The staff is verbally abusive to patients. My son observed this abuse to others and was subject to it. When my 41 year old son was admitted, there were sandwich crumbs on the floor and something sticky under the bed. He told the social worker who observed, yes it should not be so dirty, do you want a different room? My son observed electrical wires under the bed that were damaged, although the final insulation was not yet broken through to the wires, but definitely out of code. Even the hallways were in deplorable condition. He opted to be returned to the setting he was in, rather than stay a second night there.

This place is a disgrace! They are dirty, dont follow Illinois guidelines and abuse their residents in every sense of the word! When you try to see your love ones and are told that you must check with the administrator he has staff hang up one you despite the residents pleas to see his family! They are unprofessional in every way possible! Please if you love your family or whoever needs the help go elsewhere! I have contacted the state and channel 2 news team to help get them closed for good! So maybe these innocent individuals can finally go to a place that really shows love and compassion! #getthefilthoutofhealthcare

I want say from my own experience if I could give this place zero stars i would it smells dirty the staff on the 4th floor Ive seen them talk to the patients n myself with no respect I just seen a worker pull a woman out her wheelchair because she would leave the dining area I feel so sorry for these patients I pray who ever have people here please check on them this a horrible place

If you are looking for a homey environment for your loved one this is not the place. The social workers act like they are prison guards and they do not respect patients rights. They change the rules whenever they feel like it. They take personal items from patients and lock it up in their office with no clear reason why. They punish patients by making them sit in a chair in a large room for 10 to 12 hours a day. If this happens to you or your loved one please call the Office of the Ombudsman and they will help you.

Atrocious!!!! The care provided at this facility was subpar during the pandemic. Our mothers health deteriorated over the course of a year. We couldnt visit due to the restrictions mandated by the state. We understand that many nursing homes were shortstaffed due to the pandemic. But, to neglect our mom to such a weakened state is unacceptable!!! We communicated with our mom via telephone. She suddenly became disinterested in speaking with us??? My mom had her own cell phone. We could call her or she would call us ( usually several times a week per child). We were told that she was sleeping or had not charged her phone. This set off alarm bells in our heads. When we were finally allowed to visit our mom, we found her to be in a decompensated state. She had lost weight and was unable to walk ( My God, she ambulated independently prior to the pandemic). Yes, she does have a Cadillac walker. But, she mostly used it to transport her important items. She developed difficulty breathing and was sent to the hospital. It was then that we were informed that she had an unstageable bed sore on her coccyx as big as my fist. She was diagnosed with Congestive Heart Failure ( CHF). She was hospitalized three times in approximately six weeks for infection and fluid on her lungs, for falling out of bed and hitting her head and the third time for fluid overload. She had a weight gain of about 20lbs in ONE week. Midway failed to send my mom to the hospital in a timely manner. Because she was on tube feedings, they stated that they were happy with her weight gain. Really? 20 Ibs in ONE week...Really??? When finally sent back to the hospital, she had to be placed on Bi-pap and diuresed. While in the hospital mom stated, They are not treating me right. After this 3rd visit to the hospital in such a short period of time, Mom was discharged to a different facility. DO NOT TAKE YOUR LOVED ONE TO MIDWAY NEUROLOGICAL AND REHAB!!! Even if they respond to this negative review with statements of how they have or plan to correct the problems, that will be of little solace to our family and especially to our mother.....Anyways, anything they say will be superficial, and trying to cover up their wrongdoing.

Management is extremely disrespectful. Proper care is not taken for residents, they are three to a room with curtains between them. That did not change at the start of the Covid pandemic!!! We were required to wear the same disposable PPE for two weeks and threatened with write ups if we were to lose them or request new ones. Non union workers will receive NO COMPENSATION through this covid-19 pandemic. No extra pay for the high risk!! The entire 2nd floor was full of covid cases.

This was a sad, sad place to find my estranged aunt. :( She had medical disabilities but was left here by other ‘family’ and spent 10 years in a tiny room with 3 other women with mental disabilities, some severe. Staff were seldom seen and a lot were uncaring and apathetic. There were a few who really tried to help the residents, but they were outnumbered. My aunt was relegated to a wheelchair for so long without adequate rehab that her legs atrophied and she lost the function to walk. If you care about your loved ones, find somewhere else.

I applied for a job in the dietary department. I was told they could do the drug test at the facility. I was also told that they could get me in in a few days. I kept calling and they kept telling me tomorrow or hes not here then i finally got hung up on 3 times. If you arent going to hire someone then at least have the decency to tell them no or why. Horrible management. I really had hope for this job

All the pictures of this business on this site is misleading. Its the dirtiest place Ive ever seen, it smells of urine and raid throughout the entire building. There are 3 people per room, the beds look like theyre from prison. I have no idea how this place is open. It is not a rehabilitation place, its a place to drop off your forgotten. Im sadden, disappointed, and disgusted that this place actually exists. STAY AWAY FROM HERE. This is the most disgusting place I have ever seen.

IL Department of Public Health tells me the facility:-failed to ensure timely administration of pain medication,-failed to ensure a safe environment following an allegation of R A P E for one resident,-failed to send a resident to the hospital in a timely manner for medical evaluation and treatment,-failed to secure the elevator ceiling tile from falling on one resident.

This place is horrible and the living conditions are deplorable. They have people living in conditions not fit for animals. The staff is rude and insulting, the director will never return you calls. Yes, I reporting you and your facility to the State!
