My mother-in-law has been there during her declining condition. Working with speech, and physical therapies, all CNAs and ward nurse staff have been kind and nurturing to her. She has said she likes this place and the people. Doctors and nurses have cared for as if she was their mother. Social services very responsive. I want to thank them for the care.

The only thing I can say is be careful about sending your loved ones to this place. The agency does not care about the clients, it is about MONEY!!! The fact that the state of Indiana and the ISDH is in and out of this place should be a warning to anyone who wants to send their loved one here. They have high staff turnovers and it is horrible the way that the agency does not want families too involved in their loved ones care. Could you imagine that they sent a client who was on oxygen to a doctors appointment without the proper amount of oxygen and lied to the state despite it being on videotape. Please be careful about sending your loved ones here. If they can the agency would bar families from visiting if they feel that their dirty laundry is being aired to the department of health!!!! The Executive Director is not helpful and does not approve the services necessary to take care of the clients needs!!!!!

My father in law was placed in Rosewalk (by Community East) and his care has been horrible. When his daughter told them he was having trouble breathing, they did NOTHING. The next day (today) he was sent back to Community with severe pneumonia. A week later he was released back to Roselawn, and his daughter went to visit and they hadnt even cleaned his room in the entire week he was gone. Conditions are filthy. He had a doctor appointment (at Community) at 1 p.m. this afternoon and his daughter reminded the staff of it and they assured her that they would get him to the appointment. Then, the driver claimed that he (father in law) canceled his appointment. He did NO SUCH THING. Now they are trying to kick him out because they are claiming that he violated the terms of his stay. They want to send him home, and HE CANT even walk on his own, and his wife isnt in much better condition. He has been battling cancer and needs round the clock care. We will be filing a complaint with the state department of health. Avoid this facility at all costs. They really should be shut down. If I could give them less than one star I would. They deserve zero!

Ive been employee for Rosewalk for 9 months and every since I started, I have felt welcomed. Itss a family type of work environment where you can feel easy about asking questions. I have seen how the employees go to great lengths to ensure that the residents are cared for beyond their expectations.

My grandma was in Rosewalk for almost 3 weeks. Had she been in there a few more days, she would have died. My uncle will be filing a detailed report with the health department. The staff seems friendly and her nurse seemed knowledgeable and caring, but they failed to give her baths, causing her to get a severe infection that put her in the ER with a 103 temperature. She didnt even remember me visiting her the day before and she has a great memory. The ER doctors asked whose care she was in because of how badly she was neglected and unclean.

I have had my mother in facilities with much higher ratings than Rosewalk. Each place has allowed my mothers health to decline severely. Example: In June 2019, when my mother returned to Rosewalk as a long-term resident, within a few days they discovered that her hemoglobin count was drastically low. Immediately transported her to Community Hospital. Where she was transported from detected nothing. The same a month ago due to heart fibrillation. Jackie, and the staff are hard-working considering the circumstances. At 85, my mother is still here and doing well.

Nurses do their best but their isnt any organization. It is impossible to get ahold of anyone and arranging transport to essential treatments requires an act of god.

Operated like a money mill for the owners. Extremely understaffed at night. I feel certain their bad care contributed to the death of my Father. Dad chose them because they were near the hospital if he had a problem. He ended up having several serious episodes and after hitting the button over and over I had to go out and find someone each time.I began to notice that the night staff would just let the buzzers go without answering them...ever. Thats right, they NEVER answered the patient calls. There were barely any of them there to get the work done that needed to be done. My Dad had an episode after I went home and before my brother came and passed away.Overall, most of the staff seemed untrained and poorly briefed as to my Dads condition. Wish wed gone back to Miller Manor.

Unbelievably bad... Get video documentation. Plant a clandestine video recorder. Make formal complaint to Indiana State Department of Health!My brother chose this place because of its location, sight unseen, for his end-of-life hospice care. When the ambulance dropped him off and we checked in, I got a general impression that this facility was pretty bad. His first night there, he tried to get water and pain medication at 2:00 am. He kept pushing his call button, but no staff would respond. He started videoing his situation for over two hours, until approximately 4:30 am when someone finally came to check on him. The person that came to check said that they couldn’t get water or pain pills for him, but someone would be there soon. He asked them just to fill his water jug in the bathroom and they said that they’d be back, but did not return. By 5:00 am he called 9-1-1 and had an ambulance take him out of the facility and back to the hospital. When the fire department and ambulance showed up, they called me (because I am his health care advisor/POA) and asked permission to take him to the hospital. With the FD paramedic, Rosewalk LPN and my brother all in the room on speaker phone, I asked pointed questions of the LPN, only to find out that the facility didn’t even have the pain medication that was prescribed to him in the building to give to him. His total stay was less than 12 hours. I am a chief officer for local Fire/EMS with 38 years of service. I have been in many nursing homes. I have never seen one this bad. Get documentation and file a formal complaint with ISDH!!! They lied about what happened, but the two hour video set the record straight.

