My grandmother was a patient at Heritage Park following her hip replacement. The staff was helpful, warm, and considerate. They helped her stay motivated during her recovery and helped her regain her independence. Since I couldn’t visit her because of restricted visiting access, nurses and directors helped by letting my grandmother borrow their phones to FaceTime me and my family so we could check in with her. It was very kind and I’ll always be grateful for their care and service. Highly recommended. Best rehab in the area!!

Everyone in the facility was full of empathy and compassion. I was there for 3 weeks for rehab. I went from not being able to stand/walk with severe pain now I’m going home being able to walk with a walker & get back to living my life. If I need to come back I will bring coming back here! These people were wonderful!

I have worked at heritage park for about 2 years we have a fabulous team here especially my HR Crystal Sutherland she has always been there for me an has always been there to help when needed I couldn’t ask for a better HR Person very caring an always willing to go out her way to make sure we have all our training an inservices up to date I could not imagine going through another pandemic without her she made sure everyone keep up with there PPE an we stayed safe I can actually say she was more like a friend to all of us here at heritage park then just an HR Person she always respected us an listen when we had a problem. She is the best yet when it comes to HR

Unbelievable! They say there open 24/7. I just phoned and the lady that answered was very rude, and refused to give any information about there facility. I wasnt looking for anybody. Merely was just wanting to learn what the procedure was concerning the process of how one goes about selecting a facility for a loved one currently in the hosiptal that needs to be discharged to a inpatient PT rehab facility. Personally I was shocked and left with a feeling this place has no desire to rehab people but instead your sent there to die I guess

I was quite pleased with the rehabilitation care received. Found the facility clean, food surprisingly tasty and numerous options. PT and OT a Godsend....to my recovery. Last, Director Kim and entire staff was readily available for and met all my needs. I would give a 4+ rating if allowed. Five indicates perfection which is impossible in such environments....but always the goal sought.

Very sad place.. My mother did not have a chance. the place became a death camp with Covid 19 infections. So many, infections. 19 deaths. my mother one of them. taking infected patients in the facilities constitutes negligence on the facility. What ever you do, if you really care for your loved one, do not put them in this facility.

My mother was admitted to Heritage Park for Acute Rehab 5 days ago after fracturing her hip. I have only been permitted to see her once and had to visit with her outside of the building in 85 degree weather. I was told visiting is still only 1 person and only 2X/Week. I think this is a little strict and should be eased, particularly if both the patient and the visitor are fully vaccinated for COVID19. Other facilities are not limiting visitation to this degree. Very frustrating. My Mother feels I have just left her there and am not visiting.Cant get any information on her plan of care or progress, other than the Therapy Director who was very nice and called me back. The receptionist is rather rude when I call. I have no idea who or what physician is directing her care. I have been given no information by the facility at all. I am her Durable POA, Healthcare Surrogate, Pre-Need Guardian, etc. All of this documentation was provided to the facility.Honestly, I feel like I just sent my Mom into a black hole and I may have to get her out.

God help anyone that ends up at this dilapidated facility. There are some exceptional people , such as Tosha, Miss Anne, Mary Jo, Whitney, , Lorelei and PT Team and some employees that couldnt care less,When I arrived the phones were out, TVs were out. Healting pads not available, Call Buttons not working., ice packs not frozen. The poor nurses are so overwhelmed with work that they cant possibly get to everyone Food is awful.. Many had to wait over an hour to get pain medication. Several patients that need assistance going to potty are left to sit and wait until they eventually urinate in their chair, the whole place smells like urine. The responsibility for this mess lies at the feet of the owner.. Hire some Staff !!!!!.t

Went to see my mother. Staff is very nice, especially this one lady by the name of Donna. She always has a smile on her face and asks how I am doing. Sometimes while I am there, she will take a minute or two to come to my mothers room and say hello to her and asks her if she needs anything. Donna is very kind and cares. Thank you Donna.

I had to be there for 12 days in 13 nights. I found this on a daily basics. Every day there was an issue from anything waiting for hours to take your pain pill, turning your emergency or call light on and nobody ever coming, I lost all my clothes when my nephew brought them in and they wanted to wash them for Covid. I got some pieces back but still missing six pieces into the items were made to for me by my son and he is T-shirt tape canvas business. I will never be able to replace them and they would not give me a copy of the list I made of what still missing. They kept it but never made a copy of it. And lunch or dinner be in the wrong or I wouldn’t even get silverware or condiments.The only wonderful thing about heritage Park is their physical therapist and occupational therapist. They are very caring and on top of their game.There was times I could not even get a shower. I had a nurse berate me in the middle of the night. My children were very frustrated, they would call with messages and nobody would call them back on several extensions.

