This is an amazing place! The staff made me and my sister feel welcomed and assured that we made the right decision to place our dad there. The entire team walked us through every detail and step of the rehab process. The nurses were attentive and kind. I can already see progress in our Dad after just a few days!

I must rave about the admission team at Powerback. They are the most welcoming group of people I have ever met! When I first called them to inquire about their rehab and assisted living, I was greeted with the most friendly voice and then transferred over to KB. KB immediately started my request to transfer my senior mom from another facility. And she was always in communication with me, even after hours and over the weekend when I was anxiously awaiting for paperwork from the old facility and her insurance to process. Day one and my mom is 100 % better. KB and the Powerback team is truly awesome!!

My mom was at another facility and we moved her to PowerBack in Lafayette. We definitely made the right decision! The admissions staff worked hard to get her moved immediately and kept me informed of everything. The night she arrived by a stretcher transport, I received a phone call from the front desk staff to let me know that she had made it safely and give me all of the important information. The very first day she spent there, the social worker and therapy director phoned both myself and my moms sister so we could both be a part of her evaluations. The admissions team verified all of her follow up surgical appointments and set up transportation to these appointments, which are all the way in Aurora. They are providing her with all of the therapy that she needs and she is gaining her strength back. She is getting excellent care and is in great spirits again! I am made aware of everything going on with her and dont feel like I am bothering the staff when I do need to ask any questions. This is a tremendous change from facilities that she has been in previously. I am very pleased and so grateful for PowerBack! My mom has been in several rehabilitation centers along the front range, while she has dealt with recurrent prosthetic joint replacement infections. She actually told me that this is the best care she has EVER received! We will never go anywhere else.

Since my mother got admitted I have called and called asking for an update, I have called to speak to the nurses station, I have called every department there and wasnt able to get a hold of anyone. So I finally went to the facility to get some answers. My mother is the only person that I have been able to get a hold of and she has told me they dont get her out of bed. Now when I do get a hold of someone all I hear is Im sorry but no actual plans of care. I wish I had done my research before taking my mom here. If you love your loved one, please go elsewhere and do your research. There is no care here.

Powerback processed my physicians referral promptly after it was determined that I would require extended rehabilitation before safely returning to my remotely located mountain house.I was admitted the same day to the Lafayette facility after being easily transported from the hospital. I was welcomed to a clean, well-appointed room with optimally placed safety devises.Within the first hour, I was visited by your physicians who determined therapy and medication needs. I must say that I received more attention from your doctors than those at the hospital, post-surgery.Over the course of the week, I was visited by physical therapists daily. I was pleased with each days progress as it increased in intensity and duration. Visits by occupational therapists boosted my confidence in simple tasks I had never been constrained doing before. One therapist listed six hip precautions which she posted in my room. I have since posted these on my fridge at home as daily reminders for a safe recovery.I would be remiss without praising managers for my care like Hannah Millian. Hanna was there for me every day with her rare combination of compassion and professional treatment. Support staff like Tamara and Claudia, also deserve recognition. In addition to keeping track of my vitals, they served-up Shellys fantastic food, and always had cheerful dispositions. I was even contacted by your nutritionist who recommended specific dishes to better mend my bones.From welcome to discharge, the total experience at Powerback was one Ill never forget. I developed the strength and agility to negotiate a very difficult household setting. Because of the extended care I received Im at home safely and on the next steps toward my recovery.Please extend my gratitude to my entire Vitality Team by sharing this feedback and recognizing them for a job well done. I work at an Assisted Living community down the street, and would absolutely recommend PowerBack Lafayette to help get your loved one back on the mend. Their facility is top-notch and the staff is exceptionally friendly. Strong infection-control procedures, and you can rest assured they are taking the safety precautions necessary to keep your loved ones safe.I often work with Lori, one of the Case Managers there. You can trust that she will make sure you, or your loved one, will be well cared for throughout their entire stay, from admission to discharge.

Both my mom and dad have done rehab at PowerBack in Lafayette. Its proximity to Good Samaritan and the convenience of Kaiser Doctors being right there makes it a no brainer. The care staff are terrific and the facilities are very nice.

