I do not recommend this facility to o one. The pictures on the website look nothing like the inside on those pictures. My sister went and visited my mom on a Monday she found her 90% naked, food all over her like she had threw it up, she was hanging almost off the bed, she had dialysis on a Saturday there was a bloody bandage still on her arm that Monday. She was laying in her feces and she had developed a bedsore. She was admitted there that Thursday she did not have a bedsore 4 days later she had one. I was furious when my. sister called me

If you care for your loved one, stay away from this place. Unethical staff, totally lacking a sense of humanity or respect for their patients. One star I one too many.

What a wonderful caring facility. The staff is Superfine. Special thanks to Blanca and Yosleidi who are excellent social workers. The nursing staff is great. Special thanks to Manuela and her staff who have been so wonderful in having weekly FaceTime with my brother during this most difficult time. The Nursing Supervisor David goes that extra mile. Mayu in the Business office is so knowledgeable thank you for help in answering my questions. Special thanks to Marien Perez and Dr Martinez for the excellent care they give my brother.

Wonderful place. Very caring staff. Admissions, Director of Nursing, Assistant Director of Nursing, and unit supervisors are excellent and provide continuous communication, especially during this very difficult time. We are very lucky mom has such a caring group of people taking care of her.

If I could rate this facility a zero star I would. I went to go visit my family member and noticed nursing staff wearing open toed footwear, residents nude with curtain drawn and door open. I was appalled to say the least at the care and dignity of the residents/patients. The staff was unfriendly with visitors. I prayed my loved one was cared for with more dignity and love then i was subjected too. God be with the residents.

Honestly I dont know how this facility stays in business but I have a very good idea. First of all you can never get through on their telephone and it is nearly impossible to speak to a person. You are lucky if staff voice mail includes a greeting. When my Aunt was a patient there it took me 4 or more days before I was able to get anyone to call me and provide information. In addition, staff purposely delayed my Aunts discharge in order to exceed the number of days Medicaid would cover causing my Aunt to incur out of pocket expenses at the end of her life! But worse of all is the fact that Ponce Plaza staff not only stated my Aunt was not eligible for Hospice when asked specifically for this service, Ponce Plaza Staff also denied Hospice from assessing my Aunt for services. Thankfully she was able to return to the ALF she had been living for many years and not only qualified for Hospice she was under their care the final months of her life.

This facility is below standard; from the care given patients, like bathing and diapering, to the quality of the food and the cleanliness of the facility. From personal experience, if you have a choice, take your family member elsewhere.

This place is the worst for answering your phone calls and returning your phone calls. I have been trying to get someone on the phone for 2 months and not one person has returned my calls. And since I live out of state I just cant make a trip there just to talk to someone. They just dont care or just incompetent or just Lazy.

Horrible. my grandmother was sent there for rehab and when I went to visit I found her naked and no one knew why, I understand shes sick but that should tell you that they dont look at their patients they only go in once for the meds that need to be given and wont go back until the next set. a lot can happen to old people within an hour. worst place ever

This place fall out of professional ethics their bosses mistreat the assistants and do not value the work they do with respect to the patients they are victims of inimitable abuse by nurses who do not do their job they do not give them the pills and they throw them away many assistants do not They give the food to the patients and throw it in the garbage. I personally lived it and when someone from the government comes to check, they warn each other so that everything will be fine, but the reality is another, take care of their relatives and accuse the mistreatment and mismanagement of this place if you want to know hell before dying go through Ponce plaza

I have been accompanying a family member for 2 weeks and I have seen horrors that I never imagined that a country like this would pass where there are so many laws that protect its citizens. It would happen and in a place like this they play with those rights of unprotected and sick elderly people. where it is a sin to be alert, starting with the nurses a Miss who calls herself Blanca is the darkest person I have ever known since her patients scream in pain and do not give them their medicine, violating the patients right, until she did not finish talking to her husband on the phone and a few days ago a diabetic patient almost died because she did not measure her sugar outside the established time, which according to her is 3 times a day and the patient sweating cold and disheveled, but for one cna q insisted he does not make the story, well why continue if apparently nothing ever happens and they, entrusted or protected by their superiors, continue to act ruthlessly collecting their salary inhumanly, that is their only objective, good luck in your old age Miss.
