The nurse technician was who tested me was terrific... very nice, respectful & gentle!However, the price is EXORBITANT!!!!I would recommend going to some place cheaper.I would have given a 5 star, but because of the price, I cannot.

Fast and easy to make an appointment and great customer service. The technician who assisted us was excellent. 5 star service but removed one star as it is pricey if not covered by insurance. Overall had a very good experience.

Easy, fast, clean. I was happy to pay the $195 because getting it reimbursed by my insurance was faster than waiting on line somewhere else. Ill definitely go there again.

Really cute doctor/nurse did my nose swab and the whole process was super quick and easy! He was also able to answer all my questions and was super friendly despite the fact that I was openly weepy. 10/10 would recommend :)

Wow! I cannot say enough good things about Formation health! The employees at the front desk answered all my questions with a smile and were very knowledgable. The test was super fast and painless. I was in and out within 5 minutes. I have told all my friends and family to go to formation!

I was pleasantly surprised at how fast the process was! I was seen immediately and I was in and out in about 5 minutes. After I left, my results were texted to me about 15 minutes later. Definitely recommend!

Be aware these guys charge $195 paid upfront for Covid testing... worst of all they don’t tell you when you book the appointment so just don’t trust them. Criminal to profiteer from a pandemic.