Do not use this place. My mother went over to their facility after suffering a stroke. My mom was recovering from the stroke and was able to walk, go to the bathroom, and talk independently. So well that her insurance almost didn’t approve of her to go to rehab!We decided to send her there for a few days of speech and physical therapy knowing she would get a more structured schedule than in home care. Two days later we called several times not hearing anything from the facility leaving multiple messages because my my had another doctors appointment we needed to schedule. Hours later we received a call from power back asking what my mother’s normal mental status was. We told them that when she arrived there she is independent able to walk, talk and use the bathroom on her own. Not sure why they didn’t have that information. They replied “oh that’s not how she is now, she is unresponsive, isn’t able to move or interact with anyone. Would you like us to call 911 or make her comfortable and does she wanted cpr done?”Pretty much my mother had a 2nd stroke in their care and laid there for two days before we contacted them on a completely unrelated issue. For them to ask us to see if we wanted to make her comfortable or call 911.If you want your family neglected this is the place. I don’t recommend them. If I could give them zero stars it would be well deserved. Oh and they keep all of her medications. Don’t believe me. See how easy it is to get ahold of someone over there.

I have been with the company since 2016, and in my honest opinion this company has gone from ground zero up! A lot has improved since 2017, and I couldnt be prouder of the place I work! I highly recommend giving this place a try, everyone here is amazing. The staff provide the highest quality care they can as an unskilled nursing facility(And thats saying something), they are very careful about who they choose to work here, and if youre looking for a comforting environment for your family member, this is the place! Power back is a great place to get your power back! My 93-yr old mom broke her hip 10 days ago and she is up walking with a walker. From daily occupational and physical therapy to special meals, diabetes monitoring and an upbeat tenor, the faculty has really done a great job in providing an attractive, clean and well-staffed facility all designed to maximize both recovery and dignity. I couldn’t say enough good things about the effort of the staff and kindness of all involved and we certainly got first class treatment. Thanks for giving my mom her power back! And great job!

Overall very positive experience. My 96-year old dad has lived independently until a series of small heart attacks weakened him in mid-May. After hospitalization, I asked that he be transferred to PowerBack because they 1. Focus only on rehab and 2. Are near my home.After an initial bumpy start that was quickly remedied as the care plan focused on him began to be implemented, I was impressed. I had upbeat, positive interactions with everyone (except 1 person the first night he was there). I directly observed and interacted with several PT and OT staff and had several great conversations with the social workers. With one exception, the nursing assistants also rocked! My dad also liked the food. It was a mutual process for us all, including my dad, to realize that the original goal of getting him back home was not realistic (today he moved into an assisted living facility). I will forget some names but particularly want to mention Kristine, Geoff, Anne, Haley, Sue Lin & others for their professionalism, humor, communication, skills, and guided me through a freak-out as the goal shifted and I had to get to quickly know the world of assisted living as my dads discharge date quickly approached. They helped me problem solve, gave me advice, and most importantly did their best for my dad. Thank you all!

Power back has been a top notch facility for our parents who turn 90 this summer. The rehab staff helped Mom get back on her feet after breaking a femur. The nurses attend to clients better than we could. The entire PB staff care beautifully for our family members. It has been a big relief to have them here. Food is excellent, a great change compared to other places, space is bright and full of art. I would highly recommend.

The staff is very friendly and professional. The therapy department was exceptional; I was able to meet my goals in a very timely manner. The meals are hardy, and even with special diets, they were able to accommodate likes and dislikes. I had another family member have a very positive experience at PowerBack, and that influenced my decision to seek rehabilitation here.

Staff was basically good with a few exceptions. Food was good and the accomediations were great once I had a private room. Therapy technologists were very good when they understood directions. All and all it was a very positive experience.

Exceptional staff and facility. Always felt that my mother was the most important patient there. Every time I visited her room was clean, she was so happy with her care and everyone was kind and professional. I attended a physical therapy session with her and was extremely impressed with the instructor, Ron. I trusted Powerback with my mother’s recovery and I am very pleased with her experience. Recommend highly.

Be cautious. The facilities look great. They faithfully serve some sort of meal Activities are available, but are limited.The care is poor. it is very limited. Much talk and no action. If the patient is not self advocating and/or has an advocate, good things are not likely to happen. It is great to have a plan for the goal to discharge, but they do not do any strategy between admittance and discharge other than how quickly we can get you out with as minimal work as possible. The excuse is the CNAs are not paid enough. Pay never should impact your work. If you are not working to the best of your ability, and in the medical field patients are highest priority, than do not employ there. In fact, good mothers do not get paid to care for her child. Guess what, if someone reports a bad mother the government will take the child away. I wonder if the government would take action on PowerBack and all the other failing places like them? No, because we are living in the beginning of a social welfare system that the government wants.

