My wife was admitted for physical therapy post hip replacement. The staff is too busy to check her in. It took 55 minutes and the nurse asked only 3 questions. Nothing about medical history or current condition.I stood in front of the nurses station for 5 minutes with 2 staff members sitting there. Nobody acknowledged my presence.I used to come here with an old job. The attitude of the facility has changed markedly.Id seriously think twice about coming here for any sort of rehab or long term care.If my Outlook changes Ill put an addendum.

Very nice. Friendly staff offering assistance to help me find my friend staying there. I was in my wheelchair and found St. Catherine to be accessible everywhere I went. My friend praised them for the excellant care given him. Staff opened doors for me and I was told if I needed anything to just ask. Glad my friend is being cared for as family and friends would want. Thanks for making my visit so nice.

Everyday. Visited my mother. Each day wanted to fill in a comment on how the day is. Each staff member their treated my mother well with professionalism. Greeted me each n every visit.Everyone working n going to the next patient as needed. I celebrated my 50 Birthday here visiting my mothe while in rehab. My mother perhaps told someone here when my birthday is.And I believe a meal or two or three or ... How kind it is when you know something this simple can make someones day. Believe it or not. You never know what folks are going through Good or Bad. When your treated good it can be Missed. Then when your treated good again it brings back some good thoughts of the previous.I know its hard to tell what the feeling are of a place. I do know this for sure. Its a safe place. People helping people in low places like you would never expect. Sometimes hard working folks never get recognized but when you do it can be found in a place like this.If life fails you and your in a place like this know for sure their will be a positive direction headed your way.

This place is an answer to our prayers! The staff are very attentive, professional and went out of their way to meet all of our needs. The Physical and Occupational therapist challenged and pushed my husband toward recovery. My husband was a patient there for three weeks and I could not begin to tell you how much respect and peace of mind I had knowing that my husband was in the right place and in the right hands toward complete recovery!

I cant say enough good things about my Mothers stay there. From the staff, physical therapist, and overall care in general this is a wonderful facility.A special thanks to Terry Williams for taking steps to be sure my Mother didnt fall again. My mother fell out of her bed one night. Ms. Williams called to let me know. She then put a sponge type mat and night light in her room. My grandmother was here in 2004 and if anything it is even better!

Mom has been here almost four months and we dont have a single complaint. Everyone is so amazing and kind. It really takes the pressure off knowing shes so well cared for.Everytime I mention to someone that my mom is at St. Catherines, they say, Oh, St. Catherines is the best place in Jacksonville! Im SO pleased with your care for my mom!

My mom is in the inpatient rehabilitation section and has been there a week so far.. Ive found it to be very clean, neat and pleasant. There were a few issues with her prescriptions and diet but they were rectified when I brought it to their attention. The physical therapy seems to really be helping. I think they are short staffed as are most all places currently, so there response times to assist her getting out bed and such are not always timely. However, it has not been so bad as to make it an issue. I am overall pleased with her care there. Moreso than I thought I would be from a past experience about 4 years ago. I would rate it overall 4 out of 5 stars.

Went to visit my cousin while she was recuperating from an operation. Everyone was so friendly and everything was very clean. I thought to myself if I had to go somewhere to recover from surgery, this would be the place. Its close to everything including my home so its easy for friends and family to visit. They were also very safe with all the Covid rules.

I have worked with St. Catherine’s for years and would highly recommend this place for your family. The admission Stephanies have always been extremely helpful and are first class all the way. The facility is cleaner than most that I have visited and the nursing staff is top notch.

This facility is one of the best we’ve experienced. The staff is great and really will go above and beyond for your family. The physical therapist are awesome as well. They try their best to make sure residents have what they need. When they see them, it’s almost as if they see one of their own family members and treat them as such. Glad we chose them!

Though St Catherines is very nice and the people are great, i with they would take into consideration that some people drive a great distance to visit loved ones. 30 minutes is not enough time for a decent visit!! And its not becasuse they are very busy!! On my last 2 visits there were no other people waiting....

This is an amazing healthcare center that is focused on quality care for their patients. It is a beautiful building with professional and caring staff. I recommend St. Catherine for clinical or physical rehabilitation. One tour of this building and you will be convinced this is a best choice for your family member.

