I visited a neighbor of mine who is here for rehab after a fall and hospitalization. She seems to be satisfied with the care, and I found the facility to be immaculate and the staff to be helpful and courteous.

The worst place ever I wouldnt send my Dog here.The staff is lazy and unprofessional.I hate this place but this is the only place that had a bed for my Aunt.My Aunt Developed a Bed Soar that was infected.....Please turn your patients every two hours, show compassion,treat your patients as if this was someone in your family.Retrain your staff on how to care for someone,How to communicate with patients and family members.I also wonder do youll have cameras in the rooms so a family member can ask to see how their love ones are being cared for.The whole staff need to be fired if they cant do there job

Truth and all factsMy father was taken to this facility for physical therapy and possible long term care.He was transferred from the VA Hospital to Pruitt on Tuesday 8/3/21. I did not receive a call or any information about his transfer nor the guidelines for the facility until I CALLED at which time I was given limited information and told that someone would be contacting me to give me me accurate information.This was Wed. 8/4/21.Day 3 of my father being in this facility. This was his birthday. My aunt went to go see him. They were turned away and told that no visitors were allowed unless they were scheduled visits but limited information was given as to why. It was a person in the parking lot that explained that the EMPLOYEES at the facility were facing a Covid outbreak.Day 4 - More relatives attempted to visit and were told to schedule a video conference.Day 5- My father called me saying he was very uncomfortable with the care he was getting. He stated that the residence were left in their bowel all night and most of the day. And many times theyd clean the residence bowel at the same time that dinner was served.Days 5, 6 & 7 - I tried to call my father several times and got no answer by day 7 it was going straight to voice mail.Day 8 - my uncle and aunt had a video visit with him and asked the nurse why he was so lethargic. She said because he had just cone from dialysis. HE WAS NEVER ON DIALYSIS!Day 9- We were still trying to get information about how he was doing. Also, trying to locate his wallet, identification and other personal items that were missing since he was transported from the VA hospital. Also, they stated that the phone wasnt working because his phone wasnt charged 🙄 but my uncle took his phone AND phone charger and filled out a form. But that was missing also.Day 10- I went to drop off clothes and try to figure out what type of charger he needed to charge his phone so I could speak with him. I had to be outside because still no one was allowed in not even to the front desk. The front desk clerk said they found a charger and his phone was charging and to call in a couple hours to disk with him. I called several times throughout the day with no luck.Day 11- I received calls from a PRIVATE NUMBER it was the nurse saying my father had been rushed to the hospital. She initially told me the wrong hospital. About an hour or do later I finally got the correct hospital. My father was gravely ill with a STAPH INFECTION.DAY 15 - MY FATHER PASSED AWAY. Now, what I will say is , he did have preexisting conditions which could have led to his death. However, there are so many things that could have been handled more properly by this facility. Also, it is unfortunate that his loved ones didnt have am opportunity to see him face to face while there.There is apparently limited, under qualified staff trying to care for many elderly residence. And many of the BASICS arent being covered which makes it and unhealthy environment leaving friends and family members confused and frustrated.So on top of being a family that is grieving we are trying to track down and locate his personal belongings because this facility has been accused of theft many times. Its so sad on so many levels. Welcome to death row is what I feel this place is.

Despite the fact that there were several nurses and occupational therapist I would give 5 stars - I would not recommend this facility. In my opinion I would strongly suggest you search for another location unless you want your loved one to be neglected.

Due to Covid, Im not able to see the facility however this is not my reason for the low rating. My rating is due to the staff. I have had one person that contacted me and was helpful and kind. All my other interactions they were very rude and unprofessional. Having to deal with someones love one you think they would be a little more considerate. So instead of talking about do something about it. Currently looking for another location for my father

If I could give this place ZERO stars I would. PLEASE DO NOT PUT YOUR LOVED ONES HERE!! My mother was admitted to this location on Aug. 16, 2020 by ambulance directly from the hospital. My family arrived shortly after and was notified that there were no visitations because of COVID restrictions, that she would be placed on quarantine for 14 days and in the meantime we were given a phone number to call to set up a virtual visit. I was told that I could however call to check on my mother until I could visit with her and that face-to-face visits could resume after 14 days and our loved one would be brought outside. To make a long, terrible, exhausting story short, I was NEVER able to get ANYONE on the phone to tell me how my mother was doing and after a week of NO RETURNED PHONE CALL, I went to the facility to see what was going on. I spoke with Nurse Manager Simone and Nurse Solomon who assured me my mother was ok, but still had no justification for the phone being broken on the floor that my mom was on NOR a reason that NO ONE had called me back. When I was able to make an appointment for a virtual visit, my mom wasnt at the facility instead she was still out for dialysis. I should have removed my mother then, but I was hoping things would get better once she got out of quarantine. Finally, on Aug. 26th someone called the ambulance to pick my mom up from the facility and have her transported to the emergency room, however I WAS NEVER NOTIFIED!!! No one at Pruitt Health Decatur had the decency to pick up the phone to call me and let me know that my mother was sent to the emergency room. When I called and spoke with the Nurse Manager Paul, he DID NOT EVEN KNOW WHICH HOSPITAL MY MOTHER WAS IN!! Pruitt Health had no records that he could refer to. This facility AND THE STAFF ARE TOTALLY INCOMPETENT! After 1 week in the hospital my mother passed away. I feel like if I had had a chance to see my mother or get information at Pruitt Health about my mothers health, I could have possibly gotten her to the hospital sooner and she may have lived longer. PLEASE, PLEASE do not trust these people with your loved ones life..it may be the last time that you see them alive.