Just returned from a very positive stay! The facility is less than a year old and I think they are still having some growing pains but nothing they arent working out. I thought, for what is in essence an institution, the vast majority of the food was excellent!! The home made buns I would gladly buy in a store!The staff were all very warm and friendly. If God forbid I were to ever have to return to a long term facility I would want it to be there.

This is the worst place ever. The facility is very nice, but the staff is incompetent. First day the staff failed to order my 79 year old diabetic mom a meal following her arrival. However, when you do get food it is not hot. Had medical records laying on counter at nurses station for everyone to see, which is a huge HIPPA violation. Did not give my mother a bath until requested 5 days later. Continued failure to communicate to POA mothers status even though repeatedly requested. Had several conversations with management regarding issues with facility and mothers inability to hear and remember things (slight dementia). Then facility relied on mothers memory instead of handling communication with POA as requested. Failed to have correct information on discharge plan. This place is a joke, and I hope Medicare and Kaiser do not renew contracts with this provider. They are very unprofessional, unorganized and understaffed.

There are key staff members who do a great job and make things work. Without them, Id go with a negative 5. HOWEVER there are huge communication and staffing issues. When you walk through the building, there are frequent business meetings but it is extremely common to call and have no one pick up. No option for voicemail. If you stand at the nurse station, the phone is always ringing and the call nurse signal is constantly going off. Staff do their best to be responsive but there just arent enough people. My dad said that hed call for help and no one would come. I recommend avoiding this place.

This facility has the worst nurses. I had stay here for almost a week and the nurses need to check the medications before you give it to your patients. I been itching all over the place dont bother to rub the cream that the doctor recommended. They even ask me what to do with it. Hello dont you have a computer to look it up!! Overall, food is great, and therapy also. The recreation needs to have more activities also

Im really concerned because I cant seem to get him or anyone on staff to answer the phone.My request is, if you work at Powerback. Please call me and tell me hes doing okay, and how I can reach him. My experience of this place was very positive, and I found that everyone here worked really hard to help me. All of the staff were very friendly and always available quickly whenever I needed anything. The food was good and well-balanced. The nurses and doctors seem to have a lot of knowledge - they looked after my medications and wounds and were patient in answering questions. The physical therapists were great as well - very knowledgeable about a range of issues that I had. They were good at pushing me as much as I needed but also making sure I didn’t do too much. I had major surgery on my whole right leg and was very weak when I came, but now I feel really confident coming home. They also made sure I had crutches and everything for my discharge, navigating the complex medical system. Great experience all round!

UPDATE 11/29/2019I initially wrote a terrible review for Powerback (see below) based on a rough start to my Mothers care but I wanted to give an update since I was impressed with the way they improved their care after some tough feedback. After speaking with the manager, CNAs & the PA that was in charges of my Mothers care, they were much more attentive and she had an overall good experience at Powerback. I have update my rating from 1 star to 3 stars because of the deliberate steps to improve service despite being over staffed.AUG 2019>Although this facility looks nice because it is brand new, as the manager confessed today, they are under staffed and therefore unable to provide the level of care theyd like to. My mothers pain hasnt been managed, most of the staff has been rude and unskilled, and they hadnt responded to her call button for help for 90 minutes. If you must send a loved one here, just be sure to have family or friends there as well so they can advocate for them.

I cannot imagine a more poorly run rehabilitation facility. Understaffed nurses who don’t mind telling you to get in line. CNAs who sit around on their phones and act bothered when a request is made. Powerback will only use the corporate pharmacy in Golden and if a medication is back ordered, you are out of luck. I seriously don’t know how they are in business, where is Colorado Department of Public Health? Medication errors are the norm and no one seems to care one bit. Medication dosing times ignored. A scheduled transfer from the hospital for a very ill patient is met without the room being ready and medications not yet ordered from their Golden pharmacy. A nurse says they are not allowed to order them until the patient is in the building resulting in pain for the patient. I do not know how they can have the word “rehabilitation “ in their name, it is a complete joke. Apathetic staff, no scheduling, seems like a walk down the hall is the extent of rehabilitation. Please spare yourself great grief and do not use this place for yourself or loved ones.