If you love your loved ones Do Not Allow them to go to this facility...my father-in-law was placed there friday night after a 2 week stay in St Vincents left the hospital doing better but needing some rehab for his mobility, to only go in this place for 3 days to not receiving some must have medication for a on going heart condition to having to be put back in the hospital with full blown pneumonia and also placed on a ventilator due to their neglect....my wife and mother-in-law met with Wade Taylor (executive director) who showed very little concern or compassion towards them....their actions will not go unanswered and God forbid the worse to come happens to my father-in-law heads WILL be on the chopping block ...Again if this place is ever mentioned run as fast as you can in the other direction for the sake of your loved ones

St. Catherine’s Laboure Place? What can I say except one should ever EVER put a loved one in their hands! It is not that the nurses are poorly trained and uncaring. It is simply that they are ridiculously under-staffed and therefore absolutely incapable of providing the type of health care treatment their patients so desperately require. Patients are left in urine soaked beds, their calls for assistance ignored. Medical treatments are not followed up quickly enough and their one call doctors rarely available in case of emergencies. This, however, is an administrative problem which, given the number of negative reviews on Google, is one St. Catherine’s has continuously set out to ignore and/or deny.Due to this and their inability to correct the problems they have, my mother developed a debilitating infection that could have been prevented had they but heeded my urgent phone calls and warnings; in particular, as they were made aware before she was admitted that she had just been treated for the same illness. Yet no one ever answered the phone when I tried to get through and, by sheer luck, I finally managed, they then failed to follow up in my request that she be seen by a doctor AND retested a timely and adequate manner. The double infection she developed while there then turned out to be so severe it could have caused her death and had a horrendous effect on her mental, emotional and physical well-being and the fact that they failed to check her for it as demanded was completely unprofessional and life-threatening. The state she was in when she left, at the end, was so shocking that I still have nightmares about it.Getting to see them was also absolutely impossible. One had to jump through hoops and scream and shout to do so. The person responsible for visitations was never there and the requirements were so vaguely outlined initially that by the time one realised what was required, it was another two weeks before one could do so.All in all, it made me feel as helpless as my mother while she was in their care. I was angry, frustrated and outraged at times by the response to serious health problems and personal issues. To add insult to injury, someone on staff stole her brand new iPhone on the day she was rushed back to the hospital because of their INABILITY to address her basic health care needs and I had to threaten the regional management to get it back. Furthermore, she was physically mishandled by the personnel while I was on the phone with her right before her transfer. They also bad-mouthed her to other rehabilitation centres, ignoring and deflecting their own hand / direct responsibility for her rapid mental and emotional decline.To make a long story short, St. Catherine Laboure Place is one of THE WORST rehabilitative care facilities in Jacksonville! So, if you value the lives and health of your family, my advice is to avoid it all costs!!!!!!!!!!!! The center needs a complete overhaul on EVERY level and, until then, should not be allowed to operate.

This rehab is awful. I won’t recommend it too my worst enemy. From the front staff all the way up to theDoctor they are all very rude.I had a nurse call me out in front of her staff and had me in tears. My mother was there for 10 days and got worse each day. She was not eating and never got out of bed I finally told them she needs to go back to hospital. The food is awful I won’ Even give it too my dog. I could go and on, but I think you get the point. I do have to say that the CNA, PT , Social worker and the young lady who welcomes you are Very nice. My mother will never go back to that place.

When I spoke with admitting about having my husband come there because his previous rehab was understaffed, I was assured that they had plenty of staff. That was not the case. St Catherines was way understaffed as well.I had to bring a special eye drop that they did not have, not once but twice due to it being lost.He filled out a menu but got whatever he got even being diabetic. They served 3 carbohydrates with one meal. Very unhealthy.On a good note, I will say that some nurses and CNAs went above and beyond when giving care. In the covid positive 1 North side a red head nurse, I did not get her name and Rhonda were two of those and on the non-covid 1 South side Nurse Tina and CNA Alora were also two of those people. I will also say that the therapy people were fantastic.Would I send him there again? Doubtful.

My Father has been at St. Catherines Labour for quite awhile now. All his life food has been one of his favorite things. Well the meals he has been served here have been outstanding ! He not only served in the military but also was a cook back in his day. Fresh vegetables , chicken pot pie , collard greens and desserts that made him so happy . The place is so clean . The nurses have been kind and professional . I can leave with peace of mind to know he is well fed and taken care of everyday. .

St. Catherines is one of the worst facilities that I ever been to. I have personally witnessed the regular abuse and bullying of patients under their care. The staff, in general, treated me and other patients as burdens, not human beings. Most of the time medication isnt t given on time and things such as wound dressings are only done if the patient makes it a priority to ask the nurse repeatedly. Most of the time these shortcomings were blamed on a lack of staff. Transferring the stress from nurses to CNAs and finally to the patients. After my experience with them, I truly believe that I was nothing but a number to them. Something to be used then discarded like trash.

