My name is La-Trell Smith I realide in Orange Park, FL my mother lives in Eureka, CA on the departure from the rehabfacility my mom and I were told by her social worker that we would need 24 hour care for her at her Eureka home at that point in time. I did not have a clue and did not know how to get quality service that she needed. The social worker informed me that there where several within the county.
