Here at Mission Palms everyone is friendly, kind and offers to help every day! Georgia was one of my best assistants, very attentive and checked on me often! I had a very good roommate for most of my time and we really helped each other pass the time by sharing our stories, it was nice to have a friend! Only negative was when I arrived nobody explained my meal routine, I did not know I was to pick my meals.

A family was in here recently and this is the worst place to take a family member here. If you have a family member who need medical attention and just not physical therapy DONT NOT TAKE THEM HER PLEASE. The nurses don’t know anything or help. My grandpa was trying to get mucus out from his throat and the nurse yelled at him β€œstop it” but it was really bad where he couldn’t breath and so he would try to cough it up but the nurse would go to his room to yell at him to β€œstop coughing”. I was on the phone with him when he was there and I keep hearing someone saying they need help but no one would and the next time I called him I heard a nurse yelling at a patient that would want pain medicine. We were supposed to pick him up and that morning we got a call saying he didn’t make it. And they mark it as Covid but he had already gotten over it by then. Nobody would never answer the phone they talk disrespectfully. Other patients would tell for help continually. But if you do take a family member here PLEASE BEWARE / CHECK UP ON THEM EVERYDAY.

I take pride in being a part of Mission Palms. I am very grateful that upper management has an open door policy and makes you feel welcome with any conflict that may need resolving. I have a great relationship with my peers and have immense respect for my DON, Administrator and those they work closely with to run our facility. Its the best place I have worked at as a nurse. We are not micro managed by our DONs or ADONs. The facility is not a punitive place to work, you have plenty of opportunity to move up if you so desire, and there is always opportunity for overtime if you wish. Another amazing thing I want to mention is whenever we have had code blues, other departments come to help; PT, OT, ST, along side the nurses and management. Its really an amazing feeling that you have a solid teams backup getting in line to do compressions. Ive never seen that in other facilities. I can genuinely say I love working here and being a part of this team and this family.

Everyone is so wonderful around here! Ive been to different places and none have been as great as Mission Palms! Everyone from the nursing staff to the maintenance department were so nice to me! Michelle in social services was so good to me! Visa, Vikki and Dawn took great care of me! I cant say enough about all the departments, I felt like it was a birthday party every day! Activities had so many things for us to do everyday and Angelica in activities made sure to always find me so I could participate! I will miss everyone!

I NEVER EVER write reviews. Im not the type because I always know places will have good and bad, pros and cons to their businesses. Im pretty flexible, understanding and a considerably forgiving person. I was brought up to be humble, appreciate things given, and count my blessings. This place was the WORST place to have taken my dad to. If you have a family member who needs medical attention and not just physical therapy, do not take them here. We had mistake after mistake happen,over and over. To the point we would not leave my dads side EVER, due to risk of losing him. WHICH ALMOST HAPPENED, till we called the ambulance ourselves, to get him OUT. The communication through staff is HORRIBLE. Missed medications, incorrect diet plans, missed dinners, gave him straws when his instructions clearly said NO straws, over looked many CRITICAL instructions. Was never cleaned, he was suppose to be positioned every 2 hours and staff would not move him in hours, .. almost days, he even got bed sores,i had to ask for him to be changed, we had to do our own vitals on him, Understaffed, underpaid, unknowledgeable nurses. I will say there were maybe 3 nurses who care about the patients. The rest are counting the minutes they are off, and WILL leave running without fulfilling their job responsibilities. It was the worst care giving Ive seen. I myself have worked in nursing homes myself for years, have visited many rehab facilities, and completely understand how rehabs typically run, but this was ridiculous. My dad (strong, very polite, uncomplicated type of man) was treated like he was on hospice care or out the door, had he stayed our worst of fears would of happened. His worst fear.We collectively lost sleep while my dad was there, knowing he was not in good hands. Thank the lord, they messed up so many times, that we got to never leave his side . If you need medical assistance, do NOT take your loved ones here.

The care that was provided to my mom was okay at best. Some staff really do take their job seriously, most do not. I am giving it a three only because of a couple staff members where kind an understanding. Keep your belongs and valuables at home or get them off your loved one quickly, some staff have light fingers and items go missing very quickly. When making arrangements to do a window visit the staff were accommodating. If you have choices, pick some place else for your loved one.

