Sophia, the office manager told me shed email me forms before I could make an appt. I politely called 3x to remind her, emailed twice and actually slipped a note through the door over the weekend with no results. I give up.I wonder how the doctor would feel about losing a new patient this way but I cant tell him because I CANT GET AN APPOINTMENT. :(
