I only went once to establish a relationship with a primary care doctor but I will not be going back for I did not feel cared for. I had faith as my primary to meet about a few concerns with myself for I hadn’t had an doctor since my pediatrician but as soon as i pulled out my notes, she replied with “Hmm let’s just go with two on there” for I only had four things on there but I definitely got the impression she didn’t want to go through a lot but assumed everything that I had written down was either things i felt that were wrong with me or symptoms. They were just explanations of a few things but I only went over two to listen to her directions she then asks me about personal questions about my history that had to do with another topic I wrote down. I was confused as to why she wouldn’t let me just tell her the things I was concerned about because they ended up getting brought up anyways. From then on I felt very guilty/attacked (not drinking enough water, not going to an obgyn, not eating healthy etc.) for someone who just wanted some answers and comfort. She was not a comforting person and all she said in conclusion was drink more water and also ordered me some bloodwork. After receiving my bloodwork there was no follow up appointment for one of the office members called me to tell me I am “pre-diabetic” and have high-testosterone. Following the phone call the lady on the phone told me to just eat healthier and exercise daily. Nothing else. No suggestions, no offers for possible meal plans or programs, just for me to “do better” and after I asked if I should schedule another appointment with faith to go over my results and figure out the best plan for me she said “no you don’t but if you feel like YOU need to go ahead.”. As for someone who is a young adult trying my best to get on a healthy track and and have a sense of knowledge of my body, I think it’s important for primary care doctors to show empathy. I understand doctors being serious and not sugar coating things but actually look like you want to be there and help me. I dont need a lot of guidance but a little would have gone a long way not for me to “figure it out on my own”. Again I’m a young adult trying to absorb knowledge and understand my body so this location specifically with faith and her staff gave me a bad first impression and I will be taking my business elsewhere.

I’ve been a patient of Faith’s for over six years and can’t imagine seeing anyone else. Her amazing medical team is so helpful, I’ve referred my closest family and friends. Thank you Faith and team, we appreciate you all!

I have been coming to see faith for about 5 years. She is the most amazing medical professional in the valley. I suffer from multiple ailments. For years I got the run around from Doctors. Once I started coming to this office I was able to be treated for my symptoms. It also was discovered the reason for my ailments . I highly recommend faith Cutrona she’s one of the best. The staff at this office is unbelievable they are kind and understanding and patient and helpful. Office visits are flexible they have Tele med appointments virtually that you can do from your home as well as walk-in visits. This office makes it easy to access a physician when you are Sick. Oh my! From the front office all the way to the back DMG Lake Pleasant office is run efficiently and with integrity! I have been a patient for years and Faith is amazing. She cares and truly takes the time to make sure all your questions are answered. The girls are on the same page as the Dr. and they all follow protocol and that way there isn’t any confusion. This is a walk in where you feel loved and cared about.

