I highly recommend F.O.R.M. The receptionist is very friendly and welcoming and has accommodated me with schedule changes several times. Dr. Ray is very good at what he does and speaks to you in a way that you understand what he is talking about. I always leave his office feeling better than when I went in!

It was nice listening to him explain what was going on, why it happens and what we are going to do to not only fix it but prevent it from happening again. My whole family has been going to Dr.Ray for years. The attention and treatment he provides is second to none - especially for athletes and/or for sports related injuries. I never hesitate to recommend him. The only thing that would make it better is if he had a secondary office on the west side of Wichita.

Dr Ray addresses the root of the problem by focusing on muscle and soft tissue. Definitely a step above a traditional Chiropractor. I always feel loose and restored afterwards!

My experience with Dr Ray started over the phone; I was searching for a chiropractor for my back pain due to pregnancy, I was worried and skeptical of finding a chiropractor. Dr Ray was beyond kind, polite, and patient over the phone. My experience in the office was NO disappointment. His team was VERY kind and professional. Dr Ray was gentle, attentive, and respectful. He listens to you and ask great questions. I’m very happy with my results and his way of approaching pain. I was worried it would only be about popping my back and moving on but he approached it a lot different than I have ever heard of when people talk about their experiences with chiropractors.I was also very thankful I was able to bring my four year son and Dr Ray was completely understanding.Great experience! I have struggled with lower back pain from a bulging disc for 30 years. Over time, the pain has gotten constant and running down my sciatic nerve into the back of my leg. It has progressed to where only an epidural would relieve the constant pain. I was referred to Dr. Mike from a friend and I can honestly tell you that I am 100% satisfied with my treatment! It is not your typical crack your back and see ya 3-times a week kind of therapy. His process focuses on your specific needs and he has got me where I dont need the epidural injection every 3-4 months anymore. I still have some discomfort now and then but after 3-4 months of therapy I do feel so much better and I am no where near needing the numbing of an epidural injection anymore. Hopefully the progress continues!

The service and hospitality was wonderful. The attitude towards trying muscle manipulation prior to moving to the tradition chiropractic adjustment process was much appreciated.

Dr. Ray has been a great help to my husband and I. He is an amazing doctor who really takes the time to pinpoint the problem and then work to resolve it. The appointments never feel rushed or routine, as he specifically works on trouble areas and doesn’t just do the same adjustments each visit. He is easy to talk to and makes you feel comfortable throughout the appointment. My husband and I highly recommend Dr. Ray to all of our friends and family, although most of them already come to see him.

I’ve always been a little nervous when it comes to my back! I’ve had multiple back surgeries and in the past most chiropractors will just pop your back, and send you on your way. Dr. Ray takes his time and breaks down those tight muscles that stops you from getting your full range of motion! His technique is top notch! Dr Ray worked on my daughter after she was injured in a soccer game. I truly believe the symptoms of her concussion improved after he worked on her neck. I absolutely would recommend him!

Dr. Ray is great! His office is warm and welcoming. He cares about his patients and remembers what is specific to you. He makes you feel comfortable and it’s easy to know he is working through the areas you need worked on. He doesn’t just work on correcting the bones and joints, he works with the muscle.

I was ecstatic to regain range of motion and experience pain relief in my shoulder after only one visit to F.O.R.M!! Dr. Ray and the staff are personable, down to Earth, and efficient! He obviously is good at what he does hence me only needing to see him once!!

Dr. Ray is an amazing chiropractor! I had such intense neck and shoulder pain one day I had to leave work, unable to turn my head side-to-side. I went to see my PA who recommended a steroid injection, muscle relaxers, and painkillers to alleviate my pain. I passed on those options and went to see Dr. Ray (who was able to work me in the same day) and about an hour later felt SO much better! I was able to go back to work the next day and was pain free, with full range-of-motion, just a few days later. I was thrilled that Dr. Ray was able to resolve the issue through muscle work instead of masking it through medication. Highly recommended!!

Dr. Ray helped me manage the pain from arthritis in my knees which eventually required total knee replacements. He now helps me with various physical therapy regimes and rehab which have resulted in a great recovery. Knees feel better than they have in years and I am able to get back to exercising. I highly recommend Dr. Ray!

Dr. Ray is absolutely amazing. I have been seeing him since he has been out of school! He has helped me in so many ways! I have really bad lower back sciatica flare ups every couple months and he helps me every time and it only takes one visit and I’m better. He is not the chiropractor that just adjusts your problem areas. He spends quality time with you and works on deep tissue ligaments. I am usually there for about 30 min unlike others when your in and out very quickly. He is very knowledgeable in what he does. I highly recommend Dr. Ray to anyone looking for a chiropractor who truly cares about his patients!
