So sorry that we let our dad get transferred there from the hospital for rehab. They just let him lay in bed 23.5 hours of the day. We called several times a day as we were not able to get in to see him for several days after he was admitted. The staff would tell us what we wanted to hear and made everything sound like it was great. Then we would talk to dad and get a completely different story. He said the food was horrible. Without proper nutrition and exercise, he was never going to get better. After a few days we checked him out against doctors orders. We can provide better care at home.

Ive been a resident at citrus for about 5 years now and during my experience Ive had a lot of ups and downs but mostly ups. Unfortunately when I first came in the staff wasnt very welcoming.. now that I have warmed up to everyone Ive grown to have great friendships with them, staff and residents. Citrus felt like home right away. The people here really care about you. Like any facility you know your good nurses from your bad ones. At citrus I was able to go out In the halls to talk to everyone, I was able to go to the stores, I participated in activities like bingo and bowling and much more. Since covid started, all of that is on pause now. Things are different but I hope its temporary. The facility itself is very clean and organized. As for the food, its not going to be the greatest but it is definitely edible! Out of my whole experience and 5 years here Id have to say I would definitely recommend.. no ones perfect but this is the closest youd get to it.

My Husband has been in 3 nursing homes in the last year and this one has been the best experience so far. I was very upset because I would not be able to visit him but the staff always made an effort to FaceTime me so that I knew how he was doing. My husband is an super picky eater and the kitchen always made sure he was satisfied. (As much as could be expected) I was able to drop off some of his favorite snacks and his Diet Coke and he was very pleased with that. I highly recommend this nursing home and I am so grateful for the care that they provided my husband.

I don’t understand how all these place loose their patients clothes, after family members pay a lot of money for their stuff all of it gets lost then when you see your loved one they look like a homeless bum!!!! These elderly don’t have much and for thier clothes to get stolen or misplaced

I am a covid 19 survivor. When I came for physical therapy here at Citrus Nursing Center I couldnt get out of bed. Within 2 weeks I have relearned walking and my oxygen requirements have dropped from 6lpm to 3lpm. CNC has an expert staff of trained pros working 24/7 to help achieve independence. The food isnt bad either. I highly recommend this facility for recovery from hospital stays.

I wish I was able to post photos here, they would certainly turn your stomach! My sister passed away at Citrus in 2018 from repeated neglect. It is worrisome that visitors are kept out during Covid and cant check on their loved ones hygiene and general well being! So many nursing notes were falsified, I didnt discover it till after her death! Please! Please! Be vigilant!

I don’t care what anyone says about this place because we all have our own opinions. My uncle was here for several years and they treated him like a King. We made and developed many relationships with nursing staff. They were always good to him, answered all our questions during the Covid-19, provided hot meals, very clean and made sure he had his cell phone charged to receive phone calls from us. The nursing staff would notify us right away if anything wrong. My uncle passed away due to old age, and underlying conditions. This convalescent home will always have a special place in my heart for taking care of my loved one who is now with the Lord.

My grandma was a patient here at citrus and unfortunately it was around the time citrus got covid thankfully my grandma recovered and is feeling a lot better.. the staff was very good with keeping me in the loop.. I knew when she had good days and bad days. My grandma has no complaints the staff did what they could and I couldn’t be more grateful.

We were referred at this place for my mother in law’s rehab needs prior to Covid and it’s an awesome facility, very clean and inviting. The Administrative staff is excellent and the girls in the front office are very helpful and sweet. Great therapists and nursing staff, very competent and attentive to the patient’s needs.I will definitely recommend this place for any rehab needs.

My 94 yr old father is currently at Citrus, and is receiving excellent care. Their pureed food is delicious, Ive tasted it. Pork loin with herbs, mashed potatoes with gravy, green bean salad, chicken chili. He is learning to eat and swallow. Kaiser would have let him die, drugged and starving, because hes 94 after all. WHAT?!!! Dr. Constantino was the only Kaiser Dr. that believed my father needed respect and proper care, and sent him to Citrus. He will be coming home soon, within a week. He is clean, shaved, receiving physical therapy and is doing awesome. I see him every other day, and am always impressed with the staff and the cleanliness of the facility. This is a good place for your loved one should they need to go to a facility like this. Highly recommend.

