I’m an employee here at Legends Healthcare in Euless, I couldn’t ask for a better place to work, We all work as a team. It’s a loving and caring environment. Since day one Mr. Chris Cantrell walked in the building, there has been lots of changes, he actually cares about the residents and his employees as well. Legends Healthcare of Euless has lots of new faces, lots of improvement. As and employee at Legends Healthcare of Euless, I give us 5 stars, and I would highly recommend us to all families that are in need of care, where we provide the best quality care, for all your loved ones....

Since I am unable to find a way to put a review on Legend Oaks of Fort Worth, I am putting it here. I loved working at Legend Oaks, other than sometimes, the staffing was an issue. Everyone worked well together. I loved the residents, loved the co-workers. I know there were a lot of changes that took place at the Fort Worth location, some not good (like some nurses unfairly let go), Ensign really works hard to solve problems. The facility, about a year old, is beautiful. There is a lot of potential for things to be great. Dr. Hernandez is great! Overall, though not the perfect facility (no facility is perfect), this is a pretty good place to work. When employees are treated well, the residents are in better hands.

I give you this faculty a thumbs up I do not work there, But I applied for the medical records position. The adminitrator and DON are very nice very professional people I had 2 interviews and they are just pleasant. The front receptionist she had a smile that would light up a room she is such a sweet and kind person. Even the staff they greet you which is good I like that. But I said that to say this its a good place just pleasant .

I was a little weary about admitting my mom here because of the reviews and it being an older building. Once we toured and seen how strong the therapy team was/is we decided to give them a shot. We had a rocky start but what I can say is that whenever I had an issue it was immediately addressed by the staff.

Avoid this place at any cost unless you want your loved ones to feel neglected. We took my grandpa here yesterday and the nurses were super unprofessional, non-caring and rude. This place will make you feel like anything less than a human. The other patient in my grandpa’s room was screaming in pain for at least 20mins before anyone showed up. My grandpa also fell from the bed knowing that he was at fall risk. No one gave him a bed with railing for his protection. This could have been easily avoided if the nurses cared enough, they didn’t want to listen to what the family members had to say. When we tried telling the nurse in charge that my grandpa is vegetarian, she rolled her eyes and walked off without acknowledging. My voice will probably go unheard and I doubt the facility cares, but I want at least the future patients/family members to be aware of the care they will receive. They just don’t care. It’s just absolutely disturbing to see how they treat vulnerable people.

The admissions process was easy. The medical team is patient for the most part. Mom had her favorite staff members of course. This is our third time coming & I highly recommend this facility. Therapy team is great & always consistent.

My mom Alice Johnson Ivey was at legend rehab about a month and half ago. I have nothing but good to say about the staff. The admission process was smooth and easy from the nursing, aides, therapy, physician to the social worker. The care was excellent and staff was very compassionate. We had the best experience ever and I would highly recommend anyone to go to Legends for therapy. Thank you Nancy, Sean, Okey and the nursing and rehab staff. We appreciate you.Kyla Johnson

This place is a living HELL! My father was there for 3 days and they almost killed him every day. He is oxygen dependent, they moved him around the facility by untrained employees that told him “you don’t need it, we will be right back”! We had other employees assist him to his room after rehabilitation, but couldn’t help him out of his wheel chair, because she was pregnant?! Why is she helping, when she can’t help if someone falls or needs assistance?! Another employee was to bath my father, could not get him into the shower chair because “she was short handed”. I helped get him on the chair and she wheeled him in the shower and left him there to shower himself, which they know if incapable doing. My sister and I had to take him a bath ourselves. One of the reasons we picked this location, was due to the in house dialysis option. The dialysis employees are under trained and unsupervised. My father almost passed out and couldn’t breathe during dialysis. He somehow managed to get one of the employees attention to revive him. The attending doctor tried to justify their actions, but all of them failed to report the incident to anyone. These were just some of the issues that happened to my father. Not to mention other ederly patients crying for help in the hallway and in the rooms. No one seem to care until they see I was there late at night. I reported all of this and more, the explanation to me was, we were unaware and we should have told them sooner and the other patients don’t have family members, as if this justifies the inhuman treatment?! I requested to have my father transferred, they refused, lied and even hide from me and the other facility to collect as much insurance income as they could. I have recorded all of this via video and audio recordings. I promise no one from the facility will refute any of these claims! This is a very unsanitary, unfriendly and should be shut down. It’s very unsettling that there are people still in that facility. The information collected has been sent to the local news stations.

If I could give a ZERO, I would! This facility is one of the worst ever.If you need a nurse to check on your loved one, good luck finding one.They will be a little more attentive at first, but that changes quickly. I was shocked to see the PATIENTS answering the PHONE! This happened several times.Very uncaring personnel.