PLEASE BEWARE / CHECK ON YOUR FAMILY MEMBER REGULARLYMy aunt wanted to go to this facility because she had been there previously and she thought the physical therapy services was the best however, the facility has changed management/owners.Her stay was a disaster from the day she was admitted.1. Nobody knows anything2. Call lights go unanswered for hours3. My aunt had to beg for pain medicine4. My aunt had to beg to take a shower5. My aunts bed sheets didnt get changed for four days and finally I had to ask when the linens would be changed6. The food is canned/processed food7. Staff was angry because we bought outside food for my aunt to eat8. Staff spoke very disrespectfully when they even bothered9. Other patients would yell out for help continually10. Case management threatened to discharge my aunt as a retaliation tactic after we complained about the care and treatment of my aunt.Please, please be sure your loved one is being cared for appropriately.

My mom was sent her as part of hospice care. I cannot tell you enough good things about this facility. I was there a lot. They took such amazing care of my sweet momma in her final days. Shout out to Her nurse Vicky who was super compassionate while I was a crying mess most days. I got to be there with my mom in her final moments and not once did someone tell me I was not allowed to be there. It takes a special kind of gift to do what this staff does. Taking care of people as they pass or are just in excruciating pain takes a heart of gold. You all are amazing!

During the Covid shutdown, a family member was at this facility. The care he received and the shape he was in once we finally got him out, was horrendous! They failed to give him medicine that HIS DOCTOR prescribed, and was needed DAILY! He had feces dried on his backside, he had a sore that took FOREVER to heal once we got him home. Because family members were NOT allowed to actually go in to see their loved ones, they didnt care what kind of care they gave them. DO NOT recommend this place to any family member! They also marked his chart with covid information, which he NEVER had.

My mother was placed in Mission Palms for respite through Hospice. I trusted this would be a place that would take care of my mother. I was disappointed in the care they gave my mother. I was very frustrated when trying to find out how my mom was doing, no one seemed to have any answers to my questions. I asked how she was eating and if she had been incontinent while her stay at Mission Palms and the nurse had no information to give me, indicating they didnt know their residents and obviously resident care wasnt priority. I extended her stay to get me through my father-in-laws funeral and when they returned her home; 1) she smelled very strong of urine, 2) came home with wrong clothing, 3) was missing clothing items, 4) and most disturbing of all, she came home with a different wedding ring. I echo the other comment, please dont send any personal items of worth to this place with your loved one because there are personnel with sticky fingers. Also, because this was supposedly a place for respite for me, I was hoping she would come home clean and showered. Her hair had not been washed and she smelled strong of urine. I would not recommend this place for your loved ones.

This place isnt the worst but definitely isnt 5 stars, my love one was left in a bed for 2 days asked to get up, was told they had to wait to physical therapy before they could get up. Once physical therapy came they did bed exercises when my love one asked to get out of bed again to sit up in a chair, physical therapy said ok we can do that ill be back, no one ever returned. This place say they have daily activities from morning till night........ well no one talked about activities or even showed my loved one a calendar I get it covid probably canceled group stuff but offer a puzzle or something, you can expect people to be separated from friends and family, to be ok with total isolation. Not all of the staff here are terrible but alot of them are like drons and to be honest i can understand why since management is a joke. Good luck getting a call back and expect to be on hold for 30min or longer. My loved one tried to make a report about the care they received. What my love one was met with management who Argued didnt bother to listen and said if its that bad the call 911 when my love one said ok and started to dial they ran off and said they would get there director who wasnt much better, and still did not take my loved ones report, after that staff started ignoring my loved ones call light, my loved one sat in a solid bed for more than an hour before someone came into the room. Thats ok though since they didnt want to listen to my love ones report I made sure to report it to the respected agencies. I wouldnt recommend sending some who has trouble communicating here.