I am a current resident and i think Therapy is outstanding. Entertainment is aswesome.great privacy and infection practice. Cleanliness is also outstanding. Meal is balanced. I would definitely recommend this facility.

First of all, I think Iyour institution as a whole is good and your people do a great work ! However, those assigned as gate keepers could exhibit MORE COMPASSION AND UNDERSTANDING when people and trying to visit with or check on loved ones. Delays or AGGRESSIVE DENIAL , I simply do not tolerate. I do not appreciate speaking in person with senior staff and receiving the respect of EYE CONTACT ! I do not appreciate when staff hangs up on me ! I don’t appreciate not being given the respect of listening to my explanation of events ! So than , just because I can’t come up with a name of who I talked to about visitation for a particular day , I am greeted MOST INAPPROPRIATELY! Moving on, I appreciate my Fatther’s nurse ! She does very well with him and keeps him cheerful! Your prior administrator, Mrs. Davis was wonderful ! I will miss her. As for the for mentioned individuals, I realize that your administration staff is stressed but I should not ,nor should any loved one who comes to see their parent, be subjected to such treatment nor should anyone hang up on family of loved one !

Im currently a resident here Im here to rehab my left arm and knee. It had a stroke 2 years ago and recently some complications. NEVER COME HERE! The staff for the most part dont care about taking care of the patients. I wouldnt want any human here as a nursing home resident. My insurance paid for 100 days. Ive been here about a month and found a small growth on my elbow. The doctor said not to worry about it. It would be gone in 10 days. 9 days later it was 4 times as big and is now painful. For the last 5 days I have been asking literally every person who works here to see the doctor. No one knows when or where the doctor will be. Or claim to have told the doctor about this growth on my elbow. Therapist say if it is painful I can not do therapy. Im leaving tomorrow to go to a doctors office because I cant get a doctor to see me here at the facility.

Everyone was genuinely nice! Seemed like my friend, their client, was happy there! Place was clean and social distancing was practiced! Property was well groomed!🐞

Worst place ever dont be fooled by those actors on the commercial. One of the biggest mistakes i ever made in life was sending my mother here. DO NOT SEND YOUR FAMILY MEMBERS HERE FOR NOTHING THEY DONT CARE ABOUT THEM!!!

Dealing with a love one in a facility is always going to be difficult on and for the family. I try my best to be objectively and look at everything through clear lens before commenting. With that being said, once I was able to establish relationships with key people at the facility who I felt genuinely cared, the communication was honest, sincere, straightforward and helpful in my opinion and I really appreciated that greatly. The downside is that I didnt feel that it was across the board, which in turn can make things difficult. So I say this to say, seek out the helpful and genuine individuals who has empathy for all. Stay strong and never give up. Best.

This place they still their clients merchandise/items. They lie to their clients families me personally didnt have a investigation on this place. My aunt have been there for quite some time she has now been moved to a different facility they dont know when theyre not none of her things are which is weird.I had some stuff to her for Christmas that I ordered online do I know where thats at and I accidentally ordered something and had it sent there by mistake for myself and they said they never received it even though I have all the tracking information saying my aunt things was delivered with the correct address and my things as well now I have to make a police report.they dont even call you back from the complaints you have. If your family member is not vocal and can send for the ultimate complaints I will not send them there unless you want them to die quicker than the time God gave them to neglect in how I understand they are. Did not have a phone that the clients can use at the spare of a moment because its always lost let them tell it and they also would drug your family member If they raise hell I dont witness and have a threatened to call the police when I could not speak to my aunt for 2 weeks because she was drugged which is illegal by the way but they stopped giving her whatever they was giving her to make her sleep everyday and I have talked to her ever since then God shes out of there now. and he also opened your family members mail which is illegal to open residents mail without been present and a lady told me that by mistake.