I am well on my to recovery because of this team of therapists. Without their push and drive I would not have found my inner strength to get better. I cant put into words how much I truly love and appreciate this group. God Bless Director Natalie, Deb, Megha, Lyn, Hillary, Patrick, Kim, Jolly, Glenn, Melissa, Lauren, and Luna (therapy dog). I would also like to thank the healthcare staff for making me feel both comfortable and important. Director Sean, Angie, Brooklyn, Angela, and Lucys positive personalities have been simply intoxicating. Last but not least, I want to thank Fiona my transport driver and singing partner. She actually makes going to see the doctor an enjoyable experience, which isnt always an easy accomplishment.

Cant believe how small the beds are.Who would want to sleep on it! Id fall off its to narrow for older people even me! Its the most important thing for being comfortable.

They claim they offer safety and comfort in a home-like setting. They are supposed to be in the business of caring, they could have cared less. My grandmother was killed here, ask me how.

I was there after a below-the-knee amputation. The staff, physical therapists, and nurses were all top notch; professional, courteous, concerned with the patients dignity, and very good at their duties.I recommend Legend highly for you or your loved ones. /// Phillip Kurtis Stallings

If I could give this place a zero or even LESS than a zero I wouldnt think twice. My grandfather was transferred there from the hospital but ended up right back in the ER within 12 HOURS. He fell off the bed, but the nurses at the rehab said he himself made a bed on the floor which is not only impossible to believe but a very bad lie. My heart hurts so bad just thinking about how long he was there lying on he floor because they barely even answer any call lights. This place can not be compared to anything but hell. Both the nurses & the aides there are super unprofessional, unsympathetic, and negligent. Please please PLEASE do not send your loved ones to this place if you think youre going to do them any favor.

My mom has been a resident here for nearly 3 months for rehab and then long term care. The nurses are so very caring and wonderful. The food is not so good. (My mom is diabetic, and at first they were serving her sweet deserts). Now her food has no seasoning, sandwiches consist of 1 slice of meat and cheese and a plain bread. (No lettuce or tomato or condiments).If you can get someone to answer phone you are lucky. If you leave a message, calls are never ever returned. On a Friday they told me my mom would be moved from rehab, and on Monday, no one knew anything about it. Now that she is moved, they want to move her again in 2 weeks for some unknown reason. This is upsetting for an 88 year old woman who is trying to mend emotionally and physically. While in a Care meeting they had my moms records mixed up with some other resident, and the social worker kept getting up and leaving the room. Communication with administration office is rude and extremely frustrating.Here we are a few weeks later, leg infected... I went to visit yesterday and they took her to a hospital early in the morning, but failed to contact anyone and did not know what hospital (3 towns away) she was taken to. We had a preferred hospital on record, but that was ignored. This is all unacceptable.

Im only giving this one star because negative stars are not an option. DO NOT LEAVE YOUR LOVED ONES IN THIS FACILITY. I cannot stress that any harder. Dont do it. Their staff do not care for the patients. They leave the patients sitting in their own feces and urine for over 24 hours at a time, on a routine basis. Their admin do not answer their phones or return phone calls. They brutally injured my grandmother by yanking her around, causing torn muscles in her shoulder and never notified us of the injury. On another occasion they dumped my grandmother out of her wheelchair and caused her to gash her forehead so bad that she needed 10 stitches and 6 staples. I COULD SEE HER SKULL it was so bad! We finally got my grandmother moved, paid the final bill and 4 months later they sent another bill for the services we have already paid. NOBODY AT THIS FACILITY KNOWS WHAT THEY ARE DOING. By the way, dont bother calling the 1-866 number they provide, nobody answers, the voicemail is either not set up or its full. This place is the pits of hell and I have filed a complaint with The Texas Department of Aging and Disability Services.

Dont ever expect a return phone call. We called on Sunday 4/8/18 and had to leave a message. We have yet to receive a return call. They took my father in law to a Doctors appointment that wasnt even his. They only change the sheets when you force them to and only give showers to the residents when you have asked about 5 times. The food is horrible and the care given is sub par. The rehab portion is good but that is about the only good we could find. I hate writing this while he is still a patient there cause Im afraid of the action that will be taken. I mean, dont know that it could get any worse but, I guess it could.

My grandmother has been to several rehabs and this place is the worst. The bathrooms are dirty, doesnt look like theyve been mopped in months, mirrors stay dirty, drains full of hair that looks like its been hanging out for quite some time. No liners in the trash cans. Aids are putting dirty clothes in bags on top of neatly folded clean clothes. Old linens being left laying in the bathroom sink, used gloves being left on the table in her room that she eats off of. Nurses that only move from behind a computer screen to administer meds. Wrong meds given. Under staffed. Unfriendly (very unfriendly) aids. Aids more often than not are sitting in front of a computer screen. Folks being left places alone for long periods of time where they have no access to anyone for help or conversation, let alone a restroom. Folks being left without fresh water for days. Rooms look bare, sad and lonely. AC in the rooms dont work properly. Condiments are not to be found in this place. Food is bland and gross 75% of the time. I could go on and on. Im thankful my Grandmother survived his place, is home now and I PRAY she never has to return!!