Its better than a lot of places in the area but needs work. They ALWAYS seem to be understaffed. Always. And perhaps some sensitivity classes for a better part of the workers there would be a good idea. Im sure that most are used to seeing people in such a state, but it can be jarring for someone who has never seen it or is used to seeing their family member in better shape. If you are applying for Medicaid for your loved one, STAY ON TOP OF IT, or youll fall through the cracks.

The food and nutrition staff including the chef manager go above and beyond for every meal everyday! Greatly appreciate how they have taken care of my grandfather and grandmother! Love this staff and environment, the piece of mind I have while they stay here is above what I could ever expect! Thank you for everything!

Do not put your loved ones here. The care service is terrible. The front office people who you first speak to will sell you on the facility. But the people who actually take care of your loved ones are horrible. They only give them a shower twice a week and if they miss their shower day or say they dont want one they miss out. My dad nose had been bleeding all day and when the nurse finally answered the page she told me she had 19 patients and everyone had to wait their turn. I reported her to management. The Dr. called me they sent my dad to the emergency room and the cauterized his nose and found out he had Aspiration Pneumonia. I came to visit everyday and thats the treatment he received. Just imagine if you couldnt go see your loved ones everyday what would happen. Please put them in another facility!!!

While some of the staff was very caring the management turns their head to abuse and problems. My mother had a broken hip while she was in their care. They claimed it just happened during the night. However they also claimed they woke her up. Who sleeps with a broken hip. I stayed with her and witnessed nursing ignoring her roommate while she called and requested her medicine for hours they continued to put her off until she was shaking. They refused to help mom brush her teeth until I complained repeatedly and then they lost her partial plates. When they were found they blamed mom for hiding them in her dresser when she was not able to get into her dresser. I found her without a way to get water after a bladder infection they had placed her pitcher on her dresser where she could not reach it. The first roommate they gave her watched TV all night with the volume loud. Mom could not sleep and of course her health declined quickly. She died after one year in that horrible place.

Visited a family friend last week. I would do EVERYTHING you possibly can to keep yourself and your family members out of this place. They are nice to visitors, but I saw past that within an hours visit. There are too many issues to even list here, but I am compelled to leave a google rating in hopes that others will think twice before subjecting their loved ones to this type of facility.

An absolute disgrace. My Mother fell while at St Catherine’s. Over three weeks she never saw her attending physician due an extended vacation. I became aware of this (after repeated attempts to contact the doctor and I use the term loosely) only after my Mother fell. I removed her immediately. Beautiful buildings, terrific physical therapists. Great dietitians. I wouldn’t board my dog there

Have worked here for over 20 years. Used to enjoy coming to work.Love my residents and co-workers.New management is horrible. Spending money on aesthetics but it’s superficial.Staffing is always understaffed unless corporate visits then they have plenty of help.We work with 1 nurse to 30-40 patients. Often these are recent orthopedic patients. They need to be monitored closer for complications, pain management.On long term units we have several confused,wandering patients. Only 3 or 4 CNA’s to care for 60 people.It saddens me to see the quality of care diminished. I would not recommend this facility ... never thought I would feel this way.Also have recent scabies outbreak. It took getting the Health Dept involved and DCF to get treatment for residents and staff.Even then the dosing was incorrect.Quarantined unit is a joke!I know this is long but needs to be said.Pray for us.

I spent 2 + months between the hospital & this place. Can’t really say much good about either place but St. Catherine’s was the absolute worst. I gave it 2 stars just because the occupational & physical therapy were top notch, but their only contracted to be there. A few & I do mean very few of the employees were very friendly, however 90 % of the stuff we rude & just there for a check. They were never able to get me the care I needed, which is more than likely a re-replacement of my hip. Many employees witnessed the constant dislocation as well as multiple x-rays, and docs confirming it needed redone. I could say so much more regarding this place, including food, proper medications which were never on time, especially at night. You’d get them anywhere between 8:30 and midnight and practically had to beg. Go anywhere but here. When I do actually have the procedure done, I’ll go home before I let them put me in here again.