AVOID!!! ZERO CARE & WORST REHAB. Social club for employees, takes hours for staff to respond to help buttons. Rehab consists of checklists teaching how to use a grabber, microwave tea, go up stairs. Third back surgery, all this was known. Needed place to heal and get strong. Kicked out after a few days. They know how to submit paperwork to get paid and ensure their backsides are covered.

Poor quality rehabilitation, staff do not communicate with each other except when there screaming at each other, fighting, in the hall way. Lots of false promises, I will be back or let me get back to you is what you hear a lot of with no follow through. The infrastructure or proper management is non existent, thats not my department is normal response from management. Not sure if powerback does not train there employees sufficiently or the staff just hate working there, but there is serious issues with the Lafayette location. The facility is actual nice, but staffing issues are a plague there. If you care for you family take them somewhere else.

If you think you are going to get a planned Physical Therapy - Go elsewhere.If you think that there is a program that actively encourages interaction and improvement - go elsewhere.This is a modern and pretty new facility but it is more like a skilled nursing facility than a rehab center. If you are looking for a place to park somebody this might be OK. The Physical Therapists are stretched thin and do not have a schedule - they roam around looking for who they can give PT to rather than having a plan and schedule to work from. A clue should be when walking in, the PT exercise facility was rarely being used and never busy. I think the staff was stretched too thin across all services.The food was average but there was no active encouragement to get the residents to the dining room and the service was incredibly slow.

My father had a horrible experience at this place when it first opened almost 4 years ago and was refusing to go back. After listening to his complaint, the case manager assured him that she would do two things - not allow something like to happen again AND put TLC on his case as he entered this time. He was there for 18 days and was treated royally from the first day to the last day by the nurse and care givers in rehab. The physical therapists helped him to regain much strength and mobility that he had lost. The social worker did an incredible job of setting up follow up home nursing and PT help. Even the cleaning crew were exceptional in their oversight of his room and his physical needs. His assessment of Power Back went from a -10 to a + 9!

Do not send your family or friends here. They did not give my Mom medicine for 3 days. She was mentally stable when she got there and now is not. They dont care that they missed her meds and were about to administer a lethal dose of meds that could have killed her. Thank god I consulted an outside Doctor.My Mom is now in a psychiatric hospital in diapers and cannot eat on her own. They will not give us information on the nurse that missed her meds.

I have very mixed reviews about this facility. When my friend was admitted she had to go over 8 hours without her pain meds after a very painful back surgery. Then when they finally got that figured out they gave her the meds at 8:00, when I arrived at 9 a.m. she had not had any breakfast, how do you give pain meds without something to eat? Every time I called over there they would transfer to me the nurses station and no one ever answered. I called just now to the front desk to check to see if they received a package that was sent to my friend from her work team and she told me to call the Post office. I asked her if she had a record of receiving the package and she said she doesnt check them in. I suggested that she do that then when people were looking for packages she would know if they received it or not. She then apparently did not want to talk to me anymore and transferred me to another number. That person of course did not answer so I called back. She saw my number come up and just transferred me to another number without even picking up the phone. Her name was Nathalie, how rude! With everything that we experienced, I would not recommend this facility. There is more to the story but dont have the time to write it.

Mom has been here 2 days without any wound care on an amputated foot. Over half the staff called in sick on Super Bowl Sunday leaving my mom without care and over 30 minute response time after buzzing for a nurse. The food is horrible! Looking into pulling her early but leaving us nervous as to what we need to do to get her back up and gong! Very disappointed!