She doesnt listen to you when you say thats alittle rough I can feel that (hooking IV up to pic line) As a nurse, you should be able to take feedback. Well, she continued to do it, no apologizes nothing. Instead she brought someone else in the room to be a witness. Basically, Julia will talk over you and not listen. When my grandmother was first admitted to Mission Palms she tested positive for Covid, and was roomed in the Covid portion of the facility ( towards the entrance and front of the building). The staff and nurses were all kind and interactive with her. They all complimented how my grandma is so sweet and great to talk to. My family and I visited her a couple of times and called everyday.After her 10 day isolation she was moved towards the back of the facility for non Covid patients. It doesn’t even look like the same establishment. It was dirty, non welcoming and the staff in that unit were not as kind as her previous caregivers. She was there for 2 days and there was a complete change in her behavior and condition. We had an appointment to see her today and she wasn’t talking, couldn’t look at us and was shaking. We asked several staff members what happened and they all said this was their first shift day with her so they didn’t know. We wanted a full evaluation on her health and the paramedics transported her to the hospital. We’re currently waiting to see if it’s a possible UTI or dehydration (which is the usual reason there’s a change in behavior in the elderly). I hope that questions will be answered so she can be on the mend. She will not be returning to this facility upon discharge from the hospital. The communication is horrid-it was so hard to get a hold of my grandma and/or speak to anyone about how she was coming along in there. I’m just hurt and disappointed.

This place has been nothing but wonderful. They have been extremely attentive to my dad. We have had some pretty bad experiences in the past with a shove nursing homes, but this one has turned out great. Alyssa calls me daily to update me about my dad and will even FaceTime me to see him, as I live across the country. She is absolutely wonderful. Nursing homes can be tricky as we have experienced horrendous ones, but I highly recommend this one.

This facility was recently taken over by an experienced company on December 1. Unfortunately my family member ended up there on the 3rd. After a rough start (which is why 4 not 5 stars) they sorted out staff and got on track. The people in charge truly care and want to improve the facility. The staff that stayed, or came new were honestly fabulous. Especially Randy! They definitely won us over by the end and we strongly recommend Mission Palms.

This is just a money grab facility. They are understaffed and incompetent. My attorney is investigating why they treated my wife for 3 days without us signing admission papers. He is also filing a complaint with the state board.When she started having issues, they called up to say they could no longer treat her. She was originally evaluated and approved by their staff to receive occupational and physical therapy. They did nothing ! They put a mattress in the corner of the room and told us we were responsible for watching her.They were trying to transfer her to Mesa Christian’s Alzheimer/Dementia unit.While my wife’s condition got worse they kept shoving admission papers in my face to sign.Don’t let these people bully you!They are evil !Make sure you consult an attorney before you deal with any care facility. Get educated and know your rights.

I should have believed the negative reviews before letting my mom get transferred to Mission Palms. She stayed there for 30+ days. Maybe 2 or 3 RN or CNA’s or whomever they were actually were nice. Doctors are seen once a week and take no time with each patient/resident. He treated my mom based on her file history NOT how she actually was. Online pictures, activity list and calendar are all deceiving. But TOP complaint is the COMMUNICATION AND COORDINATION= BELOW 1 Star!! Yup this is a β€œfamily owned business” meaning that family members get to keep working there even if they don’t give a damn about doing their job. I am in process of filing several complaints with state departments. Photos to follow. It’s sad because no matter how many true honest reviews are posted people will still let their loved ones be transferred here and the patients and their families will continue to suffer.

Adding a 1 star is giving this facility too much credit. They have the most unprofessional employees that do not consider anyones well being. A family member had been transferred from the hospital to this place and he was better off staying at the hospital. The nurse assisting him was the rudest individual, Nara/Nora to be more specific. New ownership Ensign has taken a caring patient centered facility and turned it into a profit only corporate organization leaving patients, residents and staff to suffer their greed. They pride themselves on customers second (how do you proclaim customers second when the customers are vulnerable patients you are SUPPOSED to be caring for!) Employees were sold this culture of staff first but in reality the true culture represented by Clay the administrator and Angie the inexperienced DON is bullying, segregation, hostility and defamation of character. Staff were coerced into writing google and indeed reviews claiming quality care and excellent working environment and of course complied to keep their jobs. Resident rights are not observed and certainly not respected. When looking for a facility to provide care and compassion to your loved one PLEASE ask will my loved one aka your customer come first?..... Not in an Ensign building they wont!!!!!