Relative of a patient at this facility who was transferred there after back surgery for rehabilitation. Unfortunately, what these patients receive cannot be considered care. The bed provided my relative is so uncomfortable that she is in more back pain now than when she arrived at this God forsaken facility. Additionally, the food leaves a lot to be desired especially when the staff has been informed time and again that this patient cannot eat pork yet its given to her time and again. I just pray that, if any of you has a mother, she will never need to depend on anyone of you or anyone like you for care because, obviously, these employees are oblivious as to the meaning of care and compassion.

I found the workers extremely helpful and friendly . I had problems with my medications not given to me on time. The nursing staff was great with exception of one who took her time administering the patients medication and didn’t like to be reminded they were duePlace is kept very clean, food is ok

Total the food is horrible its served cold.the facility discharged my mother with physical therapy and then never sets up physical therapy and then when I call three times trying to get a hold of them they dont even call you back very very poor care for their patients dont send your relatives there or anyone you know

My aunt was taken here after having back surgery. She was put on a bed with no rails, she cannot move due to the surgery because she cannot hold onto anything since there are no rails. The mattress is garabage, so she is getting worse not better. Whenever we call we are put on hold and no one returns the calls in a timely manner. No one seems to care about the patients complaints, she spends the nights crying because the pain is intolerable and no one seems to care. To add insult to injury, we dont know how many times we have told the supervisory staff that she cannot eat pork, she sends it back and they send another still with pork, so she is not eating. Please do not send your love ones here.

My sister is staying there and while she is rather rude (which I understand, wasn’t what she planned for in life- still not their fault) the staff still takes good care of her. In fact David, the activities director went well out of his way to help my 84 year old mother have a meeting with her at the window for her birthday. During covid, they have worked overtime to insure the safety of the residents.

Overall a fair place to work, however I am disgusted with the HR personal. Working for signature health care for many years in many different states I am in complete awe at the way this woman, Crystal speaks to the employees. In the middle of a pandemic some of the hardest years working as a nurse I find it completely unacceptable for this woman to speak to healthcare providers the way she does. Signature Healthcare please address the issue- this should not continue.

I fractured my foot 4 weeks ago and was told I needed some therapy so I signed in to Heritage Park for out patient therapyThe facility was clean , inviting, friendlyAnd the girls were so wonderful they had patience, explained the procedure of what we would be doing as far as therapyI did a lot of foot movement, walking , balancing even rode a bikeLearning to balance and walk again with a brace and a shoe was difficult but I have finished up this past Monday 12/30/19And the experience of it all for 3 1/2 weeksWas wonderfulMy choice again would to head back to Heritage Park for my therapy if I would need it again.Thank you girls Loraie, and Barb for the great workout and helping me get back on my feetTina

My husband was sent to this Rehab from Blake Hospital. He was in Blake for 10 days and Heritage Park for 7 days. Worst rehab center!! They discharged him with large red welts on his back, that manifested in to a full scale itchy rash, all over his torso. He spent two 4 hour stints at an Urgent Care over the proceeding two days. He was to be scheduled for Home Health care upon discharge, which was sorely neglected!! I have reached out to Heritage Park for answers, and they also neglected to return my calls. There is much more to this nightmare, but if you love your family member, do not allow them to be sent to this Rehab Center. Their care borders on Elder Abuse!!!!

My mother has been placed here after having back surgery and she was told that she had to quarantine for 2 weeks when she just did a corona virus test 3 days ago and it came back negative. Not only that but, if she is to be quarantined she shouldn’t have a roommate. That defeats the purpose in trying to prevent the spread of the virus. Every time I call no one can give me answers and I’m told I’m to receive a call back but I don’t. We were told that there would be someone there that speaks Spanish and no one there does. This place is very disorganized and no one seems to know what’s happening and no one seems to be able to answer my questions. When I asked to speak with a MOD I was told there wasn’t one available. I need answers sooner than later and I will be contacting whomever needs to be contacted so that this organization can be dealt with in a legal matter.