I admitted my mother there because she had hip replacement surgery. Staff was amazing. We had a hiccup in the begging but I what I love about this facility is that when I brought my issue to them it was addressed right away.

Never, ever, take your loved ones here. Through their incompetence and lack of care very nearly killed my father after hip surgery. We told them nearly the first day there was a problem. What did they do? nothing until he nearly coded on them. We asked for at least weekly updates during the 3 weeks there; again nothing. They should be closed down by the state.

If you care about your loved one YOU WILL NOT LEAVE THEM HERE. My sister came in here bedridden and they would not provide the physical therapy ordered to get her legs in use again. She came in with no bed sores because the hospital would turn her and treat her. Not here. She has them now after being her 3 weeks. They also threaten to take food trays away from the patients if they complain or repeatedly bother them with request for hygiene treatment or medical attention. The staff will not refrain from yelling at the patients either. I have never dealt with a more rude, lazy and bullying staff in my life. Whats the staffs excuse? We are short handed. Really? Every time I am there, more than once a week, appears the staff is too busy visiting and playing on the internet. Dont both talking with the director. All talk and excuses but no action. If the employees dont like dealing with the elderly or ill why do they work here? Please, for the sake of your loved ones..Move them out of here!!!

I dont feel that Legend appropriately cared of my mother in her final days. TAlso, if you place a loved here be ready for a constant fight to get the care they need and BE SURE and get an itemized statement before you pay. My mother was in Legend 23 days as a terminal resident and 3 days after my mothers passing they called asking for payment and then again a week called and threatened me with small claims if I didnt pay that day. I have been trying to get an itemized statement since June 16 and Sheri in billing will not answer my calls or my certified letter. Also they misinformed me that Medicare would not pay for my moms stay but yet I talk with Medicare and yes they will. I cant even settle with Medicade without a statement. I tried to the hotline twice but nothing.

I worked at this facility and I was very upset with the care that was provided to patients. I did what I could to try to make the expereince better however I am only there 8 hours a day. Residents complained about a variety of things including the food, the rudeness of the nursing staff, and not responding to call lights in a timely manner. Many residents left before they were ready due to the poor service. How would you feel if you had to use the bathroom and no one answered you for hours. There are a high number of falls due to poor service and lack of attentiveness of staff. Not all the staff is bad; there are some really good nurses and nurses aides however they often leave due to being overworked. I would not recommend this place for anyones loved one until they clean house with the employees and get people who care in this facility.

THIS FACILITY SAVED OUR MOMs LIFE!!! This is the place to go for Skilled Nursing (Rehab) or Long Term Care. I recently had my loved one there for rehab with the possibility of transitioning to long term on Hospice. As I type this, it bring tears to my eyes. You see, my loved one was transferred to this facility from HEB hospital, and was very sick. Due to her comorbidities, she was a very complex patient to take care of. No other skilled facility would accept her, but Legend Healthcare did and I thank GOD everyday that they did. From day one, the nursing staff, rehab services, dietary, social worker, activities director, and physicians started assessments, implemented treatment, and developed trust. The nursing staff, especially the Director of Nursing, Tenisha Hardisty ensured that she received safe and confident care. The department heads did patient rounds everyday as a team on all the patients in the building, we had meetings to discuss plan of care, they educated our family along this tough process. As a family, we were never surprised with what was going on, they kept us well informed. The physicians and nurse practitioners are great. At the hospital, we were not given much hope. We were scared that we would have to put her in long term care. But this facility rehab my loved one and we were able to bring her home. She didnt need Hospice or long term care, as previous suggested by the hospital. I thank GOD that we chose this facility, they have a DYNAMIC team of Nurses, CNAs, Therapists, Physicians, Social Worker, and Dietary. I wish I could rate this facility with 10 STARS, they deserve it. I will forever be thankful for the care you provided. If there is anyone out there reading this and considering this facility....PLEASE TAKE YOU LOVED ONE HERE!! I will forever use this company, and should she need Long term care in the future, she will come back to Legend healthcare to live. What more can I say, YOUR FACILITY SAVED HER LIFE!! WE APPRECIATE YOU AND THANK YOU 10 FOLD FOR YOUR CARE DURING OUR TIME OF DESPAIR!! May GOD continue to bless the team of Legend Healthcare in Euless, TX.