My Mother had a bad seizure, was hospitalized & admitted to Powerback Rehab, in Lafayette Colorado, around Christmas 2018.She was mentally stable when she arrived at Powerback, but not physically stable & that’s why she went there. To build her strength & get her “Powerback”. I met with Tina Hunter, (director of nursing) & a social worker. We went over everything. I was asked what my main concerns are & what I’d like them to specifically work on with my Mother & I clearly said, she needs to get stronger, independent & she can’t miss any of her meds. Tina told me that Mom was in great hands & that I didn’t have to worry. I visited often & she was getting better. One night at bedtime, I received a call from Gretchen (floor manager). She left a message saying it was about my Mom, it wasn’t urgent, but asked me to call her back. I immediately called her back. She said that my Mother wasn’t given a certain med for going on 4 days. My heart dropped. I knew it wasn’t good. I asked what medication & she said, it was her psych med. (The message she left, made me think it was just a dose of Tylenol, not a psych med). I asked how this could happen & she said it was somehow overlooked & no one knew until it was too late. At this point the staff at Powerback asked if we had this medication, if we could drop it off & they would just “give it to her”. We thought that was the safe thing to do, as they didn’t even have her medication in the building. So, we had her medication dropped off. The staff was just about to administer the med. Shortly before that, my brothers friend (a doctor) called him & said not to give her the full dose. (Thank goodness) If she took that full dose, her body could have went into shock & she may have went into cardiac arrest. Moving forward, the staff seems to be on top of this now. Not sure why it took so many days, when her med was in the computer. The plan now is to slowly give her little by little & keep a close eye on her, to make sure her body doesn’t have a bad reaction to this. I was told it may take a week or two, until she’s back at her full dose. In the meantime, she’s getting worse. Very mentally unstable. Staff is saying they can’t control her. Most of the staff were unaware of this medication error. Our family reached out to upper management. They are saying they are aware & doing everything they can to make things better. My mom continued to go down hill. The last day she was there, I got a call & was asked if I could go sit with her. Staff said they couldn’t get her to calm down. I spent an hour with her & left when she went to sleep. An hour later I got a called from her nurse. I could hear my Mom upset in the background. I was told they were calling 911, because at this point she was becoming a danger to herself, staff & other patients. She was takin to Good Samaritan. Our family was upset & wanted answers, that we weren’t getting. We wanted the nurse responsible for this error, to be held accountable. The staff kept saying they weren’t a psych facility & she didn’t belong at Powerback. Well, she wasn’t a psych patient, until the staff messed with her meds. I didn’t understand. I mean, my Mother wasn’t takin care of & she is currently hospitalized, due to this medication error & it seems like Powerback is brushing it under the rug. My mother is currently in a mental facility, in diapers & can’t even tell you what day it is. We were told the nurse was fired & later told that she wasn’t. All I asked for, was the person responsible to be held accountable. John Adams (the executive director) & Human Resources, is refusing to give me the nurses name & license number, so I can file a complaint with the state. I’m still filing. That’s the least that can happen at this point. If you are reading this & looking for a place to send your loved one, please don’t send them here. It is a beautiful place inside & out, but that doesn’t mean anything. My Mother could have died & they didn’t seem to care at all. Shame on Powerback Rehab & the terrible staff that our family had to deal with!

My wife was there recovering from a Coma. The bed was like concrete and I have slept in jails with better beds. Food horrible. They treated my wife as just another number and rushed her out too quickly. Half the time you eat voicemail. For DAYS, not a single call or update. I think the people are OK, and my guess is they are quite understaffed.UPDATE JUNE 2020:This Update is based off of my visit this month.What you need to know is, is that any patient coming into this facility needs to be a very strong advocate for themselves healthwise.This facility seems to be woefully understaffed. Generally speaking, if one hits the call button in their rooms you can expect it to take 15+ minutes for someone to respond. And oftentimes, one has to make multiple calls to have an issue resolved.If you need pain medication and hit the call button like I did, there were times where it took 45 minutes to over an hour to get them. This causes unnecessary discomfort and some dont seem to care. Other times a CNA will tell you that they will let the nurse know, then seem to vaporize.It did not take long to realize that if I needed assistance to use the restroom per doctors orders, that it was pointless to call. I ended up having to struggle with just me and my walker which was extremely difficult. Sometimes I would hit the call button and someone came to check after I had already used the restroom. Again I believe this points to chronic understaffing. One time, after waiting for 45 minutes after pushing the call button, I used my walker to get up and hobble to the door and stood in the hall. That got the attention I needed.It gets frustrating as hell.I found the food surprisingly good with a wide variety of meals. The beds are pretty hard, but you get used to it. Otherwise they work just like a hospital bed.However, there are truly some rock stars in the staff. You will be lucky if these folks are assigned to you:***Bessie from PT, Rebecca LPN, Chris Ghizzone, Merry Finer, Connor, Nina Agudo, and Dr. Ahalem. ***Occupational therapists and physical therapists are really top-notch. They will push you, but not too hard and they understand completely if you need to take a break.Of course there are some new challenges which I believe would affect any facility due to covid19. But I am very glad that they take such precautions so seriously.Again,if someone is elderly or do not have all of their faculties, I would have a lot of trepidation about it. You really do need to be a very strong advocate for yourself.

