He verbally and physically assaulted me during my visit. Firstly making sure I was not recording the conversation on the phone which raised a huge red flag. I thought maybe I should. While he is doing all this the nurse is in the corner of the room in shock, and Im overwhelmed with the words coming out of his mouth. The only thing I heard was that there were a hundred people out there with the same condition I was in, living full and productive lives. If I find out I can sue, that is what I will do.

Worse docter ever. Like to here himself talk about the opioid crisis. Wont prescribe medical care. Just want his money. His staff OMG they are so rude. Avoid this clinic for your own good.

Ive read all the other reviews and Im floored by them. Ive been seeing Dr. Hazelwood 12+ years and all 12+ has been nothing but positive for me. Hes very attentive and keeps it real and maybe thats the part most people dont like. Hes, definitely, not a pill pusher, nor doing dirt for the insurance companies, so, dont believe that. Dr. Hazelwood and his entire staff are amazing. And no, Im not a staff member posting this, nor any relation to any of them, Im his patient, keeping it real. Hey guys, keep up the above and beyond good work.

What a JOKE of a doctor!!! DO NOT SEE HIM...REPEAT...Do NOT! I wonder if he actually has an MD license. He obviously tells everyone the same thing, very similar reviews including my experience. If you ask any questions he appears defensive and gives stupid answers. He is definitely working for the Insurance companies and I plan to investigate his credentials further. If you are forced to see him...good luck! Oh...the one star is only because it is the least you can give. He actually rates NEGATIVE 10!!!

If you are involved in litigation and sent to dr hazelwood by an insurance company or your work. Either ask for another dr or be sure to record the office visit. There are reasons he has signs that say... no recording devices in his exam rooms. He will tell the patient what they want to hear and then dictate a report and testify to what the company that hired him wants to hear. Not to be trusted.

I have been a patient of Dr Hazelwoods for approximately four years now and he is a very honest Doctor. With all of the problems we have in Tennessee, I realize he has a very tough job treating folks with chronic pain. After having four different cervical fusion surgeries since an injury in the summer of 2011, I have nothing but praise for the work he has done with me not only with prescribing medicines but also getting me connected with a wonderful Physical Therapist!I understand that we are all different as to how pain affects our lives but I truly believe he has a gift when it comes to figuring out what is best for the patient in managing pain control.If all Pain Management Doctors in Tennessee were as strait forward and caring as he is, I dont believe we would have the Prescription Drug Problem we now have.He is a Class AAA Doctor!

If I could give this guy negative stars I would. My mom got hurt at work and has been forced to see this man for the last 5 almost 6 years. His nurses are always rude, he doesnt listen to a thing my mother says about how her pain is just getting worse just blames the weather and give her more pain meds. He keeps putting her on the same meds to help the nerve damage even though they do nothing for my mom.. oh and if you go to see him be ready for a 2 to 3 hour wait each time. Never in the years that my mom has seen this doctor has it been a quick in and out trip.. man is just in it to get paid and not to actually help anyone.

I went in for a nerve conduction test.The receptionist was very friendly and kind.The nurses were also friendly and nice.Dr. Hazelwood was very friendly and nice and made sure that I knew exactly what was going on during the procedure and answered any questions that I had and any concerns I might have been worried about.

He is unbievable! Does not care about his patients at all. He’s only in for the money and obviously paid off by work comp insurance. It’s so sad that this man is allowed to mess with peoples lives because he’s a crooked “doctor.” He will lie and falsify records to avoid treating a patient.

Dr. Hazlewood is a great pain management doctor. He listens to what is going on with you and offers the best available for your treatment. He also follows the very strict rules by the state for pain medications.

I was hurt at work and Dr. Hazlewood was one of Their Doctors to choose from. First visit he pushed opiods on me after I told him I was allergic to two different types. And he , hands pills out from his office not send you to a pharmacy shady.... Then I came back for several more visits still same symptoms and he stated that there was no reason for me to have the symptoms that I was having...I begged for ex-rays that took days and then he said he saw nothing so I continued to beg for test and got MRI after weeks and weeks all while trying to work and Night shift none the less. And he stated he saw nothing, so I demanded second opinion. and finally got one with a Neuro that looked at MRI and saw one and a possible second herniated disc that caused all my nerve pain , numbness tingling. he gave meds that were not narcotics but were geared toward nerve pain and suggested surgery. I went back to Hazlewood and he said that the surgeon was cut happy and that was all he got paid for was to cut on people.. and that I was fine go back to work. and I did I tried but couldnt hold things and it got so sever that after almost two and a half years i couldnt work and then at almost the three year mark I finally got my surgery and by this point the surgeon stated the nerves may never repair themselves and I will be like this forever. so Thanks to this man and his lack of empathy and knowledge, I was declined workers comp and denied my case so I went to the legal system and got the care only three years too late and the surgery success but the damage as I have found is too far gone due to the denial of my case and length of time comp took to get to a trial and long litigation. So NO I wouldnt choose this man again ! He works for the Company your work for so his intrest is put a band aid on a disk and go back to work,

Would not even look at my MRI . He only does with Workmen’s Comp. tells him to do horrible doctor would not recommend anybody seeing him

I went to him because I got hurt at work .And my time there was bad he dose not helping me he is for the company not for you . And I didnt want pain pills I just wanted him to help me with my back and it sad because I am still in the same way I was when I went to him sad that he makes money off of people like me