This place is so frustrating! My mother was there for long term care for several months. The workers are low paid and and have no respect for the residents living there. Her belongings were constantly lost and I personally had to go to their lost and found to find her clothing and bedding on several occasions . Cov 19 made things even worse. March 11 was the last day I was able to visit her. 4 months later my mother contracted Cov 19 and was moved to Blake Hospital. Heritage Park packed up my mothers things in boxes for storage. When I learned this I called to let them know I would be picking up her things. Not surprising that many of her belongings are missing. I left a message for Pat Richardson who is the social worker asking for help locating the things most important to my mother and she didn’t even care enough to call me back. Instead she had the front receptionist call me to pick up the rest of the things they found. Still missing however is her quilt, the purse and wallet she was using and her Iphone. There’s gotta be better options for our elderly parents!

We had our mother here for several months and we couldnt have asked for better service and care. All nursing homes have their issues from time to time but they are doing an impossible job and Heritage park went above and beyond our needs. The staff was very friendly and helpful. If I need to be admitted in the future this will be my first choice to live. They made her stay as enjoyable and pleasant as could be under the circumstances. Thank you Heritage park for all your help and hard work.

My father is 2 days in & Im not happy. They provide a call button however, dont expect a response in under 20 minutes & thats being nice. Im sitting here now with him & pressed the call button over 40 minutes ago with still no response which is unacceptable. Come to find out the aide has yet to show up for his/her shift. In the meantime, my father is suffering & literally sitting in his own feces. The RNs dont want to be bothered & I heard thats beyond my pay scale one too many times. This breaks my heart & angers me beyond belief.

I had nightmares after leaving this place was only in for a few days miserable experience. A lot of people need to find a different job. They are not happy and very frustrated with people who need patience. Strictly money making business to get Medicare money.

My dad recently spent a couple of weeks in the rehab section. He always had great things to say about his treatment. Everyone he came in contact with was wonderful including his physical therapists, occupational therapists, CNAs, and his nurses. He also enjoyed all of of his meals! I felt that I was kept up to date with his care. If I had any questions, the nurses were always willing give me an explaination in terms I would understand.

I was a patient here in the fall of 2016. After back surgery I had been in the acute rehab at the Blake Hospital for about 3 weeks. I was still unable to move myself well enough to be home so I transferred to Heritage Parks rehab for further therapy.My experience there was a very positive one. I had great aides who certainly earned their pay because I was heavy, incontinent, couldnt tell if I was having a bowel movement and generally an extremely messy patient who needed a lot of physical and medical care. The nurses were quickly tuned into my needs with regard to pain and spasms, dealt very well with my specialty meds (I also have MS) and the physical and occupational therapists were wonderful, getting me to do things I was afraid to do by being so focused on me and my fear of falling again. The other staff, from the fix it man to the laundry lady to the spiritual and administrative and front desk staff , they were all genuinely warm and caring. If they were serving a food you werent fond of they had an alternative menu you could choose from ahead of time so there wasnt any problem there either.My stay was a long one, I came in well before Halloween, stayed through Thanksgiving and finally came home just in time for Christmas. I think I had a good amount of time to see how the people and facility really were and I can say I am very glad that I had chosen to go there.

Im a current patient, several workers shouldnt be in this type of business. Im to get pain meds every 4 hrs as needed. 7 and a half hours finally received meds. I will repost through my journey.

I can only echo the comments of the previous negative posters. My Mother in Law was transferred there from Blake Hospital. They were nasty and told a Woman that could not move her arms to feed herself. If she soiled herself because too much medication was given, she was screamed at. She stopped eating and drinking and was in terrible pain. She was taken to Blake Hospital again. An ultrasound showed he bladder was amazingly distended. Blake drained a quart or more urine from her. Checking urine output is basic care. Stay away from this place if a family member is at all important to you.

Check the place out first. Food was real bad they should have to eat it. Some of the people were ok. Sandra was the worst. She made my mother set in a dirty diper because she doesnt like to change she wanted to find someone else to do it. She also wasnt nice. The care was poor. There are some nice people. The therapy people are nice. When you are there for therapy you need more then them being nice. If you dont like the job quit. I would never want another family member there again. Please check this place out first. I wish we had a chance to read the reviews first . There was another woman who was also very mean to her cant remember her name. Please check the place out first.