Just returned after a 12 day stay. delighted with the continuity of care from hospital to power back. also happy with kaiser support and care while there. have lymphoedema and numerous allergies so things can be difficult. liked the flexible ot and pt schedules allowing me to go to the dining room or social activities without missing therapy. independence was stressed in a positive way. it does consider itself a short term facility. this philosophy is probably not for everyone but certainly worked for me.

Walls are paper thin, very noisy, thank goodness my husband stayed with me at nite to make sure I got to bathroom. I also had some family member with me all day. Drugs were anissue and keeping me on narcotics too long. Left to have home care. Best thing we did was leave.

My 74 year old father was sent here last week after a knee replacement. The first red flag was that they transported him from the hospital - after just having had a fairly difficult knee replacement - in the back of a van with him sitting up in a wheelchair. It proved to be a painful trip for him. When asked why they didnt use an ambulance, they said it would have been too expensive.The also failed to get a complete list of medication from the hospital. As a result, they didnt properly administer medication for epilepsy that he has been on for his entire life.After the surgery he had been given too many laxatives and was having issues with bowel control. Since he had impaired mobility related to the surgery he wasnt able to get up to use the toilet and wasnt easily able to use the in-bed urinal. One of the nurses told him that he should just call when he needed to urinate so they could help him avoid accidents. But when the shift changed, the next nurse on duty told him that she was not doing anything to assist - she just handed him the urinal. After that the staff at PowerBack left him overnight in a soiled bed. When he was finally able to get up to use the toilet they had insufficient toilet seat extenders and he wound up getting feces on the toilet. Despite being told on more than one instance, they failed to clean it for over two days even though housekeeping had been in the room more than once and the head charge nurse was made aware of it.There were other issues as well. They openly stated that they were understaffed. They seemed to think that the fact that they had only been open for 2 weeks somehow excused this when the fact of the matter is that they shouldnt have been allowed to start accepting patients if they werent prepared.My sister and father ultimately decided that they would rather have him at home than at this sub-par facility and left without the doctors release.I would advise anyone considering this place for rehab to look at other options.

PLEASE READ FULL REVIEW……(GOOD but 1 BAD CNA)……..I recently was at PowerBack in Lafayette for 17 days in October after getting a Total Hip Replacement. Prior to my surgery I did a lot of research online and also visited a few rehab facilities. As everyone is aware there are GOOD and BAD REVIEWS for EVERY facility in the surrounding area of Denver. I am also aware that there are more people that do bad reviews to vent then good because HAPPY people most of the time do not want to take the effort to write GOOD Reviews but I am…….. I received great care at this facility along with having great female therapists to get me home again. I was located on the West wing and of course everyone has their FAVORITE AWESOME NURSE and CNA and mine were NURSE JULIE and CNA NICOLETTE. If you are Lucky enough as I was to have both of these woman help with your care; then you are BLESSED! I found out that both of these women go out of their way to help any way they can; and have an AWESOME attitude in doing so. On the FLIP SIDE of this……if you have a CNA named CONNIE, ASK for a different CNA as quickly as you can to say yourself from the GRIEF…..this woman left me stranded in the bathroom and when I asked her to pick up the remote that I dropped on the floor and could not reach it with my REACHER device. She said did you try to use your REACHER and I replied YES but it was too far….she then said did you try to bend over and reach for it”…..needless to say since I had a Total Hip Replacement and there was a paper on the wall reminding me not to break the 90 degree angle and what NOT TO DO. I was more than upset when she asked me to reach for it and to add insult to injury……LAUGHED about it…… and that was RUDE! The fact that she worked there and that it was her job to know my restrictions; really upset me. So it is MY OPINION that she has NO business WORKING there and having a “I DO NOT GIVE A DAMN” ATTITUDE!

Three days after arrival I came down with suspected norovirus. In the middle of the night I could hear other patients groaning. It took weeks for my GI system to completely recover. Staff would never confirm that was the diagnosis but I was put on quarantine. I had to repeatedly request thorough cleanup in the bathroom area. I checked in on a Monda night and was not offered a shower until Thursday. The place is overstaffed by nursing assistants and you never saw a registered nurse twice. I spent five minutes with a doctor who was only concerned about pain meds. They give out ophoids like candy. There is also little regard to other prescriptions. I was sent home with an inhaler that contradicted a cardiac condition. Obviously there will be food issues at a medical institution but this place kept serving meat despite repeated requests for a vegetatian diet.Bottom line. They do nothing but warehouse patients and it is one big medicare scam! DO NOT LET YOUR MEDICAL PROVIDER SEND YOU THERE!!!!!!!!!!