Some of the staff members appear to be incompetent and none compassionate. It is hard sometimes to get the phone answered. Very few return phone calls from leaving messages. No follow ups from doctor. too lack communication about love ones. I think all of these concerns should be investigated. I am constantly having to complain

My brother came to PruittHealth Dekalb COVID-19 free(negative) and left Covid-19 (positive). When I visited him virtual the person acted like she couldn’t set the phone, iPad what ever device she was using up. She put the camera at the top of his head and said can you see him several time she did this. I was able to talk to him, he sounded very weak. It just seem because we can’t visit our love ones💕 and they know this. They really are not given our love ones the best of care. That how they made me feel. I am so sorry I had to write this and give such a low rating. But I love my brother and he is disabled double amputee . So, I hope that they will start being more careful and show more love and start given good customer service .

My mother been at pruitt health decatur about 6yrs now. the staff takes good care of my mother Ms.Earlene Brumdage. Supervisor Nurse Harriett keep me inform with all concerns about my mother. I was very disappointed with the care my loved one received. I understand there are circumstances around the world at this time, but patient care should always be top priority. Staff ignored concerns regarding health and pain. Please check on your family and hold the facility accountable for care of your loved ones. I would not choose this facility again.

I have not visited the facility but the staff is very professional and seems to take my husbands care as being important. I like the video chats that I have with him since I can not visit him. He has only been there a few months. The staff when ask how he is doing will give me a update. I would have to give 4 stars since I have not visited the facility.

I don’t like that I have to make an appointment to visit my father. Makes me think you are not treating him right and only doing right by him when someone comes to visit.

Someone I know was very much abused and neglected and possibly abused here. Went in with partial paralysis, now has no mobility, vision loss from possible and unexplained brain swelling, horrible sores from not being properly changed, clearly weak and has a very bad cough. Do not send your loved ones here.

Bad communication, no trust = need to staff care givers that know how to give good basic care with compassion . Medical attention to the residence need improvement .

I was forced to falsify my answers on the COVID screening per the director. They are rude unprofessional and disrespectful to their visitors. This place needs to be shut down!

My dad was placed at this facility because it was on VAs list as contracted facilities and it was the closest facility to us. Weve only heard from his nurse once, whenever I call I get transferred and no one on his floor picks up ever. It keeps ringing and then your call gets disconnected.This doesnt make any sense knowing that family members cant visit their loved ones, now they cant call because no one wants to deal with the family and answer the phone. If thats not the case, it sure seems like it. Ill be glad when this Covid gets under control because the residents probably think their family has abandoned them.

FIND SOMEWHERE ELSE!! Terrible care I believe Dr Gruber or whatever his name is. Should be in jail for malpractice. Thats one retired veterans opinion.

I think this facility has great potential, i see the employees working hard, i think that the employees should be acknowledge more by the company, .my sister was there and her outcome was amazing she went there after surgery and came home without any problems. The communication between departments can be better.When going there bring practical clothing and nonskid footwear also leave your valuable at home there is no place to store expensive items there and they clearly tell you not to bring valuables

If you love your loved one, please find another place, any place! My mother passed under their care and she was only there for rehab. She wasnt even there a month. Its not like we just dropped her off and never went there. No, someone in our family went to visit her EVERY SINGLE day! The nurses were rude to her and didnt treat her right. Her own roommate stated so. My father witnessed it a time or two also. They should shut this place down. Or fire EVERYONE and start over. I really urge you not to go here. Please read all the negative reviews, even the older ones because nothing has changed at all except some nice renovations on the outside, the inside is still ratchet! This isnt the place for anyone!!! Let alone someone you love and care about!

This place is horrible my mom had huge open wounds inside her bottom she dehydrated and neglected please don’t send family here they told me her insurance wouldn’t cover after so many days so I should just bring her check (ssi) so she could stay

My grandmother spend the last month of her life her. They messed up some paperwork (which was frustrating). However, they took excellent care of her and they were really compassionate in her last days. She just passed three days ago.

I was sent here for wound care , worst place ever . Poor attitudes , roaches , stench , neglect . The rehab department was awesome , totally separate from Pruitt. I checked myself out as soon as I could get home health. I would not recommend this place to anyone .

As a former employee of this company I can truly say that this is the worst company I have ever worked for in my entire life. I would not leave my dog to be cared for in this facility. I am in no means exaggerating in the least. Everything about this facility (minus the patients) is awful. The managers were mean and crooked. Me and all the staff who were in the almost 4 year time I spent there only got a raise once and it was only for 10 cents. They never paid us for staying over and working for 24 hours during the ice storm even though they said they would pay us double. The LPNs that work there were among the laziest and cruel I have ever seen. The turnover was high in all departments, but despite this, the place was okay food-wise and cleanliness was mediocre. Although, there was a roach or two seen crawling up walls from time to time. To sum it all up, the owner of this facility should been ashamed of himself and should not be allowed to open or operate another business. The state of this particular building makes me wonder about the state of the other buildings he or she owns. He/she needs to take responsibility and do their job instead of sitting on their glorious asses all day thinking about their yacht. Theres only so long he/she can go sending bribes to the state regulators. Eventually something serious will come up.

