This place is the worst . Don’t ever put your loved one here. Lived here for 3 years this place has had the most turn over of all there staff that I’ve ever seen.They use agency staff whom the nurses and CNA do not care about the patients. The nurses give out medicine when ever they feel like it, management does nothing to fix this issue. State has been in this home so many times nothing changes.All during covid the administrative staff changes the rules how they deal with covid on each case. Staff is ones with covid none of the patients have had covid but they treat patients like there the ones spreading this virus ! Patients have died living in this home from neglect. Stay far away from any Liberty Health & Rehab homes! There all the same

My family member was here for about 2 weeks. The building looks and smells clean, however looks can be DECEIVING! Interesting to see a response to negative comments about the facility from what appears to be an administrative staff person. Forget about getting an intelligent response from the administrator who doesn’t even know who the patients are. Forget a timely call back from Amanda, who never answered the phone while my family member was there. Her contact number ALWAYS went to voicemail. Visitors are required to give 24hr notice before visiting and the visit must be confirmed by Amanda. Good Luck with that one! This Rehab facility definitely has staff shortages and uses contractors to fill some of those gaps. We did have a few good CNA’s and other sporadic certified help, but only a couple here and there. This definitely will make a difference in your loved ones care and well being. Remember this is a Rehab facility and anything outside of those perimeters, like your loved one getting an assisted bath at least once a week will be your responsibility! Clearly, the low ratings aren’t the result of just one unhappy and hard to satisfy family member, but comes from those who were hoping for a positive outcome. What any reasonable human being would expect! Y’all have a BLESSED DAY!

My husband came here after a 2 month hospital stay. He is a amputee that needed to be able to transfer to and from his wheelchair. He used to transfer independently with me holding his chair. Now, after being there over 2 week is weaker than when he left the hospital. Its is a beautiful place but needs alot of improvement in care. The nurses and aids are wonderful but short handed. My husband says the food is good. Everything else needs work. Phones arent answered. Calls arent returned. Its a better place for respite care. A place if a caregiver needs a break. Your loved one would be clean and feed. Not for rehabilitation.

Worst place in Kernersville. My step father was left in soiled clothing for 6 or more hours. We could not get in to see him but took new clothes and the they lost them for over 24 hours. Even though his name was on the bag. He said the nursing staff was not friendly or helpful. The head nurse would not return our calls.He went home with no oxygen and it took 5 days and repeated calls the the social worker. Needless to say he is back in the hospital and his pneumonia is back.

My aunt was discharged from SS. Very clean building but nursing care was NOT good. Attitude was poor. Call bells frequently not answered. In the past I had great communication with the administrator Howard who was always willing to help even giving his cell phone number. Today I dont even know who the administrator is. I realize Covid has been a problem but the needs of the residents still need to be met!

My aunt was a resident at this facility recently. She really liked the care she received there. She praised the hard work that the nurses and aides done. She told us that despite the hard times in the world right now she felt comfortable and safe there. Most the staff greeted her smiling and was attentive to her needs. She did not care for the food there but it’s mainly because she loves the foods she shouldn’t have and while there she followed the type of diet the doctor ordered. Hearing her praise about the facility was shocking. I say this because my family and I was skeptical about sending her there because of some of the reviews. This facility is highly recommended by our family. We will definitely make others aware of the positive experience she had. Thank you to the staff there for comforting her during this difficult time in healthcare. We know your jobs are not easy, but thank you for doing your best to make a difference in one persons life. We will forever be greatful.

I have been to Summerstone myself a few times in the last couple of years; and, although the nursing staff isnt perfect, they did allow me to have my picture taken with my now 93-year-old grandmother pre-Covid back in 2019. It was one of the most incredible experiences of my young life.Another thing Ive noticed when I talk to them over the phone is that they care about my grandmother and even have a caregiver for her. They give her puzzles to work on, and they even let her play games like Bingo. Sure, I called them a few times to check on my grandmother yesterday and they were willing to prepare her for a video call with me; the only problem with that, however, is that I dont own a device thats compatible with an iPad, so that couldnt be possible.Finally, they managed to give me my grandmothers rooms phone number (they had already told me which room she was in when I asked), and from there I finally got to talk to my granny over the phone for the first time since getting to see her physically a few months ago. So, as I said before, the staff isnt perfect -- but they always do their very best, even if they do get a bit busy with other things. And, that is all I ever ask. C:

I was here for 2 weeks I was happy to leave. The staff was nice but always short handed. They were slow in delivering pain medication. Sometimes Id have to wait a couple hours before I got any. Look long and hard before deciding on this place.