One of the worst experience in health care my husband has had. Lack of competent staff, lack of equipment, the worst food ever was just a few of the things wrong. It is a nice, new facility and it is unfortunate that the care was so sub-standard. They should not accept new patients if they can not take care of them. They want the patients 20 days of 100% paid Medicare and then they want you out. It is bad, bad, bad. Dont go there if you can help it.

This place os so disfunctional. I cant begin to tell you. I reported them to all the local case workers in the Hospitals and they were horrified at our story and lack of medical competency. They will not be getting refferred by any local hospitals soon if i have anything to do about it. Im not going into detail but i had to call the cops on the facility to get my wife out of there before they caused her any more physical harm.

My mother was here for over a month. She had some fantastic care, including her CNAs Amber, Isabella, and Jaunita. She had some great nurses like Jonda and Nina. She had at least one fantastic doctor, Dr. Bezecny .If it were not for him, we might not have found out about some issues she had and ultimately led to correction. She enjoyed her physical and occupational therapists as well and they helped her get stronger.The facility is very nice. There were a lot of good things to say about it. However, I would really give this place a 3.5 stars.It our particular situation there were issues with consistency of care with the wound care my mother received and some pain issues she had as a result. There was a lot of miscommunication and a few people who did not seem on board with the idea that she was in a lot of pain and that this wound needed more care than some nurses cared to give. (not all). One doctor, Megan, finally got the pain situation resolved. (maybe last name Mohott). Dr Bezecny also took her concerns very seriously and worked to find the issue with her leg. I am very grateful for the both of them.I think if Powerback worked on the few issues they have, they would be a great organization. They have many wonderful people in place. If your needs are therapy, I would recommend without hesitation.They need to be brought up to speed in some other areas such as wound care. I wish them luck and thank them for the areas they helped my mother in and wish continued growth in the areas they need to improve.

Discharged, July, 3, 2017Very poor rehabilitation facility! This place is so UNDERSTAFFED, and the nurses are very open about the problem. Most of the time the nurses claim to have 30 patients, and they are lucky to even have one CNA. There were many times her nurse said I have 30 patients and NO CNA!Upon admission, my mothers nurse stuck herself with a needle that required my 80 year old mother to be poked again to test for H.I.V. And other diseases. Her nurse was then sent home leaving them even more understaffed. Understandable, stuff happens!I received numerous calls over her 3 week stay that her meds were 1 hour and 40 mins late. I spoke with charge nurses every time this happened. On one occasion, the nurse came into the room and stated to my mother you are not going to cause me any problems and threw her medicine at her. She was removed from the facility and investigated. I never heard another word about it, and my mother was discharged about a week later.The first few nights she was there, the nurse was only giving her half of her narcotic pain medication.🤔 I would receive a call around 1:30am from my mother for a dose of medication that was due at midnight. I kept getting the same excuse when i would call to address the issue. oh, i over looked her other dose.They are extremely unprofessional, unorganized, understaffed, and they make promises without following through.The majority of the reviews from people are 1 star. They have started since day one of a 2 year old facility, and the poor facility reviews continue to current day. They are hiding behind a Medicare/Medicaid rating system. It appears that the admin does not give a hoot about the poor care they are providing or what their patients or family members think about their facility.Do yourself and your loved one a favor, and avoid PowerBack Lafayette at all costs!

This place doesnt even deserve one star. My grandfather was put into this rehabilitation for a surgery that required him to get his foot removed, this place does absolutely ZERO rehab, he was there a week and did rehab twice in that week. We immediately removed him from this facility because obviously their definition of rehab and our definition of rehab are two completely different things. After removing him from this establishment we received phone calls from this place about information that was wrote on his paper work. One stating he suffers from dementia... No one EVER brought that to our attention and its completely false. Im 100% positive this place literally does not listen to the families or the patients needs whatsoever and not to mention the fact, there is literally no staff on duty whatsoever, my grandfather would have to wait 30-45 minutes at a time after clicking the nurse button to arrive. They also forgot his insulin the first 2 days he was one there, considering the fact that he is a diabetic you would think that would be a top priority. Please do not contribute to this establishments lack of attention they give your loved ones, they dont deserve them and they definitely do not look out for them.