This place should be closed down immediately. The nursing staff, and the CNAs are tertible. They go out of their way to avoid interaction with the patient and their families. The doctor came in and changed my wifes medicines all around. She had just gotten control with the changes that were made at the hospital. He is a know it all and very condescending when you ask him about the changes. This place simply warehouses patients whose families cant care for them themselves. This place is terrible.

My mom-in-love was a patient here. The physical therapy, occupational therapy, and speech therapy are AMAZING, PHENOMENAL, OUTSTANDING! They are VERY caring and thorough. The administrative staff, CNA staff, nursing staff, and the food leave A LOT to be desired. This place could be as great as the PT/OT; but for some reason they lack the will or the ability (even staffing) to do better. It doesnt feel warm and friendly; most of the time the attitudes feel forced OR if they dont feel like forcing - you get what you get! The first day I had to write up a complaint in order to get someone to change her brief as she is incontinent.There were so many issues along the way and I dont have time to detail them here. Including the promises of home healthcare - which still hasnt happened in the 3 weeks since shes been home. And every time I call to inquire about it - I get mixed signals or some excuse why it hasnt happened. Sigh...But I will say that if youre able to check on and spend time with your loved one DAILY or better yet have someone sit with them (as we did) throughout the day, then and only then would I recommend this facility because as stated PT is AMAZING! They got our mom back to where she was prior to her sickness and EVEN BETTER! Kudos and bravo to the PT/OT/ST team members! The rest of the team needs a desperate recharge.p.s. The food is mediocre at best! The first evening she arrived - dinner was a pimento cheese sandwich and a bowl of beets.

Now I give respect where its do. But honestly this place is a nightmare. Literally a circus with a horrible . I wouldnt wish this place on anybody. Ill rather die in prison than be sent to this place for my last days. The place reeks of a strong urine smell. My father is always smelling like urine. They wont clean him until you say something. They have an attitude when you tell them. And do a half ass job at that. Not all of the nurses ate that way. But majority do not have the best interest in the patients.

My dad was in this place for 10 days ( Really 8 full days) he left there and has been in the hospital 6 days now and counting. He had fallen and broke his hip and shoulder so he needed a place for rehab the hospital staff gave us two choices this place and another one with a worse ratings they transported him there from the hospital. If you love your family member dont put them here. The place is unclean the nurse told my sister not to put her purse on the floor in my dads room the first day. Keep in mind I was there everyday ans I would hate to see if I wasnt. As you search and read reviews you will find that you have to go to these places everyday to check on your loved one.

The staff is great i had my mother there and they did a great job, the administator and dhs is very responsive to concerns and is always there late hours to ensure that people are cared for.thanks to decatur for helping us in our time of needplace needs to be updated in the rooms outside looks great but the rooms are still in need of some tlc

Working here was very different.allways short staff. But higher up was very nice an understanding.my only problem was pay. N no matter how long you been a cna dont make a difference. Co/workers was great an we worked as a team.but i made the best out of it. But i love new jersey better was home sick so dats why i left.

DO NOT PLACE YOUR LOVED ONES HERE!!!! I hate this place. Staff is lazy and you can’t get anyone on the phone. They have this meeting when your family is first admitted with the “team”. They tell you everything they’re supposed to do but don’t do it. This place needs to be closed down.

The physical therapist are skilled and kind. However, the nursing staff are absolutely miserable. When my grandmother stayed here, wait times for a nurse call could take 10-15 minutes. One time when my grandmother needed help, we called the nurse, waited a long time, and when I fetched them in person they were gathered at the nurses station conversing like it was social hour. One nurse helpfully suggested that my grandmother only go to the bathroom with nurse help, but she always had difficulty acquiring this assistance. One nurse who helped asked my grandmother to wait 20 minutes while the nurse prepped dinner. The nurse in question did not even wear her name tag.Also, the patients do not have room phones, and most phone calls to Pruitt Health will have you transferred multiple times and eventually disconnected.

Worst place on earth. Full of roaches and full of ratched ppl. I wouldnt put any of my family members here. This place is very stressful and unhappy. Management are crooks and administrator dont even know her own ppls names. Unfriendly!!!!!!!! Dont take ur ppl there.

The place should be shut down it run down for a big corporations this place sucks and i have alot of concerns and issues and if they dont get it together i am calling state. And fox 5. They have bad food just feed the residents anything.the food they serve i wouldnt feed it to a dog. Than housekeeping needs attention every day for cleaning.they have roaches bad .this place is horrible.need to some changes need to be done asap.
