems are brought to the attention of the management. In my opinion, just DO NOT SEND ANYONE YOU CARE ABOUT TO HAMILTON TRACE. If anyone has any questions, please feel free to email me.

Absolutely awful!! My brother was here for rehabilitation for over 2 1/2 months following a hospital stay and the care given was so incredibly poor. The entire wing where he was placed wreaked of urine and feces to the point we would have to cover our noses when in this area. He sat for over a week before they even began his therapy and was only given 33 minutes of therapy a day and would sit in a wheelchair or lie in his bed for the remainder of the day. We were private pay and I repeatedly requested more therapy and meetings to which very little was done. Finally, after many tries to get these issues corrected, I removed him from the facility. I could get no one to take action except for a the woman in billing. There were so many issues that went wrong here...stolen clothes, unkept rooms, showers and/or bathing not regularly given, (I actually gave him showers myself on a few occasions because the staff was not doing this.) and the list goes on and on. The lack of care and amount of disorganization at this facility were unbelievable. I wrote a letter to the CEO of CarDon and CCed the Director at Hamilton Trace expressing these concerns and more and neither ever responded or contacted me. Simply unbelievable! The Independent Living portion of this campus (The Reserve at Hamilton Trace) seems to be good, but this rehabilitation section is not. At this point, my brother is requiring rehabilitation again and it will not be here - now or ever again.This is in response to Hamilton Traces acknowledgement of my review:Dear Hamilton Trace,So you respond to a post on social media where my review of your facility is out there for all to see, but when I wrote letters to you expressing all my concerns, no one bothered to respond? And now you want me to call you? Im afraid youve missed the boat on this. Your response on here makes it look as if you are engaged and concerned. That is absolutely NOT the case. My letter was written to the CEO of CarDon, Dr. Moore and Brian Lindsay, the Director at Hamilton Trace on March 12 and never acknowledged. This is incredulous.

DO NOT COME HERE! Do not let your loved ones come to this facility for rehabilitation. Our grandmother was neglected, ignored and left unattended to for hours upon hours at a time without medication or physical therapy. She was left helpless in her room calling and yelling for help but nobody assisted her. There were two staff members for 30 patients in her area and she and many other patients were left ignored all day. We spoke to other families in her hallway experiencing the same neglect and frustration. The director of the facility did not even have the decency to call our family back about our complaints. Please do not bring your loved ones here. I would give this place zero stars, but you cannot post a review without selecting at least a one star rating.

I was there from a homecare facility with a client overnight she had no sheet on her bed she was laying on a plastic mattress till morning when I requested a sheet which the young lady bought that had a blood stain on it even though it had been washed.The nurse and other employees except for 2 were unpleasant & irritable when ask to do something.I felt they had TERRIBLE bedside manners.Sad to say these young people have no concern for the client or their jobs