I DO NOT recommend this place at all. My grandma was placed here over a year and her health declined so much while there. She fell and broke her hip ... nurses didn’t know how. After that she fell multiple times. Over the last 4-5 months she has lost 25 lbs got a bad skin tear and is currently in hospice. Don’t send your loved ones here at all.

Let me just say this is a beautiful place and it has so much potential but when trying to reach someone it is like going through hell. The sweet young lady at the front desk really needs to look into getting a new job she is too intelligent and gifted to be working the front desk she can always point you in the right direction and tries her best to answer your questions but some of those others I don’t see how they still ha a job! The nursing staff is terrible they don’t have anyone there on the weekend and their attitudes are the worse. I would not put my dog in a place like this. Unless they make changes this place will never thrive. They need to just replace all the nurses and nurses managers and start over.

This place is beautiful, it reminds me of a hotel. my mothers best friend was here for rehab and ended up moving to long term. they have some really good people working there during the day time. they are helpful and caring. I would recommend them to friends and family.

Mother-in law went to Summerstone after being released from hospital for rehab. From the very beginning of her stay there were numerous issues. The nursing staff did not agree with the medications she was taking, so they did not give her the medications as prescribed by her doctors. Instead of helping with the patients needs when they would ring the nurse button, the staff would come to the room, turn the nurse bell off and leave the room without providing any assistance. My mother-in-law would be left sitting in soiled cloths for hours on end. She was unable to walk and therefore had no choice but to ask for help. The staff says they are just too busy and running behind. Beds were not made and there were days where the staff would not help her remove her night cloths and get dressed until the afternoon. It took was two whole days just to get a cup of ice water in the room. We had put money on her account to get her hair done while there and they would tell her she did not have the money. Upon discharge we discovered the financial office never recorded the money, so the hair dresser had no idea there was money on file. Although I never ate any of the food, my mother-in—law did not like it at all. I did make my concerns known many times, even meeting with head of nursing and administrators, but absolutely nothing changed. I absolutely do not recommend you or your loved one stay at this facility. The quality of care is extremely poor at best.

My mother in law was there a month ago for physical therapy. She was treated well by the therapy people they helped her so much. She had 2 major complaints: 1- they did not give her the medication that she was taking at home. She went thru terrible withdrawal because of it. 2- the food was terrible. She was vomiting after several meals. Our personal opinion as family members is that its totally under staffed. There are 2 employees that were awesome. The case worker and one male CNA first name starts with an A they were both very kind and helpful. But getting anything done after 5 or on weekends is like pulling teeth. Its really terrible. Yes we would use them again but only because the other choices were much worse. Please do something about the food these people need good nutrition.

The facility is going through a difficult time with the c19 virus, I have a family member in the facility. We are frustrated trying to understand the process but the staff has been very helpful in communicating the situation. We appreciate the fact they are protecting the residents.Ms Smiththe director was very informative and professional in answering our questions. We know this will pass

My grandmother was here for a little over a month, and I must say it is a beautiful facility. she really liked it here. The staff were really friendly. we also learned they have long term care as well, which is a huge plus.

Very nice facility, staff were very friendly and attentive. Front Desk Receptionist very pleasant and welcoming always smiling. A family friend was there for three weeks and I would visit twice a week.

Very familiar with this facility. I have visited many patients. It is clean residents are well taken care of. Staff is very friendly and compassionate.When my mother has surgery this will be my first choice for rehab.

If it weren’t for the doctor here, we would be planning a welcome home party rather than a funeral for my grandmother. We did not send her here to die, we sent her here for REHABILITATION. She received the opposite. Do not send your loved ones here.

Very understaffed. Terrible response time when patients push their button for assistance. It is like a 20 - 45 minute wait (at least) for patients that are not able to get up and down on their own. Food served is terrible and it is never served on time for the patients. Place is nice looking inside but it is not nice for those who need help that are in there. Management and staff never return your call .

This is an excellent place to have you family member(I would highly recommend this facility for short term stays only) for the length of time my husband has stayed here I heard both good stories and horror stories. Lucky for us my husband had only good cnas and lpns. I just feared him having to stay as a permanent patient and being moved to the other halls as I overheard many horrible stories about that side and how it was ran. There was always a mention of a woman named Monique that would yell up and down the hallways I know my husband could not take that mentally. So I praise Jesus every day he is out of there

My brother in law was a recent patient on the 200 hall here recently passed away. Most of the care staff was wonderful and for that I would give 3 stars however there was one nurse who I think was the unit manager who had a nasty attitude toward the staff and toward her job in general. She brought the whole experience down.