I enjoyed my stay at the Citrus Nursing Center for several reasons, which I have stated below. First, I should say that I spent two weeks at Citrus due to suffering a stroke on my left side (I have never had a stroke before). When I arrived, I had no mobility in my leg and arm. This is the placement my health provider provided thus, we did not have chance to research, it was a true learning experience.Positives:Caring Nurses and CNA’s (Certified Nursing Assistants) even during the most difficult timesMany bilingual staffClean basic living and facility areas (beds changed daily)Food is what you would expect, low salt/fat. Appropriate portionsAccess to management when neededExcellent therapists, very knowledgeable (A+ for me)Excellent Covid-19 rules are in place for all who are in the buildingStaff always took my wife’s phone calls and deliveriesConcerns:Time it took for Nurses or CNA’s to come and help/answer questions, I had to be patient. (I will add, the average CNA had anywhere between 12-18 patients to attend too at any given time. What I found, some of these patients required constant monitoring, thus other patients where left to wait, this also pertains to the nurses. I highly suggest to the management that more staff is hired due to the type of high-need patients Citrus serves)This facility serves both high and low functioning patients. At times, the needs of the low functioning patients had a negative effect on the ability of the staff to attend to the high functioning patients.Inside gray wall paint is very institutional (dark). Wall pictures of old Fontana…why, who cares! All staff wear gray institutional scrubs (yuk). I viewed this atmosphere as counterintuitive to patients wanting to get healthy. Suggestion: a new paint job with positive colors, scrubs for the staff that are bright, cheerful, place slogans of positivity on the walls and hang pictures of famous people who have overcome tragic health/life events. Make sure to educate all staff about the slogans and pictures so they may share and possibly inspire their patients.Final thought: I have read the other reviews, I see their point of view. What I observed and this is from my point of view, families are dropping off family members and thinking that the Citrus staff has a magic-wand that will automatically make their loved-one well. This is not the case, the patient has to want to try and families need to be involved as best as they can (I understand, Covid rules make this difficult). For me, my goal is to be healthy, self-reliant and get back to what I consider normal. The Citrus made a difference in my life… now I’m getting better!Many thanks to Citrus and thanks for reading.

My mentally and physically handicapped son recently spent two weeks in this facility to get rehab for a broken hip. I was with him daily for several hours. It is not the Hilton, but the best place to recover I have come across in my 82 years. I would highly recommend it. The staff is dedicated and friendly to everybody. The place is light and clean, the meals are appetizing.I am grateful for the good care my son received. As I mentioned before I can highly recommend this nursing home. I do not think there are any better ones. At least I have not seen any when I had to visit several of my elderly friends.

First impressions are lasting. No one at front desk to start, had to ask administrator for assistance, this facility needs to have better housekeeping, dust on furniture (alot), trash on floors, etc. The only good thing was staff. Very friendly and accommodating. Dont have much confidence in this place.

When you speak of quality of care,leave this place off that list. Their incompetence is only outmatched by their inability to perform basic services for the patients. Things that we take for granted that we trust these people to handle they dont. I would Strongly recommend that anybody that maybe considering using this facility,NOT TO. Be aware that if you leave your loved ones at Citrus Careits like leaving them in hell. Here is a short list of things he dont do, properly bath their Patients, brush their teeth, change their catheters, we wont even mention whatwhat dont do for their patients that are incontinent. Once again in capital letters DO NOT USE THIS FACILITY YOU ARE JEOPARDIZING YOUR LOVED ONES LIFE.

If you must put your loved one in a Nursing home, Do not sign anything unless its explained to you! I was tricked into signing arbitration papers that were stuck in with a stack of other papers. My sister was physically and mentally disabled and being her next of kin, I signed her admission papers without knowing anything about arbitration. My sister passed away on June 23rd 2018 at Citrus Nursing Center. She was full code and they did nothing to save her! They called me for every issue night or day, and the one time she needed me the most, they didnt bother to call! Please be very active and vigilant if you have to put your loved one in a nursing home because they require 24 hour nursing. I discovered alot of falsified nursing notes. I discovered they were giving my sister opioids but charting zero pain levels!! This was after me telling them to stop giving her anything other than tylenol. Theres so much more to this but my main reason for posting is....Please Please Please!!! Do not sign Arbitration agreements! SIGNING ARBITRATION IN CALIFORNIA NURSING HOMES IS NOT MANDATORY!!! Anyone in their right mind would not sign if properly explained and told its optional!

*NO STAR* WOULD RATHER DIE THEN TO GO TO THIS PLACE, I WAS THERE 3-3 1/2 WEEKS WORST PLACE EVER WAS VERY LOUD ALL DAY ESPECIALLY AT NIGHT. THE STAFF TREATS THE PATIENTS VERY BADLY ESPECIALLY THE ELDERLY WHO CANT DEFEND THEMSELVES. I COMPLAIN ABOUT EVERYTHING TO HEAD OF NURSES MARILYN, I COMPLAIN TO SOCIAL SERVICES, EVERYONE I COULD COMPLAIN 2 & NOTHING EVER GOT DONE ABOUT IT. SUPPOSE BE THERE 6 WKS & SO BAD I DISCHARGED MYSELF IN 3 1/2 WEEKS I TOLD HEAD OF NURSES MARILYN I RATHER TAKE MY CHANCES AT HOME W/NO MEDICAL CARE. My grandma was here from June -mid September of 2016 and it was horrible. The staff is usually playing on their phones, watching tv or gossiping the whole time and they steal clothes/belongings from the patients. Technically anyone can walk in because theres never anyone at the receptions desk and the employees never ask where your going. On top of employees not doing there job the a.c. is usually out in most of the building so it gets really hot inside and what makes it worse is they dont always wanna change the patients that soil themselves so the smell of feces and heat does not mix too well in a building with little air flow circulating. My grandma would receive dialysis 3 times a week and the van would come pick her up at 5 in the morning, the driver would ring the bell so the nurses could let him in but most days the nurses would see the guy through the window and walk away because they didnt want to open the doors. Overall this place is horrible I dont recommend sending loved ones here!!

The worst service ever. The RNs belittle all staff around them and Infront of visitors as well. No security what so ever. No one is on the same page when it comes to patients care. Many patients are rooming around the faculty with no supervision one almost left the faculty in the wheel chair complaing they were hungry.
