My husband was taken to this facility from Memorial Hospital on November 30 about 2:30 in the afternoon. He only had a hospital gown on. They brought in a straight back wheel chair and placed him in it. He was wheeled outside to a parked unheated van in 37 degree weather, with a thin sheet partially covering him. He had 5 broken ribs and was on oxygen. They loaded him on a metal grate to put him on the van and I could see the grimace on his face from going over the rough grate with the pain to his ribs. I went to the nursing home to fill out paperwork and was told I would have to stay outside to fill this out because of the virus. I had asked where his room (302) was located because I wanted a room with a window that I could at least see what kind of care he was receiving. I said I would only send him to a facility that could provide that, I had been told he would have that at st Charles. I was then told he was not in a room with an outside window but one that looked into a hall but they would try and get one for him as soon as possible .I was not comfortable with this at all, I should have refused to leave him. They lied to me about the room. The staff said they would notify me later of the phone number for the room, in case I wanted to call.He was unable to talk on a phone because of his condition, so that was useless. I received a phone call that evening about 10:30 from a male nurse saying he was just arriving on his shift. He said, “I’m sorry but I went to check on your husband and he has expired” . He was not even there for eight hours.I yelled, how can that be he just got there, what happened?” I have not received any information as to what happened. This was supposed to be skilled care where he was to receive therapy so he could return home. They asked if I wanted to come up and see him. REALLY??? now that he was dead ,I was allowed in to see his room. Was he kept on oxygen? Did anyone check on him? We’re there any monitors there to check his vitals? Was he bleeding into the cavity by his lung again, causing him to not be able to breathe? He should never have been released from the hospital in his condition, he could not even feed himself. He was terrified to go to a nursing home where he was just put in a room to die alone. I blame this facility for his death along with Memorial Hospital for not caring about him.DO NO HARM

My family was so blessed that my grandfather was able to call St Charles “Home” for the last four years of his life. The facility is well kept and the staff are awesome! My whole family felt at peace having him there, knowing he was well cared for!

My grandfather was a resident at st Charles. He loved it there and he was treated like family. There’s nowhere he would have preferred to spend his last days. I’d recommend this place to anyone. I knew he was well taken care of.

St Charles is one of the best nursing homes I’ve ever been able to experience. The facility smells and looks clean, fine dining meal services, and they have resident retreats where the residents get to stay the night in a hotel and experience another city! They care for their employees just as much as their residents, quarterly wage increases, monthly parties, and free health insurance is offered. Voted number one nursing facility in Dubois county!

This place is an absolute joke for a physician namesake. I was released from this place after missing an appointment that was scheduled after they had canceled and rescheduled, canceled and rescheduled, and canceled then rescheduled so many times that I forgot which day they wanted me to cone in. I was prescribed clonazepam for frightening panic attacks I suffer from which the NPR complained about prescribing at every visit. When let go from the clinic I was therefore forced cold turkey from the medication. Its a benzodiazepine which when cut off is very dangerous, even deadly. I had an FMLA claim from the clinic and had missed work assuming she would still cover the rest of the month. Nope. Now I risk losing my job. I had to award 1 star since youre not able to give NO stars. IF Looking for a new doc or an NPR then stay away from this place. No care, no compassion, no ethics. What a way to treat someone whom suffers. To be fair I dont know how the rest of the caregivers treat patients, but since I cannot say her name directly Ill just say it rhymes with Nervous-son. Good riddance.

I have had the pleasure of being employed here since 2001. We had a previous rehab to home patient visit us today, and these are his exact words: This was home for 5 months. My care was excellent, and Im doing things now that I never thought I would. I even miss this place. I still do my exercises everyday, and am grateful for care I received from St. Charles Health Campus.. Ron F.

My mom went to St. Charles for rehab. The first night she fell & broke her hip. She fell 2 more times. She got pneumonia & finally a UTI before she passed away. We did not send her there to die. A few months earlier she was riding her 4-wheeler. This place was a nightmare she would call us crying because no one would answer her call light we would call & ask them to check on her. Very under staffed with no compassion.
