Trying to get a hold of the Dr. or nurse it’s just impossible. The Humana pharmacy was trying to get a prescription approved by the Dr. and couldn’t get thru. It looks like all calls are going to a centralize place when each clinic should take care of their own patients. Terrible service…..

I was grateful that I had a place to go to get my shots. I am thankful for Oak Street health center. It was fast and easy ,I went I. Got my shot waited for about 20 minutes to see if I had a reaction, then I went home. It was a nice experience, no problems. THANK YOU 😊

I just feel comfortable about everything and your service in the doctors office yesterday she helped me with all my needs

It was fast to get back in an examine room and shortly thereafter i saw my health care person. Everyone listens to what you have to say.

I have been requesting assistance to be able to access my portal, since Nov. 2018. Im still waiting, this is my only grief.

The care was excellent the examination room that I was placed in was dirty. I asked the nurse why was the room dirty and she said to me that other people have used the room. I asked her well isn’t the room clean after each patient. I asked why is there dry blood,on the floor, the exam table wasn’t changed the room was dirty.. Not saying that because I was in that room I got sick afterwards. I was growing up for a week and not able to eat. Please make sure this does not happen again. That room was awful. I only sat in the chair but if the room was dirty the chair was too.

I was in and out in no time! The process start to finish was professional, quick service, no lines to wait in. Very pleased with services rendered. Thank you.

They actually have zero stars but I had to submit one star to write this review . Just awful!!! I’m a veteran on disability transferring back to Indiana from Florida. My first goal was to get all required documentation’s transferred to the state of Indiana. I was needing my handicap placard form signed in order to replace my Florida placard . Melissa Jones-Saah refused such request demanding I take about five or six test echocardiogram colonoscopy mammogram blood test etc... Before I could obtain the form. Which by the way has nothing to the fact that I need a handicap placard.I had already signed for all my medical records to be transferred there they kept telling me they did not have them. Can you say Medicare fraud wanting to charge for a bunch of tests demanding I take a bunch of test before I can get the documentation I need for my placard. I’ve already been been deemed disabled!! Those tests are irrelevant!!!! I did file a grievance with Medicare I sure hope they look into that B...H.

Just satisfied with the service. There is room for improvement hard to get an appointment with the physician Dr. Shah.

They seem to have a record keeping problem. I was just there the other day and I was sitting there waiting for an updated script and the front desk forgot all about me until I reminded her and then I was told that they would mail the papers to me. And as usual they got my city address wrong. They also had a doctors appointment in my chart when I didnt have a doctors appointment.

They provide me with the care I need to stay healthy. Rides to n from appointments. Meds that keep my body going strong. They set me up with #Humana in 2018 to be able to provide me with better healthcare. I have a Medical Assist K9 helping me stay Alive. Do not seperate my dog from his handler. He goes wherever I go per Doctors orders. Oak Street got meds covered for me where other Doctors could not. I get a ride to my pharmacy after my appointment. I am thinking bout having my meds dropped off too me at my apartment. Not many doctors/pharmacys offer that........Oak Street Health Does.They do Labs in Office. Trust me when I say she was fast. It did not brusie after. Thank u for the speediness of the blood draw.Come out for a Bingo game, watch a movie, use a computer, listen to music, get healthcare info in their patient lounge.#EclecticMocha is my drink of choice. It is special made by me. Only a few know the secret to how it is made. Otherwise I will drink tea or flavored tea. Oak street usually has lots of flavors to choose from to keep me healthy. Warm, awesome and fun feels like your at home great doctor and an awesome staff they will treat u like family! Also pick up drop off is available upon request! A good place for the Latino community they have a good doctor and a wonderful team that treats them like family! They also have people to translate and answer all your questions. They also have transportation, so dont be sad.