I came here from St Vincents yesterday for rehab, and boy do i wish I were back at St Vincents!!!They put me in a room and thats where I stayed. They didnt have my meds and I was in serious pain, resulting from a total hip replacement. The only one I saw was the aid, never a nurse. I was put in a private room that seemed far from population. I was stuck back there and I quess it was too.far for the nurse to walk. The first time they never even answered.the call bell. Im left alone in pain. Maybe I should call the news. My third day here was much much better, and I must give credit to two of the most courteous and caring nurses I know. Nurses aide Maria, who always answered the call bell. I only had to ring once becuz she would periodically check on me. The other nurse is the RN Annie. Shes always on time. No waiting on these two nurses!! Always with a smile. I love them

After knee replacement surgery, I was sent to St. Catherines for the next stage of rehab. From the very start, I found the nurses and the CNA to be very caring and helpful. I have no complaints regarding food, care, service and concern for my well-being. These professionals are confronted with a variety of challenges as each patient requires differing levels of care. For them to do the job that they did with each and every one of us was amazing, to say the least. EVERY aspect of my being there was of concern to all who were there and I never had any doubt that my best interest and speedy recovery was a goal shared by all. Please accept my heartfelt gratitude. Everyone was very nice. I was taken care of with great professioalism. Had physical therapy after surgery and had to regain my strenght. Was fed three good meals every day, if they were serving something I didnt like they would gladly give me something else. I would recommend St. Catherines to anyone. Great nursing care.

The only reason I am giving this one star is because I do not have the option to give it zero. My father was here in long term care from 2011 until he passed away in 2013. St. Catherines was wonderful then; however, my mother was recently there for rehab following hospitalization for a heart issue, and it was AWFUL! Ironically, we chose St. Catherines based on our prior experience with my dad. Little did we know that since he was there they are under new management. My mother was not supposed to get up by herself so she had to call for assistance to use the bathroom and depended on someone to bathe and dress her. She was left lying in the bed for sometimes as long as 18 hours at a stretch. Ringing the call button was useless. It was hardly ever answered. They are so short staffed as to be ridiculous. I dont know how they get away with it. There were times when there were only two CNAs on the entire wing. They have to deliver meals as well as assist patients with personal hygiene needs and feeding and dressing. If you complain, all you get are hollow apologies and promises that they are working on the issues. For the most part, the nursing staff, therapists, and CNAs are caring and competent; however, management is atrocious or the situation wouldnt be nearly as bad as it is. My mother was there for 2 1/2 months, and it was like this the whole time. I believe that managements only motivation is profit, and the patients and residents are suffering for it. Trust me. No one in our family will ever ask to be admitted there as long as Ascension is in charge. The nightmares started when they took over. I know this from personal conversations with residents who have left because of the decline in care and from employees who have left for the same reason; that and the fact that they are being overburdened because of short staffing. What a sad misuse of a beautiful facility. It was once a treasure, and now it is a nightmare.

The entire staff should grow a heart. Employees there to collect a check, lazy and rude! The turnover rate is unbelievable with staff and the framework staff will concoct to get rid of patients is shocking! Those 1 star ratings speaks volumes and the replies are ridiculous and not sincere. Time to get the (I Team) or big wigs to come out and shed a light on corrupt nursing homes. Rehab is one thing but those who are long term, my heart goes out to them.

Negative 0 is my scoreAids are racist, and selective on whom they choose to “care” for. Sarcasm is the norm. This place should be shut DOWN!!TV not working. My family member was so afraid to ask for help or anything because her roommate warned her that there would be problems if she did. And sure enough when the nurse button was pushed the reaction was “ I know you didn’t just ring that call bell”!The Aid was non responsive when ask for something, she would just mumble and walk out .I am not stopping here !!!!

Neglect, broken bones just a warehouse for the elderly. Some of the staff are wonderful people trying their best but the management covers up accidents and neglect. The Doctor ignores the obvious and helps with the cover up.

I know times are hard but our elderly cant speak up so we have to for them. Some people are very friendly but what we need most of all is more RNs. Patients are in pain and have to wait for meds. St Cath says Your dad is with hospice. Hopice says St Cath should be doing most of the care. Someone in the family comes everyday to make sure he is taken care of. Sad but necessary.

This place is awful. My elderly brother was there about a month, recovering from hip surgery. He fell several times and was released with a stage 4 heel wound and a long deep cut on his arm and they stated that they knew nothing about the heel and that he just had a little scratch on his arm. No one would even follow up about his condition. He had his heel cut to the bone (debreaded) because it was inflected.He is now on a ventilator. PLEASE DO NOT SEND YOUR LOVED ONES TO ST CATHERINES Not a 2 star. More like 0