Saw Faith for a sick visit recently and mentioned that I am wheezing and having shortness of breath. I asked her for a refill of my inhaler Symbicort during the visit and she informed me to go to symbicort.com where I will get a discount card for my inhaler. Turns out this wasn’t true and it would be cheaper for me to get the medication through my insurance. I called the office to let them know and was told that Faith said I must have another visit and be charged again. Why do I need to see her AGAIN when I just saw her and specifically asked for this prescription originally and was misguided by her. This is medication for my breathing NOT a controlled substance. Now while I am running out I am told to wait til Monday to talk with the manager and can’t have my inhaler refilled unless I pay for a second office visit after just recently seeing her. Was told that Stacy would reach out to me. Stacy never reached out I had to call the office twice to get a response back. Then I was told that Faith said that during my appointment I was asking for my rescue inhaler and not a maintenance drug. Looks like Faith needs to be educated on the difference because Symbicort is indeed a maintenance drug and not a recuse inhaler. Additionally, Stacy informed me that there are several pieces of information that needs to be explained when prescribing symbiocort. If things were documented correctly by Faith the “NP” it would be clear that I have had Asthma all my life and been on Symbicort for more than a decade now, which has been prescribed by Faith in the past. Be that as it may. I asked for a refill of this medication recently during a sick visit and never received this so called “important information” instead Faith brushed me off and told me to go to Symbicort.com to get a discount card during this appointment. So my point is if there is important information that needs to be given with this medication why direct me to go get a discount card from the website? Was the information that needs to be given not as important at that time due to her rushing through her appointments like she always does Or is it all just about the money?Stacy responded and said I understand your side and will reach out to Faith and give you a call back in regards to this matter. Well I never received a call back which is not surprising to me. I would never recommend this place to anyone who in undergoing chronic or acute issues. At the end of the day I have a severe pulmonary disease. I am not taking these inhalers just for fun or due to boredom! And I’m not here to play any games and experience delays in receiving my inhalers due to a money hungry Nurse Practitioner. And if I had it my way, I wouldnt chose to have to depend on inhalers to live comfortably. They are needed to save my life. I shouldn’t have to go through all this drama for an inhaler to help me with breathing normally I am not asking for Morphine or an opioid. I will gladly take the time to find a physician that knows the difference between a maintenance and rescue inhaler.Update* Turns out they had a meeting about my situation and they came to terms that Faith was 100% WRONG in several ways what a BIG surprise but I was told that I should still pay the price. What I do find amusing though is that Stacy informed me that during this meeting they asked some of the providers if they could prescribe the medication to me. If there is such important information that needs to be given why would you ask another provider to prescribe this medication w/o seeing me? If they had agreed, would you have just let them take the fall? Also Stacy informed me that during this meeting they came to the conclusion that I should not be taking symbicort. If this is the case why did you ask other providers to prescribe it to me? Also why did Faith prescribe it in the past and give me access to a year+ supply of this medication afterwards? All I’m saying is if you are experiencing anything more serious than a paper cut do not bring it to Faith. BTW I’m a nurse so I know how the health care industry works ;) !

Bad experience! Did not accept my insurance, after I was with them for 2 years! Because place had traded hands I had to have a drug test, I had already done less than a year ago. They dont care about anyone and are so irresponsible they havent even updated their own address on goggle! GO ELSEWHERE!

Clean,fast, friendly, easy in and out. No wait but sometimes that’s out of everbodies control depending on how busy it is I’m sure. I like that I can still make an appointment or walk in! Great concept! Faith is by far the best and she deserves a huge raise. Smart, kind, attentive, a real professional! Front staff was great as well! Easy paperwork when you come in for the first time, not like those other places that require you to fill out a novel! Overall it was great and I’ll be going back for all my medical needs if needed!

Faith is by far the BEST provider I have ever encountered! She has a true passion for patient care! I was extremely impressed that she had my 13 yr old son leave a UA for medication management before she prescribed and while she was still prescribing a man ADD medication. Not only can she manage how he metabolizing the medication, but I also found out he had a metabolite of nicotine in his system. I would have NEVER known this if she wasn’t such a thorough provider! The best of the best!

From the initial greeting by a pleasant receptionist, to the efficient woman who took my history and finally to meeting with Faith (who was absolutely wonderful) this facility delivers prompt, thorough, empathetic and well thought out service. Faith went the extra mile in searching the most cost effective alternative for my prescriptions (I am Canadian and all expenses for me are out of pocket - made no sense to access insurance due to deductible)......Frys Pharmacy at Happy Valley also delivered in spades. Ive been to several other Urgent Cares in the West Valley, mostly large chains. Ive usually been disappointed in the quality of the care so Im always very hesitant to go to Urgent Cares. But, Choice Medical is wonderful. I felt I got an excellent diagnosis and a day later I am feeling much better. My thanks to the doctor and staff there. My only suggestion for improvement is to give a written summary at the end of the visit, that would be helpful. The wait was a little long, but worth it. I plan to use Choice Medical for all of my family Urgent Care needs in the future.

They have guided me through issues with my blood pressure, weight gain, dehydration and all of the normal other stuff that comes up as you get older. I highly recommend them!

I recommend this medical clinic but not this location. The actual doctor only comes in once every two weeks, therefore the office is run by a nurse practitioner. I would not recommend an appointment with Faith ever. Every time I go in and talk to her about anything important I feel very belittled. She insists she knows more about me after talking to me for 5 seconds than I know myself. She refused to prescribe be anti-depressants and gave the excuse “if you’ve been depressed for 10 years what’s a few more months”....???? If you plan to get insight on anything more pressing than a cold I would not go to her. She does not know what she’s talking about and you end up wasting at least $25 just for her to say she won’t do anything for you.