It’s the worst place that you can take your parents to if you want them to live. The first time she was there it took her a week in the hospital to recoup because they almost let her die. Since the Kovic 19, she went back for rehab so we could not go visit her. But when we went around to her window she was laying face down in the floor with her head bleeding And no one in site. We had to Bam on the window to get somebody’s attention to get my mother out of the floor. If you check with people you noticed that people go in pretty healthy and then die while they’re there. My mother will never go back there.

I would not put my loved one here. When I checked her chart and seen the neglectful ways they treated my mom, I checked her into the ER. They could not find iv access because she was so she was so dehydrated. She was gone in less than 2 weeks. They sent me a bill for 800$. I would not treat a dog this bad and yet

I guess this establishment is used to dealing with people who are slightly senile or just wont stand up for themselves. My mother, who was there due to being in recovery from spinal surgery had an awful experience there. Theyre understaffed, and lacking in compassion. I trusted them to take care of her while I was at work only for her to tell me how she fell three times because nobody came for her when she waited for half an hour twice and once 45 minutes for assistance with bathing and using the restroom. Unacceptable.

I have a family member who is there for rehab and the CNAs are very rude to her and has nasty attitudes. I am telling you now if I hear one more thing about her getting disrespected I will drive from Gaston County and and show out. If your employees do not like their jobs they need to find a new one.

This is a horrible facility. I just recently had a family member here and was not impressed with the quality of care. This facility is very unorganized and it starts from the top. I voiced my concerns to a staff member and was met with a ‘get over it’ attitude. I write this review, because I want others, who are potentially looking for a facility for themselves/loved one, to know what they are getting into.

The place is really nice one of the prettiest facilities Ive seen some of the staff are pretty friendly and caring but some I have experienced over the times Ive been there they come in with strange attitudes as if theyre only there for a paycheck it frustrates me because of certain CNA become certified just to get the paycheck you have to have a heart to do this kind of care and sadly to say some of them dont and they dont like to come in and check on their patients like they should but like I said there are some CNA there that do a very good job the food is not so great but the place is pretty much Pleasant for most of the part!

My mom was there for a little over a year, when she was on the therapy hall, she was happy, all nurses and aids were wonderful due to a great nurse who has now left for a better opportunity. Before she left she told me alot of what was going on there that tied her hands. Another reason she left. They moved my mom to long term care and she went down from the first week. Only on good nurse cared for her, others would hand me a diaper for me to change my own mom. Not that I didnt want to do it because I helped them numerous times to change her, take her to the bathroom, even other patients family ask me to come change there mom into a pajama shirt, because the staff wouldnt. Thats not my job. Its there job to treat all these people with loving care. The Dr there diagnosed mom wrong twice and the last time caused her death. I will be following up with the state board of medical examiners about her. If you leave your loved one at this place, make sure you keep up with the symptoms and what they are doing. Even having POA, they gave me the run around to what I could get. Their was always an excuse for the reasons she wasnt taken care of. My mom. passed on July 7th of 2019, I never got a call from anyone there. Guess they needed the bed. Hospice of the Piedmont took the most awesome care of her and are still helping me with the loss of my mom. Think twice before putting your loved one in their care.

I would rate this place nothing but the rehab nurse that took care of my mom during the day was great. She always took time to talk to my mom about any concerns and was real loving and cheerful every time she came in the room. A real heart for the work. In my experience the nurse manager was lazy and just barked orders but never saw her working except once for a meeting we had and the director of nursing was either sitting in her office eating or no where to be found when needed. Some of the CNAs have real attitudes and they need to realize what kind of job their doing and why it’s important to have a good attitude. On the weekends it’s ghost town in there. Can’t get no help. The food is bad and my moms food/coffee was always cold. Mom was really well taken care of by the day shift nurse with the brown hair and glasses can’t remember her name but the facility as a whole needs some work.

The main nurse is a BULLY. They look down on CNAs. Refuse to sign slips. Don’t care about their CNAs. They don’t care about their patients I feel bad because my grandma is here.

Food horrible, no schedule for anything, if you cant do for yourself, I pray you have family. It looks nice and clean. Looks can deceiving. If your not in your room when meds come around they leave them laying on the table. Pain meds you have to ask for. There is more....but Ill leave it alone.