My 89 year old father was sent to Heritage last week because hes been in the hospital and needed help gaining his strength so he can walk again. Unfortunately he didnt have a good experience at Heritage. He told the nurse that he had been constipated for 8 days ever since he left the hospital and she ignored this important fact and didnt do anything. Because he was so constipated he had to be admitted back to the hospital. He said he didnt want to go back to Heritage because he was not well taken care of, the food was bad and the bed was incredibly uncomfortable.

My father had a terrible experience at this facility. The nursing staff for the short term rehab showed no sense of compassion, caring or professionalism. Because he was admitted late on a Friday evening, little attention was paid to him despite my frequent request for assistance. I was constantly told his vitals were normal as was his temperature. By Monday he suffered from complete dementia/delirium due to extreme infection that went unnoticed by the nursing staff. Again, this was in spite of my continual request for aid - I was treated as if I was annoying them and when I told the charge nurse we want him transferred to the hospital her only response was whos we and itll have to wait because there is paperwork involved. Because his infection went untreated he has potential permanent swallowing problems that are life altering if not life threatening. The experience was a heartbreaking nightmare that I wouldnt wish on anyone.

I visiting my mom several times when it was necessary to use the call bell for nurse or CNA care. On 3 separate occasions the response time was more than 25 min. Twice I ended up walking out to the nurses station to request aid & was told both times that someone would be right there. Once they did respond within several min and the other time I had to go back out after 15 min to demand that someone come and assist my mom. This was not acceptable but the administrator feels like the care is not lacking. Never will I put a loved one in that place again. That isnt even the worst of my complaints. She only went to Heritage to rehab from a bad bout of flu and pneumonia. She was there gaining strength and had been walking with a walker for over a week when she developed severe diarrhea. She had to forego rehab for about 5 days when they finally sent her back to the hospital. She had emergency surgery that night and had to have a total colectomy due to having contracted c-diff. Upon returning to the facility we found out that they dont even have a wound care nurse to show staff or patients who have this new stoma how to care for it. I met with Katie, the administrator who tried to assure me that a spattering of people had tried to educate my mom but none of the people were able to tell me exactly what their teaching consisted of. I did ask to see documentation about education and none was produced. I will be following up with a state complaint agency and one of the hospital joint commision information lines.

My father in law had surgery at FL Hospital Tampa and was transported to Heritage Park in Bradenton. Heritage Park forgot his clothes and couldnt give me an update for days. We received a phone call at 2 AM the very next day telling us that he had been taken to Blake Hospital around 10 PM because his blood pressure had dropped. They waited almost four hours to let us know. We never received a call to let us know his progress. We called the next morning and he had already been brought back. That very next night (New Years Eve) my husband received a call that his father was gone. They said that his blood pressure had dropped again and that he just died. When I went to collect his personal items, the medical records person told me that he bled out and that he was encouraged to sign a DNR before he died. We were never aware of this. I have worked for a hospital and have medical knowledge. My husband and I had medical surrogacy for him and no one contacted us prior. I questioned what she told me and she immediately took it back. I have been waiting since the beginning of January for the director to call me and have received nothing. I want to get his medical records but in order to do this we have to sign that we will not take legal action. We have someone looking into this for us. WE DO NOT RECOMMEND this facility to anyone!

One of my family members stays here.One of the staff members is down right rude and ignorant. Her name is Rebecca. This rep. has been very unprofessional on several occasions. Before being transferred to my Aunt one evening, I asked for the address to Heritage Park since I needed to mail something to my Aunt. The operator Rebecca refused to give me the address and told me to look it up in a phone book. I then asked if she wouldnt mind looking it up and she started arguing with me. I then asked for her supervisor and was hung up on. I also requested for a supervisor or HR to call me back more than once and nothing happens.The staff here is aloud to be ignorant, rude, and lack professionalism. I always try to be polite and have manners, however that will change for now on if the staff members will talk to me in this manner again. No respect will be shown to this poor excuse of an employee in the future.

It is the worst in the world. I wood not put your love one here. They never do they job right. They take hours to answer the bell when they are needed.Take hours some times to do any thing. Cnas bad. Nurses too. Management. They are not family oriented. Not nice with family at all.

Do not bother to call after hours as you will be put on death hold, you will just get hold after hold with no answer to your inquiry.