My family go to Choice medical Peoria. The staff is always friendly and even if I am a same day walk in I am never rushed. We all love NP Faith and have referred her to many friends which all feel the same and became regular patients.

The only reason I gave a 1 is because I couldn’t put 0!! I was in an auto accident and had a shoulder injury, I gave them $100 for them to not do anything for me!!! I got an MRI referral and that was it!!! I would like my $100 back!!! All this place is, is for weight loss. I don’t know why it’s called a “medical walk in” horrible service, no calls back until the next day! Absolutely HORRIBLE! I DO NOT RECOMMEND THIS PLACE to anyone!!! Oh and by the way I was in for a shoulder injury and yet they kept asking me if I was dieting! Are you kidding me????? I went here 5 years ago to the 1 in Carefree highway. They were amazing. I don’t know what happened to this place since then...

It was the worst experience I’ve ever had a doctors office. Faith the nurse practitioner there it was downright rude. She wasted my time to send me to get a test it was not needed, rather than listening to me. When I called her office the next day to follow up on what I should really do, she told me to make another appointment. She also would ask me the same questions 4 to 5 times. I’m not sure if she’s used to working with elderly patients but I’m not elderly and I expected her to listen when I answered the first time.

The staff is always prompt and professional. I never have to wait that long. Faith is so professional and knowledgeable. She always knows what the problem is and spends enough time with you.

I walked in and gave the front desk my insurance and asked how much my visit would be. She said around $60-$80 tops. I decided that was fine and saw Dr. Faith. I get the bill in the mail and it was $140?! I called into the billing department and she said it was because it was charged as a level 3 visit. I went in for a sore throat and they shined a light in my mouth and prescribed me antibiotics....

I have been going to Choise Medical in Peoria for years and will never change. Great Doctors and Nurse Practitioners. Never a long wait time, with or without an appointment! I give them and the staff *****!

We really appreciate how friendly and warm the staff is at the lake pleasant office. We used to live in anthem and unfortunately the staff at the anthem location were not at all friendly and warm. Especially the woman up front who also collected co pays etc. She had no personality and seemed to be a grump most of the time. I had some other encounters that resulted in rudeness. We are glad to have moved and will never step foot in that location. The girls who answer the phone and check you in at the lake pleasant office, are very kind and welcoming. Something you should expect when dealing with the medical field.

I absolutely love Choice Medical and especially Faith Curtrona! She goes out of her way to help my family stay healthy! She literally has the best bedside manner out of any Dr. I have ever met!!! All of my good friends and family go to Lake Plesant Choice Medical because of how amazing that office is! My kids love Faith and she takes very good care of them! This used to be such a great place. The staff now is extremely rude. Robin - faiths medical assistant - laughed at my very sick son as he was crying in my arms. Super unprofessional. Then it was a couple mins past 430... and then they decided they didn’t want to see my son who was running a high fever because they didn’t want to be there a few extra minutes. So much for medical providers that care.

Unfriendly receptionist. Very inconvenient system because of a machine that you have to enter your own information into to check in. It didn’t work so a human had to get up and help me.Then they told my provider that I told them not to run my blood tests even though I gave them my providers instructions, and said we need whatever he said.

I highly recommend choice medical. They take the time to sit and listen to any concerns. The staff is all very nice and attentative. The wait to be seen is never long.

I had been seeing Faith Cutrana for several years. She was my favorite pcp. This last visit however, was awful. She was not the same caring NP that I had trusted for eight years. Sadly, she is not knowledgeable in something as basic as sending off a blood test for lyme disease and told me theres no test for that. That is not true. I immediately went to see another Dr who even said that was unprofessional of her, and he sent off my blood work. The way I was treated was as if I was asking for her to do the impossible and treated me like I was crazy. I do believe that her ego is bigger than her knowledge.She lost a few patients over this as I recommended her to six or so people, all of which wont return. Bummer choice medical walk in.

Closed for the holiday, that by the way isnt until Monday.......I guess that having a head start on a Holiday two days prior is Standard Operating Procedure.. Whatever you do....DONT get ill on a weekend or Holiday around this place....Have a Great Holiday

I have been using Dr Faith Cutrona for about 8 years, prior was Dr Foy for 6 years. Neither have ever steered me wrong. I have always been sceptical about doctors but with these two I am completely confident. Cant go wrong here.