My father was placed here for hospice care. His end of life journey was not at all pleasurable @ Rosewalk. Hospice is supposed to be a time of comfort and care from caregivers. My family feels cheated because we did not have a good experience as most families do when they have a loved one in hospice care. The staff is unprofessional, they have tattoos and piercings and look at you like a rival gang member. My family has walked in on our dad in respiratory distress, genitalia exposed, flat on his back with tube feedings running and his face in the bedrails. Each of these accounts were witnessed on separate occasions! Many times, my dad was left in his room with the door shut. He cannot walk or talk. We rely on the staff to make sure he is comfortable during his transition from this earth. My family feels that hospice to Rosewalk means, We do nothing for the patient. He is clearly neglected! Mr. Link, the administrator is aware. However, his efforts to change what is happening to my dad are not effective. DO NOT SEND YOU LOVED ONE TO THIS FACILITY. SPARE YOUR LOVED ONE!!!!!!!!!!!!!

My wife was recently in the rehab center for a month. While the staff was helpful and attentive, any requests we made, while passed on to the staff, was ignored by anyone above them. My wife was given 22 units of Lantis when her doctor had prescribed 33. She told that to the staff, but nothing was ever done. As a result, her blood sugar spiked quite often over 400. She says the staff recorded it but nothing was done. She called her doctor and was told they had no control or influence over the center.I received a call in the afternoon last Tuesday and was told that our insurance ran out. I was asked whether we would appeal or leave. We had to leave that day. I asked the person on the phone how much it cost to stay, she didnt know. I asked her to check and call me back. Instead of calling me, my wife was told the price was 220/day. She told me she had one more appointment with her therapist and had to arrange for oxygen. I told her to pay for the next three days if she wished to. She paid the next morning and was THEN told therapy was not included. She was tested for oxygen Wednesday at 1:30 and no one told her the results until Thursday. At that point we were so disgusted that she came home. We were never refunded for the lost day.

Mom has been her twice for rehab after a hospital stay. In 2017 she received excellant care. This year is different. She has been ignored by the nursing staff. A catheter was disconnected for therapy and not reconnected. When she used her call button she was told a nurse would have to inspect her catheter before they would change her urine soaked linens. Three hours later she was still waiting. When she asked for her pain meds, once again, a nurse would be back. Its been over an hour with no nurse.The therapists are caring people and so are a few of the nurses and cnas but 70 percent of the staff just want a paycheck.Avoid Rosewalk if at all possible. They promise the world but deliver very little.

Ive read some of the reviews, and to say the least, its heartwrinching! But I also was a short term resident at Rosewalk Village. I had 2 surgeries on my spine, and I was in very bad shape! In fact, I ended up staying there for 6 1/2 months. As a result of that, my husband and I was left homeless, and I was suppose to be released after 100 days, but they knew about me being homeless, and they allowed me to stay, until I found a place of my own. So, I do agree that in every place there are bad seeds, like my bad run ins with the social worker( Jackie )😒😒😒 but I am grateful for the caring nurses,(Susan⭐💜⭐💜⭐(evening), Sharon(night),Tameka(weekend), Cheryl(day), and my favorite......Shanelle!(day)💜💜💜💜💜⭐⭐⭐⭐⭐) and therapists(Karen(PT), Albert(PT), Sarah(PT), Chris(OT), and my favorite....Jake(OT)!💜) at Rosewalk! So, my prayer is that the Administration can weed out the bad seeds and hire more of the people that genuinely care.💗

My mother in law was put in the bathroom without her oxygen and left there for several minutes with it. The staff is rude and very unpleasant. She tried to explain what was going on and the staff didnt bother to listen and didnt care.

This place is very horrible my father in law is currently in there now. Husband has tried several times to contact administration there and left several messages with no response back .very unprofessional .my husband has tried to get poa paperwork sign on his fathers behalf with having case management escorting my husband out of the building and them saying he is not able to sign paperwork. We have been looking for to move him some where else so he could get the proper treatment that he should be receiving. Please dont take any of your love ones to this place stinks and management is very horrible

This place is horrible. My father passed away in there due to low supervison and no proper care. My ex husbands grandmother wasnt propped up correctly. She was given a peice of food and it was lodge in her throat and she died due to choking....

A relative is there now for rehab after a surgery. Call light on for 45 min, no response to use bathroom. 35min wait to get a wheelchair leg rest. Hairballs & gunk on floor.
